Effect of Preemptive Epidural Analgesia on Blood Levels of Glucose, Plasma Cortisol and Nitric Oxide in Patients Undergoing Gastrectomy

Abstract:Objective To investigate the effect of preemptive analgesia on serum glucose (Glu),cortisol (Cor) and nitric oxide (NO) and its mechanism. Methods Thirty ASA class Ⅰ or Ⅱ patients undergoing selective gastrectomy under general anesthesia were randomly divided into two groups. Group A was injected 0.05mg/kg morphine via epidural catheter 45min before incision and then maintained with 0.02mg·kg-1·h-1.Group B was given no any analgesia. Visual analog scales (VAS), sedation score, pethidine consumption and incidences of side effects were recorded. Plasma levels of Glu,Cor, NO were measured before anesthesia, at 90min after incision and 6h after operation. Results VAS and pethidine consumption in group A after operation were significantly lower at every time point compared with those in group B (P0.05). The plasma Glu in group B increased significantly at 90min after incision (P0.05). As compared with group A, plasma Cor content in group B increased significantly at 90min after incision and 6h after operation.Conclusion Preemptive analgesia with epidural morphine could relieve pain better and inhibit the stress response to operation. Preemptive analgesia with epidural morphine may be involved in inhibiting NO release.
