Abstract:Objective To study the effects of MRCP on the operative therapy of bile duct.Methods 281 cases with history of common bile duct explor ation in Tongji Hospital of Shanghai were collected from October 2000 to Decembe r 2003.The cases from October 2000 to September 2001 were contrasts of remnants study (group A).156 cases were collected from October 2001 to December 2003.68 c ases of them (group B) had no MRCP examination,88 cases (group C) had MRCP exami nation before operation.The operative exploration was taken as gold standard and the positive rate of choledochus exploration in group B was contrasted with tha t in group C.Cases whose choledochus diameter were less than 1.0 cm in explorati on were also contrasted with their positve rate.Remnants rate after choledochus exploration was studied in all the groups.Results The accuracy of choledochus exploration with MRCP examination before operation was higher than that of the group without MRCP,the difference was statistically significant (P0.05).The choledochus explorati on rate of whose choledochus diameter were less than 1.0 cm was not statisticall y significant in the two groups,but the positive rate of cholelithes was differe nt,.the difference being statistically significant (P0.05).The remnant rate after choledochus exploration with or without MRCP examination was also differe nt,the difference was statistically significant (P0.05).With MRCP examinati on before operation,the malformation of bile duct could be found,and the injury of bile duct in operation could be avoided,too. Conclusion With MRCP examination before operation,the acc uracy of choledochus exploration can be enhenced;the unnecessary choledochus exp loration decreased;and patients' pain alleviated,especially increase the positiv e cholelith rate of cases whose choledochus diameter are less than 1.0 cm.The ma lformation of bile duct can be found and the injury of bile duct in operation wi th the bile duct tree provided by MRCP before operation can be avoided,and it is helpful to decreases remnants after operation.
