Distribution of Endometrial Cytology by Age Groups:A Hospital-Based Study

Abstract:Background and purpose:Cytology is a valid, convenient and inexpensive method for the diagnosis of endometrial pathologic lesion, and is appreciated for its potential to be applied in endometrial cancer screening. While a screening test must detect a considerable number of precursor, which for endometrial cancer is hyperplasia, to ensure its cost-effectiveness. The purpose of this study is to explore the detection of endometrial cytology among Chinese women on different age groups and other factors (area, hormonal replacement therapy, symptoms, etc.). Methods:After obtaining informed consent,1 903 women who went to health centers for medical treatment or regular check-up were recruited to the program from August 2005 to November 2007. Endometrial cells were collected by miniature noninvasive device. Diagnostic outcome was classif ied as normal endometrium, in? am mation, simple hyperplasia, atypical hyperplasia and endometrial carcinoma. A description of endometrial cytology was made and further presented within different age groups and other factors. ANOVA, Fisher’s exact test and Multinomial logistic regression were applied to test the significance between cytological type and the factors. Results:1 903 women were enrolled in the program, and 1 770(93.0%) of the specimens were successfully prepared for cytological diagnosis, of which 81.8%(1 448/1 770) were classified as normal endometrium, 2.7%(48/1 770) as inflammation,9.0%(159/1 770) as simple hyperplasia, 6.3%(112/1 770) as atypical hyperplasia and 0.2% (3/1 770) as endometrial carcinoma. The proportion of atypical hyperplasia was 2.4% for women under 40 years old, while corresponding proportions were 6.8%, 9.2%, 8.5%, 8.0% for age groups of 40-49, 50-59, 60-69 and above 69 years old, respectively. There were 3 cases of endometrial carcinoma for women 40 years old or above, and no endometrial carcinoma was detected among those under 40 (P0.001). With other factors such as areas and BMI controlled, women within age groups of 40-49, 50-59 and 60~69 years old are 2.9 (95% CI: 1.5-5.7), 3.6(95% CI:1.9-6.8) and 3.7 (95% CI:1.1-12.3) times more likely to be detected as atypical hyperplasia. Conclusions:Compared those women under 40 years old, women with 40 years old and above had obviously large proportions of atypical hyperplasia and endometrial carcinoma.
