0047: Impact of Contrast-Enhanced Ultrasound on Diagnosis of Hepatocellular Carcinoma in Cirrhosis: A Large Series Report

Abstract:The aim of this retrospective study was to assess the impact of the introduction of contrast-enhanced ultrasound (CEUS) into the diagnostic clinical algorithm for diagnosis of hepatocellular carcinoma (HCC). CEUS has been included in the work-up for diagnosis of HCC at our three affiliated hospitals of the university and we evaluated its performance. The diagnostic criteria for HCC were according to American Association for the Study of Liver Diseases (AASLD) guidelines and the characteristic vascular pattern of CEUS for characterization of HCC was according to European Federation of Societies for Ultrasound in Medicine and Biology (EFSUMB) recommendations. During the past 4 years CEUS was utilized in total 5842 of patients for detection or characterization of focal liver lesions. HCC diagnosis was proven in 2110 cases and among them 1883 (89.2%) had liver cirrhosis. There were 1782 (94.6%) cases showed the characteristic vascular pattern of CEUS accordingly, namely, intense arterial contrast uptake followed by washout in the portal/late phase. With CEUS combined another imaging modality, conclusive diagnosis could be achieved in 67.1% (149/222) of the patients with tumors 2 cm or smaller. CEUS used solely as radiological criteria was able to achieve the diagnosis in 94.5% (1570/1661) of the HCCs lager than 2 cm. CEUS has changed the diagnostic imaging protocols for HCC. Because of its safety, easy to approach and cost effectiveness CEUS has great impact on daily practice.
