Abstract:Stress is a nonspecific reaction to everything the body needs. Although occupational stress exists in every occupation, it is seen with more frequency and intensity amongst those occupations related to human health. In this study, we aimed to investigate the relationship between occupational stress and social support (SS) among hospital nurses. A cross-sectional survey was conducted among 1144 hospital nurse in China. They were investigated with a self-administered questionnaire about socio-demographic characteristics, occupational stress and social support. A validated version of the revised Occupational Stress Inventory (OSI-R) was applied to evaluate occupational stress; SS was measured by nine questions. Logistic regression analysis was used to study the association between occupational stress and SS and adjusted for income, gender, marital status, working years, educational level, and profession. Of 1144 nurses, the majority age group was less than 30 years, and the mean age across participants was 31.8 years. Further correlation analysis indicated that score of ORQ and PSQ had a significant negative correlation with score of SS (P<0.05), and a significant positive correlation was found between Score of PRQ and score of SS (P<0.05). The univariate analysis and multivariate analysis results also revealed that high SS increased significantly with decreasing ORQ score and increasing PRQ score after controlling for income, gender, marital status, working years, educational level and job title. SS significantly influences occupational stress in hospitals nurse. We also should pay more attention to occupational stress of married and long working years nurse.

An Analysis of Characteristic Occupational Stressors for Medical Staff in General Hospitals

Abstract:Objective To explore the status, characteristics and factors in relation to occupational stress for medical staffs in tertiary general hospitals. Methods A total of 2460 medical staff were sampled in five tertiary general hospitals in Beijing, with their occupational stress levels evaluated with the Occupational Stress Inventory. Results The top ten stressors as found ranked as heavy duty, high risk exposure, high workload, low wages, setbacks in the health care management system, insufficient staffing, excessively frequent inspections and examinations, strained doctor-patient relationship, price inflation, frequent overtime, and pressure from continuous learning. Occupational stress is seen as moderate and above by 95.2% of the surveyed. Differences in age, gender, marital status, professional title, education, work experience, as well as those of different organizations, departments, professions, and duty were found to be statistically significant in regard of professional stress. Conclusions Stress management should be in place targeting demographic and stress characteristics. Effective measures are recommended to alleviate the pressure on medical staff, in order to maintain their physical and mental health, hence improving their work efficiency and organizational cohesion.

Abstract:[Objective] To assess the condition of occupational stress and related impact factors among medical staff.[Methods] The job content questionnaire(JCQ) and the effort-reward imbalance questionnaire(ERI) were distributed to 1 200 medical staffs by random sampling,and 1 128 questionnaires were collected.[Results] High level of occupational stress was reported by 916 medical staffs(81.2%) in JCQ and 262(23.2%) in ERI.According to the results of JCQ,higher occupational stress was found in the staffs reporting family monthly income per capita 1 000 Yuan RMB or 1 000-3 000 Yuan RMB groups(both P 0.05,OR=4.34 and 1.95 respectively,compared with the staffs reporting family monthly income 3 000 Yuan RMB per capita) and the job rotation staffs(P 0.05,OR=2.54,compared with the no job rotation staffs).According to the results of ERI,higher occupational stress was found in the staffs reporting family monthly income 1 000 Yuan RMB group(P 0.05,OR=3.29,compared with the family income 3 000 Yuan RMB group),the secondary or tertiary hospital staffs(P 0.05,OR=2.70 and 1.98 respectively,compared with the primary hospital staffs),nurses(P 0.05,OR=2.50,compared with the other staffs),and the staffs reporting 40-60 or 60 working hours per week(both P 0.05,OR=2.55 and 6.83 respectively,compared with the 40 working hours per week staffs).[Conclusion] Medical staff may suffer from high occupational stress and the common impact factors are family monthly income per capita,hospital level,job position,working hour per week,and rotation system.Relevant health policies are necessary to control their stress at work to promote occupational health.
Influence of Effort-Reward Imbalance on Turnover Intention in Nurses

Abstract:Objective To investigate the relationship between effort-reward imbalance and job burnout and turnover intention.Methods Six hundred and eighty nurses in three municipal comprehensive hospitals in Hangzhou were surveyed utilizing Questionnaires on general conditions,Effort-Reward Imbalance Inventory,Maslach Burnout Inventory and Turnover Intention Inventory,and 470 effective questionnaires were obtained(69.1%).Results Among the nurses surveyed,31.9%of the nurses were in the state of effort-rewards imbalance,and 82.3%of nurses were overloaded in job. Effort-reward imbalance was significantly related to job burnout and turnover intention(P0.01),and effort-reward imbalance and job burnout had directly impact on turnover intention(P0.05).Conclusion To decrease the turnover intention of nurses and improve the nursing development,the measures of salary increase and job promotion optimization should be adopted to inverse the state of effort-reward imbalance and prevent nurses from job burnout.In addition,more attention should be paid to young nurses and contract nurses to decrease the turnover intention.

[Association Between Occupational Stress and Presenteeism among Medical Staff at Grade A Tertiary Hospitals in Shanghai, China].

Abstract:OBJECTIVE:To investigate the current status of occupational stress and presenteeism among medical staff at grade A tertiary hospitals in Shanghai, China, and to analyze the association between occupational stress and presenteeism.METHODS:A total of 2356 healthcare workers from eight grade A tertiary hospitals in Shanghai were investigated by stratified random sampling. All the subjects were asked to complete self-administered questionnaires with informed consent. The occupational stress was assessed by the occupational stress core questionnaire. The presenteeism was evaluated by the Stanford Presenteeism Scale.RESULTS:In all subjects, the average score of presenteeism was 15.23 ± 3.89, and 72.5% felt occupational stress in self-evaluation. There were significant differences in the score of presenteeism between subjects with different ages, education levels, occupations, lengths of service, job titles, and shifts (all P<0.05). There were significant differences in the score of presenteeism and the rate of high presenteeism between subjects with different indices of occupational stress, including job demand, control, social support, and demand-control ratio (P<0.01). High job demand and high demand-control ratio were risk factors for high presenteeism (OR=2.56, 95% CI=2.01~3.27; OR=1.53, 95% CI=1.23~1.90). High social support was a protective factor for high presenteeism (OR=0.23, 95% CI=0.18~0.29).CONCLUSION:The intensity of occupational stress in medical staff from grade A tertiary hospitals is positively correlated with the level of presenteeism. It is important to promote social support to reduce the loss of work efficiency due to presenteeism.

Abstract:[Objective]To study the relationship between occupational stress and mental health of staffs from business buildings,and provide scientific basis for further intervention.[Methods]A total of 1 033 workers from 11 business buildings were selected for questionnaires survey about general condition,JCQ,CHQ and CES-D.Their occupational stress and mental health were also investigated.The data were statistically analyzed by means of ANOVA analysis,rank correlation,stepwise regression and unconditional logistic regression.[Results]Internal consistency analysis showed that the JCD,GHQ and CES-D had high reliability in the investigation.The prevalence rate of high occupational stress,psychological problem tendency and high depression tendency in the investigated subjects was 32.33%,12.49% and 57.89% respectively.ANOVA analysis showed that scoring of each JCQ dimension,especially job demand and job control scoring,varied significantly between different individual characterizations(P<0.05 or P<0.01).Rank correlation indicated that spearman rank correlation coefficient of GHQ and CES-D with occupational stress was-0.08 and-0.21(P<0.05 or P<0.01) respectively.Stepwise regression analysis showed that variables predicting psychologic status were age,gender,educational background,job demand,autonomy and variables predicting depression tendency were age,job demand and social support,with job demand having the strongest predicting capacity.Logistic regression showed that risks of psychological problem and depression tendency induced by high occupational stress was 4.46 times(OR 95% CI=2.36~8.46) and 3.38 times(OR 95% CI=2.36~4.85)of that induced by low occupational stress,while social support could mitigate the risk of depression tendency,OR=0.61 and OR = 0.44(P<0.01),while confouding factors influenced the effect of social support on mental health obviously.[Conclusion]Occupational stress is affected by individual factors.High occupational stress,especially job demand,is an important factor resulting in high risks of psychological problem and depression tendency.Measures should be taken on the individual level and on organizational level to reduce occupational stress,then workers' psychological health can be protected.
Appraisal of Occupational Stress and Its Influential Factors in Technical Staff

Abstract:Objective To assess occupational stress and its influential factors in technical staff.Methods A test of occupational stress and its influential factors was carried out with revised occupational stress inventory(OSI-R)for 857 technical staff and 319 controls.Results Among 6 scales of occupational role questionnaire,the scores of role overload,role insufficiency,role boundary,role ambiguity,and responsibility were obviously higher in technical staff(P<0.01).The scores of vocational strain,psychological strain,interpersonal strain and physical strain in staff were significantly higher(P<0.01).The results of personal resources indicated that self-care scores in technical staff were superior(P<0.01).Occupational orle and personal strain were positively correlated(r=0.518,P<0.01),both of which correlated negatively to the personal resources(r=-0.254,P<0.01).The major influential factors of personal strain,vocation strain,psychological strain,interpersonal strain,physical strain were different.Conclusions Different measure should be taken to reduce the occupational stress so as to improve the work ability of technical staff.
Survey and Analysis of Occupational Stress and Job Burnout among Community Health Service Workers in Shanghai

Abstract:Objective To identify the situation of occupational stress and job burnout among community health care workers.MethodsA survey on occupational stress and job burnout was carried out among 1397 community health care workers in Shanghai.Self-administered questionnaire was used to assess job burnout and occupational stress based on Maslach Burnout Inventory and the Job Demand-Control model as well as Effort-Reward Imbalance Model.Multiple logistic regressions were used to analyze the association between occupational stress and job burnout.Results77.1% participants were assessed having occupational stress,19.3% subjects showed effort-reward imbalance and the positive rate of job burnout occupied as high as 77.3%.The mean of job burnout was 2.17±0.91 and the mean of its three dimensional emotional exhaustion was 2.23±1.46.Depersonalization was 1.04±1.15 and the sense of personal accomplishment was 3.78±1.36.High demand-low self-control was a protective factor for personal accomplishment.With low D/C group as control,odd ration equaled to 0.54(95%CI=0.37~0.78) for the middling D/C group,and 0.42(95%CI=0.25~0.72) for high D/C group.Social support was a protective factor of job burnout.With low social support group as control,odd ration equaled to 0.69(95%CI=0.47~1.00)or 0.52(95%CI=0.35~0.77) for medium or high social support groups.Effort-reward imbalance was a protective factor of personal accomplishment.In comparison with low effort-reward imbalance group,OR of the medium effort-reward imbalance group was 2.48(95%CI=1.33~4.62),the OR of the high effort-reward imbalance group was 0.52(95%CI=0.35~0.77).Over-commitment was a risky factor of emotional exhaustion and depersonalization.ConclusionsIn order to prevent job burnout,it's better to control occupational stress,improve effort-reward imbalance and increase social support.

Conclusion Different measures should be take to reduce the occupational stress to improve working ability of the technical workers.

Abstract:This paper explores the complex idea of work pressure among Chinese hospital administrators and how it affects job happiness and overall effectiveness of the organisation. Hospital directors confront a wide range of intricate issues within the framework of China's distinct healthcare system because of their twin responsibilities as administrative and medical leaders. The dimensions of role pressure are examined in this study, which include role gap, role overload, where the workload is excessive, role ambiguity, and the role of conflict. Understanding these aspects is essential to appreciating the effectiveness and well-being of hospital administration. Given that job happiness is a major factor in both employee performance and retention, these pressures have a significant effect on job satisfaction. The study also looks at the wider effects of pressure on organisational performance, which is crucial for the long-term viability and efficiency of healthcare facilities. Through an examination of these facets, the paper illuminates the noteworthy, albeit frequently disregarded, psychological and operational obstacles encountered by hospital administrators in China. This, in turn, offers valuable perspectives on the possibility of policy and administrative changes targeted at augmenting job contentment and organisational results.

Abstract:The objective of the present study was to investigate the occupational stress of mental health practitioners and the affecting factors.769 mental health practitioners completed a questionnaire that consisted of 93 multiple choice questions.Among them two scales with like format were related to occupational stress,and had acceptable psychometric properties.Factor analysis indicated that the occupational stress of mental health practitioners could be divided into three categories: professional insufficiency,lack of support from one's organization,and work overload.Regression analysis indicated that the non-professional factor affecting occupational stress included age,gender,education,full-time/part-time status,whether the workplace is for exclusive use,and hours spent on mental health service per week.Among the professional factors,supervision and training significantly alleviated stress,whereas difficulty in interacting with clients gives rise to higher occupational stress of mental health practitioners.

Abstract:Aim. This paper is a report of a study investigating nurse burnout and its association with occupational stressors in Shanghai, China. Background. Burnout is described as feelings of emotional exhaustion, depersonalization and reduced personal accomplishment. Measuring burnout among nurses and its association with occupational stressors are important in providing appropriate intervention to reduce stress and burnout. Method. In total, 527 nurses from 41 hospitals in Shanghai returned self-reported questionnaires in June 2008. Their burden of burnout was measured by the Chinese version of 'Maslach Burnout Inventory', and their stress by the Chinese version of 'job content questionnaire' and 'effort-reward imbalance questionnaire'. Results. Most nurses (74.76%) had a Demand/Control ratio higher than 1 and 27.13% had an Effort/Reward ratio higher than 1. The nurses showed a high level of emotional exhaustion, moderate level of depersonalization, and low level of reduced personal accomplishment. Linear regression analyses showed higher burden of stress and burnout among nurses at younger age, or on shift duty or from higher grade hospitals. Both the job content questionnaire and effort-reward imbalance questionnaire models had good predictive powers of the nurses' burnout, especially the effort-reward imbalance questionnaire model is more powerful than the other in predicting two 'Maslach Burnout Inventory subscales of emotional exhaustion and depersonalization. Conclusion. Nurses in Shanghai were suffering from high levels of burnout, which was strongly associated with work-related stress. Interventions in reducing occupational stress are needed to reduce the burden of burnout in Chinese nurses.
Work-related Stress and Stress Management Practice among Chinese Hospital Staff Working Night Shifts: A Mixed-Methods Study.

Abstract:Health care workers (HCWs) are susceptible to cumulative stressors and their detrimental effects. Hence, it is important to identify the factors in HCWs' stress management behaviors to help them better cope with daily stress. Inspired by the health belief model (HBM), we decided to focus on examining the psychosocial pathway between stressors and stress outcomes among HCWs. A sequential explanatory mixed-methods study was conducted in two general hospitals in Wuhan, China. A constructive questionnaire was applied to assess the psychological measures of stress, and the Holter devices were used to evaluate the physical effects of stress. Subsequently, a multiple case study was conducted to better understand factors related to stress management attitudes and behaviors among HCWs. A total of 292 HCWs were included in this study. The results showed that 67.1% of participants were overstressed, with 57.5%, 69.2%, and 78.4% of participants suffering from depression, anxiety, and stress symptoms, respectively. Only half of the participants adopted stress management behaviors, and stress management attitudes and behaviors played a mediating role between stressors and stress outcomes for both physical indicators (beta = 0.014, p = .006) and psychological measures (beta = -.001, p = .052). Analyses of the results of this qualitative study consistently found that knowledge, skills, and positive feedback were critical elements for stress management behaviors. Promoting positive health beliefs around stress management, and emphasizing, in particular, the positive role of stress management in HCWs' personal lives, was critical to help HCWs build stress management behaviors.
Path Analysis Between Work Pressure and Quality of Life among Medical Professionals in A General Hospital

Abstract:Objective The study was to build the influencing path from work pressure to life quality among medical staff in a general hospital.Methods Using stratified-cluster sampling,404 medical staff of a general hospital in Beijing were investigated by questionnaires,including individual characters,organizational support,work pressure,job burnout,job satisfactions and quality of life and so on.Results(1)The centesimal-system revised scores of organizational support,job control,job satisfaction(including salary,promotion,reward,operation procedure,communication,interests and work-itself)and life quality(including psychology and surroundings)which those medical staff perceived subjectively were not more than 60;(2)The results of path analysis showed that self-efficacy and organizational support,in contrast with A-pattern character,had negative impacts on task needs and job burnout and had positive impacts on job control,job satisfaction and quality of life.Task needs and job burnout had negative impacts on job satisfaction and quality of life(including physical,surroundings and social relationship).Job burnout had negative impacts on quality of life on psychology.At the same time,there was certain relevance between job satisfaction and quality of life.Conclusion Chinese medical staff in general hospitals are facing heavy work pressure which have seriously influenced their physical and mental health and quality of life.It should be taken to strengthen training for community health workers to relieve work pressures of medical staff in general hospitals thereby to realize reasonable utilize and allocation of various levels' health resources and improve efficiencies of the whole health system.

Abstract:Objective To analyze occupational stress and its influencing factors among working populations in urban China.Methods Employees aged 18-65 years were selected from 5 types of workplaces in Beijing,Xiamen,Quanzhou and Hohhot using stratified cluster sampling and surveyed with a questionnaire between March and October 2015.Results For the 3 553 employees with valid responses,the average age was 34.2 ± 9.7 years;62.3 ％ (2 213/3 553) of the participants had high degree of occupational stress,and there was a significant difference in the proportions of the occupational stress among the participants from the 5 types of workplace (x2 =100.19,P ＜ 0.01).The results of analysis of variance (ANOVA) showed that there were significant differences in job demand scores among the employees categorized by gender,age,education,job type,income,household registration,occupational group,type of contract,work shift,and working condition (all P ＜ 0.05);there were significant differences in job control scores among employees categorized by age,education,job type,marital status,household registration,occupational group,and working condition (all P ＜ 0.05);and there were significant differences in social support scores among employees categorized by age,education,job type,income,household registration,occupational group,type of contract,working hours,and working condition (all P ＜ 0.05).Logistic regression analysis indicated that employees who were female (odds ratio [OR] =1.20,95 ％ confidence interval [95 ％ CI:1.03-1.38),with education of college and above (OR =2.14,95 ％ CI:1.73-2.65),being a administrator (OR =1.37,95 ％ CI:1.09-1.71),having working hours of＞ 8 hours/per day (OR =1.24,95 ％ CI:1.06-1.47),and experiencing unfavorable working condition (OR =1.48,95 ％ CI:1.28-1.71) were more likely to have occupational stress.Conclusion The occupational stress is high and mainly influenced by gender,education,occupational group,working hours,and unfavorable working condition among urban working population in China.Interventions should be taken on individual and organizational level to reduce the occupational stress.

Abstract:Objective: To know primary job stressors between emergency room nurses and ward nurses in county level hospital,and provide scientific basis for alleviating nurses' stress, and improve body and mind health level of nurses. Method: To Make an investigation to 50 emergency room nurses and 50 ward nurses through an Job Stressors Questionnaire. Result: The high level of job stressors among all staff nurses were the professional and career issues workload and time pressure and patient care and interaction. The common level of job stressors among all staff nurses were resource and environmental problems, interpersonal relationships and management issues. For resource and environmental problems, was significantly difference between stress level of emergency room nurses and ward nurses ( P 0.05). Suggestion: To bring forward measures alleviating staff nurses' stress including:①improving emergency room environment;② reasonably arranging ward staff nurses; ③decreasing paper work of ward staff nurses;④strengthening all staff nurses' coping stress ability .

Abstract:OBJECTIVE:To assess occupational stress and its influential factors in executive group.METHODS:A test of occupational stress, its influential factors, was carried out with revised occupational stress inventory (OSI-R) for 263 executive group and 319 controls.RESULTS:(1) Among the 6 items of occupational role questionnaire, the scores of role overload, role insufficiency, role boundary, role ambiguity, responsibility are obviously higher in the executive group (P < 0.01). The scores of vocational strain, psychological strain, interpersonal strain and physical strain in the executive group are significantly higher (P < 0.01). The results of personal resources indicate that social support scores in executive group are superior (P < 0.05). (2) Occupational role and personal strain were positively correlated (r = 0.337, P < 0.01), both of which correlated negatively to the personal resources (r = - 0.311, P < 0.01). (4) The major influential factors of personal strain and psychological strain were: responsibility, personal resources, role boundary and working environment. The major influential factors of interpersonal strain were role ambiguity, role boundary and personal resources. The major influential factors of physical strain were personal resources, responsibility, role boundary and education.CONCLUSION:Different measure should be taken to reduce the occupational stress so as to improve the work ability of executive group.
