Predictive Value of Vascular Endothelium Dysfunction and Insulin Resistance to Metabolic Syndrome in Obese Subjects

Abstract:Objective To investigate the relationship of insulin resistance (IR) and vascular endothelium function to the incidence of metabolic syndrome (MS),to probe into the predictive values to MS.Methods A seven-year follow-up study was conducted in 583 simple obese subjects(BMI≥25 kg/m2,376 males,207 females) chosen in 2000.Body mass,blood pressure,blood lipids,blood glucose,insulin resistance index (HOMA-IR) were determined in the subjects,and vascular endothelium function was observed.The subjects were followed up and the above-mentioned indicators were reviewed after 7 years.Results Five hundred twenty-four subjects were followed up in 2007.MS was noted in 194,with a cumulative incidence of 37.03%,and the MS annual incidence was 5.29%.There was significant difference between males (n=356) and females (n=168) in MS cumulative incidence(45.22%,19.64%,respectively)(P<0.01).Significant difference was found in waist circumference,waist-hip ratio,triglycerides(TG),fasting plasma glucose(FPG),fasting insulin(Fins),Homa-IR,EDD in base line between MS subjects and non-MS (P<0.05).Seven-year cumulative incidence of MS was 41.53% in EDD group and 46.22% in EDD with IR group,higher than IR group(29.35%) and normal EDD and non-IR group(15.73%,P<0.05).Conclusion IR and endothelial dysfunction,occurring earlier than MS,may be the underlying reasons causing MS.It may become an early predictor for MS.
