Changes and Significance of T Lymphocyte Subsets and C-reactive Protein of Acute Pancreatitis in Acute Reaction Period

Abstract:Objective To estimate the severity of acute pancreatitis by observing the dynamic changes of T lymphocyte subsets and C-reactive protein level in acute inflammatory response phase.Methods 50 patients with acute pancreatitis from May,2010 to October,2010 were included in the study.The C-reactive protein and T lymphocyte subsets concentration in serum were measured on day 1,2,3,4,5,6,7 after admission.Contrast computed tomography was used in the acute pancreatitis.According to the severity of the disease,patients were divided into groups separately.Average C-reactive protein and T lymphocyte subsets values were compared between groups by t test for unpaved data.The difference was assumed statistically significant when P0.05.Results The fluctuation range of each index(CD3,CD8,CD4/CD8)of T lymphocyte subsets of acute light pancreatitis was smaller than severe pancreatitis.There were more apparent differences between the acute light pancreatitis and the severe ones about CD3 on the second and fourth days(P0.05).CD4 in the light and severe group was not statistically significant.CD8 was significant lower in the severe group than in the light group.In the first day,the ratio of CD4/CD8 was lower in acute severe pancreatitis than the light ones.But the ratio was higher than the acute light pancreatitis on the next day.This relationship was more obvious,especially after the fourth day.Serum c-reactive protein level began to rise on the first day,reached the top on the second day,and then descended gradually.It is statistically significant in the light and severe group.Conclusion The results of our study show that the T lymphocyte subsets and the C-reactive protein level in acute inflammatory response phase is an important predictive marker of severity of acute pancreatitis.
