Abstract:Editor, We read with interest the recent article by Drs Corbanese and Possamai1 regarding awake intubation with the Bonfils fibrescope in patients with a difficult airway. In managing difficult airways, we have also often used the Bonfils fibrescope to perform airway topical anaesthesia and awake orotracheal intubation.2,3 Based on our experience, we agree with their view that awake orotracheal intubation with the Bonfils fibrescope is highly successful, and the Bonfils fibrescope is one of the most promising devices to assist in the orotracheal intubation of patients with difficult airways. However, we suggest that several clinical aspects of this article should be clarified. First, in the abstract, the authors concluded that awake intubation with the Bonfils fibrescope was well tolerated by awake sedated patients. However, we found no evidence to support this conclusion in their article. This is concerning because it has been our experience that many readers do not read beyond the abstract of an article, and, if so, this could inadvertently mislead some readers. In only one failed intubation reported in this article, the second intubation attempt was actually abandoned because the patient complained of severe discomfort. We do not know how the discomfort levels of patients during awake intubation were assessed. Also, the authors did not describe the patients' reaction and coughing severity during awake intubation, and the intubating conditions.3–6 Second, a total of 30 patients were included in the study. In the results of the article, however, topical anaesthesia methods used in the larynx and trachea were described in only 19 patients (transcricothyroid membrane injection in nine patients and ‘spray as you go’ technique in 10 patients). Which techniques were used to anaesthetize the larynx and trachea in the remainder of the patients? It would perhaps be more informative to provide the difference in efficacy of airway topical anaesthesia between the transcricothyroid membrane injection and the ‘spray as you go’ technique. Third, when the ‘spray as you go’ technique was used to provide airway topical anaesthesia, 2% lidocaine was injected by means of the working channel of the Bonfils fibrescope in this study. However, this administration method is ineffective and sometimes difficult for several reasons: the stream of lidocaine injected by the large working channel (inner diameter = 1.2 mm) cannot be directed, with a great deal of accuracy, to the targeted sites, especially for the supraglottic and glottic areas; the large working channel can cause significant wastage of lidocaine (also, lidocaine injection via the working channel can often obscure vision with the fibrescope because lidocaine solution remains on the lens); and when the Bonfils fibrescope is positioned immediately in front of the glottis, it is sometimes difficult to achieve a precise intratracheal lidocaine spray by means of the working channel in awake patients because of the patient's head movement, swallowing, or glottic closure and coughing. When the Bonfils fibrescope is used to provide airway topical anaesthesia for awake intubation, therefore, we prefer to spray lidocaine solution into the targeted airway areas by means of an epidural catheter placed in the working channel of the Bonfils fibrescope.2,3,7 Fourth, during airway topical anaesthesia with the ‘spray as you go’ technique, an interval of 1–2 min between consecutive endoscopies was used in this study. Also, only one lidocaine spray was performed to anaesthetize the vallecula and epiglottis or the glottis and trachea. We feel that, as with other local anaesthesia methods, ‘tincture of time’ is one of the most useful supplements to airway topical anaesthesia. Inadequate airway topical anaesthesia due to slow onset of maximal effect often results in repeated drug use within a short time, leading to overdoses of lidocaine.4 Therefore, we recommend that repeated lidocaine sprays in each targeted area should be done to achieve satisfactory airway topical anaesthesia.2,3 Also, a 3–5-min waiting period after each lidocaine spray should be allowed to provide adequate penetration of local anaesthetic into the airway mucosa for maximal effect.2,4 Our experience suggests that a long interval between consecutive endoscopies can improve the patients' tolerance and acceptance of the airway procedure. We emphasize that when the fibreoptic technique is used to anaesthetize the airway by the ‘spray as you go’ technique, adequate anaesthesia of the larynx must be obtained by repeated lidocaine sprays before further injection of local anaesthetic through the glottis into the trachea.3,4 Otherwise, intratracheal injection of lidocaine under inadequate anaesthesia of the larynx is not only very difficult but can also cause severe cough, choking, or even laryngospasm. Finally, it must be pointed out that, unlike the flexible fibreoptic bronchoscope, the Bonfils fibrescope is not suitable for nasotracheal intubation. In patients with severe limited neck extension or mouth opening, or an anterior larynx, a small fixed anterior curvature of this device (40°) would make it difficult to negotiate the oral–tracheal angle and may decrease its efficacy for the intubation.8 Acknowledgement All the authors have no financial arrangement with the makers of the Bonfils fibrescope.
