Abstract:Tuberculosis is the most common major infectious disease worldwide. The ongoing HIV/AIDS epidemic and failure of public health programs in many resource-poor countries, along with the resultant development of drug resistance, means that TB will continue to be a major health problem for the foreseeable future. Although improved chemotherapeutic regimens or vaccine development are the best hope of controlling and eradicating this disease, a small but difficult group of patients will remain that are best treated by a combination of resectional lung surgery and prolonged chemotherapy. The ultimate goal of surgical intervention remains conversion of positive to negative sputum, and the removal of sequela(ae) to avert or prevent future problems, both with reinfection and/or worsening of pulmonary function. It would appear that establishing regional centers of excellence, in which surgeons and pulmonologists, experienced with this difficult problem, can work collaboratively, would be a practical and effective way to manage these challenging patients. The challenge for thoracic surgeons is to gain or increase their basic understanding of tuberculosis, and to acquire the necessary skills and experience to treat a complex and challenging group of patients. © 2008 Mosby, Inc. Tuberculosis: mother of thoracic surgery then and now, past and prospectives: a review

Abstract:Knowledge on ontogenesis of thoracic surgery is essential not only for understanding present concepts and debates on surgery for tuberculosis, but it also contributes to the further developments in operative treatment of lung cancer. Both diseases have been the leading cause of death in their respective ages. History of tuberculosis follows the classic algorithm: diagnostic, casuistic and therapeutical stages. Villemin followed by Virchow, and, finally, Koch revealed the pathoanatomy and the cause of tuberculosis. The therapeutic phase of lung cancer has been reached without identified cause of the disease. Chest surgery, eradication of the macroscopic focus by physical interference with the involved tissue mass, in both diseases preceded medical treatment. Identification of phenotypes of lung cancer-if it is a single disease at all-does not contravene the concept: the tumor mass should been eliminated. However, causation is not an absolute sine qua non of an effective treatment, as the tuberculosis-lung cancer analogy also proves. Surgical approach of both diseases suffered from the same paraoxon: eradication without direct interference with the causative factor. While lung cancer seems to be controlled by an emerging array of new drugs, tuberculosis poses a new challenge, as multidrug resistant and extensively drug resistant Koch bacteria are emerging and fragile societies' immunity is weakening. Thoracic surgery has a significant share in the fight against tuberculosis, when drugs and/or society fail. Palliative and radical adjuvant surgery multiplies the chance of cure in those cases, where not much hope is left. The jury is still out in a series of questions, but it is obvious, that surgery is only an option and not a panacea where medicines and their providers fail. Deeper understanding of our past and present failures with tuberculosis and its surgery might contribute to new concepts in coping with lung cancer as well.
Recent advances in the management of pulmonary tuberculoma with focus on the use of tubeless video-assisted thoracoscopic surgery

Abstract:Pulmonary tuberculoma are benign solitary pulmonary nodules representing up to 25% of all resected solitary pulmonary nodules. However, the differentiation between pulmonary tuberculoma and lung cancer, as well as between active tuberculoma and inactive tuberculoma remains a clinical challenge. The present review summarizes the recent advances in the management of pulmonary tuberculoma, including radiological findings, the response to anti-tuberculosis treatment and surgical treatments. Application of the novel tubeless video-assisted thoracoscopic surgery (VATS) technique in both the diagnosis and treatment of pulmonary tuberculoma has been found to be safe and feasible and leads to less surgical trauma, which results in reduced length of hospitalization and better post-operative quality of life.

Abstract:Background: Abdominal tuberculosis comprises all forms of tuberculosis that involve the gastrointestinal tract. Controversies exist regarding the surgical approach and timing and type of intervention for complicated forms of abdominal tuberculosis. The aim of this systematic review is to define the rate of surgical treatment, the type of surgical procedures performed and the role of minimally invasive surgery in the management of abdominal tuberculosis. Methods: The literature in MEDLINE, Scopus and Google Scholar and forward and backward citations for studies published between database inception and July 2022 were searched without language restrictions. All prospective and retrospective studies were included. The electronic database search yielded 2440 records. Additionally, eight records were identified through snowball searching. Following duplicate removal (45 duplicates found), 2403 records were screened for titles and abstracts. After screening for titles and abstracts and exclusion criteria, 38 reports were included for systematic review, 27 retrospective studies and 11 prospective studies. Data extracted included the general and demographic characteristics of the studies, diagnostic methods used, clinical presentation, site of involvement and details on surgical treatment. Results: In total, 2870 patients with a diagnosis of abdominal tuberculosis were included, and 1803 (63%) underwent a surgical procedure. The majority of patients underwent an open surgical procedure (95%). The most commonly performed procedures were adhesiolysis (21%) and small bowel resection with primary anastomosis (21%). Conclusions: The results of this review suggest that whenever surgery is required, there is a tendency to perform open surgical procedures in patients with complicated abdominal tuberculosis, both in emergency and elective settings, despite advances in minimally invasive surgery. Abstract:The Society for Translational Medicine and The Chinese Society for Thoracic and Cardiovascular Surgery conducted a systematic review of the literature in an attempt to improve our understanding in the postoperative management of chest tubes of patients undergoing pulmonary lobectomy. Recommendations were produced and classified based on an internationally accepted GRADE system. The following recommendations were extracted in the present review: (I) chest tubes can be removed safely with daily pleural fluid of up to 450 mL (non-chylous and non-sanguinous), which may reduce chest tube duration and hospital length of stay (2B); (II) in rare instances, e.g., persistent abundant fluid production, the use of PrRP/B <0.5 when evaluating fluid output to determine chest tube removal might be beneficial (2B); (III) it is recommended that one chest tube is adequate following pulmonary lobectomy, except for hemorrhage and space problems (2A); (IV) chest tube clearance by milking and stripping is not recommended after lung resection (2B); (V) chest tube suction is not necessary for patients undergoing lobectomy after first postoperative day (2A); (VI) regulated chest tube suction [−11 (−1.08 kPa) to −20 (1.96 kPa) cmH2O depending upon the type of lobectomy] is not superior to regulated seal [−2 (0.196 kPa) cmH2O] when electronic drainage systems are used after lobectomy by thoracotomy (2B); (VII) chest tube removal recommended at the end of expiration and may be slightly superior to removal at the end of inspiration (2A); (VIII) electronic drainage systems are recommended in the management of chest tube in patients undergoing lobectomy (2B).

Abstract:Background:: Chest wall tuberculosis may develop if tuberculous (TB) lesions spread through the chest wall and invade the thoracic cavity. The presence of a mass on the patient's chest wall may be the first indication of TB, and a chest CT scan can help diagnose external penetrating chest wall TB, the incursion of tuberculosis from the lungs into the chest wall. Objective:: This study examines the safety and efficacy of thoracoscopic-assisted surgery for the treatment of penetrating chest wall tuberculosis as a means of exploring novel concepts of minimally invasive surgery. Methods:: Our hospital conducted a retrospective study of 25 patients with penetrating chest wall TB who underwent thoracoscopic surgery between January 2020 and June 2021. General demographics, CT scan data linked to surgery, and postoperative patient outcomes were compared between the two groups. The data was also evaluated to determine the range of operation time and the volume of bleeding from different foci in the thoracic cavity. Results:: All procedures went well after patients took conventional antituberculosis medication for at least two weeks prior to surgery. CT scans showed that thoracoscopic surgery needed a smaller incision than traditional chest wall TB surgery, with no discernible increase in surgical time. Postoperative tube use, length of hospital stay, and blood loss were all significantly lower than they would have been with conventional surgery. In addition, thoracoscopy was associated with a significantly reduced rate of subsequent treatment. Fibrous plate development and calcification caused the longest operation times in the thoracoscopic surgery group, whereas multiple pleural tuberculomas generated the most hemorrhage. Thoracoscopic surgery usually reveals tuberculous foci hiding in the thoracic cavity. Conclusion:: Thethoracic surgery can eliminate the TB focus in the chest wall and intrathoracic while treating penetrating chest wall tuberculosis. The CT scan is a crucial part of the diagnostic process for these patients. Minor surgical trauma, low complication and recurrence rates, and good results. There is a greater distinction between the two surgical approaches for patients with penetrating chest wall TB as opposed to those with basic chest wall tuberculosis.

Abstract:Objective To investigate clinical characteristics and outcomes of primary thoracic tuberculosis and pyogenic spondylitis.Methods From January 2001 to January 2011,102 patients with thoracic infectious diseases were involved in this study.There were 68 males and 34 females with an average age of 47.2 years.Of them,85 patients suffered from tuberculosis spondylitis and 17 patients with pyogenic spondylitis.Clinical data,injured vertebral body,Frankel classification,radiological features,choice of treatment,surgical approach,operation time,blood loss,clinical outcomes,complications and pathogenic bacteria were retrospectively analyzed.Results Among the patients with tuberculous spondylitis and pyogenic spondylitis,there was no significant difference in terms of patient age and gender,and preoperative erythrocyte sedimentation rate(P>0.05);however,there were significant differences in terms of onset time,range of lesion,and the incidence of chronic disease,neurologic symptoms,paravertebral abscess and preoperative fever(P<0.05).The patients were followed up for 6.9 months on average,clinical outcomes were satisfied and only 2 patients received secondary surgery.Frankel classification was improved in the patients with nervous symptoms.Conclusion There is distinguished difference of clinical feature between thoracic tuberculosis and pyogenic spondylitis.To avoid delaying treatment,differential diagnosis of the diseases is necessary before surgery.Antimicrobial therapy according to susceptibility test is the treatment principle of spinal infectious diseases.Once conservative treatment is invalid,it is essential to perform operation to those with surgical indications,especially with neurological defects.Early-stage surgical treatment can achieve good clinical outcomes and prognosis.

Clinical Efficacy of Different Surgical Approaches in Treatment of Thoracolumbar Tuberculosis in Tibet

Abstract:Objective To assess clinical outcomes of surgical management of adults with thoracic and lumbar tuberculosis in Tibet by comparing posterior only, anterior only, and combined posterior and anterior surgical approaches. Methods The medical records for 62 patients treated for thoracic and lumbar tuberculosis between January 2010 and September 2018 were retrospectively reviewed. Among them, 28 patients were treated with single-stage anterior debridement and interbody fusion with instrumentation (Group A), 11 patients with posterior instrumentation, anterior debridement, and bone graft in a single or two-stage procedure (Group B), and 23 patients with posterior debridement and strut grafting with instrumentation (Group C). Operative time, blood loss, Visual Analog Scale score (VAS), complications, recovery of neurological function, Cobb angle, correction rate, and loss angle were compared among all groups. Results The patients were followed for 5-96 months, with an average of (10±5) months. The operative time of Group C [(213.5±42.3) min] was significantly shorter than that of Groups A [(260.9±58.1) min] and B [(292.7±45.2) min] (P 0.05). There was no statistically significant between groups in length of stay (P>0.05). The correction rate was superior in Groups B [(73.4±11.2)%] and C [(72.5±13.4)%] compared with Group A [(45.2±16.8)%], whereas the VAS score showed no statistically significant difference among the groups (P>0.05). Two patients developed recurrence 4 and 7 months after operation, one in Group A and one in Group B. During the operation, there were five cases of pleura rupture leading to hemopneumothorax (3 in Group A and 2 in Group B) and two cases of peritoneum rupture in Group B. Conclusion For patients with thoracic and lumbar tuberculosis, the use of single-stage anterior debridement and interbody fusion with instrumentation approach is limited for the unsatisfactory corrective effect and high complications. Although the combined approach produces satisfactory outcomes, it remains more traumatic and is associated with longer operative time and more blood loss. Therefore, the posterior approach is recommended, not only because it achieves good results, but also because it has reduced complications, shorter operative time, and less blood loss. One-stage Surgical Management for Thoracic Tuberculosis by Anterior Debridement, Decompression and Autogenous Rib Grafts, and Instrumentation.

Abstract:BACKGROUND CONTEXT: Patients with thoracic tuberculosis have been not always successfully treated by radical debridement and bone grafting with or without supplementary posterior instrumentation and fusion, although most surgeons use posterior instrumentation to support anterior strut grafts.PURPOSE: To determine the efficacy of anterior instrumentation after radical debridement and autogenous rib grafts in patients with thoracic tuberculosis over a 6-year period at a single institution.STUDY DESIGN: A retrospective clinical study of one-stage surgical management for thoracic tuberculosis by anterior radical debridement, decompression and autogenous rib grafts, and instrumentation.PATIENT SAMPLE: Procedure was performed in 42 patients.OUTCOME MEASURES: Kyphotic angle was measured to assess the severity of the thoracic tuberculosis. Neurologic outcome was evaluated using Frankel grade, infection activity using erythrocyte sedimentation rate value and C-reactive protein value, and pain using a visual analog scale pain score.METHODS: In this retrospective study, the authors evaluated 42 patients (25 men and 17 women; average age, 47.7 years) with thoracic tuberculosis, who underwent one-stage anterior radical debridement, decompression and autogenous rib grafts, and instrumentation. The average follow-up period was 50.8 months (range, 24-85 months).RESULTS: A solid fusion was achieved in all cases. Of all 42 patients with preoperative kyphosis, the deformity was corrected from an average of 19.8 degrees on admission to an average of 3.7 degrees after surgery and to an average of 4.6 degrees at the final visit. No significant loss of deformity correction was noted in these patients. There was no other recurrence of the tuberculous infection.CONCLUSION: The authors think that the one-stage anterior autogenous rib grafts and instrumentation are safe and effective methods in the surgical management of thoracic tuberculosis. Abstract:Tube thoracostomy (TT) placement belongs among the most commonly performed procedures. Despite many benefits of TT drainage, potential for significant morbidity and mortality exists. Abdominal or thoracic injury, fistula formation and vascular trauma are among the most serious, but more common complications such as recurrent pneumothorax, insertion site infection and nonfunctioning or malpositioned TT also represent a significant source of morbidity and treatment cost. Awareness of potential complications and familiarity with associated preventive, diagnostic and treatment strategies are fundamental to satisfactory patient outcomes. This review focuses on chest tube complications and related topics, with emphasis on prevention and problem-oriented approaches to diagnosis and treatment. The authors hope that this manuscript will serve as a valuable foundation for those who wish to become adept at the management of chest tubes.

Abstract:Objective The type of surgical treatment that should be adopted for thoracic tuberculosis (TB) remains controversial. This study was performed to compare the clinical efficacy of surgery via the single anterior and single posterior approaches for treatment of thoracic spinal TB. Methods Seventy-eight patients with thoracic TB undergoing surgical treatment were divided into two groups on the basis of the surgical methods employed: Group A (single anterior debridement + bone graft fusion and internal fixation) and Group B (single posterior debridement + bone graft fusion and internal fixation). Results of clinical and imaging examinations were analysed and compared between the two groups. Results The surgical duration and mean hospital stay were significantly longer and the perioperative bleeding volume was significantly higher in Group A than B. At the last follow-up, changes in the American Spinal Injury Association grade showed no obvious differences between the two groups. Before and after the surgery and at the last follow-up, no significant differences were detected in the Cobb angle change or correction rate between the two groups. Conclusion Surgery via the single anterior and single posterior approaches achieved good clinical efficacy in the treatment of thoracic spinal TB.
Pleural tuberculosis: an update

Abstract:Purpose of review: Pleural tuberculosis accounts for fewer than 1% of all exudative effusions in western countries, occurring in only 3-5% of tuberculous patients. However, in developing countries like India, it is responsible for 30-80% of all pleural effusions encountered and may complicate tuberculosis in 31% of all cases. Thus, tuberculous pleuritis remains a major contributor to global morbidity and mortality. Tuberculous pleurisy presents a diagnostic and therapeutic problem due to the low sensitivity of the diagnostic tools. There have been many advances in the field that we have attempted to update. Recent findings: Due to the limitations of conventional tests and the delay of several weeks for mycobacterial culture results, there has been a resurgence of interest in newer rapid tests and biomarkers. This review highlights the deficiencies of the traditional tests, while providing an overview of the newer tests. In addition, we provide an update on the changing epidemiology, clinical features and current treatment options. Summary: Newer immunological tests and scoring systems will revolutionize the way tuberculous pleurisy is diagnosed in the years ahead. Future research needs include validating the available diagnostic tests in larger randomized studies, finding newer specific biomarkers that are simple, accurate and cost-effective for the developing world and determining treatment regimens specific for tuberculous pleurisy.

Abstract:Lung cancer has become one of the key life-threatening factors and is the most common type of tumor. With the improvement of equipment and the improvement of lung cancer awareness, the surgical treatment of lung cancer is becoming more and more mature. Surgery can provide an important part of individualized treatment strategies for patients with different stages of lung cancer and different wishes. In addition, the concept of minimally invasive, precise, and intelligent has triggered a revolutionary change in the surgical treatment strategy for lung cancer. Therefore, this review focuses on the development of lung cancer surgery history, summarizing the era from traditional surgery to the era of minimally invasive thoracic surgery based on TV-assisted thoracic surgery. The operating methods and treatment effects of different surgical methods are comprehensively introduced, and the optimization and improvement of different surgical methods in the future are also discussed. Along with the concept of minimally invasive surgical techniques, there are more and more explorations of nanotechnology in the surgical treatment of lung cancer. The application of nanotechnology in lung cancer imaging, and the combination of surgery with nanomedicine is an effective solution for cancer treatment today. So, this review also summarizes the application prospects of nanotechnology before, during and after lung cancer surgery.

Abstract:Posterior transfacet approach has been proved to be a safe and effective access to treat thoracic disc herniation. However, the therapeutic effect and safety of modified transfacet approach for treating thoracic spinal tuberculosis (TST) has not been reported in the clinical literature. In this study, the clinical efficacy and safety of a single-stage posterior modified transfacet debridement, posterior instrumentation, and interbody fusion for treating TST were retrospectively evaluated.

Abstract:Even though the majority of tuberculosis (TB) programmes consider their work completed when a patient is ‘successfully’ cured, patients often continue to suffer with post-treatment or surgical sequelae. This review focuses on describing the available evidence with regard to the diagnosis and management of post-treatment and surgical sequelae (pulmonary rehabilitation). We carried out a non-systematic literature review based on a PubMed search using specific key-words, including various combinations of ‘TB’, ‘MDR-TB’, ‘XDR-TB’, ‘surgery’, ‘functional evaluation’, ‘sequelae’ and ‘pulmonary rehabilitation’. References of the most important papers were retrieved to improve the search accuracy. We identified the main areas of interest to describe the topic as follows: 1) ‘Surgery’, described through observational studies and reviews, systematic reviews and meta-analyses, IPD (individual data meta-analyses), and official guidelines (GRADE (Grading of Recommendations Assessment, Development and Evaluation) or not GRADE-based); 2) Post-TB treatment functional evaluation; and 3) Pulmonary rehabilitation interventions. We also highlighted the priority areas for research for the three main areas of interest. The collection of high-quality standardized variables would allow advances in the understanding of the need for, and effectiveness of, pulmonary rehabilitation at both the individual and the programmatic level. The initial evidence supports the importance of the adequate functional evaluation of these patients, which is necessary to identify those who will benefit from pulmonary rehabilitation.
Comparison of Anterior, Posterior, and Anterior Combined with Posterior Surgical Treatment of Thoracic and Lumbar Spinal Tuberculosis: a Systematic Review

Abstract:Background This study was performed to compare different surgical approaches in the treatment of spinal tuberculosis. Methods We conducted a literature search to identify and analyze papers published from January 1966 to April 2018 relevant to comparison of the anterior, posterior, and anterior combined with posterior approaches in the treatment of spinal tuberculosis of the thoracic and lumbar regions. Results Twenty-five studies involving 2295 patients were identified in this systematic review. The operative time was significantly longer in the anterior combined with posterior approach than in the other two approaches. Blood loss was significantly greater in the anterior combined with posterior approach (1125.0 ± 275.5 mL) than in the posterior approach (710.4 ± 192.4 mL). The difference in correction of the kyphosis angle among the three procedures was not significant. The overall surgical and transthoracic complications were significantly lower in the posterior approach. The clinical outcome of all patients improved, but there was no significant difference among the three procedures. Conclusions Blood loss, overall surgical and transthoracic complications, and the operative time are different among the three approaches. Therefore, different factors must be carefully assessed in deciding among the three procedures.

Abstract:Purpose: Pulmonary tuberculosis (PTB) has clinically significant sequelae, even after recommended treatment completion. It is important to recognise these sequelae for accurate assessment of severity and treatment planning, if indicated. Material and methods: We retrospectively analysed contrast-enhanced computed tomography (CT) scans of chest of 100 patients with previous history of treated pulmonary tuberculosis, excluding those with active pulmonary disease. CT findings were analysed based on parenchymal, airway, pleural, mediastinal, and vascular sequelae of PTB. Results: Parenchymal sequelae included fibrosis with architectural distortion and volume loss (90%), cavities (21%) (with aspergillomas noted in 19% of these cases), and tuberculomas (54%). Airway involvement was noted as bronchiectasis (77%) and bronchial stenosis (4%) but none with broncholithiasis. Mediastinal sequelae included lymph node calcification (74%), fibrosing mediastinitis (1%), and pericardial tuberculosis (2%). Pleural sequelae included pleural thickening (22%), with 40.9% of these patients showing calcifications, and one patient with chronic chylous pleural effusion. Vascular sequelae included Rasmussen aneurysms (4%), enlarged bronchial arteries (3%), and systemic bronchial collaterals in 1% of our patients. Conclusions: PTB has multiple appalling sequelae, which require due attention and appropriate treatment in symptomatic cases. Radiological evaluation forms an integral part in patient assessment and decision making.
Video Thoracoscopic Surgery Used to Manage Tuberculosis in Thoracic Surgery

Abstract:BACKGROUND:The aim of this study was to evaluate the indications and results of video-assisted thoracic surgery (VATS) for the management of tuberculosis in 10 patients with unusual clinical and radiologic presentation for the disease.METHODS:From March 2000 to March 2002, 96 diagnostic VATS operations for unclear thoracic lesions were performed at the authors' institution. Their final diagnosis for 10 (10.4%) of these patients was tuberculosis. The suspected preoperative diagnoses were pancoast tumour (n = 1), pericardial effusion (n = 1), pleural mesothelioma (n = 1), pleural empyema (n = 2), mediastinal lymphoma (n = l), and lung cancer (n = 4).RESULTS:For all the patients, the diagnosis of tuberculosis was achieved by VATS. The duration of drainage was 2.5 days. There have been neither morbidity nor mortality since surgery. The hospital stay was 3 to 5 days.CONCLUSION:Thoracoscopy is a safe and effective procedure for the management of tuberculosis. Tuberculosis should be kept in mind during the differential diagnosis of unknown thoracic lesions, and also for patients who live in economically well developed countries and are not immune compromised.
