Abstract:The prevalences of asthma, allergic rhinitis (AR), and eczema were analyzed in relation to retrospective risk factors from birth in a questionnaire study of schoolchildren in two areas covering the whole climatic span of Sweden: the Göteborg area on the southwestern coast (7‐year‐olds, n= 1649) and Kiruna, a mining town in the northernmost inland mountains (7–9‐year‐olds, n= 832). The strongest background factor, a family history of the diseases, was more common in children with another strong risk factor, particularly for asthma: high frequency of upper respiratory tract infection (URTI). Other significant risk factors related to high indoor humidity caused an increased prevalence of both allergic diseases and URTI. Active mechanical ventilation of the homes caused a slight reduction of the prevalence of allergic diseases, and repainting or new wallpaper in the bedroom of the child after birth caused a moderately increased risk of allergic disease. This study illustrates the interaction between genetic and environmental risk factors with special emphasis on factors related to an unventilated indoor climate, which may have substantially contributed to the current increase of the diseases in the country.

Abstract:The aim of the study was to test the hypothesis that a low-ventilation rate in homes is associated with an increased prevalence of asthma and allergic symptoms among children. A total of 198 cases (with at least two of three symptoms: wheezing, rhinitis, eczema) and 202 healthy controls, living in 390 homes, were examined by physicians. Ventilation rates were measured by a passive tracer gas method, and inspections were carried out in the homes. About 60% of the multi-family houses and about 80% of the single-family houses did not fulfill the minimum requirement regarding ventilation rate in the Swedish building code (0.5 air changes per hour, ach). Cases had significantly lower ventilation rates than controls and a dose-response relationship was indicated.

Abstract:Background-During the last 30-50 years, the prevalence of children’s asthma and allergy has been increasing dramatically worldwide. Indoor environment, especially in homes, is suspected to play an important role in this increase. A global network study on “asthma and the home environment” started in Sweden, followed by Bulgaria, Singapore, Taiwan, Denmark, USA, South Korea and China. Aim-To picture a global map on the prevalence of asthma and allergy in children as well as on characteristics of homes and indoor environmental exposures; to identify general and specific risk and protective factors for asthma and allergy among children. Methods-The “Children-Health-Homes” global study consist of two phases. In phase I, information on children´s health and their home environment is collected in a baseline questionnaire survey. In phase II, a case-control study is conducted including measurements of indoor environmental parameters as well as medical examinations. This paper includes phase I data. Results-In Phase I more than 100,000 children up to 8 years old participated with response rates from 35% to 79% in eight countries. The prevalence rate of doctor diagnosed asthma varied from 4.4% (Bulgaria) to 15.6% (USA), hay fever from 2.0% (Denmark) to 27.2% (USA), Current rhinitis from 10.2% (Denmark) to 50.9% (USA). The rate of PVC flooring ranged from 0.9% (Denmark) to 57% (South Korea). Air conditioning units were installed in 46% Singaporean families and in 81% in USA, while natural and micahnical ventilation system were reported by 84% Swedish families. The rate of visible mold on indoor surfaces ranged from 1.5% (Sweden) to 22% (South Korea). The rate of breast feeding less than 3 months ranged from 15% (Sweden) to 65% (USA). Conclusions-These results suggest substantial worldwide variations in the prevalence of asthma and allergy, home environment and life style. These differences may offer important clues to environmental influences on allergy.

Abstract:The risk of indoor exposure to volatile organic compounds (VOCs) on allergic airway diseases in children remains unknown.We examined the residential concentrations of VOCs, emitted from building materials, paints, furniture, and other lifestyle practices and the risks of multiple allergic diseases as well as the IgE-sensitization in pre-school age children in Sweden.In a case-control investigation (198 case children with asthma and allergy and 202 healthy controls), air samples were collected in the room where the child slept. The air samples were analyzed for the levels of eight classes of VOCs.A natural-log unit of summed propylene glycol and glycol ethers (PGEs) in bedroom air (equal to interquartile range, or 3.43 - 15.65 µg/m(3)) was associated with 1.5-fold greater likelihood of being a case (95% CI, 1.1 - 2.1), 1.5-fold greater likelihood of asthma (95% CI, 1.0 - 2.3), 2.8-fold greater likelihood of rhinitis (95% CI, 1.6 - 4.7), and 1.6-fold greater likelihood of eczema (95% CI, 1.1 - 2.3), accounting for gender, secondhand smoke, allergies in both parents, wet cleaning with chemical agents, construction period of the building, limonene, cat and dog allergens, butyl benzyl phthalate (BBzP), and di(2-ethylhexyl)phthalate (DEHP). When the analysis was restricted to the cases, the same unit concentration was associated with 1.8-fold greater likelihood of IgE-sensitization (95% CI, 1.1 - 2.8) compared to the non-IgE sensitized cases. No similar associations were found for the other classes of VOCs.We propose a novel hypothesis that PGEs in indoor air exacerbate and/or induce the multiple allergic symptoms, asthma, rhinitis and eczema, as well as IgE sensitization respectively.

Abstract:There have been no worldwide standardised surveys of prevalence and severity of asthma, rhinoconjunctivitis and eczema in school children for 15 years. The present study aims to provide this information. Following the exact International Study of Asthma and Allergies in Childhood (ISAAC) methodology (cross-sectional questionnaire-based survey) the Global Asthma Network (GAN) Phase I was carried out between 2015 and 2020 in many centres worldwide. The study included 157 784 adolescents (13–14 years of age) in 63 centres, in 25 countries; and 101 777 children (6–7 years of age) in 44 centres, in 16 countries. The current prevalence of symptoms, respectively, was: 11.0% and 9.1% for asthma; 13.3% and 7.7% for rhinoconjunctivitis; and 6.4% and 5.9% for eczema. For asthma ever, hay fever ever and eczema ever prevalence was 10.5% and 7.6%; 15.2% and 11.1%; and 10.6% and 13.4%. Centres in countries with low- or lower-middle- gross national income (LICs or L-MICs) had significantly lower prevalence of the three disease symptoms and diagnoses (except for hay fever). In children, the prevalence of asthma and rhinoconjunctivitis symptoms were higher in males, while the reverse occurred among adolescents. For eczema, while the prevalence among female adolescents was double that of males, there was no sex difference among children. Centre accounted for a non-negligible variability of all disease symptoms (10% to 20%). The burden of asthma, rhinoconjunctivitis and eczema vary widely among the limited number of countries studied. Although symptom prevalence is lower in LICs and L-MICs, it represents a considerable burden everywhere studied.

Associations Between Parental Health, Early Life Factors and Asthma, Rhinitis and Eczema among Pre-School Children in Chongqing, China

Abstract:Purpose: To study associations between parental health and children’s asthma, rhinitis and eczema.Methods: Parents of 3-6 years old children from randomized kindergartens in Chongqing, China answered a questionnaire on parents’ history of asthma/allergies, current symptoms and children’ doctor-diagnosed asthma and rhinitis, wheeze, cough, rhinitis and eczema. Associations were analyzed by multiple logistic regression.Results: Among 4250 children (response rate: 74.5%), 8.4% had doctor-diagnosed asthma (DD asthma); 6.2% doctor-diagnosed allergic rhinitis (DD rhinitis); 20.4% current wheeze; 19.4% cough; 37.9% rhinitis and 13.6% eczema. Among reporting parents (females 70.4%, males 20.6% ), 16.2% were smokers; 47.4% had any current rhinitis; 54.2% cough; 47.8% skin symptoms; 70.5% fatigue and 48.7% headache.Parental asthma or allergy was associated with children’s DD asthma (OR=3.64) and DD rhinitis (OR=4.23). The associations were stronger for paternal asthma or allergy. Children of mothers who were salespersons during pregnancy had more rhinitis (OR=1.49), and children of white-collar worker mothers had more DD (OR=1.49) and DD rhinitis (OR=1.92). Rural children had less DD rhinitis and current rhinitis. Parental current symptoms were associated with wheeze, cough, rhinitis and eczema among the children with OR ranging from 1.37 to 2.28 (all p<0.001).Conclusions: Parental asthma or allergy can be a risk factor for children’s asthma or allergy, especially paternal asthma or allergy. Growing up in rural areas can be beneficial for rhinitis. Mothers’ occupations during pregnancy may influence asthma and rhinitis in offspring. In studies on children’s asthma or allergies, based on parental reporting, parents’ current symptoms may influence the results.

Abstract:Background: The reported impact of day-care attendance on respiratory and atopic symptoms has varied between studies from different countries. Regarding to the 'hygiene-hypothesis', day-care attendance may lead to less sensitization later in life, but the question still is whether day-care attendance and subsequent exposure to more frequent early infections is a risk or a protection against future allergic disease or asthma (atopic and nonatopic).Methods: A cross-sectional postal questionnaire was replied by parents of 10 851 children, aged 1-6 years, in the year 2000 in a Swedish region (DBH-phase 1). The questionnaire focused on respiratory and atopic symptoms, the home environment and information on day care of the children.Results: Children in day care were reported to have more symptoms than children in home care: adjusted odds ratio (AOR) for wheezing last 12 months, AOR 1.33 (CI 95%: 1.12-1.58), cough at night apart from colds last 12 months AOR 1.56 (CI: 1.17-2.07), doctor diagnosed asthma AOR 1.23 (CI: 0.88-1.71), rhinitis last 12 months AOR 1.15 (CI: 0.92-1.44), doctor diagnosed hay fever AOR 1.75 (CI: 0.94-3.23), eczema last 12 months, AOR 1.49 (CI: 1.24-1.79), allergic reactions to foods, AOR 1.27 (CI: 1.07-1.52), > 6 colds last 12 months of 2.57 (CI: 2.12-3.12) and ear infection ever AOR 2.14 (CI: 1.87-2.45). The increased risks were mainly seen and reached significance in the youngest group of children, aged 1-4 years. Adjusting and stratification for the number of airway infections last year did not change the risk associated with day-care attendance for allergic diseases.Conclusions: Attending day care was associated with an increased risk of symptoms related to airways infections as well with eczema and allergic reactions to food. No sign of protection from day-care attendance for allergic diseases was found up to 6 years of age. Multiple airway infections and day-care attendance were found to be independently associated with asthma and allergic symptoms.

In a case-control study, 53 Hdm-sensitized children (cases), a group of non-Hdm-sensitized atopic children (N = 54), and a group of nonallergic neighborhood children (N = 53) were included. Mattress dust was analyzed with ELISA for content of Hdm allergens. Indoor humidity and temperature were measured, and questionnaire data were collected. Asthma was significantly more common in the Hdm-sensitized group than in the atopic control group. Hdm allergens were found in 40%, 19%, and 23% of the dust samples of the case group, atopic group, and neighborhood control groups, respectively. Only 15% of the dust samples of the Hdm-sensitized children contained Hdm allergens above the recommended threshold levels of sensitization of 2000 ng/gm of dust. Inadequate ventilation, which partly appeared to be a consequence of energy saving, appeared to be a risk factor for Hdm sensitization. This result may apply also to other temperate regions and suggests that the problem is preventable.

Abstract:Low ventilation rate have been shown to increase the risk for health and comfort problems in offices. However, very few studies have investigated the impact of ventilation rate at home on health effects, (Wargocki et al. 2002). The aim of this study was to investigate if low ventilation in homes increases the risk for asthma and other allergic symptoms among pre-school children in Sweden. The first step of the study “Dampness in Buildings and Health” started in March 2000 with a questionnaire investigation including all children (1–6y) in Värmland, Sweden (n = 14 077). The questionnaire included 84 questions concerning family background, symptoms among children and parents, home characteristics including dampness indications and food habits. Parents to 10 851 children responded the questionnaire, corresponding to a response rate of 79 %. In a second step 400 children were selected for a case-control study. The children were selected from the questionnaire data in the first step. Cases (n = 200) were children with at least two symptoms of three possible (i.e. wheezing, rhinitis and eczema). Controls (n = 200) were children without any symptoms at all. The 400 homes were investigated by professional inspectors and measurements were made of ventilation rate. Measurements were made with passive technique during one week during wintertime from October 2001 to April 2002. The 400 children lived in 352 single family houses (SH), (88 %) and in 45 multi family houses (MH), (11 %) and 3 children lived in other type of buildings. Valid samples for ventilation rate were collected for 396 children (198 cases and 198 controls). Mean values for ventilation rate were 0,37 Air Change Rate per Hour (ACRH) with significant differences (p < 0.001) between SH and MH, (SH:0.36 and MH:0.48 ACRH respectively). Cases had lower mean ventilation rate than controls, (0.36 and 0.39 ACRH respectively), However, the difference were not significant (p = 0.13). When only children living in SH were included in the analysis (n = 349) there was significant (p < 0.05) lower ventilation rate among cases compared with controls (0.34 and 0.39 ACRH respectively). Furthermore, data on ventilation rate in single family houses were divided into quartiles: (0–25%, n = 87) 0.17 ACRH (min 0.05–max 0.24); (25–50%, n = 88) 0.29 ACRH (0.24–0.33); (50–75%, n = 87) 0.38 ACRH (0.33–0.44) and (75–100%, n = 87) 0.62 ACRH (0.44–1.43). Logistic regression indicated a dose response relationship when the highest quartile was used as reference: (0–25% quartile) OR 1.92 (1.05–3.50); (25–50% quartile) OR 1.49 (0.82–2.70); (50–75% quartile) OR 1.39 (0.76–2.53). Concerning multi family houses no difference in ventilation rate was found between cases and controls.

Abstract:The aim of this study is to investigate the status of child's health including allergy and asthma and home environment in Korea and to explore the association between them. This study is based on self-administered questionnaires and 2740 out of 5107 surveys had been collected and a response rate was 53.7%. Frequent symptoms were rhinitis last 12 month, doctor-diagnosed rhinitis and eczema. Children living in single-family or row houses have shown significantly more eczema and old housing residents has shown more wheezing and coughing than newly built housing residents. Dampness showed different distribution by housing characteristics. Condensation, mold and damp stain were found more often in single family houses and houses built earlier. Children living in homes with dampness showed statistically significant association with allergic symptoms. From these statements it could be expected that dampness is found by different housing character and the found dampness is related to allergy symptoms.

Abstract:BACKGROUND: Little is known about the prevalence of atopic eczema outside Northern Europe.OBJECTIVES: We sought to describe the magnitude and variation in the prevalence of atopic eczema symptoms throughout the world.METHODS: A cross-sectional questionnaire survey was conducted on random samples of schoolchildren aged 6 to 7 years and 13 to 14 years from centers in 56 countries throughout the world. Those children with a positive response to being questioned about the presence of an itchy relapsing skin rash in the last 12 months that had affected their skin creases were considered to have atopic eczema. Children whose atopic eczema symptoms resulted in sleep disturbance for 1 or more nights per week were considered to have severe atopic eczema.RESULTS: Complete data was available for 256,410 children aged 6 to 7 years in 90 centers and 458,623 children aged 13 to 14 years in 153 centers. The prevalence range for symptoms of atopic eczema was from less than 2% in Iran to over 16% in Japan and Sweden in the 6 to 7 year age range and less than 1% in Albania to over 17% in Nigeria for the 13 to 14 year age range. Higher prevalences of atopic eczema symptoms were reported in Australasia and Northern Europe, and lower prevalences were reported in Eastern and Central Europe and Asia. Similar patterns were seen for symptoms of severe atopic eczema.CONCLUSIONS: Atopic eczema is a common health problem for children and adolescents throughout the world. Symptoms of atopic eczema exhibit wide variations in prevalence both within and between countries inhabited by similar ethnic groups, suggesting that environmental factors may be critical in determining disease expression. Studies that include objective skin examinations are required to confirm these findings.

Abstract:BACKGROUND:Few data exist on asthma and rhinitis across China in relation to indoor and outdoor air pollution, climate and socioeconomic factors. The main aim was to study associations between asthma, rhinitis and current respiratory symptoms among pre-school children across China and selected indoor and outdoor exposure and indicators of socio-economic status (SES) in mutually adjusted models. METHODS:Chinese children (3-6 yr.) (n = 39,782) were recruited from randomly selected day care centres in seven cities in China. Data on asthma, respiratory symptoms, rhinitis, indoor and outdoor exposure at home and SES were assessed by a parentally administered questionnaire. Lifetime mean ambient temperature, PM10, NO2, and GDP per capita on city level were calculated. RESULTS:Totally 7.4% had ever doctors' diagnosed (DD) asthma and 8.7% DD-rhinitis, 19.7% had current wheeze, 45.0% rhinitis and 16.9% cough. DD-asthma was associated with ambient temperature (OR = 1.15, 95% CI 1.11-1.20 per °C), NO2 (OR = 1.16, 95% CI 1.02-1.33 per 10 μg/m3), indoor mould/dampness (OR = 1.25, 95% CI 1.13-1.39) and living near major roads (OR = 1.13, 95% CI 1.02-1.25). DD-rhinitis was associated with ambient temperature (OR = 1.07, 95% CI 1.05-1.10 per °C), NO2 (OR = 1.20, 95% CI 1.09-1.32 per 10 μg/m3), GDP (OR = 1.03, 95% CI 1.01-1.06 per 10,000 RenMinBi/year), indoor mould/dampness (OR = 1.23, 95% CI 1.11-1.35), passive smoking (OR = 1.11, 95% CI 1.01-1.21), and living near major roads (OR = 1.14, 95% CI 1.03-1.25). Children in suburban or rural areas, in larger families (≥5 persons) and with prenatal farm exposure had less DD-asthma and DD-rhinitis. CONCLUSIONS:Economic development level of the city, higher SES, ambient temperature, NO2, PM10, traffic air pollution and mould/dampness can be risk factors for asthma and rhinitis and respiratory symptoms among pre-school children in China. Breastfeeding, large family size and early-life farm exposure could be protective factors.
Epidemiological characterization and risk factors of rhinitis and rhinoconjunctivitis among preschool children in Shanghai, China

Abstract:Background Previous studies have reported an increasing prevalence of childhood allergic rhinitis in developing countries. There is still a lack of the recent epidemiology of allergic rhinitis among Chinese preschool children. Therefore, this study explored the prevalence of rhinitis symptoms and identified their associations with potential risk factors among children at the age of 3–6 in Shanghai, China. Methods Validated International Study of Asthma and Allergies in Childhood (ISAAC) questionnaire was adopted to collect information about rhinitis symptoms and potential risk factors. Univariate and multivariate logistic regression analyses were used to assess associations between risk factors and allergic rhinitis and rhinoconjunctivitis. Results A total of 6183 questionnaires were included in our study. The prevalence of rhinitis ever, current rhinitis, and physician-diagnosed rhinitis were 32.6%, 29.2%, and 14.3%, respectively, while the prevalence of current rhinoconjunctivitis was 11.3%. The higher prevalence was observed in boys than in girls in terms of rhinitis ever, current rhinitis, current rhinoconjunctivitis and doctor-diagnosed rhinitis. Autumn had the highest prevalence among four seasons. In our multivariate logistic regression analyses, history of allergic diseases and paracetamol use in the last year showed positive associations with the increased risk of both current rhinitis and rhinoconjunctivitis, and antibiotic use was an independent significant risk factor only for current rhinitis. Genetic factors, including maternal and paternal rhinitis, asthma, and eczema, were significantly associated with the prevalence of current rhinitis. Similar associations were seen between these factors and current rhinoconjunctivitis, except for paternal eczema. Among environmental factors, smoking exposure at home, heavy truck traffic in home's street, floor heating system were independent risk factors for both current rhinitis and rhinoconjunctivitis in the adjusted model, while cleaning the house less than once a week was only associated with current rhinitis. Conclusion The prevalence of current rhinitis was 29.2% among children aged 3–6 in Shanghai, China. Sex differences and seasonal variations were observed in the prevalence of rhinitis symptoms. The identified risk factors would provide a basis for policy makers and medical experts to take intervention measures to prevent allergic rhinitis and rhinoconjunctivitis.

The epidemiology of eczema in children and adults in England: A population-based study using primary care data

Abstract:Background: Whilst eczema is a common inflammatory skin condition, we lack contemporary estimates of disease incidence and prevalence across the lifespan. Objective: To estimate the incidence and prevalence of eczema in children and adults in England and variation by sociodemographic factors (sex, socio-economic status, ethnicity, and geography). Methods: We used the Royal College of General Practitioners Research and Surveillance Centre primary care research database of 3.85 million children and adults registered with participating general practitioner practices between 2009 and 2018 inclusive. Eczema incidence was defined as the first-ever diagnosis of eczema recorded in the primary care record, and eczema prevalence was defined as fulfilment of criteria for active eczema (two eczema records appearing in the primary care record within any one-year period). Results: Eczema incidence was highest in infants younger than 1 year (15.0 per 100 person-years), lowest in adults aged 40-49 (0.35 p/100 person-years), and increased from middle age to a second smaller peak in people 80 years or older (0.79 p/100 person-years). Eczema prevalence was highest in children aged 2 (16.5%) and lowest in adults aged 30-39 (2.8%). Eczema incidence was higher in male infants (<2) and male adults older than 70; for all other ages, incidence was higher in females. Eczema was more common in Asian and black ethnic groups than in people of white ethnicity. Higher socio-economic status was associated with a greater incidence of eczema in infants younger than 2, but the reverse was seen for all other age groups. Both incidence and prevalence of eczema were greater in urban settings and in North-West England. Conclusions and clinical relevance: Eczema has a bimodal distribution across the lifespan. We observed differences in incidence and prevalence of eczema by ethnicity, geography, sex, and socio-economic status, which varied in magnitude throughout life.

Abstract:PP-31-077 Background/Aims: Allergic disease has become an issue due to rapidly increasing prevalence in children and environmental factors are known to be possible causes of the disease. The aim of this study is to explore the associations between indoor environmental factors and asthma and allergy symptoms among pre-school children. Methods: This study is based on self-administered questionnaires to survey the home environment and the children's health. The standardized International Study of Asthma and Allergies in Childhood (ISAAC) questionnaire used. Results: A total of 2145 surveys have been distributed and 1102 of them have been returned (response rate 51.4%) and 1067 of them were valid. The prevalence of doctor-diagnosed rhinitis was 18.4% and eczema was 23.6%. More than 70% of children were living in multi-family buildings or town house and living in multi-family house increased the risk of doctor-diagnosed rhinitis and cough at night. There was a significantly increased risk of all symptoms as the size of residence is smaller. More than 50% of homes used PVC material and it tended to be associated with all of the investigated symptoms. Of the parents, 53.9% reported visible mold and 39.4% reported damp stains. Discolored wood floors, water leakage/flooding and floor moisture had strong association with cough at night, doctor-diagnosed asthma and Rhinitis. The risk was higher for children living in homes with condensation more than 25 cm. At least one of the family members smoked in 66.5% of the families. Smoking habit and keeping pets are significantly associated with all symptoms. Attending day-care center earlier increased the risk of doctor-diagnosed Asthma. Conclusion: The present survey found that Type of dwelling, size of residence, flooring material, dampness, smoking habit, attending day care were associated with an increased risk of allergic symptoms.

Abstract:Background Very long‐chain (VLC) polyunsaturated fatty acids (PUFA) have been hypothesized to influence the risk of allergic disease. The aim of the study was to investigate the role of plasma levels of omega‐3 (n‐3) and omega‐6 (n‐6) PUFA in childhood and adolescence, for the development of rhinitis and allergic sensitization up to young adulthood. Methods The study included n = 933 participants from the BAMSE cohort. Proportions of n‐3 and n‐6 PUFA in plasma phospholipids were analyzed at 8 and 16 years using gas chromatography. Associations between PUFA and rhinitis as well as allergic sensitization, analyzed by IgE reactivity against airborne allergens, up to age 24 years were analyzed by generalized estimating equations and logistic regression models. Results High plasma levels of VLC n‐3 PUFA as well as the n‐6 PUFA arachidonic acid (AA) at 8 years were inversely associated with rhinitis (overall OR up to 24 years: 0.72, 95% CI 0.55, 0.0.93 and 0.69 [0.53, 0.89], respectively) and aeroallergen sensitization (0.64 [0.49, 0.83] and 0.71 [0.54, 0.92], respectively). However, excluding prevalent cases at 8 years attenuated the associations. Conclusion Plasma levels of n‐3 and certain n‐6 PUFA in childhood were inversely associated with allergic sensitization and allergic rhinitis up to young adulthood. The association may to some extent be explained by persistent childhood disease, rather than new incident cases in adolescence and young adulthood. To what extent these associations are driven by dietary PUFA intake versus metabolism remains to be clarified for the prevention of rhinitis and allergic sensitization.
