Progress on Compensation for Tumor Motion Induced by Respiration in Radiotherapy

Abstract:Objective: Respiration induces significant movements of tumors in the vicinity of thoracic and abdominal structures, which greatly degrade the efficacy of intensity modulated radiation therapy and harm the surrounding normal tissues and organs, so it is necessary to compensate tumor motion in radiotherapy. Methods: Main compensation methods are breath-hold, respiratory gating, four-dimensional radiotherapy and real-time tumor tracking, which reduce tumor motion, synchronize radiation with a certain respiratory phase, take time into consideration during radiotherapy and make radiation beam following with the moving tumor respectively. Results: Breath-hold is hard for patients to endure. While respiratory gating is easy for patients to endure, but it lengthens the radiation time. Four-dimensional radiotherapy and real-time tumor motion tracking is the development direction of dynamic radiotherapy. System delay makes tumor motion prediction indispensable. Since traditional predictive algorithms all have disadvantages, modified or new algorithms are needed. Conclusions: Tumor motion induced by respiratory needs to be compensated for accurate radiation, which is of great significance to improve efficiency of radiotherapy and reduce harm to patients.
