Abstract:Recent advancements on interventional molecular imaging aimed to further complement the advantages of two imaging fields, namely, interventional radiology and molecular imaging. Interventional molecular imaging significantly improved the early diag-nosis of cancer, local treatment, and monitoring of tumor treatments. Interventional radiology has continuously extended the capabili-ties of the currently available molecular imaging techniques to (i) obtain deep-seated targets;(ii) thoroughly examine small targets;(iii) precisely guide the delivery of non-targeted imaging tracers or therapeutics;and (iv) selectively enhance the effectiveness of targeted imaging and treatment with high accuracy. Molecular imaging has been used to guide interventional therapies and assess the thera-peutic efficacies of medical interventions. The continuous efforts on interventional molecular imaging extend molecular imaging from benches or small animal laboratories to large animal suites and ultimately to certain clinical applications in humans and enhance the diagnosis and treatment of cancer.

Abstract:The tumor microenvironment plays a critical role in tumor initiation, progression, metastasis, and resistance to therapy. It is different from normal tissue in the extracellular matrix, vascular and lymphatic networks, as well as physiologic conditions. Molecular imaging of the tumor microenvironment provides a better understanding of its function in cancer biology, and thus allowing for the design of new diagnostics and therapeutics for early cancer diagnosis and treatment. The clinical translation of cancer molecular imaging is often hampered by the high cost of commercialization of targeted imaging agents as well as the limited clinical applications and small market size of some of the agents. Because many different cancer types share similar tumor microenvironment features, the ability to target these biomarkers has the potential to provide clinically translatable molecular imaging technologies for a spectrum of cancers and broad clinical applications. There has been significant progress in targeting the tumor microenvironment for cancer molecular imaging. In this review, we summarize the principles and strategies of recent advances made in molecular imaging of the tumor microenvironment, using various imaging modalities for early detection and diagnosis of cancer.

Abstract:Tumor growth depends on angiogenesis,and therefore anti-angiogenesis has been widely used in the treatment of tumors.Microvessel density(MVD) is taken as the "golden standard" for assessing angiogenesis,though it has pitfalls in clinical practice.Routine imaging methods can neither reflect the real condition of tumor angiogenesis nor objectively evaluate the clinical effect of the anti-angiogenetic therapy.The authors reviewed the related literature and,based on the clinical experience,analyzed various imaging methods for assessing tumor angionenesis,including tumor angiography,double energy CT enhancement,CT perfusion,MR perfusion and MR diffuse weighted imaging(DWI),MR blood oxygen level dependent imaging(BOLD),ultrasonic strategies(such as Doppler blood flow imaging,color Doppler energetic imaging,and ultrasonic contrast enhancement),PET/SPECT,and optic imaging.Molecular imagery approaches to vascularization have developed rapidly.Each method is unique for its imaging mechanism,features and applicability.In clinical practice,we should choose among all these techniques the most suitable one to avaluate the effect of anti-angiogenetic therapy.

Abstract:This paper summarizes the new progress of the research of the targeted microbubble ultrasound contrast agents and ultrasound molecular imaging in tumor angiogenesis.There are two constructing methods of targeted microbubble ultrasound contrast agents.Targeted new blood vessels microbubble unltrasound contrast agents are mainly divided into the connection of alpha(V)beta3 antibody,αVβ3 integration antibody and connecting VEGFR2.The quantitative analysis of targeted microbubble adhering to tumor target and the evaluation of the correlation between targeted microbubble adhering to tumor neovascular endothelial cell and tumor local pathological microvessel density suggest that ultrasound molecular imaging can be noninvasive,quick,repeated,dynamic and quantitative evaluate tumor angiogenesis and indirectly reflect the malignant tumor tissue proliferation activity and its biological characteristics.

Abstract:Brain tumors are important diseases which are harmful to human health. Because of their growth in the extremely important part,themortality and morbidity are very high. So early diagnosis is very important to guide treatment and prognosis. Medical imaging is entering the era of microscopic molecular imaging from the times of Macroscopic anatomical images. With the rapid development of molecular imaging, the tumor biological processes can be displayed from the cellular and molecular level in vivo, providing molecular-level information for the early diagnosis and treatment of non-invasive monitoring of disease. This article reviews the clinical applications and new advances for nuclear medicine,nuclear magnetic resonance,optical and multimodal molecular imaging in brain tumor diagnosis and therapy.

Abstract:Molecular imaging is an emerging field that integrates advanced imaging technology with cellular and molecular biology. It can realize noninvasive and real time visualization, measurement of physiological or pathological process in the living organism at the cellular and molecular level, providing an effective method of information acquiring for diagnosis, therapy, and drug development and evaluating treatment of efficacy. Molecular imaging requires high resolution and high sensitive instruments and specific imaging agents that link the imaging signal with molecular event. Recently, the application of new emerging chemical technology and nanotechnology has stimulated the development of imaging agents. Nanoparticles modified with small molecule, peptide, antibody, and aptamer have been extensively applied for preclinical studies. Therapeutic drug or gene is incorporated into nanoparticles to construct multifunctional imaging agents which allow for theranostic applications. In this review, we will discuss the characteristics of molecular imaging, the novel imaging agent including targeted imaging agent and multifunctional imaging agent, as well as cite some examples of their application in molecular imaging and therapy.

Abstract:Tumor neoangiogenesis plays an important role in growth and metastasis of human hepatocellular carcinoma (HCC).Angiogenesis induced by tumor cells is different from the normal vessels.Microvessel density measured by immunohistochemistry is used as the gold standard for assessment of angiogenesis,but it is an invasive diagnostic method and its clinical application is restricted.Digital subtraction angiography and noninvasive imaging examinations such as multi-slice spiral CT,MRI,ultrasound,nuclear medicine diagnosis are widely used in clinic for assessment of angiogenesis induced by tumor cells,which can play an important role in clinical diagnosis of HCC and assessment of the treatment effect of inhibiting angiogenesis induced by tumor cells.Molecular images such as optical imaging and synchrotron radiation X-ray imaging are gradually used in the experimental study of assessment of angiogenesis induced by tumor cells.

Abstract:Tumor precision medicine is an emerging approach for tumor diagnosis, treatment and prevention,which takes account of individual variability of environment, lifestyle and genetic information. Tumor precision medicine is built up on the medical imaging innovations developed during the past decades, including the new hardware, new imaging agents, standardized protocols, image analysis and multimodal imaging fusion technology. Also the development of automated and reproducible analysis algorithm has extracted large amount of information from image-based features. With the continuous development and mining of tumor clinical and imaging databases, the radiogenomics, radiomics and artificial intelligence have been flourishing. Therefore, these new technological advances bring new opportunities and challenges to the application of imaging in tumor precision medicine.

Abstract:Tumor growth and metastasis require neovascularization, which is dependent on a complex array of factors, such as the production of various pro-angiogenic factors by tumor cells, intercellular signaling, and stromal remodeling. The hypoxic, acidic tumor microenvironment is not only conducive to tumor cell proliferation, but also disrupts the equilibrium of angiogenic factors, leading to vascular heterogeneity, which further promotes tumor development and metastasis. Anti-angiogenic strategies to inhibit tumor angiogenesis has, therefore, become an important focus for anti-tumor therapy. The traditional approach involves the use of anti-angiogenic drugs to inhibit tumor neovascularization by targeting upstream and downstream angiogenesis-related pathways or pro-angiogenic factors, thereby inhibiting tumor growth and metastasis. This review explores the mechanisms involved in tumor angiogenesis and summarizes currently used anti-angiogenic drugs, including monoclonal antibody, and small-molecule inhibitors, as well as the progress and challenges associated with their use in anti-tumor therapy. It also outlines the opportunities and challenges of treating tumors using more advanced anti-angiogenic strategies, such as immunotherapy and nanomaterials.

Abstract:Molecular imaging agents and a parallel progress in instrumentation ofimaginingtechnology have demonstrated to be effective in improving diagnosis, prognosis, planning, and monitoring of personalized medication. Molecular imaging modalities include positron emission tomography (PET), single photon emission computed tomography (SPECT), magnetic resonance imaging (MRI), computed tomography (CT), ultrasound (US), and optical (Raman, quantum dots, bioluminescence). Among these imaging modalities, PET and SPECT agents could provide target specific information as well as function, pathway activities, and cell migration in the intact organism. Furthermore, the radiotracer could noninvasively assess diseases treatment endpoints which used to rely almost exclusively on biopsies and histopathological assays. New leads on the development of personalized theranostic (image and treat) agents would allow more accuracy in the selection of patients who may respond to treatment. Topics covered in this special issue include advances in biomarkers in preclinical drug discovery, PET/CT and SPECT/CT in disease management, image-guided therapy approach of diseases, imaging technology in drug development, and progress in instrumentation ofimagingtechnology. For instance, upregulation of transporter expressions has often been observed in tumor cells to facilitate their accelerated rates of uptake. Advances in biomarkers in transporter system-based imaging in oncology and neurological diseases such as amino acid (FL. Kong et al.), glucose (YH. Zhang et al.) and dopaminergic (LH. Shen et al.; HF. Hou et al.) transporters were reported. These biomarkers showed that specific uptake in lesions compared to clinic imaging agent. In addition, these biomarkers were capable to be labeled with theranostic isotopes for personalized medication. Most tumors have a considerable proportion of hypoxic cells that are resistant to radio/chemotherapy, with a high propensity to metastasize, and result in worse therapeutic outcome. The contribution by M. Ali et al. and H. Fuji report their newly developed 99mTc-N4-NIM and 125I-IPOS probes for tumor hypoxia. The preclinical findings showed that these biomarkers could image tumor hypoxia by SPECT. The ability to quantify tissue hypoxia would allow the physicians to select patients for additional or alternative treatment regimens that would circumvent the ominous impact of hypoxia. Along this line, L. Jiang and coworkers report the peptide based radiotracer targeting tumor angiogenesis via VEGF integrin alpha(v) beta3. J. Sims-Mourtada and co-workers report 131I-labeled derivatives of the sonic hedgehog (HH) protein for detection of cancer cells via HH receptors. Molecular imaging of cancer stem cell trafficking was reported by T. Xia and co-workers. Hybrid molecular imaging modalities (PET/CT, SPECT/CT) provide high-sensitivity functional and high resolution anatomical imaging, which are important in design personalized treatment. To avoid radiation exposure from multiple slices-CT, K. Tang and co-workers report a reduction of CT tube voltage from 120 kv to 80 kv, the radiation dose could be reduced by 32–42% without losing low-contrast detectability. F. Chao and H. Zhang report the value of using PET/CT in the staging of nonsmall cell lung cancer. Y. Hu and co-workers report the application of PET/CT in assessment of treatment outcome. The application of SPECT/CT in the differential diagnosis of bone metastasis (Y. Zhang et al.) and Parkinson's disease (L. Wang et al.) was reported. Miyake K and co-workers report the application of FDG, MET, and FLT-PET/CT for the management of gliomas. L. Yang and co-workers report the value of using myocardial contrast echocardiography quantitative analysis during adenosine stress over visual analysis before therapy in acute coronary pain. The quantitative analysis correlates well with thrombosis in myocardial infarction. In summary, molecular imaging could integrate metabolomics and chemical biology. Molecular imaging agents could characterize target expressions, understand the disease progression, prediction for drug response and toxicity, staging, grading, and micrometastasis, and support animal studies. This special issue provides a platform of efficacy of personalized medication from molecular imaging technology which may have high impact on drug discovery, delivery, and development. Abstract:The growing use of molecular imaging helps to control and monitor dosage for increased safety and effectiveness. Molecular imaging agents become the scientific tools for moving a concept from bench work to clinic product development. This special issue provides original research and review of new trends in molecular imaging agent development. Among clinic molecular imaging modalities, radionuclide imaging technique is the most sensitive and could provide target-specific information. The radiotracer could non-invasively assess disease treatment endpoints which used to rely almost exclusively on biopsies and histopathological assays. L.-H. Shen et al. (Taiwan) reviewed molecular imaging in the diagnosis and management of CNS disorders. F.-L. Kong et al. (USA), N. Tsao et al. (USA), and C.-L. Ho et al. (Taiwan) report pharmacokinetics and dosimetry of molecular imaging compounds in oncology. M.-H. Liao et al. (Taiwan), X. Wang et al. (China), C. Wu et al. (China) and M. Tian et al. (USA) reported that molecular imaging techniques play a major role in the development of novel therapies since they measure target expressions (apoptosis, glycolysis) as well as function, pathway activities, and cell migration in the intact organism. Y.-F. Wang et al. and M.-Y. Chao et al. (Taiwan) described the kit probe for 99mTc-labeling which is crucial for centralized drug process. The kit probe may allow theranostic concept. A theranostic agent would be more accurate in the evaluation of patient response to treatment. The radiotracer could also be used for radionuclide therapy as reported by C. Wang et al. (China). Hybrid molecular imaging modalities (PET/CT, SPECT/CT) provide high-sensitivity functional and high-resolution anatomical imaging which are important in design-personalized treatment. M.-C. Chen et al. (Taiwan) reported the value of using I-131 NP-59 SPECT/CT to diagnose patients with subclinical or atypical features of primary aldosteronism. To proof of concept for molecular biology findings using different imaging modalities (optical, sonography) were described by Z. Pu et al. (China). In summary, molecular imaging enables the comprehensive characterization of therapeutic intervention and can be used in preclinical studies, pharmacokinetic studies, dose-finding studies, and proof-of-concept studies. Abstract:Early diagnosis and precision medicine generally show significant differences in the prognosis of patients with carcinoma. Angiogenesis not only plays a key role in tumor pathophysiology but also acts as an important drug target. Peptides with specific se-quences can target specific molecules on the endothelial cellular membrane during tumor angiogenesis. Radionuclide-labeled molecu-lar probes exhibit many advantages in oncotherapy. This article focuses on the progress of radionuclide-labeled RGD and RRL in radio-immunoimaging and radioimmunotherapy targeting tumor angiogenesis.

Abstract:Integrin is often significantly upregulated in activated endothelial cells during tumor angiogenesis. The arginine-glycine-aspartic acid (RGD) peptide sequence is a specific recognition motif to αvβ3 integrin. In this study, a RGD labeled, Poly lactic acid (PLA) coated ultrasmall paramagnetic iron oxide (USPIO) (referred to as RGD-PLA-USPIO) were developed and the ability to detect tumor angiogenesis was investigated in vitro and in vivo. Increased uptake of RGD-PLA-USPIO by human umbilical vein endothelial cells (HUVECs) was detected by Prussian blue stain and transmission electronic microscopy (TEM). Pronounced signal decrease in T2*-weighted magnetic resonance image (MRI) and heterogeneous arrangement of neovasculature of tumor tissue were clearly identified in Vx-2 tumor model. The MR signal of contralateral muscle only could be seen a slight background change after either RGD-PLA-USPIO or PLA-USPIO injection. These studies demonstrate the efficiency of RGD-PLA-USPIO to visualize αvβ3 integrin in activated tumor endothelial cells and its potential for detecting and monitoring tumor vasculature change after therapy

Abstract:Molecular imaging of cancer is a collaborative endeavor, uniting scientists and physicians from diverse fields. Such collaboration is actively developing and translating cutting-edge molecular imaging approaches to enhance the diagnostic landscape of human malignancies. The advent of positron emission tomography (PET) and PET imaging tracers has realized non-invasive target annotation and tumor characterization at the molecular level. In surgical procedures, novel imaging techniques, such as fluorescence or Cherenkov luminescence, help identify tumors and enhance surgical precision. Simultaneously, progress in imaging equipment, innovative algorithms, and artificial intelligence has opened avenues for next-generation cancer screening and imaging, augmenting the efficiency and accuracy of cancer diagnosis. In this review, we provide a panorama of molecular cancer imaging and ongoing developments in the field.

Abstract:Molecular imaging is an advanced technology that utilizes specific probes or markers in conjunction with cutting-edge imaging techniques to observe and analyze the localization, distribution, activity, and interactions of biomolecules within living organisms. Tumor molecular imaging, by enabling the visualization and quantification of molecular characteristics of tumor cells, facilitates a deeper and more comprehensive understanding of tumors, providing valuable insights for early diagnosis, treatment monitoring, and cancer biology research. However, the image quality of molecular imaging still requires improvement, and nanotechnology has significantly propelled the advancement of molecular imaging. Currently, nanoparticle-based tumor molecular imaging technologies encompass radionuclide imaging, fluorescence imaging, magnetic resonance imaging, ultrasound imaging, photoacoustic imaging, and multimodal imaging, among others. As our understanding of the tumor microenvironment deepens, the design of nanoparticle probes for tumor molecular imaging has also evolved, offering new perspectives and expanding the applications of tumor molecular imaging. Beyond diagnostics, there is a marked trend towards integrated diagnosis and therapy, with image-guided treatment playing a pivotal role. This includes image-guided surgery, photodynamic therapy, and chemodynamic therapy. Despite continuous advancements and innovative developments in molecular imaging, many of these remain in the experimental stage and require breakthroughs before they can be fully integrated into clinical practice.
Multimodality Molecular Imaging of CD105 (endoglin) Expression.

Abstract:Since most solid tumor growth depends on angiogenesis, non-invasive imaging of tumor angiogenesis can allow for much earlier diagnosis and better prognosis of cancer, as well as more accurate treatment monitoring, which will eventually lead to personalized molecular medicine. CD105, also known as endoglin, is required for endo-thelial cell proliferation. The currently accepted standard method for quantifying tumor angiogenesis is to assess microvessel density based on CD105 staining, which has been shown to be an independent prognostic factor for survival in patients of almost all solid tumor types. In this review, we will summarize the progress to date on multimo-dality molecular imaging of CD105 expression during tumor angiogenesis which includes targeted contrast-enhanced ultrasound, molecular magnetic resonance, near-infrared fluorescence, single-photon emission computed tomography, and positron emission tomography. Although molecular imaging of CD105 expression is surprisingly understudied, non-invasive imaging of CD105 expression has already been achieved with every single molecular imaging modality. In the future, significant research effort should be directed towards non-invasive visualization of CD105 expression, such as quantitative imaging, the use of long-lived isotopes for antibody-based imaging, development of peptide, small molecule, or antibody fragment-based imaging agents, multimodality imaging of CD105 expression with a single agent, the application of nanotechnology, among others.

Abstract:Recent advances in molecular imaging have facilitated early disease detection, diagnosis, and therapeutic efficacy monitoring. Clinicians aspire to achieve prompt diagnosis, provide personalized treatments, and accurately monitor and quantify therapy effectiveness. This has fueled a growing interest in tracing biomarkers and biochemicals associated with disease progression. Identifying crucial biomarkers and refining accurate, minimally invasive monitoring methods are the pivotal focuses of ongoing molecular imaging research. Consequently, there is a notable surge of interest in developing molecular probes and multi-modal systems to enhance imaging capabilities. This review is intended to provide an overview of the promise and limitations of different modalities employed in molecular imaging for patient care, along with the ongoing research aimed at innovating novel imaging agents and devices. Molecular imaging holds the potential to revolutionize disease diagnosis and treatment.

Abstract:Objectives This review describes the current status and progress of immune checkpoint targets for imaging of malignancies. Immune checkpoint blockade holds great potential for cancer treatment, and clinical implementation into routine is very rapidly progressing. Therefore, it is an urgent need to become familiar with the vocabulary of immunotherapy and with the evaluation of immune checkpoint and related treatments through noninvasive molecular imaging. Currently, immune target-associated imaging mainly includes PET, SPECT, optical imaging, and MRI. Each imaging method has its own inherent strengths and weaknesses in reflecting tumor morphology and physiology. PD-1, PD-L1, CTLA-4, and LAG-3 are the most commonly considered targets. In this review, the current status and progress of molecular imaging of immune checkpoint targets are discussed. Conclusion Molecular imaging is likely to become a major tool for monitoring immunotherapy. It can help in selecting patients who are suitable for immunotherapy, and also monitor the tumor response. Key Points • Immune checkpoint blockade holds great promise for the treatment of different malignant tumors. • Molecular imaging can identify the expression of immune checkpoint targets in the tumor microenvironment at the molecular and cellular levels, and therefore helps selecting potential responders, suitable for specific immunotherapy. • Molecular imaging can also monitor immunotherapeutic effects, and therefore participates in the evaluation of tumor response to treatment.
