Abstract:Background: Awake fiberoptic bronchoscope intubation (AFOBI) is the gold standard technique for the management of patients with difficult airways. Adequate sedation and analgesia are essential for successful AFOBI. The aim of this study was to evaluate the sedative and analgesic validity and administration routes of dexmedetomidine and fentanyl combined with ketamine in awake fiberoptic intubation. Methods: Patients undergoing head and neck surgery under general anesthesia with predicted difficult airways were included. Participants were randomly assigned to 6 different groups (n=6): groups 1-3 were intravenous (IV), while groups 4-6 were intranasal(IN) (group 1: dexmedetomidine (DEX) 1 μg/kg + fentanyl (FEN) 1 μg/kg; groups 2-3: DEX 1 μg/kg+ FEN 0.7 μg/kg + ketamine (KTM) 0.1/0.2 mg/kg; group 4: DEX 1.5 μg/kg + FEN 1.4 μg/kg; and groups 5-6: DEX 1 μg/kg + FEN 1 μg/kg + KTM 0.4/0.6 mg/kg). The visual analog scale (VAS) score during intubation, time required for the modified observer’s assessment of alertness/sedation scale (OAA/S) score to reach above 2 and for the bispectral index (BIS) to decrease to 60-80, motor activity assessment scale (MAAS) score, changes in vital signs and adverse effects were recorded. Results: Among the IV groups, the VAS score of group 1 (5.65±2.11) was higher than those of group 2 (1.89±2.16, P =0.012) and group 3 (1.15±0.98, P =0.001). Among the IN groups, the VAS score was lower in group 6 (0.86±1.27) than in group 4 (7.20±2.70, P <0.001) and group 5 (3.93±2.73, P =0.031). Participants in group 5 and group 6 were less likely to cough when intubated than those in group 4 ( P =0.002), while the differences among IV groups were not significant. There were no significant differences in the other endpoints. Conclusions: Our study indicates that the addition of subanesthetic doses of ketamine, either intravenous or intranasal, could reduce the fentanyl and dexmedetomidine consumption used in AFOBI and provide better sedative and analgesic effects. Comparison of Intravenous Dexmedetomidine-Propofol Versus Ketofol for Sedation During Awake Fiberoptic Intubation: A Prospective, Randomized Study

Abstract:Background: Management of difficult airway due to laryngeal mass is a major challenge to the anesthesiologists, and awake fiberoptic intubation (AFOI) would be the technique of choice. Objectives: The current study aimed at comparing the effects of administration of dexmedetomidine-propofol or ketofol for sedation during AFOI in terms of intubation conditions, hemodynamic stability, and patients and anesthesiologist's satisfaction. Methods: Eighty adult patients, 18 - 60 years old, ASA (the American Society of Anesthesiologists) physical status I-III, with difficult airway due to laryngeal mass and planned for AFOI were enrolled. Sedation was randomly given to the patients according to their assigned group by either dexmedetomidine-propofol (group D; n = 40) or ketofol (group K; n = 40). Outcome variables included time to reach Ramsay sedation scale (RSS) ≥ 3, intubation time, number of patients in need of rescue propofol, patient's discomfort score, tolerance to endoscopy and intubation, hemodynamic parameters, patients and anesthesiologist's satisfaction, and occurrence of side effects. Results: Time to reach RSS ≥ 3 and intubation time were significantly shorter, and fewer patients required rescue propofol in the K group as compared to the D group (P = 0.000*, and 0.035*, respectively). Higher discomfort score and better tolerance to endoscopy and intubation were noticed in group K than group D, but with no statistical significance (P = 0.132, 0.137, and 0.211, respectively). Patients in group D had significantly lower mean arterial pressure (MAP) and heart rate (HR) after the loading dose till five minutes after intubation (P = 0.000*). There was no significant difference in patients' satisfaction between the two groups (P = 0.687), while anesthesiologist's satisfaction was higher in group K compared with that of group D (P = 0.013*). Cough score as well as incidence of unfavorable respiratory outcomes were not significantly different between the two groups (P = 0.611, 0.348, respectively). Conclusions: Ketofol and dexmedetomidine-propofol combination were suitable and satisfactory for AFOI. However, ketofol was more advantageous in offering faster onset of sedation, shorter intubation time, stable hemodynamic profile, as well as higher anesthesiologist's satisfaction when compared to dexmedetomidine-propofol. Abstract:Background The purpose of this study was to compare the efficacy and safety of dexmedetomidine versus remifentanil for sedation during awake intubation using a Shikani optical stylet (SOS). Methods Sixty-eight patients with cervical trauma or severe cervical spondylosis undergoing cervical spinal surgery were enrolled in this prospective study. They were randomly assigned to receive dexmedetomidine (Group D) or remifentanil (Group R). In Group D, the patients received an intravenous loading dose of dexmedetomidine 1 μg · kg −1 over 10 min followed by a continuous infusion of 0.7 μg · kg −1 · h −1 . In Group R, a target-controlled infusion of remifentanil was administered to achieve a plasma concentration of 2.5 ng · ml −1 , increased to 3 ng · ml −1 10 min later. An endotracheal tube was inserted using a SOS under dexmedetomidine or remifentanil sedation after topical anesthesia to the airway. Midazolam was given as rescue sedation. We recorded the first attempt intubation success rate, the dose of midazolam, duration of intubation, Ramsay Sedation Scale (RSS) score, tracheal tube tolerance score, duration of drug infusion, adverse events and patient satisfaction score. Results The RSS score was significantly higher in Group D than in Group R. First attempt success rate, rescue midazolam dose and the duration of intubation did not differ between the groups. Patients in Group R were significantly more tolerant of the tracheal tube. The incidence of hypoxia was significantly higher in Group R than Group D, but there was no significant difference in the incidence of other adverse events between the groups. The hemodynamic responses of the two groups were similar, but more patients in Group R were able to recall airway instrumentation. Conclusions Both dexmedetomidine and remifentanil are effective sedatives for awake intubation using an SOS. Although the first attempt success rates were similar, patients sedated with remifentanil tolerated the tracheal tube better after intubation with moderately increased risk of desaturation. Abstract:Dexmedetomidine has been widely used in the intensive care unit (ICU), with the primary aim to keep patients on an appropriate level of sedation. Both observational and randomized controlled trials have observed that the use of dexmedetomidine is associated with improved outcomes for mechanically ventilated patients [Citation1]. In ICU patients receiving prolonged mechanical ventilation, dexmedetomidine was not inferior to other sedatives in maintaining sedation level, but was associated with shortened MV duration and improved ability to communicate pain [Citation2]. MV is an important factor for delirium and dexmedetomidine was found to be associated with lower risk of delirium [Citation3, Citation4]. Prophylactic low-dose dexmedetomidine is able to reduce the occurrence of delirium during the first 7 days after surgery for patients aged over 65 years who are admitted to the ICU after surgery [Citation4]. Thus, the beneficial effect of might be explained by the reduction of delirium in the treated group. In fact, delirium can be considered as a type of acute organ dysfunction mediated via inflammatory response. There has been evidence that inflammatory biomarkers such as C-reactive protein was positively correlated with the occurrence of delirium [Citation5].

Abstract:BACKGROUND: Severe mouth opening difficulty may increase the risk of airway management during anesthesia induction, and awake fiberoptic nasotracheal intubation (AFNI) is the first option for patients with orofacial anatomical changes.CASE SUMMARY: A 54-year-old man was scheduled to undergo wedge resection of the right upper lung in August 2021. The patient had a history of enlarged right maxillary lesion resection and partial right maxillary resection surgery in April 2020, which led to orofacial anatomical changes and severe mouth opening difficulty. To avoid difficult airway-related emergency scenarios, the AFNI was successfully performed through intravenous injection of sufentanil and dexmedetomidine combined with lidocaine topical anesthesia under a conscious state without any uncomfortable feeling or complications.CONCLUSIONS: Intravenous injection of sufentanil and dexmedetomidine combined with lidocaine topical anesthesia can be used as an alternative medication scheme to relieve uncomfortable suffering for AFNI in patients with severe mouth opening difficulty.

Evaluation of different doses of dexmedetomidine for awake fibreoptic nasotracheal intubation in patients undergoing oromaxillofacial and oral malignancy surgeries: A randomised, double-blind study

Abstract:Background and aims: In patients undergoing cancer surgeries with anticipated difficult airway, awake fibreoptic nasotracheal intubation (AFONI) is critical for securing the airway. However, different doses of dexmedetomidine (DEX) are yet to be evaluated in these patients. Thus, we compared three doses of DEX for AFONI in patients undergoing oromaxillofacial and oral malignancy surgeries. Methods: In this randomised, double-blind study, 90 patients aged 18-60 years of either gender, with American Society of Anesthesiologists physical status I/II, and undergoing elective oromaxillofacial and oral malignancy surgeries were randomised to three groups: Group D1 (0.5 µg/kg DEX), Group D2 (1 µg/kg DEX), and Group D3 (1.5 µg/kg DEX). The primary outcome measure was the airway obstruction score. Secondary outcome measures were intubation scores (including vocal movement, coughing, and limb movements) and a 5-point fibreoptic intubation comfort score. Sedation was assessed using the Ramsay sedation score (RSS). One-way ANOVA and Chi-square test were used to assess the association between quantitative and qualitative variables, respectively. A P value of <0.05 was considered statistically significant. Results: The airway obstruction score was comparable between the groups (P = 0.78). Similarly, vocal movement (P = 0.15), coughing (P = 0.31), limb movement (P = 0.51), and 5-point fibreoptic intubation comfort score (P = 0.49) did not differ between the groups. The mean RSS was significantly greater in Group D3 than in Groups D1 and D2 (P = 0.001). Conclusions: In combination with topical spray and airway block, all three doses of DEX resulted in comparable airway obstruction scores and thus provided favourable conditions for AFONI.

Effects of Dexmedetomidine on the Electrocorticography Monitoring in Patients Undergoing Epilepsy Surgery

Abstract:Objective To investigate the effects of dexmedetomidine on the electrocorticography ( ECoG ) monitoring in the different dose.Methods Sixty patients with epilepsy , were randomly divided into four groups , n=15for each group.A loading dose of 1 μ g / kg dexmedetomidine was slowly injected in group D2 , D4 , D6.Then the maintenance dose of dexmedetomidine was injected with speed of 0.2 μ g · kg -1 · h -1 in group D2 , 0.4 μ g · kg -1 · h -1 in group D4 , 0.6 μ g · kg -1 · h -1 in group D6 respectively.The equal volume of physiological saline was injected for group C.The anesthesia was maintained with sevoflurane and remifentanil. Time from drug discontinuance to definitely electrocorticographical location , the amplitude , classification and frequency of electrocorticography waveform in operation were recorded.Results Compared with group C , the time of electrocorticography location in group D6increased ( P0.05 ) .Classification of electrocorticography in groups D2and D4were very similar with group C.Compared with group C , the incidence of severe abnormality in group D6significantly decreased ( P0.05 ) .The spike wave showed in all of four groups during electrocorticography recording.Compared with group C , the frequency of spike wave decreased in groups D2 , D4and D6 , but there was no significant difference.There was no significantly difference with the amplitude of spike wave in four groups.Conclusion Dexmedetomidine , injected with 0.2-0.4 μ g · kg -1 · h -1 , is eligible during anesthesia in epilepsy surgery , which neither evokes nor inhibits spike wave during electrocorticography monitoring.

Abstract:BACKGROUND: Non-intubated video-assisted thoracoscopic surgery combines a minimally invasive technique with multimodal locoregional analgesia to enhance recovery. The mainstay sedation protocol involves propofol and fentanyl. Dexmedetomidine, given its opioid-sparing effect with minimal respiratory depression, facilitates sedation in non-intubated patients. This study aimed to evaluate the efficacy of dexmedetomidine during non-intubated video-assisted thoracoscopic surgery.METHODS: A total of 114 patients who underwent non-intubated video-assisted thoracoscopic surgery between June 2015 and September 2017 were retrospectively evaluated. Of these, 34 were maintained with dexmedetomidine, propofol, and fentanyl, and 80 were maintained with propofol and fentanyl. After a 1:1 propensity score-matched analysis incorporating sex, body mass index, American Society of Anesthesiologists classification, pulmonary disease and hypertension, the clinical outcomes of 34 pairs of patients were assessed.RESULTS: The dexmedetomidine group showed a significantly lower opioid consumption [10.3 (5.7-15.1) vs. 18.8 (10.0-31.0) mg, median (interquartile range); P = 0.001] on postoperative day 0 and a significantly shorter postoperative length of stay [3 (2-4) vs. 4 (3-5) days, median (interquartile range), P = 0.006] than the control group. During operation, the proportion of vasopressor administration was significantly higher in the dexmedetomidine group [18 (53) vs. 7 (21), patient number (%), P = 0.01]. On the other hand, the difference of the hypotension and bradycardia incidence, short-term morbidity and mortality rates between each group were nonsignificant.CONCLUSIONS: Adding adjuvant dexmedetomidine to propofol and fentanyl is safe and feasible for non-intubated video-assisted thoracoscopic surgery. With its opioid-sparing effect and shorter postoperative length of stay, dexmedetomidine may enhance recovery after surgery.

Dexmedetomidine for Sedation During Intubation Period in Postoperative Patients Receiving Orthognathic Surgery in Intensive Care Unit

Abstract:AIM To study the effectiveness and safety of dexmedetomidine for sedation during intubation period in postoperative patients receiving orthognathic surgery admitted to intensive care unit(ICU).METHODS Forty post-operative patients undergoing orthognathic surgery with tracheal intubation in ICU were enrolled and randomized into 2 groups to receive either midazolam or dexmedetomidine.The dexmedetomidine group starting dose was 0.4μg·kg~(-1)·h~(-1) without a loading dose and adjusted 0.2 to 0.7μg·kg~(-1)·h~(-1).The midazolam group starting dose was 0.1 mg·kg~(-1)·h~(-1) and adjusted 0.05 to 0.2 mg·kg~(-1)·h~(-1),the dose of sedation was regulated by Ramsay Sedation Score(RSS) maintain 2 to 4 sedative score.During the course,RSS,heart rate(HR), blood pressure,respiratory rate(RR) and pulse oxygen saturation(SpO_2) were observed and registered continuously.The amount of the drug,and incidence of adverse reactions,such as hypotension,bradycardia, delirium,etc.were recorded in two groups.RESULTS The expected sedative scores were obtained in all patients in two groups.The HR and mean arterial blood pressures(MAP) showed no significantly different between two groups before sedation(P0.05).The HR in the dexmedetomidine group at 1,2,3,4,6,8, 12 and 16 h were lower compared with those in the midazolam group(P0.05).The MAP in the dexmedetomidine group at 1,2,3,and 4 h were lower than those in the midazolam group(P0.05).The times of dose adjustment needed were significantly lower in the dexmedetomidine group(2 patients with 1 adjustment each) than those in the midazolam group(3 patients with 1 adjustment,4 patients with 2 adjustments).Atropine was administered to 2 patients in the dexmedetomidine group because of bradycardia.No serious adverse reactions occurred in both groups.CONCLUSION Dexmedetomidine 0.4μg·kg~(-1)·h~(-1) is effective sedatives for post-operative patients undergoing orthognathic surgery with tracheal intubation in ICU. Dexmedetomidine required fewer adjustments in dosing compared with midazolam to maintain adequate sedation.

Abstract:Dexmedetomidine is a new intravenous drug gaining popularity in neuroanesthesia and neurocritical care practice. This alpha2-adrenergic receptor agonist offers a unique "cooperative sedation," anxiolysis, and analgesia with no respiratory depression. Cerebral effects are generally consistent with a desirable neurophysiological profile, including neuroprotective characteristics. In addition, sympatholytic and antinociceptive properties allow for hemodynamic stability at critical moments of neurosurgical stimulation. This review will address the neuropharmacology and neurophysiology of alpha2-adrenergic agonists and will specifically consider the rapidly evolving applicability of dexmedetomidine as an adjuvant to neurosurgical case management.

Clinical application of oxycodone combined with dexmedetomidine in awake tracheal intubation

Abstract:Objective To observe the analgesic and sedative effects of oxycodone combined with dexmedetomidine and its effects on respiratory and cardiovascular functions during endotracheal intubation guided by fiberoptic bronchoscope at conscious state.Methods Forty patients with endotra-cheal intubation difficulty were randomly divided into sulfentanil group and oxycodone group.An initial induction dose of 1 μg/kg dexmedetomidine was given followed by an maintaining infusion of 0.2 μg·kg-1 ·h-1 in sulfentanil group,additionally,0.2 μg/kg sulfentanil was given intravenously. Patients in oxycodone group received intravenous infusion of oxycodone 0.08 mg/kg instead.The changes of HR,MAP and SpO 2 were monitored during medication and intubation.The Ramsay scores before intubation, the PET CO 2 and comfort degree after intubation were recorded. Results There were no statistically significant differences in hemodynamic changes,Ramsay scores and comfort level between the two groups during drug-injection and intubation.SpO 2 of patients in oxycodone group was higher than that of sulfentanil group after injection of drugs,but PET CO 2 of oxycodone group after endotracheal intubation was lower.Conclusion The combination of oxycodone and dexmedetomidine can be safely used in awake tracheal intubation,for it shows effective sedation and analgesia,less respiratory depression and other side-effects compared with sufentanil.
Application of dexmedetomidine in difficult airway patients with blind tracheal intubation

Abstract:Objective To assess the value of dexmedetomidine in difficult airway patients receiving awake nasotracheal intubation with blind intubation device under topical anesthesia.Methods Fifty patients with difficult airway from oral maxillofacial department were randomly divided into two groups (n=25).The patients in dexmedetomidine group received intravenous injection of 1.0 μg/kg dexmedetomidine for 10 mins at 15 mins before intubation (T0).The patients in midazolam group received intravenous injection of 0.02 mg/kg midazolam and 2 μ/kg fentanyl at 5 mins before intubation (t0).Awake nasotracheal intubation was applied with blind intubation device.The success rates of intubation,intubation time,and heart rate (HR),mean arterial pressure (MAP),saturation pulse oxygen (SpO2),respiratory rate (RR) and observer's assessment of alertness/sedation (OAA/S) score at the time point of T0 or t0,T1 (immediately after inserting lightwand) and T2 (immediately after inserting the tracheal tube guided by lightwand) were observed.Results There were no significant differences in age,gender,body mass inder (BMI),success rates of intubation or intubation time between the two groups (all P>0.05).HR at T1 and T2 in dexmedetomidine group were significantly lower than those in midazolam group (all P<0.01).Compared with those in midazolam group at T1,RR was significantly increased and OAA/S score was significantly decreased in dexmedetomidine group (P<0.05).There was no significant difference of MAP at each time point between the two groups (P>0.05).SpO2 maintained at 95% to 100% during intubation in both two groups.Conclusion Under the induction of dexmedetomidine,stable hemodynamics and ideal sedation can be achieved in patients receiving awake nasotracheal intubation with blind tracheal device.(Shanghai Med J,2012,35:994-997)

Abstract:Background: Awake fiberoptic intubation is the standard of care for difficult airway management. Quality and success of this technique depend on the experience of the intubating physician and the proper preparation of the patient. The aim of this study was to compare remifentanil (R) as single agent to the combination of fentanyl (F) and midazolam (M), which have been the drugs for analgesia and sedation for this procedure. Methods: Seventy-four adult patients requiring nasotracheal intubation were randomly assigned to one of two groups. In group I, (n=37) R was administered in incremental dosages (0.1-0.25-0.5 microg/kg/min) by an infusion pump according to comfort, level of sedation and respiratory depression. In group II, (n=37) analgesia and sedation was achieved by F 1.5 microg/kg and doses of between 1 and 10 mg M, titrated to the individual needs. Patient reactions like grimacing, movement and coughing during intubation were assessed, as well as patient recall of the procedure. Haemodynamic and respiratory parameters were continuously recorded. Results: Group I patients better tolerated nasal tube passage (P<0.001) and laryngeal tube advancement (P<0.001) than group II. Remifentanil better suppressed hemodynamic response to nasal intubation (P<0.001). No significant difference in respiratory data was recorded. In group I more recall of the procedure was observed (six vs. zero patients, P<0.05). Conclusion: Remifentanil in high doses, as the single agent for patient preparation for awake fiberoptic intubation seems to improve intubating conditions, quality and reliability of the procedure. However, a higher incidence of recall is to be expected.
Effect of Dexmedetomidine on the Postoperative Cognitive Function of Patients Undergoing Radical Resection of Esophageal Cancer

Abstract:Objective: To investigate the effect of dexmedetomidine on the postoperative cognitive function of patients undergoing radical resection of esophageal cancer. Methods: A total of 76 patients undergoing radical resection of esophageal cancer were randomly and equally divided into observation and control groups. The patients in the two groups were then treated with continuous intravenous pumping of fentanyl to ease their pain. In the control group, the patients were initially treated with 0.05 mg/kg and then with 0.02-0.08 mg/kg.h(-1) rhythim to remain anesthetized. In the observation group, the patients were intravenously infused with 1.0 mu g/kg dexmedetomidine and then with 0.2-0.7 mu g/kg.h(-1) to retain their anesthetized conditions. The sedative effect and incidence of adverse reactions in these patients were compared. Results: The sedative effects did not significantly differ between the two groups (P>0.05). The recovery time after the drug was withdrawn from the patients in the observation group was significantly shorter than that in the control group (P<0.05). The incidence of adverse reactions in the observation group was 13.16%, which was significantly lower than that in the control group (39.47%; P<0.05). The postoperative SDS and SAS scores of the patients in the two groups were significantly higher than their preoperative scores (P<0.05). The postoperative SDS and SAS scores of the patients in the observation group were also significantly higher than those in the control group (P<0.05). Conclusion: The sedative effect of dexmedetomidine on patients subjected to general anesthesia and undergoing radical resection of esophageal cancer was good. Dexmedetomidine could improve the cognitive function of patients and reduce the incidence of adverse reactions. Therefore, this drug should be recommended for these patients.

Abstract:Objective To study the effect of dexmedetodine during extubation period afer general anesthesia.Methods Sixty ASA Ⅱ-Ⅲ patients undergoing elective surgery were randomly assigned to receive either intravenous infusion of 0.5 mg/kg dexmedetodine(group D) or intravenous of the same amount of normal saline(group N) at 30 minutes before the end of operation.Systolic blood pressure(SBP)、 diastolic blood pressure(DBP)、 heart rate(HR) and pulse oxygen saturation(SPO2) were recorded before infusion、 before extubation、during extubation and 5 and 10 minutes after extubation,and also time of stating breathing 、time of eyes opening、 time of extubation and the number of patients with restlessness.Results Compare to group N,SBP 、DBP and HR before and after extubation in group D had not obviously changed,P>0.05,there were no statistically significant differences between two groups.The number of restlessness in group D obviously less than group N,P<0.05,there were statistically significant differences between two groups.There were no statistically significant differences in time of starting breathing、time of eyes opening and time of extubation between two groups.Conclusion Dexmedetomidine given anesthesia,extubation have a significant effect,stable hemodynamics,reduce the incidence of restless anesthesia cupping of Security,the ideal drug.

Abstract:Purpose: To determine whether dexmedetomidine sedation in the postanesthesia care unit (PACU) could decrease agitation and delirium after free flap surgery.Materials and Methods: Eighty patients were randomly divided into 2 groups. In the experimental group, dexmedetomidine was given at an hourly infusion rate of 0.5 mu g/kg for 1 hour before the operation was completed and continued in the PACU at 0.2 to 0.7 mu g/kg continuously until the next morning. In the control group, normal saline was given during the same periods. Patients in the 2 groups received sufentanil and midazolam for sedation and pain relief when necessary. Agitation was monitored with the Riker Sedation-Agitation Scale in the PACU and delirium was monitored with the Confusion Assessment Method for the Intensive Care Unit for 5 days postoperatively.Results: The overall incidence of agitation was similar between the 2 groups. However, when the influence of patient shiftingwas excluded, the incidence of agitation in the dexmedetomidine group was apparently lower than that in the control group (10.3 vs 30%; P =.029). No difference was found in the occurrence of delirium between the experimental and control groups (5.1 vs 12.5%; P =.432).Conclusion: Dexmedetomidine does not change the overall incidence of agitation after free flap surgery; however, it does decrease agitation after PACU admission. Abstract:Objective:To determine the impact of Dexmedetomidine on sedative effects of Fentanyl for percutaneous tracheotomy in laryngeal carcinoma.Methods:Between April 2010 and October 2012, a total of 42 patients with laryngeal carcinoma received percutaneous tracheotomy before surgery. By random table method, the patients were randomly divided into the study group ( Dexmedetomidine plus Fentanyl ) and the control group ( Fentanyl) , with 21 patients in each group. Before the procedure ( T0 ) , at completion of drug injection ( T1 ) , after tracheal dilatation and placement of the cannula ( T2 ) , and immediately after the procedure ( T3 ) , the patients were recorded for mean arterial pressure (MAP), heart rate (HR), oxygen saturation (SpO2), and respiratory rate (RR). The observer′s assessment of alertness/sedation (OAA/S) scores at T0 and T1 were monitored. Adverse events during tracheotomy were also recorded.Results:The OAA / S scores at T0 and T1 were 5 and (4.11±0.47) in the control group versus 5 and (4.23±0.53) in the study group, with no significant difference between the two groups ( P>0.05) . In either group, the OAA/S score at T1 was significantly lower than the score at T0(P<0.05). In the study group, the values of MAP, HR, SpO2, and RR at T1, T2, T3 were statistically comparable to those at T0(all P>0.05). At T2 and T3, the values of MAP, HR and RR in the study group were significantly lowered than those in the control group (all P<0.05) However, at any time point, the SpO2 was significantly comparable between the study group and the control group (P>0.05).Conclusion:Use of dexmedetomidine in combination with low-dose fentanyl during percutaneous tracheotomy may reduce agitation and coughing in the patients but is not associated with suppressions in breathing and circulation.

Abstract:Purpose: To compare the safety and efficacy of sedation induced by dexmedetomidine and propofol after oral and maxillofacial surgery.Materials and Methods: In this trial, 66 patients 18 to 50 years old received oral and maxillofacial surgery and required postoperative nasal endotracheal intubation under overnight sedation with dexmedetomidine or propofol. The dexmedetomidine group (group D) received dexmedetomidine 1.0 mu g/kg intravenously for 10 minutes after entering the recovery room. The dose was maintained by giving an intravenous injection of dexmedetomidine 0.4 mg/kg. The injection rate could be modulated from 0.2 to 0.7 mu g/kg/hour in the intensive care unit (ICU). If the Ramsay score was lower than 2 and involuntary limb movement occurred, other sedatives were used. The propofol group(group P) was given propofol 0.1mg/kg intravenously for 10 minutes after entering the recovery room and then maintained with intravenous injections of propofol 1 to 2 mg/kg/hour in the recovery room and ICU.Results: The oxygen desaturation of group P was higher than that of group D (at the first sedation time of 30 minutes), but the mean blood pressure of group P was significantly lower than that of group D at the 10-minute time point. The Ramsay score was higher in group D after the first 3 hours of sedation than in group P (P <.05).Conclusion: Dexmedetomidine showed similar safety and efficacy as propofol and could be used for tube-retaining sedation after oral and maxillofacial surgery.
