Hyperglycemia Affects the Prognosis of Acute Myocardial Infarction

Abstract:Objective: To observe the influence of admission blood glucose on the prognosis of acute myocardial infarction(AMI) in patients without a history of diabetes mellitus during hospital stay.Methods: A total of 737 AMI patients without a history of diabetes mellitus were divided into 3 groups according to the level of admission blood glucose(BG): GroupⅠ(n=357) with BG6.1 mmol/L,Group Ⅱ(n=228) with BG between 6.1 and 7.8 mmol/L,and Group Ⅲ(n=152) with BG 7.8 mmol/L.The basic clinical data,infarction sites,incidence of main complications and case-fatality were compared among the three groups.Results: In those with higher BG,there were more females,older in age,and with fewer initial angina symptoms,faster heart rate and lower blood pressure.The incidences of heart failure,severe arrhythmia and cardiac shock and case-fatality were obviously higher in Group Ⅱ than in Ⅰ(P0.05).And the rate of main complications and case-fatality were significantly higher in Group Ⅲ than in the other two during the hospital stay(P 0.01 and P0.05).No statistically significant differences were observed in the infarction sites among the three groups(P 0.05).Conclusion: Non-diabetic AMI patients with elevated admission BG had higher incidences of heart failure,severe arrhythmia and cardiac shock,higher case-fatality and worse prognosis during the hospital stay.
