Abstract:Question: A 16-year-old girl was admitted to the hospital because of progressive diarrhea, fever, abdominal pain, and weight loss for 1 month after she ate “takeaway food” for 15 days. Seven days before presentation she was admitted to the local hospital and treated with broad-spectrum antibiotics, which resulted in mild improvement in her symptoms. Then she came to our hospital for further evaluation and treatment. She has no history of travel or family history of diarrheal illnesses. On examination, the patient appeared pale and thin. There was diffuse tenderness of the abdomen with no rebound tenderness. Erythrocyte sedimentation rate was 55 mm/h (reference, <18 mm/h), C-reactive protein was 118 mg/L (reference, <8 mg/L), white blood cell count was 17.79 × 10⁹ cells/L (reference, 3.5–9.5 × 10⁹/L), and the neutrophils count was 84.90 × 10⁹ cells/L (reference, 40–75 × 10⁹/L). Stool cultures yielded Pseudomonas aeruginosa and gram-positive cocci. Blood cultures were negative. Tests for thyroid function, TSPOT, cytomegalovirus DNA, B2-GP1, HLA-B51, and rheumatic series were unremarkable. Computed tomography enterography of the abdomen after the administration of contrast material showed a hepatic cyst, multiple enlarged lymph nodes, and thickening of the ileocecum, ascending colon, and transverse colon walls (Figure A). Esophagogastroduodenoscopy showed multiple ulcers in the lower part of esophagus and non-atrophic gastritis (Figure B). Colonoscopy revealed an erosion of terminal ileum and multiple ulcers located in the colon (Figure C). What is the diagnosis? In the multidisciplinary team meeting, the radiologist found a significantly enlarged lymph node next to the ileocecum (2.7 cm in the longest axis), and suspected the presence of lymphoma (Figure A, arrow). The pathologist found chronic inflammation of mucosa with focal granulomas in the terminal ileum, which is not typical in Crohn’s disease or tuberculosis (Figure D). It is reported that fever and regional lymphadenopathy with inflammatory granuloma formation were suggestive of cat scratch disease. Then a history of cat exposure 1 month ago was confirmed. Warthin-Starry staining was subsequently performed and revealed the presence of small bacilli compatible with Bartonella infection (Figure E). She was treated with intravenous piperacillin-tazobactam and oral vancomycin and cured after 7 days. At the 6-month follow-up, her overall condition has improved significantly with a 20-kg weight gain, which further supported our diagnosis. Cat scratch disease is caused mainly by Bartonella henselae, and cats are the natural reservoir. Uncomplicated cat scratch disease is usually self-limited and characterized by lymphadenopathy, especially in the neck. The characteristic histologic change is an inflammatory granulomatous process with central microabscess surrounded by a ring of macrophages and rare giant cells.1Angelakis E. Raoult D. The abdominal lymphadenopathy presented hypermetabolic in positron emission tomography and computed tomography scans has been reported, which is easy to be misdiagnosed as lymphoma.3Imperiale A.
