Abstract:Objective To analyze the clinical and pathological characteristics of patients with MPO-ANCA positive Wegener granulomatosis (WG)and to investigate the difference between patients with MPO-ANCA and PR3-ANCA. Methods Patients with WG were selected according to both Chapel Hill Consensus Conference definition and American College of Rheumatology (ACR) classification criteria in 700 Chinese patients with ANCA-positive systemic vasculitis. The clinical manifestions were compared between patients with MPO-ANCA and with PR3-ANCA. Results Eighty-nine patients fulfilled the diagnostic criteria of WG, 54/89(60.7%) were MPO-ANCA positive, 34/89 (38.2%) were PR3-ANCA positive, and the rest one was both positive. Patients with MPO-ANCA were female predominant compared with patients with PR3-ANCA, their male/female ratios were 23:31 vs 24:10, respectively (P 0.05). Patients with MPO-ANCA also had multi-system involvement. However, the prevalences of arthagia, skin rash, ophthalmic and ear involvement were significantly lower in patients with MPO-ANCA than those in patients with PR3-ANCA (46.3% vs 70.6%, P 0.05; 20.4% vs 44.1%, P0.05; 27.8% vs 58.8%, P0.01; 40.7% vs 67.6%, P0.05; respectively). The prevalence of elevated initial serum creatinine was significantly higher in patients with MPO-ANCA than that in patients with PR3-ANCA (81.5% vs 61.8%, x2=4.2, P 0.05). The mean score of Birmingham vasculitis activity score (BVAS) was significantly lower in patients with MPO-ANCA than that in patients with PR3-ANCA (22.2±6.21 vs 24.7±6.87, P0.05). Conclusions Patients with MPO-ANCA positive WG were common in Chinese. The significance of patients with WG with different ANCA serotypes still needs further investigation.

Abstract:Background. Proteinase-3 antineutrophil cytoplasmic antibody (PR3-ANCA) was the serological marker for Wegener's granulomatosis (WG), while myeloperoxidase (MPO)-ANCA was the serological marker for microscopic polyangiitis (MPA). However, our previous study suggested that patients with MPO-ANCA positive WG were common in Chinese. This study aimed to analyse the renal histology of patients with MPO-ANCA positive WG.Methods. Patients in our centre with WG were selected according to both the Chapel Hill Consensus Conference (CHCC) definition and American College of Rheumatology classification criteria. Patients with MPA were selected according to the CHCC definition. The renal histology was compared between patients with MPO-ANCA positive WG and with PR3-ANCA positive WG as well as patients with MPO-ANCA positive MPA.Results. Sixty-one patients with WG had complete renal histological data, 39/61 with positive MPO-ANCA and 22/61 with positive PR3-ANCA. Among patients with crescents in glomeruli, those with MPO-ANCA had fewer cellular crescents and more fibrous crescents than those with PR3-ANCA (P < 0.01 and P < 0.05, respectively). Interstitial fibrosis and tubular atrophy were more prevalent and severe in patients with MPO-ANCA than in those with PR3-ANCA (P < 0.01 and P < 0.05, respectively). Compared with 44 patients with MPO-ANCA positive MPA, patients with MPO-ANCA positive WG had fewer glomeruli with crescents and more normal glomeruli (P < 0.01 and P < 0.01, respectively).Conclusion. Patients with MPO-ANCA positive WG are common in Chinese. In renal histology, chronic lesions were more severe and prevalent in patients with MPO-ANCA positive WG than in patients with PR3-ANCA positive WG. Glomerular lesions were less severe and less prevalent in patients with MPO-ANCA positive WG than in those with MPO-ANCA positive MPA.
IgG Subclass Distribution, Affinity of Anti‐myeloperoxidase Antibodies in Sera from Patients with Wegener's Granulomatosis and Microscopic Polyangiitis

Abstract:SUMMARY:Aim: Our previous study suggested that patients with MPO‐ANCA (myeloperoxidase antineutrophil cytoplasmic autoantibodies)‐positive Wegener's granulomatosis (WG) were common in Chinese, indicating that patients with MPO‐ANCA could manifest as either WG or microscopic polyangiitis (MPA). The aim of this study was to compare the immunological characteristics of MPO‐ANCA in patients with WG and MPA.Methods: Fifteen patients with WG and 21 patients with MPA were enrolled in the current study. Anti‐MPO IgG subclasses and their titres were detected by antigen‐specific enzyme‐linked immunosorbent assays (ELISA); affinity was assessed by antigen‐inhibition ELISAs. The sera from all patients were employed to inhibit biotin conjugated affinity‐purified human anti‐MPO antibodies (Probe‐biotin), from plasma exchange of a patient with MPA, in a competitive inhibition ELISAs system.Results: All four anti‐MPO subclasses could be detected in sera from patients with WG and MPA. The titres of anti‐MPO IgG4 subclass in patients with WG were significantly higher than those with MPA (1:1878 vs 1:218, P < 0.005). The affinity constants of MPO‐ANCA were comparable between patients with WG and MPA (0.3–70/M vs 0.3–140/M respectively). Eleven out of the 15 sera and 18 out of the 21 sera could inhibit the binding of the Probe‐biotin in patients with WG and MPA respectively. The average inhibition rate was 47.7% ± 11.5% and 61.7% ± 14.5% respectively (P < 0.05).Conclusion: MPO‐ANCA IgG4 subclass might play a role in the development of WG. The MPO‐ANCA in WG and MPA might recognize overlapping but different epitopes on native MPO molecule. The difference in immunological characteristics of MPO‐ANCA might contribute to different disease entities.

Abstract:Background: Antineutrophil cytoplasmic autoantibodies (ANCA) are serological markers of ANCA associated systemic vasculitides (AASV), which is one of the most common multisystem autoimmune diseases. Features of Chinese patients with AASV have not been fully investigated.Objective: To analyse the clinical and pathological characteristics of Chinese patients with AASV.Methods: 426 Chinese patients with AASV diagnosed in the past eight years were retrospectively studied and their clinical and pathological data were analysed.Results: Of the 426 patients, 87 (20.4%) were Wegener's granulomatosis, 337 (79.1%) were microscopic polyangiitis and two (0.5%) were Churg-Strauss syndrome. Only 201 of 426 (47.2%) patients were diagnosed within three months. Clinically, the patients had multisystem involvement. Altogether 371 of 426 (87.1%) had kidney involvement and 260 of 426 (61.0%) had lung involvement. The prevalences of renal involvement and fatigue were significantly higher in patients with MPO-ANCA than that in patients with PR3-ANCA; the prevalences of ophthalmic, nasal involvement, rash, and arthragia were significantly higher in patients with PR3-ANCA than those in patients with MPO-ANCA. The one and five year death rates were 13.1% and 22.4%, respectively. The percentage of patients progressing to end stage renal disease at one and five years was 15.9% and 27.1%, respectively.Conclusions: AASV is not a rare autoimmune disease in Chinese people. Kidney and lung were the most vulnerable organs. For patients with multiorgan damage, an ANCA test should be performed to make an early diagnosis and to start treatment in time.

Abstract:Objective To investigate the clinical features and risk factors of patient with Wegener's granulomatosis complicated with pulmonary infection.Methods Patients with Wegener's granulomatosis admitted to our hospital in the past 11 years were retrospectively analyzed.Comparisons between groups were performed by t tests or Fisher test.Results Pulmonary infection occurred in 27 cases with an incidence rate of 29％.Twenty-six percent of pulmonary infections occurred at the initial diagnosis,and 44％ occurred within 6 months,while 30％ occurred later than 6 months.The clinical manifestations of pulmonary infection were productive cough (89％),hemoptysis (63％),fever and fatigue (56％),chest pain and pactoralgia (33％).The most common causative pathogen were bacteria(59％ ),fungi(37％ ),and tubercle bacillus(37％ ).Sinus infection (P=0.01),hypoproteinemia (P=0.03),hypoimmunoglobulinemia (P=0.007),and methylprednisolone pulse therapy (P=0.002) were the risk factors for pulmonary infection.Conclusion The occurrence of Wegener's granulomatosis complicated with pulmonary infection is high within 6 months.The most common clinical manifestation is productive cough.The most common causative pathogens are bacteria,tubercle bacillus and fungi.Sinus infection,hypoproteinemia,hypoimmunoglobulinemia,and methylprednisolone pulse therapy are risk factors of pulmonary infection.

Abstract:Objective To investigate the clinical features of patient with Wegener granulomatosis(WG) complicated with pulmonary infection.Methods Twenty three patients with WG complicated with pulmonary infection admitted to Peking Union Medical College Hospital in 12 years were retrospectively analyzed.Results Among 23 patients,21.7% pulmonary infection occurred at the first diagnosis,and 52.2% occurred within 6 months,while 26.1% occurred more than 6 months.73.9% were generalized,and 26.1% were localized,accompanied with erythrocyte sedimentation rate(ESR,63.6%),C-reactionprotein(CRP,65%)increased,and immunoglobulin(30%)decreased.Antineutrophil cytoplasmic antibodies(ANCA)was positive in 82.6%,and C-ANCA was positive in 69.6%.The clinical manifestations of pulmonary infection were productive cough(86.9%),fever and fatigue(52.2%),hemoptysis(52.2%),chest pain and pactoralgia(34.8%).The causative pathogen mainly were bacteria(56.5%),tubercle bacillus(43.5%),and fungas(39.1%).Diabetes mellitus or impaired glucose tolerance(34.8%)and hypoproteinemia(65.2%)were the common complication.Most patients(95.7%) were treated with corticosteroid and cyclophosphamide,21.7% receive methyllprednisolone pulse therapy.Three patients died.Conclusions Patients with WG tend to complicate with pulmonary infection in the first six months,most of whom were generalized.The clinical manifestations were productive cough.The causative pathogens mainly were bacteria,tubercle bacillus,and fungas.Most patients were treated with corticosteroid and cyclophosphamide.

Abstract:Wegener' granulomatosis is an autoimmune diseases, often involving the lung and kidney, has a high mortality rate in nontreatment patients. The low incidence and nonspecific features, often lead to misdiagnosis and delayed treatment. This paper reported the diagnosis and treatment of a 55-year-old female patient with primary Wegener' granuloma of the lung diagnosed by percutaneous lung biopsy of pulmonary nodules, and reviews the relevant literature. .
Expression of Immune Cell TLR4 in Peripheral Blood of Patients with Wegener's Granulomatosis

Abstract:Objective The aim of this study was to assess whether the expression of the Toll-like receptor-4(TLR4) on peripheral blood immune cells including dendritic cell(DC) of Wegener′s granulomatosis(WG) patients is abnormal.Method Flow cytometry was used to analyze the expression of TLR4 protein on peripheral blood immune cells of health controls(HC) and WG patients.Results There was no difference in TLR4 expression on PMN,monocytes,BDCA3+DC,CD11c+DC,and DC-like monocytes-CD14+CD16+ monocytes between WG patients and HC.However,TLR4 expression on lymphocytes is much higher in WG patients compared with HC.There was no difference of TLR4 expression on these cells between active and inactive WG,WG treated with cyclophosphamide and methotrexate.Conclusions TLR4 expression on immune cells,DC and DC-like monocytes in WG was not abnormal.

Abstract:Aim: To analyse the clinical and pathological characteristics of Chinese patients with immune complex deposition in kidney in anti-neutrophil cytoplasmic antibodies (ANCA)-positive vasculitis.Methods: Enrolled in this study are patients with immune complex deposition in kidney in ANCA-positive vasculitis diagnosed in Peking University First Hospital. Their clinical and pathological data were collected and analysed.Results: Twenty-three patients were eligible. Fifteen patients were with microscopic polyangiitis and eight patients were with Wegener's granulomatosis. The mean age was 48.8 years and with a male/female ratio of 10/13. The interval, between onset of disease and the diagnosis of disease, was 429.6 +/- 693.3 days. All patients had clinical evidence of renal involvement. The major immunoglobulin deposited was IgM and the main locations were mesangial and sub-epithelial area. Four patients also presented features of membranous nephropathy and six patients presented features of IgA nephropathy. About 52.2% of patients had hypocomplementaemia. All patients received immunosuppressive therapy and all of them achieved clinical remission. Patients were followed for about 28.8 +/- 25.3 months. Nine patients kept clinical remission, nine patients progressed to end-stage renal disease and five patients died. When these patients are compared with patients who had classical pauci-immune vasculitis, they had greater proteinuria (P < 0.05), higher prevalence of hypocomplementaemia (P < 0.05) and greater glomerular cellularity (P 0.05).Conclusion: The present study showed that the features of patients with ANCA-associated vasculitis with immune complex deposition in kidney were similar with classical 'pauci-immune' vasculitis except for more proteinuria, more hypocomplementaemia and greater glomerular hypercellularity.

Abstract:Anti-neutrophil cytoplasmic antibodies (ANCA)-associated vasculitis (AAV) and anti-glomerular basement membrane (GBM) disease, the typical autoimmune diseases causing necrotizing crescentic glomerulonephritis (CrGN), have been largely reported in Caucasian patients. However, since the establishment of standard assays in China to screen for serum ANCA and anti-GBM autoantibodies over 10 years ago, Chinese patients with the two diseases were increasingly identified and studied. This editorial comment focuses on the updated information of Chinese patients with the two diseases.

In this study, we presented a patient with sinonasal symptoms, hematuria, and proteinuria, who had been misdiagnosed as a patient suffering from WG, but finally turned out to be a patient suffering from non-Hodgkin’s T cell lymphoma. Diagnostic approach of WG was also discussed.

Abstract:Background: Patients with anti-glomerular basement membrane (GBM) disease were predominantly reported in Caucasian population and reports from Chinese were lacking. The general picture of Chinese patients with anti-GBM disease was still unclear. This study is to investigate the characteristics and prognosis of Chinese patients with anti-GBM disease. Methods: Data from 105 patients with anti-GBM disease diagnosed in our hospital, between 1997 and 2002, were analyzed retrospectively. All the 105 sera were screened by enzyme-linked immunosorbent assay (ELISA) using highly purified bovine alpha(IV)NC1 as solid phase ligands. Clinical and pathological data of 69 patients with complete clinical remission (n = 5), partial remission (n = 10), and treatment failure (n = 54) were compared and the prognostic factors were evaluated. Results: Patients increased chronologically and three quarters of the 105 patients were diagnosed in the last 3 years. Most of the patients were between 20 and 29 years (n = 31) and a smaller second peak was found in patients over 60 years. 25/105 (24%) were also ANCA-positive. Patients with both anti-GBM antibodies and ANCA positive were elder (50 +/- 19 vs. 34 +/- 15 years, p < 0.01) and female predominant (15/25 vs. 16/80, p < 0.05). 56/97 (58%) patients presented as Goodpasture syndrome, 40/97 (41%) patients presented as rapidly progressive glomerulonephritis and one patient had pulmonary hemorrhage only. The following factors predict poor prognosis: (1) serum creatinine more than 600 mumol/l on diagnosis (p < 0.01); (2) oliguria or anuria on diagnosis (p < 0.01); (3) a high percentage (185%) of glomeruli had crescents (p < 0.01), and (4) renal involvement before pulmonary hemorrhage (p < 0.05). Patients with serum creatinine over 600 mumol/l on diagnosis had higher levels of anti-GBM antibodies (106 +/- 48% vs. 73 +/- 40%, p < 0.01). Intensive plasma exchange therapy predicts a better prognosis in the patients with serum creatinine less than 600 mu mol/l (p < 0.05). Conclusions: Anti-GBM disease is not rare in China and behaves similarly to elsewhere. Early diagnosis and intensive plasmapheresis might be the most promising approaches to improve the outcome. Abstract:Objective. To investigate clinical features, independent associated factors, treatment, and outcome of patients with peripheral neuropathy (PN) in eosinophilic granulomatosis with polyangiitis (EGPA). Methods. We retrospectively analyzed clinical data of 110 EGPA patients from 2007 to 2019 in Peking Union Medical College Hospital. The independent factors associated with PN in EGPA were analyzed with univariate and multivariate logistic regressions. Results. In EGPA with PN, paresthesia and muscle weakness were observed in 82% and 33% of patients, respectively. Both the upper and lower limbs were involved in 51% of patients. 30% of EGPA patients had symmetrical multiple peripheral neuropathy, whereas only 16.4% presented with mononeuritis multiplex. Compared to patients without PN, patients with PN had a higher erythrocyte sedimentation rate, C-reactive protein, rheumatoid factor, Birmingham vasculitis activity score (BVAS), and positivity of myeloperoxidase-antineutrophil cytoplasmic antibodies (MPO-ANCA). Regarding manifestations, patients with PN tended to develop weight loss and arthritis or joint pain. Notably, ANCA positivity, arthritis or joint pain, and higher BVAS were found to be independent associated factors for PN in EGPA. Patients with PN more frequently need glucocorticoid pulses and intravenous infusion of cyclophosphamide. With the longest follow-up of 11.0 years, we found that age and cardiac involvement were risk factors for survival, and female was the protective factor. Conclusion. PN in EGPA frequently displays with symmetrical multiple peripheral neuropathy in China. Positive ANCA, arthritis or joint pain, and higher BVAS are the independent associated factors of PN in EGPA. Glucocorticoids with immunosuppressants are vital therapeutic strategy.

Abstract:Objective The study was aimed to explore the otology manifestations of Wegener's granulomatosis(WG),the overlooked causes and measures to avoid delay in treatment,in order to raise the awareness of the disease among otologist community.Methods The clinical information and treatment process of one case of was which otology manifestations was the first symtors,was analysed.Results Otomastoiditis was diagnosed according to the otology manifestations,followed by mastoid surgery without hearing improvement.Several days later,the clinical manifestations of kindey damage occurred.The diagnosis of WG was made by renal biospy and the testing of anticytoplasmic antibodies versus neutrophilpolymorphonucleate granules(c-ANCA).The combined treatment of cyclophosphamide and prednisolone was contributed to the disease controlled and hearing improvement.Conclusion A high level of suspicion of this disease is needed,especially whas WG presents as a local manifestation and a prolonged classic therapy for otitis is ineffective.It is important to cooperate with other related departments and overall tests,in order to avoid wrong diagnosis and treatment.

Methods The clinical data of 8 cases diagnosed as Wegener granulomatosis with involvement of lungs were analyzed retrospectively. Results Of 8 patients, 5 were male and 3 were female. The patients age ranged from 21 to 62. The major symptoms of respiratory system included hemoptasis in 5, chest pain in 5, dry cough in 4, and dyspnea in 3. 4 cases were fever and 5 cases were positive with ANCA. The chest X-ray showed pneumonocirrhosis in 1, mass and nodular shadow in 4, and patchy shadow in both lungs in 3. The patients were misdiagnosed as pulmonary infection(3 cases), primary or secondary lung carcinoma(4 cases), and systemic lupus erythematosus with lung infection(1 case). Pulmonary lobectomy was carried on 2, Wedge-shaped excision of lung under VATS on 3, lung biopsy on 3, and renal biopsy on 3 with nephrosis. Nursing management included psychologic nursing, airway nursing, fever and skin nursing, and self nursing to hemorrhage. All 8 patients were cured. Conclusion It is difficult to the comfirmation of Wegener granulomatosis with involvement of lungs. The pathological examination is the key to diagnose. Early diagnosis、proper treatment and careful nursing can improve the qualities of patients life.

Abstract:Granulomatosis with polyangiitis (GPA) is a necrotizing granulomatous vasculitis occasionally affecting central nervous system (CNS), and GPA patients with initial CNS symptoms are even rarer, whose diagnosis is further confused by an absence of positive antineutrophil cytoplasmic antibody. The authors described the characteristics of antineutrophil cytoplasmic antibody -negative GPA with CNS onset in a patient and discussed on its management, which may contribute to future diagnosis and treatment of patients with similar conditions.

Abstract:Background. Anti-neutrophil cytoplasmic autoantibodies (ANCA) are serological markers of ANCA-associated small vessel vasculitis (AASV) which is one of common autoimmune diseases in Caucasian elderly population.Objective. To analyze the clinical and pathological characteristics of Chinese elderly patients with AASV.Methods. One-hundred forty one Chinese patients with AASV over 65 years old, diagnosed between 1997 and 2001 in the Institute of Nephrology of Peking University First Hospital, were retrospectively studied and their clinical and pathological data were analyzed.Results. Patients diagnosed with AASV patient increased chronologically with the yearly ratio in 2000 and 2001 significantly (P < 0.05) higher than that in 1998 and before. Of the 141 patients, 72 were male and 69 were female with an average age of 68.2 years. 13 of the 141 were cytoplasmic ANCA (cANCA) positive and all recognized proteinase 3 (PR3). The other 128 were perinuclear ANCA (pANCA) positive and 120/128 recognized MPO, 8/128 recognized both PR3 and MPO. Less than 50% of the patients were correctly diagnosed within 3 months. Clinically, 78% of the patients had fever and fatigue, 52.5% had body weight loss, 96.4% had kidney involvement, of which 75% had elevated serum creatinine and 30.8% had acute renal failure. 76.6% had lung involvement, over half of them had hemoptysis or lung infiltrates. Other clinical manifestations included arthralgia (48.2%), muscle pain (39.7%), gastrointestinal symptoms (39.7%), eye involvement (28.3%) and ENT involvement (31.2%). In laboratory examinations, 94.4% of the patients had anemia, 62.4% had increased WBC count, 93.6% had increased ESR and 55.1% had increased CRP.Conclusions. More and more patients with AASV were diagnosed in Chinese elderly. Kidney was the most vulnerable organ to be involved and lung was the most important extra-renal organ to be affected. For elderly patients with multi-organ damage, an ANCA test should be performed in order to make an early diagnosis and start therapy in time. (C) 2004 Elsevier Inc. All rights reserved.

Abstract:Objective The aim of this study was to assess the status of protease-activated receptor-2(PAR-2) expression on the peripheral blood immune cells including dendritic cells(DC) of Wegener's granulo matosis(WG) patients.Methods Flow cytometry was used to analyze the expression of PAR-2 protein on peripheral blood immune cells,DC and DC-like monocytes of healthy controls (HC),inactive,active and different medicine-treated WG patients.Two-tail Mann Whitney non-parametric test was used for statistical analysis.Results There was no difference of PAR-2 expression on PMN,monocytes,lymphocytes and DC between WG patients and HC.However,PAR-2 expression on CD14+CD16+ (t=3.823,P=0.004) and CD64+CD16+ (t=2.652,P=0.024) DC-like monocytes was much higher in WG patients compared with HC.In active WG,expression of PAR-2 was up-regulated on the cell surface of PMN (t=2.690,P=0.013 ),monocytes (t=1.688,P=0.047),lymphocytes(t=1.742,P=0.038),BDCA3+DC(t=2.582,P=0.016),CD11c+DC(t=1.828,P=0.044),CD14+CD16+ (t=3.419,P=0.002)and CD64+CD16+ (t=3.494,P=0.005) DC-like monocytes when compared with inactive WG.PAR-2 expression on these cells was similar between cyclophosphamide (Cyc) and Methotrexate (MTX) treated WG patients.Conclusion PAR-2 expression on immune cells,DC and DC-like monocytes correlates with WG occurrence and development.There is no difference in the effects on PAR-2 expression between various medications.
