Abstract:Background and purpose:Transurethral resection of bladder tumor electroresection(TURBt)has been a routine treatment for superfi cial bladder tumors for many years,but it is not suitable for all patients.Endoscopic holmium laser has recently been found to be useful for the treatment of superfi cial bladder tumors as a novel technique.Our purpose was to discuss the experience of Ho:YAG laser in the treatment of superfi cial bladder tumors.Methods:From July 2005 to February 2008,92 patients with superfi cial bladder tumors underwent endoscopic holmium laser treatment with postoperative intravesical instillation therapy.There were 69 male patients and 23 females with an average age of 56 years.The tumor lesion is single in 56 and multiple in 36 patients.The diameter of the tumor was 0.5-4.0 cm,the patients were pathologically confi rmed to be low-grade papillary urothelial carcinoma by biopsy.11 patients went through the operation with local anesthesia due to either their age or other disease.Results:All the patients could tolerate the surgery,cold cup biopsies of the tumor base and incisional margins were negative.Postoperative intravesical instillation therapy was done to all the patients who underwent cystoscopy every three months.The mean operation time was 18 minutes(10-50 minutes),with few bleeding.Neither intraoperative bladder perforation nor secondary hemorrhage occurred.Indwelling urethral catheter time ranged from 1-3 days(minimum 14 hrs).Patients with local anesthesia could recover immediately after operation.Hospitalization stay after operation ranged from 1 to 3 days,including 43 patients who stayed only one day in the hospital.All patients have been followed up for 2 to 31 months with a mean of 15 months.12 patients(13.3%)had out-of-fi eld recurrence,and were retreated with laser treatment or RE-TURBt.Conclusion:Ho:YAG laser treatment for superfi cial bladder tumors provides a useful clinical therapeutic method that has the advantages of safety and minimal invasion,the outcome is similar to curative TURBt.
