Abstract:BACKGROUND:Microsurgical Transcranial approach (mTCA) is the primary choice for the resection of giant Tuberculum Sellae Meningiomas (TSM). The objective of this study is to explore surgical details of unilateral subfrontal approach.METHODS:Ten patients with giant TSM treated by unilateral subfrontal approach were included from January 2018 to June 2021. Demographic characteristics, surgical data, post-procedure complications and outcomes of patients have been descriptive analyzed, combined with systematic literature review to explore the surgical details and the prognosis of unilateral subfrontal approach.RESULTS:Ten patients include six male and four females, age range from 35 to 77 years, duration of visual impairment from 1 to 12 months, were all performed unilateral subfrontal approach. Nine patients achieved radical resection (Simpson grades I-II) through post-operative imaging confirmation, and Simpson IV resection was performed in the remaining one due to cavernous sinus invasion. The postoperative visual acuity was improved or maintained in 8 patients. Visual acuity decreased in 2 cases, including 1 case of optic nerve atrophy and the other case of optic canal not opening. Five cases with frontal sinus opened were repaired during the operation and there was no postoperative cerebrospinal fluid leakage or intracranial infection. One patient suffered from postoperative anosmia, one patient developed left limb weakness, but their symptoms have improved in the follow-up.CONCLUSION:Summarize the experience of our center and previous literature, unilateral forehead bottom craniotomy is a feasible surgical approach for giant tuberculum sellae meningioma. Intraoperative application of EC glue and pedicled fascia flap to repair the frontal sinus can prevent complications associated with frontal sinus opening. Optic canal unroofing has huge advantage in visual improvement.
Microsurgical Treatment for Para-Superior Sagittal Sinus Meningiomas in Middle Cortex Area

Abstract:Objective To investigate the operative technique for the para-superior sagittal sinus meningiomas in the middle cortex areas and its curative effect.Methods The clinical data of 28 patients with para-superior sagittal sinus meningiomas in the middle cortex areas,who underwent surgery in our hospital from July 2000 to December 2005,were analyzed retrospectively. The clinical data included imaging data,the operative approach,and microsurgery method.Results According to Simpson's classification of glioma resection,of the 28 patients,17 received Grade Ⅰ resection of the tumors(60.7%),6 received Grade Ⅱresection(21.4%),3 received GradeⅢ resection(10.8%),and 2 received Grade Ⅳ resection(7.1%).No patients died from the operation.The following-up from 6 to 24 months showed that the meningiomas recurred in 4 patients.Conclusion The para-superior sagittal sinus meningiomas in the middle cortex areas can be totally resected by using microsurgical techniques,the measures including sufficient preoperative imaging evaluation,good protection of the vein of central sulcus,excellent management of superior sagittal sinus,and avoiding damage to functional cerebral cortex during the operations.It can significantly increase the percentage of total removal of the tumors,decrease postoperative complications,and improve survival outcome.

Abstract:Objective:To explore the microsurgical technique for the parasagittal meningiomas in the middle and posterior 1/3 of sagittal sinus and its operative outcome.Methods:The imaging data of 36 patients with parasagittal meningiomas in the middle and posterior 1/3 of sagittal sinus microsurgical operative technique and appropriate treatment of the involved sagittal sinus were analyzed retrospectively.Results:According to Simpson classification of glioma reopertion,of the 36 patients 29 received grade Ⅰ resection of the tumors,6 grade Ⅱ.No patients died from the operation.No tumor recurrence was found in 28 patients during the following-up from 7 to 36 months.Conclusion:Using microsurgical techniques in the resection of parasagittal meningioma in the middle and posterior 1/3 of sagittal sinus may improve the percentage of tital tumor resection,decrease the injures of important functional area of the brain,reduce complications and improve survival outcome.

Abstract:Objective:To investigate characters of microsurgical resection of meningioma of lateral ventricle.Materials and Methods:Eight patients of this entity received operations via transcortical sulcus approaches and retrospective analysis of their clinical,radiological characters and operative procedures was made.Results:The patients' average age was 64 years old and all had no significant symptoms and objective signs of high intracranial pressure.Their major clinical presentations were chronic headache and local neurological deficits.Operation via transtemporal sulcus approach was performed in 5 patients and the transparietooccipital sulcus approach was performed in 3 patients.The size of tumors was no more than 3.8cm.Tumors of these 8 cases were resected completely with no epactal speech disturbance,hemiamaurosis and amplified defect of visual field.There was no death in these 8 patients.Conclusions:Operation via transcortical sulcus approach was suitable for patients who suffered from meningioma of lateral ventricle in old age,smaller tumor and without higher intracranial pressure.

Abstract:Objective To discuss the efficacy of microsurgery in meningioma around central gyrus region. Methods Seventy-one patients with parasagittal and parafalcine meningioma around central gyrus region were treated with microsurgery technique. Special attention was paid on the vein of central sulcus and other compensative venous pathways to protect them from being damaged; the affected sagittal sinuses were duly handled. Results Simpson grade Ⅰ resection was achieved in 45 patients (63.4%) ,grade Ⅱ in 17 (23.9%), near total resection in 9 (12.7%), and hemiparalysis happening in 5patients, and no patient died. Fourty-two patients was followed up for 1 to 5 years without recurrence.Conclusion choosing the strategy of preserving the function of central gyrus region and improving the microsurgical technique in treating meningioma around central gyrus region can decrease the injuries to significancant domain of the brain.

Abstract:Background: Falcotentorial meningiomas (FM) are surgical challenges for protecting sinus, and the technique notes on the management of superior sagittal or transverse sinus are required for good results. Methods: We improved the technique notes on the management of superior sagittal or transverse sinus in three FM patients with signs of increased intracranial pressure or chronic headache. Results: All patients underwent surgeries in the prone position, and occipital/sup-occipital/sub-occipital craniotomy was performed. In one patient, the skull was removed traditionally with exposure of the confluence of sinuses, superior sagittal, and transverse sinus, while the longitudinal skull bridge was left to suspend the dura for protecting the superior sagittal sinus in one patient, and the transverse skull bridge was left to suspend the dura for protecting the transverse sinus in one patient. The dura was opened infratentorially or supratentorially to spare the sinus and then the "skull bridge" was suspended. The tumor was then removed completely without brain swelling or significant venous bleeding. Complete tumor resection was confirmed by early postoperative imaging, and all patients recovered well without postoperative morbidity. Conclusion: The authors recommend the "skull bridge" to suspend the dura for optimal control of the venous sinuses during FM surgery (less venous bleeding).

Abstract:Objective:To study the curative effect of microsurgery in treatment of para-meningiomas of sagittal sinus.Methods:150 patients with meningioma of the sagittal sinus who were treated in our hospital from July 2014 to June 2015 were selected and randomly divided into the observation group and the control group,with 75 cases in each group.The patients in the observation group were treated with microsurgery,and the patients in the control group were treated with conventional eraniotomy.Then the operation indexes,surgical resection rate,clinical curative effect and complication rate were observed and compared between the two groups before and after the treatment.Results:The amount of blood transfusion and blood loss in the observation group were significantly lower than those of the control group (P＜0.05).There was no significant difference in the operation time,the improvement rate,the morbidity rate and death rate between the two groups (P＞ 0.05).The incidence of complications in the observation group was significantly lower than that of the control group (P＜0.05).Conclusion:Microsurgical treatment of meningiomas near sagittal sinus has less blood transfusion and hemorrhage,with better curative effect and lower incidence of complications,which is worthy of clinical application.
Microsurgical Strategies of Medial Sphenoid Ridge Meningiomas

Abstract:Objective To analyze and explore the microsurgical approaches,microsurgical techniques and prevention of postoperative complications.Methods The clinical data,microsurgical course,curative effects and prognosis of 37 cases with medial sphenoid ridge meningioma admitted to our department from 2000 to 2006,were retrospectively reviewed and analyzed.Results All cases accepted microsurgery.The operative results were evaluated with Simpson grade system.Total removal was achieved in 32 cases(grade Ⅰ,Ⅱ) with 14 cases in grade Ⅰ,18 cases in grade Ⅱ;grade Ⅲ removal was achieved in 3 cases and grade Ⅳ in 2 cases.No patient died of operation.Postoperative visual disturbance occurred in 3 cases and postoperative hemiplegia in 2 cases.One year after operation,MRI scan showed no recurrence in groups of grade I and grade Ⅱ resection,2 cases of recurrence in group of grade Ⅲ resection and all patients presented recurrence in group of grade Ⅳ resection.Conclusion Choice of suitable microsurgical techniques,careful identification of the arachnoid intervals between tumor and important adjacent structures and total resection with protection of the important adjacent structures may improve the therapeutic efficacy and reduce the recurrent rate.Postoperative radiotherapy can postpone the recurrence in incompletely resected cases.

Abstract:Objective To explore the method and therapeutic effectiveness of microsurgery for clinoidal meningiomas.Methods The clinical data of 26 patients with clinoidal meningiomas treated from 1999-2006 were analyzed retrospectively.There were 8 men and 18 women.The patients ranged in age from 21 to 66 years(mean 47.6 years).All patients underwent surgery through the pterional or extended pterional approach.Microvascular Doppler probe was used to guide dissection of the tumor from internal carotid artery(ICA)and its branching arteries.Results Resection were achieved in 6 cases(23.1%)in Simpson grade IV,in 4 cases(15.4%)in Simpson gradeⅢ,in 16 cases(61.5%)in Simpson gradeⅡ. No patients died.Conclusions The microsurgery can be a great help in total resection of the tumors,and reduce the postoperative complications and mortality.Combined operations with postoperative radiotherapy should be carried out for treating the tumor remained apparently invading tissues such as the cavernous sinus or ICA.

Abstract:Objective Meningiomas in the trigone of the lateral ventricle are characterized by deep location and low inci-dence. A few studies have been done on its treatment at home and abroad. This study was to explore the access,techniques,and clini-cal effect of microsurgery for lateral ventricular trigone meningiomas (LVTM). Methods We retrospectively analyzed the clinical data about 36 cases of LVTM treated by microsurgery in our hospital from December 2011 to December 2015. The operation involved lumbar cistern tube drainage, intraoperative drainage of cerebrospinal fluid, a unilateral parieto-occipital U-shaped cut, approach through the interparietal fissure,a sagittal incision about 3 cm long at the interparietal fissure for exposure and resection of the tumor. We followed up the patients for a mean of 17 months postoperatively and analyzed the results and complications. Results Simpson grade I removal of the tumors was achieved in all the 36 cases. Postoperative complications included homonymous hemianopia in 4 cases, central nervous system infections in 3,secondary epilepsy in 2,subcutaneous hydrops in 2,and intratumoral hemorrhage in 1 (which necessitated a second operation). Extended temporal horn of the right lateral ventricle occurred at 32 months after surgery,which was treated by fistulation. Homonymous hemianopia was improved in 2 of the 4 ca-ses. All the patients were capable of daily life activities and none experienced recurrence. Conclusion Sufficient preoperative evalu-ation of the tumor characteristics,rational selection of surgical approach,and expert operation techniques are the key factors for the mi-crosurgical treatment of meningiomas in the trigone of the lateral ventricle.
Clinical analysis of surgical treatment in meningiomas located in middle 1/3 superior saggital sinus with nearly total occlusion

Abstract:AIM:To analyze the surgical treatment of meningiomas located in the middle 1/3 superior saggital sinus(SSS) with nearly total occlusion and to discuss the strategy of preventing postoperative complications.METHODS:The clinical data of 10 patients with meningiomas located in the middle 1/3 SSS with nearly total occlusion were analyzed retrospectively.All the patients underwent preoperative CT angiography(CTA)/MR venography(MRV) or digital subtraction angiography(DSA) examination.Tumors were resected by microsurgery and the sagittal sinus and falx were properly managed intraoperatively.Routine treatment was given after surgical resection.RESULTS:Six cases achieved Simpson grade I resection and 4 cases achieved Simpson grade Ⅱ resection.Eight patients awoke postoperatively within 24 h.Consciousness disorder occurred in 3 of the 8 patients on the 2 nd day and CT scan revealed acute epidural hematoma in non-operative location.Two cases were in persistent coma and CT scan revealed acute diffuse brain swelling.Epidural hematoma was removed by a second microsurgery and the patients recovered well.Bain swelling was treated with dehydration strategy and patients awoke.No tumors recurrence was found during the follow-up averaging 21.5 months.CONCLUSION:Careful analysis of the imaging data preoperatively,resection of tumors by microsurgical techniques and proper intraoperative management of central sulcus vein and superior saggital sinus are of vital importance for the improvement of the prognosis and the prevention of postoperation complications in patients with meningiomas located in the middle 1/3 SSS with nearly total occlusion.
Microsurgical Treatment of Meningiomas--attached 58 Cases of Report

Abstract:BACKGROUND?The operative approaches and microsurgical effects of meningioma were reviewed and it was studied how to rise the total removal rate, reduce operative mortality and improve operative effects.?CASE REPORT?58 cases of meningioma operated from November, 1997 to September, 2001 were retrospectively analyzed. The microsurgical treatment was performed for meningioma by using 6 kind of approaches, including the pteronal approach in 4 cases, the frontal approach in 2 cases, the approach of stride over agittal sinus in 24 cases, the suboccipital retromastoid approach in 5 cases, the suboccipital central approach in 2 cases and the approach depending on the locations of tumors in 21 cases. 61 of meningioma were excised removed in 58 cases. Total excision documented by postoperative CT scan was in 52 cases (90%), subtotal resections in 2(3%) and most of resections in 4(7%), and good postoperative effects were got in 43(74%), new cranial nerve deficits occurred in 4(8%) and operative death was in 2(4%).?DISCUSSION?It could improve total resections rates of meningioma and decrease the mortality rates that the measurable operative approach chosen according to the sizes and the locations of the tumor, acquaintance of the relation of neurotomy and skilled operative technique.
Microsurgical treatment of large and giant parasagittal meningioma invading the superior sagittal sinus

Abstract:Objective:To investigate the method and effect of microsurgical removal of large and giant parasagittal meningioma invading the superior sagittal sinus.Methods:The clinical data of 154 cases with large and giant parasagittal meningioma invading the superior sagittal sinus admitted to Department of Neurosurgery, Peking University Third Hospital from May 2009 to May 2019 were retrospectively analyzed. The diameter of the tumors ranged from 4 cm to 8 cm, with an average of 5.5±1.2 cm. Microsurgical resection was carried out along the midline and longitudinal fissure with a horse-shoe shaped skin incision turning to the temporal side. The degree of resection was evaluated by Simpson grading system. The results of clinical follow-up were evaluated by Karnofsky performance score (KPS), and the tumor recurrence was evaluated by imaging follow-up.Results:Among the 154 cases, Simpson grade Ⅰ resection was achieved in 112 (72.7%) and grade Ⅱ resection in 42 cases (27.3%). All of them were proved to be meningiomas (World Health Organization grade Ⅰ). The operation time ranged from 30 minutes to 210 minutes (median: 60 minutes), and the length of stay ranged from 5 days to 10 days with an average of 7.2±1.1 days. There was no infection, cerebrospinal fluid leakage, death or permanent aggravation of neurological dysfunction. The patients were followed up for 3 months to 123 months, with an average of 63.5±0.7 months. The KPS score ranged from 80 points to 100 points, with an average of 93.6±0.3 points. Tumor recurrence occurred in 14 patients (9.1%), including 7 in Simpson Ⅰ resection group and 7 in Simpson Ⅱ resection group. There was significant difference in recurrence rate between the 2 groups ( χ2=4.01, P<0.05). Conclusion:Simpson grade Ⅰ microsurgical resection can be pursued for large and giant parasagittal meningioma invading the superior sagittal sinus which helps minimize the recurrence rate.

Abstract:Objective To explore the classification and the microsurgical resection of the clinoidal meningiomas. Methods Eleven cases of clinoidal meningioma that underwent microsurgical resection were analyzed retrospectively. Results Simpson grade Ⅱ resection were achieved in 8 cases (72.7%). Simpson grade Ⅲ resection were achieved in 3 cases (27.3%).4 patients died (36.4%). Satisfied outcome were achieved in 5 cases (45.5%). Conclusion The clinoidal meningiomas could be divided into three types: presellar, parasellar-suprasellar, diffused type. Each type could be subdivided into A and B subtype according to whether the cavernous sinus or the internal carotid artery is invaded or encased by the tumor. The clinoidal meningiomas should be resected aggressively but prudently. Whether the tumor invades the cavernous sinus especially the internal carotid artery is the key which determines the extent of resection and the outcome. The tumor which invades or attaches directly to the cavernous sinus or the internal carotid artery should not be resected constrainedly. The residual tumor could be treated with stereotactic radiosurgery or be followed up nearly.

Abstract:Objective To investigate the perioperative management strategies for medial sphenoid ridge meningioma (MSRM). Methods The clinical data of 46 MSRM patients undergoing neuro-microsurgery were analyzed retrospectively. The pterional or extended pterional approach was adopted in 45 patients and subfrontal approach in 1. Results According to Simpson resection grade for meningioma, grade Ⅰ was in 4 cases, grade Ⅱ in 16, grade Ⅲ in 17 and grade Ⅳ in 9. The consciousness alteration and hemiplegia appeared in 4 patients, neuropsychiatric symptoms in 3 and tumor cavity bleeding in 1. The recover was good in 24 patients, fair in 18, and poor in 3, and one patient died. Conclusions The surgical resection is the most effective treatment for sphenoid ridge meningiomas. The detailed and comprehensive preoperative evaluation, individualized surgical treatment strategy, solid anatomical knowledge on the skull base and skilled surgical techniques and timely treatment of postoperative complications are key factors for improving therapeutic efficacy and prognosis of MSRM.

Abstract:Objective To summarize the clinical features,surgical manners and effects on para-cavernous sinus.Methods The clinical data of 24 hospitalized patients with para-cavernous sinus tumors were analyzed retrospectively.Results All tumors were resected by interdural routes,18 pterion approach and 6 infratemporal approach. The study consisted of 11 cases of meningiomas,5 trigeminal neurinomas,5 cavernous haemangiomas,1 epidermoid cyst ,1 hemangiopericytoma and 1 fibrosarcoma. The tumors were totally removed in 11 cases, subtotally removed in 10 and partially removed in 1.Conclusion The surgery on para-cavernous sinus tumors are very difficult and highly risked, but the outcomes are quite good.

Abstract:Objective:To explore the operative technique of parasagittal meningiomas.Methods:The clinical data of 67 patients with parasagittal meningiomas was analysized retrospectively.Results:Simpson Ⅰ,Ⅱ,Ⅲgrade resection was achieved in 22,39 and 6 of the 67 patients in this series respectively.The follow-up period ranged from 6～36 months.All the patients recovered well.There were no postoperative deaths.Conclusions:Simpson Ⅰ～Ⅱ grade resection can be reached for parasagittal meningiomas.Adequate surgical exposure,efficient management of operative bleeding,protection and replacement of the sagittal sinus,and avoidence of damage to the brain tissues are the key factors to improve operative effect.

Abstract:OBJECTIVE:To investigate the surgical strategy for large and giant recurrent meningiomas near the middle and posterior third part of the superior sagittal sinus with extracranial invading. METHODS:The clinical data of 16 patients with large and giant recurrent meningioma in the middle and posterior third part of the superior sagittal sinus with extracranial invasion who underwent surgery in the Department of Neurosurgery of Peking University Third Hospital from May 2019 to May 2022 were retrospectively analyzed. All the patients underwent brain-enhanced magnetic resonance imaging (MRI), magnetic resonance venography (MRV), computed tomography angiography (CTA) and three-dimensional skull computed tomography (CT) before, to evaluate the extent of tumor invasion, the edema of brain tissue, the degree of skull damage, the blood supply of the tumor, and the degree of compression of the superior sagittal sinus, etc, and to formulate an individualized surgical plan. The neurological function of the patients was evaluated 1 week, 1 month, and 3 months after the operation, and the tumor condition was evaluated by brain-enhanced MRI 3 months, 6 months, and 1 year after the operation. RESULTS:The tumors in the 16 patients were all located in the middle and posterior 1/3 part of the superior sagittal sinus and invaded extracranially. Among them, 8 cases were operated for the second time, 6 cases for the third time, and 2 cases for the fourth time; In the last operation, the bone flap was used to repair the skull in 4 cases, and the titanium mesh was used in 12 cases; Tumor arterials of 3 cases were embolized under digital subtraction angiography (DSA). Tumors of 10 cases were resected at Simpson grade Ⅰ, and 6 cases at Simpson grade Ⅱ; 2 cases underwent decompressive craniectomy during operation, and 14 cases underwent cranioplasty at the same time; scalp incisions of 14 cases were directly sutured, and flap transposition was used in 14 cases. When evaluating nerve function after operation, the limb muscle strength was improved compared with that before operation, and the Karnofsky performance scale (KPS) score reached 100 points 3 months after operation. During the follow-up, 1 patient's tumor recurred after 1 year and received Gamma Knife treatment, and the rest of the patients had no recurrence during the follow-up period. CONCLUSION:Surgical treatment is the first choice for large and giant recurrent meningiomas near the middle and posterior third part of the superior sagittal sinus with extracranial invading. It is a safe and effective surgical method to take individualized surgical plan after detailed preoperative assessment of cerebral edema, tumor blood supply, venous sinus compression, and scalp invasion.
Microsurgical Treatment for Large Meningiomas at Anterior Skull Base

Abstract:Objective To investigate the operative approaches、microsurgical technique and its curative effect for large meningiomas at anterior skull base.Methods Clinical data including imaging data,the operative approach,microsurgery methods and curative effect of 21 patients with large meningiomas at anterior skull base who underwent microsurgical surgery in our hospital from January 1998 to June 2005 were analyzed retrospectively.Results According to Simpson classification of glioma resection,16 received Grade Ⅰ resection(76.19%),3 received Grade Ⅱresection(14.29%) and 2 received Grade Ⅳ resection(9.52%) in all 21 patients.No patients died from the operation.Following-up 6 to 24 months showed that none recurred.Conclusion The large meningiomas at anterior skull base can be totally resected by using microsurgical techniques.The measurments including sufficient preoperative imaging evaluation,good protection of the nerve and blood vessel,excellent management of the affected bone of skull base and meninges during the operations.It can significantly increase the percentage of total removal of the large meningiomas at anterior skull base,decrease postoperative complications,and improve survival outcome.
Surgical Management and Outcome Experience of 53 Cerebellopontine Angle Meningiomas

Abstract:Background Meningiomas follow schwannomas as the second most common cerebellopontine angle (CPA) tumors. We investigate the diagnosis, management, and prognosis of this disease. Methods We reviewed the cases with the CPA meningiomas in our institution in Shaanxi, China from January 2012 to December 2015. Charts were retrospectively examined and patients were divided into two groups: 1) surgical treatment with a retrosigmoid approach for tumor resection and 2) stereotactic radiosurgery treatment only. Patients were followed up and outpatient records were also reviewed. Results Forty-nine patients underwent surgical resection via the retrosigmoid approach, while the other four underwent Gamma Knife® stereotactic radiosurgery (Elekta AB, Stockholm, Sweden) only. The most common presenting symptoms included hearing loss/tinnitus, vertigo, and headache; only 8.2% were asymptomatic. The largest diameter and base of each tumor varied from 4.0 to 5.5 cm and 3.0 to 5.0 cm, respectively. The tumors extended into different directions, mainly towards the tentorium and internal acoustic meatus (IAM). Eighty-three percent of surgical patients had a gross total resection. One death occurred due to pulmonary inflammation. Tumor recurrence was noted in 6.1% of patients. Postoperative trigeminal disturbance, facial nerve palsy, and hearing deterioration or loss were the most common immediate and delayed postoperative complications; most patients partially or completely recovered after hospital discharge. Intraoperative neuro-electrophysiological monitoring, complete resection, and postoperative radiation were key factors for reducing complications and recurrence. Conclusions The retrosigmoid approach offers an ideal visual field for exposing and resecting CPA meningiomas in a large series of cases. In our experience, it is one of the most useful and commonly used surgical approaches for removing meningiomas of this region.
