Abstract:As a standard management mode for clinical diagnosis,the clinical pathway is of great significance to improve the efficiency and quality for patient nursing.With the digital hospital construction and electronic medical records popularization,the electronic research of the clinical pathway is drawing increasingly the public attention in recent years. This text presents the development and application of the clinical pathway and brings forward a new method to formulate electronic clinical pathway,i.e.electronic method of ontology-based clinical pathway.During scientificalness and feasibility analysis of such method,examples are given to illustrate the significance of the ontology in the clinical pathway research and benefits brought to medical informationization.

Abstract:It has been more than 20 years of history since the application of clinical pathway in healthcare industry.It has been demonstrated that the use of clinical pathway can improve the quality,ensure patients safety,improve work efficiency and hospital services,and the patients satisfaction.The paper reviews and summarizes the origin and development of clinical pathway,the definition,international scope of application.The problems we are facing are also analyzed and some suggestions for improvement put forward.

Abstract:In an era of increasing competition in medical care, critical pathway guidelines have emerged as one of the most popular new initiatives intended to reduce costs while maintaining or even improving the quality of care. Developed primarily for high-volume hospital diagnoses, critical pathways display goals for patients and provide the corresponding ideal sequence and timing of staff actions for achieving those goals with optimal efficiency. Despite the rapid dissemination of critical pathway programs in hospitals throughout the United States, many uncertainties remain about their development, implementation, and evaluation. In addition, serious concerns have been raised about their effect on patient outcomes and satisfaction with care, physician autonomy, malpractice risks, and the teaching and research missions of many hospitals. Underlying these concerns is the absence of data from controlled trials to evaluate the effects of critical pathways. Physicians should understand the potential benefits and problems associated with critical pathways because physicians are increasingly being asked to provide leadership for pathway programs. Physicians and other health service investigators should also develop methods to study pathways in evolving health care settings. Although the promise of reduced costs and improved quality is enticing, the gaps in our knowledge about critical pathways are extensive; therefore, like any new health care technology, pathway programs should be fully evaluated in order to understand the conditions under which that promise may be fulfilled.

Abstract:Objective To analyze the management of clinical pathways ( CP) in China. Methods Cross-sectional questionnaire surveys of 51 public hospitals with CPs in place in Shanghai, Hubei province and Gansu province were conducted from March to May of 2015. Results Among the 51 public hospitals with CPs, 48 ( 94. 1%) of them organized training on CPs, 48 ( 94. 1%) of them monitored CPs′implementation, and 40 (78. 4%) applied incentives for CPs′ implementation. But there were some issues and difficulties encountered in CPs′ implementation. Conclusions Comprehensive measures are necessary to improve the management of CPs at public hospitals of China.

Abstract:Clinical pathway management mode is an important part of new health care reform.Through regulation of hospital services,improvement of the doctor-patient relationship,adjustment of hospital management strategies,the hospital market competitiveness can be enhanced.Results showed that the implementation of clinical pathway can regulate the medical behavior,shorten the average hospital stay,reduce the medical expenses,ensure the medical quality,and improve the patient satisfaction.The clinical pathway management mode will bring new opportunities to the hospital development.The operating department as the important part of the hospital,it reflects the hospital medical service,nursing,the medical technique strength,moreover can also comprehensively reflects the level of the hospital management.The article describes the concept and history of clinical pathway,uses SWOT analysis to the military large comprehensive hospital.
Key Factors Affecting the Implementation of Clinical Pathways: a Systematic Review

Abstract:Objective To systematically review the barriers and facilitators affecting the implementation of clinical pathways for the clinical pathways.Methods PubMed, Embase, CNKI, CBM, Wanfang, Cvip databases were searched to collect articles about clinical pathways implementation barriers and facilitators from inception to January 4th, 2019. The tool of confidence in the evidence from reviews of qualitative research ( CERQual) was used to grade the confidence of each study. Results A total of 43 articles from 12 countries were included.There were 8 main categories and 31 subcategories of the barriers about clinical pathways, including content of the clinical pathways, negative outcomes of clinical pathways, physicians knowledge, physicians attitude, resource availability, implementation of activities, patients factors and social factors. The first three barriers of high confidence were lacking of time, capital, equipment, staff and other resources (15 articles, 34.9% ), increasing workload (14 articles, 32.6% ), unrecognizing pathways(12 articles, 27.9% ). There were 6 main categories and 28 subcategories of the facilitators about clinical pathways, including pathways content related, physician related, resource factor and implementation activity. The first three facilitators of high confidence were communication, education and training(25 articles, 58.1% ), supporting from managers and colleagues ( 21 articles, 48.8% ) and establishing a clinical pathway facilitation committee(17 articles, 39.5% ).Conclusions The successful implementation of clinical pathways connects with its development process, aftereffect evaluation and feedback. It will be implemented effectively only by the completely and environmentally acceptable pathways design, adequate resources, effective organizational activities, continuous audit, evaluation and feedback and physicians active cooperation.

Abstract:Clinical pathways used today are paper-based, although many different kinds of clinical pathway are used in clinical practice. However, in the development of software for clinical pathway, it is difficult to achieve cooperation between medical experts, who are not used to expressing their ideas and work in words, and system-developers whose medical knowledge is limited. As a consequence, the current situation is that medical practitioners make their own software and use it in their practice. While this may work, it is less than ideal because refinements that may make the software most effective through engineering expertise is not used. Thus, in our research team, the nurse researchers and the engineering researchers cooperated and developed a clinical pathway system. In this paper, the present conditions, problems, and future direction of the server-controlled CP system development in the psychiatric hospitals, is discussed from viewpoint of nursing as a user.

Abstract:Peking University Third Hospital is one of the first hospitals for experiments in clinical pathway selected by the Ministry of Health,which keeps optimizing the clinical pathway management mechanism and function of information system in its 4-year practice.This article described the combination strategy of clinical pathway management and information system applications,in terms of reasonable design of the management plan,standardized clinical pathway,strengthened execution control and improvement of the quality control.The purpose is to balance the relationship between clinical pathway management norms and user-friendliness of the system,enhance rationality and feasibility,improve efficiency and provide high-quality service to ensure medical safety.

Abstract:Objective To Summarize improvement methods on clinical pathway and provide reference for clinical pathway practice.Methods The PDCA cycle was used to analyze the problems during the implementation of clinical pathway which aimed to develop and implement improvement measures and check the effect constantly.Results After the implementation of clinical pathway based on information system,the proportions of discharged patients entering and completing the clinical pathway had risen to 93.4％ and 87.2％.Conclusion Based on information system and according to local condition,the clinical path management can be fully promoted.

Abstract:The writers of the article assess the benefits from practicing the clinical pathway by comparing the days of hospitalization,hospital expense,the changes in medical quality and the changes in the item expenses of myocardinal infarction,the repairs to the urinary tracts,pelvic fracture and the replacement of total hip before(2007)and after the practice of the clinical pathway(2009)myocardial infarction.After the practice of clinical pathway,the average days of hospitalization shortened,hospital expense reduced and medical safety upgraded.The writers conclude that clinical pathway can improve hospital management and save medical cost.

Abstract:Objective To analyze the status of the clinical pathways’Quality Control to domestic hospital management. Methods Surveying the history records of patients from August 2010 to December 2012, and then recording the status of clinical pathways’implementing. Results From August 2010 to December 2012,798 clinical pathways were established. The rations of using the clinical pathways,completing the clinical pathways and executing doctor’s advice in clinical pathways were all ascendent. To the end of 2012,83.6% of the patients were used the clinical pathways,and the rations was 14.5% more than the year of 2011. Conclusion Useful management modes of quality control can promoting the development of clinical pathways

Abstract:Objective The paper analyzed the variation factor in clinical pathway,classified variation factor,took effective actions in accordance with variation types,provided basical ensure for continuous improvement of clinical pathway.Method The paper investigated retrospectively the implementation efficiency of clinical pathway in clinical departments in 2009;analyzed variation factors,and discussed countermeasures.Result There were three main variation factors which were from hospital,medical staff and patients,accounting for 62%,28% and 10% respectively.Variation factor from patients could not be controlled;and variation factor from hospital could be classified to many respects,such as the poorer communication between departments,out-dated hospital equipments,etc.

Abstract:Objective:To evaluate patient satisfaction conditions and the implementation effect of clinical pathway(CP) and cost control(CC) of 10 single-disease entities.Methods: After collecting the information of the hospitalized patients after the surgical therapies from October 2005 to March 2007,we analyzed the effect of CP and CC with the evaluation indexes of average length of stay and the hospitalization charge.Results: There was significant difference in CQI,the average length of stay,the hospitalization charge,and patient satisfaction rate between the CP-CC groups and the traditional groups.Conclusion: The method of clinical pathway and cost control can markedly improve medical quality and patient satisfaction rate,decrease the average length of stay and the hospitalization charges,and fundamentally improve the doctor-patient relation.
