Analysis of Clinical Characteristics and Antimicrobial Resistance of Enterococcus in Nosocomial Infection

Abstract:Objective To explore the clinical characteristics and antimicrobial resistance of Enterococcus in nosocomial infections for guiding the prophylaxis and treatment in clinical practice. Methods Clinical data of 103 cases of nosocomial infections with Enterococcus were analyzed retrospectively,bacterial identification were performed by Microscan WalkAway-40 system,antimicrobial susceptibility tests were done by Kirby-Bauer method,vancomycin-resistant Enterococcus(VRE) and high-level aminoglycosides resistant(HLAR) isolates were carried out by agar dilution screen plate(ADSP). Results E.faecalis and E.faecium accounted for 92.9% of all Enterococcus.Infective sites were urinary tract,lower respiratory tract,skin wound,abdominal cavity and blood in turns.All patients had severe underlying diseases.The main risk factors of nosocomial infections with Enterococcus were invasive diagnosis and treatment manipulation,using broad-spectrum antibiotics,glucocorticoids,chemotherapy drugs or immunosuppressive agents.Mixed infection and mortality rate were 64.3% and 45.6% respectively.The resistant rates of E.faecium were obviously higher than that of E.faecalis,while the susceptibility rates of the microarganisms to vancomycin and teicoplanin were over 90%.VRE and HLAR strains were detected in 3.6% and 71.4% of all Enterococcus respectively. Conclusion Nosocomial infections with Enterococcus in the patients with severe underlying diseases are common,they cause principally infections of urinary tract,lower respiratory tract and skin wound.Mixed infection and mortality rate are high.More attention should be paid to these infections.Glycopeptide antibiotics are still the first choice for treating Enterococcus infection.
