Abstract:Objective Clinical features of primary biliary cirrhrosis(PBC) were reviewed in order to improve its diagnosis and treatment. Method The general conditions, clinical manifestations, biochemical and immunological changes, and pathological findings were assessed in 31 patients. Result Twenty five cases were females, the mean age at definite diagnosis was (49.2±10.7)years. Jaundice(74.2%) was the most frequent symptoms, pruritus (51.6%) and fatigues (32.3%)were the second and thrid, respectively. Three patients (9.7%)were complicated by ascites. Serum alkline phosphatase (ALP) , glutamyl transpeptidase (γ GT) and bilirubin levels were markedly elevated ((388.9±277.5)U/L, (381.6±213.2)U/L and( 176.4 ±176.1)μmol/ L, respectively).ALT and AST levels were mildly or moderately elevated ((79.7±46.3) U/L and(119.8±61.2)U/L, respectively),mean level of IgM was also elevated to (3.0±1.9)g/L. 92% (23/25) of patients had positive anti mitochondrial antibody(AMA). Ursodeoxycholic acid (UDSA) was efficitive in 61.3% of patients. Conclusions PBC most frequently affects middle aged women. The elevated level of ALP, γ GT and IgM and AMA positive may be crucial to diagnosis of PBC. Liver biopsy can help to identify the diagnosis and carry on pathological staging.

Abstract:Objective Clinical features of primary biliary cirrhosis(PBC) were reviewed in order to improve its diagnosis in our clinical practice.Methods Clinical data of 76 patients with PBC were reviewed including the clinical manifestation,laboratory tests,pathological findings and the response to therapy.Results Twelve patients were males(16%),sixtyfour patients were females(84%),and the mean age at diagnosis was(53.2±15.2) years.The most frequently complained symptoms were jaundice(64%),fatigue(60%),anorexia(48%) and pruitus(25%).Serum alkaline phos-phatase(ALP) andγglutamyl transpeptidase(GGT) levels were markedly elevated in majority patients,The serum total bilirubin level increased in fourty eight patients(64%).Twenty four patients(32%) had elevated serum immunoglobin M(IgM),and 64% of patients were antimitochondrial antibody(AMA)/AMA-M2 positive.sixty eight patients underwent pathological examinations,16.2% in early stage and 83.8% in advanced stage.Conclusion PBC most frequently affects middle-aged women and the main clinical manifestations are jaundice and some patients are asymptomatic at early stage.Elevated serum ALP and γ-GT levels together with positive AMA/AMA-M2 can help to diagnose.Liver biopsy is useful to confirm the diagnosis and differentiate histopathological stages.
Clinical Characteristics of Primary Biliary Cirrhosis: a Retrospective Analysis of 101 Cases

Abstract:Objective To investigate the clinical characteristics of patients with primary biliary cirrhosis (PBC) and provide reference for the diagnosis and treatment of PBC. Methods Clinical data of 101 patients with PBC who were admitted to the First Affiliated Hospital of Xi'an Jiaotong University between October 2005 and November 2016 were retrospectively analyzed. Among them, 7 cases were males and 94 were females, aged 17-80 years old with a median age of 59 years old. The informed consents of all patients were obtained and the local ethical committee approval was received. General data such as age, gender, symptoms and physical signs were summarized. Diagnostic indicators after admission such as routine blood test, liver and renal function, coagulation function and autoantibody, and selection of therapy were analyzed. Results The percentage of females among PBC patients was 93.1%(94/101). The incidence of asthenia, jaundice, hematemesis and melena, cutaneous pruritus was respectively 52.5%(53/101), 33.7%(34/101), 14.9%(15/101), 12.9%(13/101). The incidence of splenomegaly, peritoneal effusion, hepatic face and nevus araneus was respectively 73.3%(74/101), 48.5%(49/101), 33.7%(34/101) and 5.9%(6/101). Pancytopenia, abnormal levels of serum ALT, AST, ALP, GGT, TB and DB, and coagulation dysfunction were constantly observed. The incidence of DB abnormality was 97.0% (98/101). The positive rate of anti-mitochondrial antibody (AMA) and antinuclear antibody (ANA) was respectively up to 81.2%(82/101) and 79.2%(80/101). Among all the treatment, drug therapy such as ursodesoxycholic acid accounted for 69.3%(70/101), liver transplantation accounted for 16.8%(17/101) and other symptomatic therapies accounted for 13.9%(14/101). Conclusions Mastering the clinical characteristics of PBC patients contributes to early diagnosis and treatment of PBC. Abstract:BACKGROUND:Because of the diversity of the clinical and laboratory manifestations, the diagnosis of autoimmune liver disease (AILD) remains a challenge in clinical practice. The value of metabolomics has been studied in the diagnosis of many diseases. The present study aimed to determine whether the metabolic profiles, based on ultraperformance liquid chromatography-mass spectrometry (UPLC-MS), differed between autoimmune hepatitis (AIH) and primary biliary cirrhosis (PBC), to identify specific metabolomic markers, and to establish a model for the diagnosis of AIH and PBC.METHODS:Serum samples were collected from 20 patients with PBC, 19 patients with AIH, and 25 healthy individuals. UPLC-MS data of the samples were analyzed using principal component analysis, partial least squares discrimination analysis and orthogonal partial least squares discrimination analysis.RESULTS:The partial least squares discrimination analysis model (R2Y=0.991, Q2=0.943) was established between the AIH and PBC groups and exhibited both sensitivity and specificity of 100%. Five groups of biomarkers were identified, including bile acids, free fatty acids, phosphatidylcholines, lysolecithins and sphingomyelin. Bile acids significantly increased in the AIH and PBC groups compared with the healthy control group. The other biomarkers decreased in the AIH and PBC groups compared with those in the healthy control group. In addition, the biomarkers were downregulated in the AIH group compared with the PBC group.CONCLUSIONS:The biomarkers identified revealed the pathophysiological changes in AILD and helped to discriminate between AIH and PBC. The predictability of this method suggests its potential application in the diagnosis of AILD.
Clinical characteristics of patients with primary biliary cirrhosis

Abstract:Objective To explore the features, improve the cognition、investigate the more efficient treatment of patients with primary biliary cirrhosis. Methods The clinical manifestation, pathological change, response to therapy and the laboratory test of 46 cases of PBC were investigated retrospectively. Results The mean age was 51 ± 4 years, forty-three were women and three were men. The main presentations were fatigue(87%), abdominal distension (83%), anorexia(76%) and pruritus (56%). The major signs included jaundice(78%),bilirubinuria(74%), hepatomegaly and splenmegaly(65%). The results of laboratory test showed cholestasis and elevated IgM. ALP, GGT and IgM were 460.6 ± 251.5 U/L, 644.1 ± 246.3 U/L and 5.9 ± 3.7 g/L, most of the patients had a elevated ALT, AST and bilirubin. Forty-six patients (93%) presented positive anti-mitochondrial antibody(AMA) or AMA-M2. The main histological changes were necroinflammation of interlobular bile ducts, lymphocyte aggregation or lymphocyte follicles, smaller bile duct proliferation, bilirubinostasis in canaliculi, but the typical histological change appeared only in 33% of bioptic patients. Thirty-nine patients (84.8%) presented satisfactory response to ursodeoxycholic acid treatment. Conclusion PBC mainly affects middle-aged women, the presentations and signs have no specificity, the liver function test reveals a cholestatic pattern, the percentage for typical histological change is not high in bioptic tissue of liver, the diagnosis of PBC should be based on the biochemical and immunological indexes, the histological change and clinical presentation, and the ursodeoxycholic acid could effectively improve the clinical features and abnormal liver function.

Abstract:OBJECTIVE:To study the clinical features of primary biliary cirrhosis (PBC) in order to facilitate cognition of the disease.METHODS:Clinical data of 42 patients clinically and/or histologically diagnosed with PBC were reviewed. Anti mitochondrial antibody (AMA) negative/positive patients as well as the patients who were/were not associated with Sjogren Syndrome (SS) were compared in terms of clinical, biochemical and immunological features.RESULTS:Among the 42 patients, 78.6% (33/42) of the cases were females; the mean age at diagnosis was (61.1+/-10.8) years. The most frequent symptoms were fatigue. Serum alkaline phosphatase (ALP), gamma-glutamyltranspeptidase (gamma-GT) and total bile acid (TBA) levels were markedly elevated in the majority of the patients, whereas ALT and AST levels were mildly to moderately elevated. Thirty-one patients had a total bilirubin (TBil) level above normal. The levels of TBil and prothrombin time had positive correlationship with years of the course (P=0.000, r=0.696; P=0.005, r=0.424), whereas serum albumin level had negative correlationship with years of the course (P=0.002, r=-0.462). Thirty-seven patients had elevated serum IgM and 34 patients were AMA/AMA-M(2) positive. AMA negative and AMA positive patients were similar in terms of clinical manifestations and liver biochemistries findings. Serum IgM and IgA levels were significantly lower, whereas total cholesterol level was higher in AMA negative patients when compared with AMA positive cases. Fifteen cases were associated with SS, which were similar in terms of clinical, biochemical and immunological features when compared with the PBC patients were not associated with SS.CONCLUSION:PBC is mostly found in middle aged and old women. Elevated serum ALP, TBA and gamma-GT levels together with positive AMA/AMA-M(2) can help to diagnose PBC. AMA negative PBC patients are characterized by relatively lower serum IgM and IgA levels and higher total cholesterol level. PBC patients who are associated with SS have not substantial differences in the clinical, biochemical and immunological spectra of the disease.

Abstract:Objective:To explore the clinical and pathological traits of primary biliary cirrhosis(PBC) in different stage.Methods: Patients of PBC in the hospital were divided two groups by which were companied with chronic cholangtitis(CC) or not liver cirrhosis(LC).To compare with the biochemical and pathological index,complications in the different stage.After 1 year Ursodeoxycholic Acid(UDCA) was given,the prognosis of these patients were observed.Results: The pathological injury of CC group was less than the LC group,better to treatment.The mostly patients in LC group inclined to higher jaundice and chronic liver failure.Conclusion:Early treatment of UDCA is avail to ease the disease,but PBC with cirrhosis would be worse and have poor prognosis.

Abstract:OBJECTIVE:In order to improve the cognition and early diagnosis of primary biliary cirrhosis (PBC), we investigated clinical and pathological characteristics of PBC.METHODS:Clinical data of 37 PBC patients together with pathological findings of 20 PBC patients were reviewed.RESULTS:Among the 37 patients, 35 were women and the mean age at diagnosis was (53.4 8.9) years. The most frequent clinical presentations were jaundice (70.3%), fatigue (70.3%), and pruritus (56.8%). Serum glutamyl transpeptidase (GGT) and alkaline phosphatase (ALP) levels were markedly elevated in all patients (P50 was 467.50 U/L and 424.00 U/L, respectively). Among the 37 cases, 35 (94.6%) had total bile acids elevation, 32 (86.5%) had serum cholesterol elevation and 32 (86.5%) serum IgM elevation, 34 (91.9%) were positive for anti-mitochondrial antibody (AMA) and/or AMA-M2. Liver biopsy in 20 PBC patients mainly found: fibrosis in 17 cases (85%), interlobular bile duct lesions in 13 (65%), lobular mononuclear inflammation in 13 (65%), lymphocytic hepatocellular piecemeal necrosis in 10 (50%), and bile pigment accumulation in 9 (45%). The results of nonparametric test showed that GGT was related with pathological stage (P=0.002) and interlobular bile duct lesions (P=0.01).CONCLUSIONS:PBC is mostly found in middle-aged women. Accurate and prompt diagnosis of PBC should be based on the clinical presentation, biochemical and immunological indexes, and hepatic pathological changes. The level of GGT may partly reflect the severity of the histological lesions

Abstract:The serological hallmark of primary biliary cirrhosis (PBC) is the presence of high titer and specific antimitochondrial antibodies (AMAs). Although there is no global immune defect in patients with PBC, there is widespread dysregulated B-cell function, including increased sera levels of immunoglobulin M and enhanced B-cell responses to cytosine-phosphate-guanine stimulation. The mechanisms involved in this B-cell dysfunction have remained unknown. To address this issue, we focused on identifying the frequencies of B-cell subsets in patients with PBC and the mechanisms that lead to B-cell dysregulation, including the relationships with chemokine (C-X-C motif) receptor 5 (CXCR5)(+)CD4(+)T cells. Herein, we report that elevations of both serum and intrahepatic interleukin-21 (IL-21) were found in patients with PBC and, in particular, promoted B-cell proliferation, signal transducer and activator of transcription 3 phosphorylation and AMA production in vitro. More important, upon stimulation with recombinant E2 subunit of pyruvate dehydrogenase complex, CXCR5(+)CD4(+)T cells in PBC produced higher levels of IL-21 than healthy controls. Additionally, sorted CXCR5(+)CD4(+)T cells increased production of AMAs by autologous CD19(+)B cells. Indeed, elevated expression of intrahepatic chemokine (C-X-C motif) ligand 13 (CXCL13), a key chemokine of CXCR5(+) cells, was uniquely found within the portal tracts in PBC, accompanied by infiltrates of CD4(+), CXCR5(+), CD19(+), and CD38(+) cells. Conclusion: CXCL13 promotes aggregation of CD19(+)B cells and CXCR5(+)CD4(+)T cells, which directs the aberrant AMA response by IL-21. These data have implications for potential immunotherapy and also reflect the unique lymphoid biology in liver of PBC. (Hepatology 2015;61:1998-2007)
Retrospective study and meta-analysis:the clinical features of patients with primary biliary cirrhosis in recent 20 years in China

Abstract:Objective To investigate the clinical features of patients with primary biliary cirrhosis (PBC) in China. Methods The reported articles about clinical analysis of patients with PBC in China were searched. The quality of included studies was critically evaluated. Meta-analysis was performed using RevMan 4.2 software about controlled trials. Results ① Ninety-one literatures including 2315 patients wuth PBC were included. ②The common symptoms in PBC were fatigue (54.54％). AMA was found in 74.1％～100％ of patients with PBC, as well as the prevalence rate of anti-M2 range from 45％ to 83％. ANA antibodies present in 20％～83.78％ of patients, the most common antinuclear patterns were nuclear-envelope (38.65％).Increased levels of IgM [(2.8±0.7)～(7.3±5.1) g/L], IgG [(16.5±4.9)～(20.5±5.9) g/L] were found in these patients, and the most common liver histologic classification was type Ⅱ (36.6％). Sjogren's syndrome occured significantly more frequently in PBC (1.96％～34.61％). To the end of follow-up period (five months to nintysix months ), 3.80％ of patients were dead. ③ Meta-analysis performed in several case control studies, showed no significant differences was found in liver tests including of alkaline phosphatase (ALP), alanine aminotransferase (ALT), aspartate aminotrans ferase (AST), total bilirubin (TBil), or gamma-glutamyltransferase (GGT), IgG levels between AMA positive groups and AMA negative groups. However, lower levels of IgM presented in the latter. While it also showed that levels of ALP decreased after ursodeoxycholic acid (UDCA)therapy. The levels of TBil were lower in the elderly than in younger group, but the mortality ratio for liver diseases was higher in the former. Conclusion The clinical characteristics of the present series in China are mostly similar to those reported in other countries. Because of the low quality and the small number of included studies, larger sample-size, randomized, double-blinded controlled trials are needed.
Immunological Pathogenesis of Primary Biliary Cholangitis

Abstract:Primary biliary cholangitis (PBC) is a chronic cholestatic autoimmune disease caused by the attack of the liver by the immune system and is commonly seen in middle-aged women.The pathological features of this disease include the infiltration of a large number of innate immune cells (NK cells,NK T cells,and monocytes,etc.) and adaptive immune cells (T lymphocytes and B lymphocytes) at the portal area and injury of small intrahepatic bile ducts.In addition,PBC patients have high serum levels of anti-mitochondrial antibodies and inflammatory cytokines such as interferon-γ,tumor necrosis factor-α,interleukin-6,and interleukin-12.Although current studies have shown that autoreactive T cells are the major effector cells for PBC,NK cells,NK T cells,monocytes,and B cells around the portal vein also directly or indirectly participate in the development and progression of PBC.This article systematically summarizes the role of innate immune cells,adaptive immune cells,and related chemokines in the pathogenesis of PBC,in order to provide a theoretical basis for a deep understanding of the immunological pathogenesis of PBC.
Clinical and Pathological Analysis of Primary Biliary Cirrhosis-Autoimmune Hepatitis Overlap Syndrome

Abstract:OBJECTIVE:To explore clinical and histopathological characteristics of primary biliary cirrhosis-autoimmune hepatitis overlap syndrome.METHODS:Clinical data and pathological findings of 10 patients were reviewed.RESULTS:Serum glutamine transpeptidase,alkaline phosphatase levels,alanine transaminase, aspartate transaminase, serum IgG and IgM were elevated in all the patients. They were all positive for anti-mitochondrial antibody and AMA-M2. Nine patients were positive for anti-nuclear antibody and one patient was positive for anti liver-kidney microsome antibody. Liver biopsies in these patients revealed: ten patients had bile duct lesion,hepatitis activities ranged from moderate to severe, and fibrosis ranged from S1 to S3.CONCLUSION:PBC-AIH overlap syndrome is mostly found in middle-aged women. It has the clinical and histopathological characteristics of both PBC and AIH. Accurate and prompt diagnosis of overlap syndrome patients should be based on the clinical presentation,biochemical and immune indexes,and hepatic pathological changes.
Diagnosis and Treatment of Primary Biliary Cirrhosis-Autoimmune Hepatitis Overlapping Syndrome:An Analysis of 30 Cases

Abstract:Background: Autoimmune liver diseases include autoimmune hepatitis (AIH), primary biliary cirrhosis (PBC) and primary sclerosing cholangitis (PSC), etc. The clinical manifestation, biochemical, immunological and histological changes of these diseases are varied and overlapped, the differential diagnosis is rather difficult. Aims: To study the clinical features, diagnosis and treatment of patients with PBC-AIH overlapping syndrome. Methods: Thirty cases each of PBC-AIH overlapping syndrome, AIH and PBC were selected from 164 patients with autoimmune liver diseases, the diagnosis and response to therapy of the patients with PBC-AIH overlapping syndrome were analyzed particularly. Results: PBC-AIH overlapping syndrome constituted 18% of the patients with autoimmune liver disease recruited in this study. No significant differences were found in sex, age and the symptoms of jaundice and pruritus among the three groups. In PBC-AIH group, the levels of aspartate aminotransferase (AST) and alanine aminotransferase (ALT) were significantly higher than those in AIH and PBC group; and the levels of alkaline phosphatase (ALP) and γ-glutamyl transpeptidase (GGT) were markedly higher than those in AIH group, but lower than those in PBC group (P0.05 or P0.01). After combination therapy with immunosuppressant, all the above parameters of liver function improved significantly in AIH group (P0.05 or P0.01); while in PBC-AIH group the levels of bilirubin, AST and ALT decreased significantly as well (P0.05 or P0.01), but the time required for improvement was longer than that in AIH group (P0.05 or P0.01). Conclusions: PBC-AIH overlapping syndrome is not rare as seen in this study. Its diagnosis should be based on the clinical presentation, biochemical, immunological indices and histological changes. Long-term combination therapy with corticosteroids, azathioprine and ursodeoxycholic acid (UDCA) is effective in patients with PBC-AIH overlapping syndrome.
Insight into the natural history of primary biliary cirrhosis: A systemic review of data from placebo-controlled clinical trials.

Abstract:Background/Aims: The natural history of primary biliary cirrhosis (PBC) is extremely variable. The extraction and analysis of available information from placebo-treated patients in randomized controlled trials of PBC treatment would facilitate the study of the natural history of PBC. The aim of the present study was to determine important clinical information regarding the natural history of PBC patients without effective treatment. Materials and Methods: A search of the PubMed, EMBASE and Cochrane Library databases was performed by two authors. Twelve randomized placebo-controlled clinical trials for PBC patients without decompensated cirrhosis were retrieved for further review. Pooled estimates of biochemical measurements, histological scores and clinical outcomes associated with PBC were calculated in the placebo group. Results: Placebo-treated PBC patients displayed a significant decrease in serum alkaline phosphatase and very slight fluctuations in the other biochemical parameters during the 2-year follow-up. Meanwhile, histological progression was observed in 39.4% of the placebo-treated patients, and a moderate deterioration in histological scores was noted after 2 years. The pooled 2-year rates of death, transplantation and development of varices were 11.4%, 8.7% and 10.6%, respectively, in placebo-treated PBC patients. Conclusion: This review provides a foundation for further epidemiologic investigations in untreated patients and ursodeoxycholic acid-resistant patients with PBC. Biochemical responses after 2 years may provide some information on disease progression and therapeutic response in PBC.
[Immunological characteristics of primary biliary cirrhosis patients].

Abstract:The percentage of PBC in Chinese liver disease patients is about 1% to 2%. Most of the PBC patients have high levels of AMA and AMA-M2, IgM, ALP, GGT and several cytokines, indicating that abnormality of humeral and cellular immunity may be associated with the pathogenesis of PBC.

Abstract:Objective Analyzing the lncRNA expression profile in peripheral blood mononuclear cells(PBMC)of primary biliary cirrhosis(PBC)patients to provide new ideas for the pathogenesis,clinical diagnosis and treatment of PBC.Methods Collected peripheral blood from 30 PBC patients and 30 healthy volunteers, then separated PBMC.Four cases from each group were selected for long-noncoding RNA (lncRNA)expression microarray detection.Reverse transcription-PCR technology in a larger sample size was used to verify the microarray results.Bioinformatic analysis such as Cis-/Trans-target genes Gene ontology(GO)and pathway analysis, co-expression networks were conducted in order to provide a theoretical basis in the pathogenesis of PBC.Transfecting small interfering RNAs(siRNAs)for linc-pbc to see changes in the expression of nuclear receptor 4A group 3(NR4A3)and forkhead boxP3(FOXP3)and cell apoptosis in transfected PBMC.Results Compared to the healthy group, 749 lncRNA and 230 coding messenger RNA(mRNA)genes were abnormally expressed.Interestingly,NR4A3 gene was down-regulated by 78%.While linc-pbc, which was about 20 000 bp downstream of NR4A3 gene, increased 2.56-fold. Then design siRNA for linc-pbc.After transfection, mRNA and protein levels of NR4A3 and FOXP3 were up-regulated.Conclusions By recruiting PRC2 complex, linc-pbc may increased the methylation level of NR4A3 gene promoter region,thus decreasing the expression of NR4A3 in PBC patients and reduced NR4A3 futher downregulated the expression of FOXP 3 and reduced the amounts of immunosuppressive Treg cells in peripheral blood and liver tissue, breaking the equilibrium state of immune tolerance, and promoted the occurrence and development of the disease.

Abstract:Background To explore the correlation between gut microbiota and disease severity in patients with primary biliary cholangitis (PBC). Methods According to the natural course of the disease, 18 PBC patients were divided into two groups: early stage (asymptomatic stage and symptomatic stage) and decompensated stage. Ten patients with decompensated hepatitis B cirrhosis, 10 patients with decompensated alcoholic cirrhosis, and 10 healthy people were selected as controls. 16sRNA sequencing was performed in the fresh feces of the research subjects. To compare the composition of gut microbiota among decompensated PBC and early-stage PBC as well as other common decompensated chronic liver disease patients. Correlation analysis was conducted with clinical indicators. Results The diversity and abundance of gut microbiota in PBC patients decreased significantly compared to the healthy control group. The composition of the gut microbiota in the early PBC group was similar to that of the healthy control group, while the composition of the gut microbiota in the decompensated PBC group was less similar to that of the healthy control group (figure 1). Upon correlating clinical indicators with gut microbiota, it was revealed that genera such as Roseburia, Agathobacter, and Ruminococcus were found to be negatively correlated with the Child-Pugh score and total bilirubin (TBIL) levels, while positively correlated with albumin levels (figure 2). Furthermore, employing LEfSe for all groups comparisons, a total of 8 potential biomarkers at the genus level were identified: Agathobacter and Fusicatenibacter in the healthy control; Ligilactobacillus and Roseburia in the early PBC group; Veillonella and Klebsiella in the decompensated PBC group; Dialister in the decompensated hepatitis B cirrhosis group; Faecalibacterium in the decompensated alcoholic cirrhosis group (figure 3). Conclusions The gut microbiota of PBC patients exhibit dysbiosis, suggesting its involvement in the pathogenesis of the disease. Gut microbiota may potentially distinguish PBC from other common decompensated chronic liver diseases. Certain specific gut microbiota may be associated with disease progression.

Abstract:Primary biliary cirrhosis (PBC) is a chronic cholestatic immune liver disease characterized by persistent cholestasis, interlobular bile duct damage, portal inflammation, liver fibrosis, eventual cirrhosis, and death. Existing clinical and animal studies have made a good progress in bile acid metabolism, intestinal flora disorder inflammatory response, bile duct cell damage, and autoimmune response mechanisms. However, the pathogenesis of PBC has not been clearly elucidated. We focus on the pathological mechanism and new drug research and development of PBC in clinical and laboratory in the recent 20 years, to discuss the latest understanding of the pathological mechanism, treatment options, and drug discovery of PBC. Current clinical treatment mode and symptomatic drug support obviously cannot meet the urgent demand of patients with PBC, especially for the patients who do not respond to the current treatment drugs. New treatment methods are urgently needed. Drug candidates targeting reported targets or signals of PBC are emerging, albeit with some success and some failure. Single-target drugs cannot achieve ideal clinical efficacy. Multitarget drugs are the trend of future research and development of PBC drugs.

Abstract:Background: Primary biliary cholangitis (PBC) is a chronic autoimmune cholestatic disease, which can progress to cirrhosis. It mainly affects middle-aged women. Its most frequent form of presentation is asymptomatic with biochemical cholestasis and the presence of antimitochondrial antibodies (AMA). Aim: To describe the epidemiological characteristics, clinical presentation and treatment for patients with PBC at a clinical hospital. Material and methods: Descriptive, observational, retrospective study, carried out between January 2015 and December 2020. Results: 179 patients (158 women) were cared in the study period. At the time of diagnosis, the median age was 54 years (range 24-76), 55% of them were asymptomatic, 45% had fatigue and 28% had pruritus. Positive AMA were present in 65% of patients, antinuclear antibodies (ANA) in 51%, and anti-smooth muscle antibodies (ASMA) in 9%. Immunoglobulin M (IgM) was elevated in 30% of the patients and 50% of patients were biopsied. Splenomegaly and esophageal varices were present in 24 and 22% of patients, respectively. PBC was associated with Sjogren's syndrome in 15%, hypothyroidism in 14%, osteoporosis in 13%, and scleroderma in 8%. Conclusions: The epidemiological characteristics of our patients agree with those published abroad. Laboratory cholestasis associated with the presence of AMA, currently allows diagnosis without the need for histological study. Ursodeoxycholic acid (UDCA) is the first-line treatment for patients with PBC. The use of biochemical response criteria is essential to identify patients who require other UDCA alternatives for isolated or combined treatment.

Abstract:Primary biliary cholangitis (PBC) is an autoimmune liver disease characterized by elevated serum anti-mitochondrial Ab and lymphocyte-mediated bile duct damage. This study was designed to reveal the clonal characteristics of B lymphocyte repertoire in patients with PBC to facilitate better understanding of its pathogenesis and better management of these patients. Using high-throughput sequencing of Ig genes, we analyzed the repertoire of circulating B lymphocytes in 43 patients with PBC, and 34 age-and gender-matched healthy controls. Compared with healthy controls, PBC patients showed 1) a gain of 14 new clones and a loss of 8 clones; 2) a significant clonal expansion and increased relative IgM abundance, which corresponded with the elevated serum IgM level; 3) a significant reduction of clonal diversity and somatic hypermutations in class-switched sequences, which suggested a general immunocompromised status; 4) the reduction of clonal diversity and enhancement of clonal expansion were more obvious at the cirrhotic stage; and 5) treatment with ursodeoxycholic acid could increase the clonal diversity and reduce clonal expansion of the IgM repertoire, with no obvious effect on the somatic hypermutation level. Our data suggest that PBC is a complex autoimmune disease process with evidence of B lymphocyte clonal gains and losses, Ag-dependent ogligoclonal expansion, and a generally compromised immune reserve. This new insight into the pathogenesis of PBC opens up the prospect of studying disease-relevant B cells to better diagnose and treat this devastating disease.
