The Feasibility of Karyocyte Scavenger for Intraoperative Blood Salvage During Oncosurgery

Abstract:Objective To test whether the tumor cells in salvaged blood during oncosurgery can be completely removed or destroyed by a new type of karyocyte scavenger.Methods A total of 20 patients with hepatoma and scheduled to tumorectomy were selected.Blood was salvaged beginning from skin cutting until tumor was removed,and the salvaged blood was treated with karyocyte scavenger after it was washed.The counts of nucleated cells and RBC,the morphology of nucleated cells in blood were detected before and after scavenger treatment.Cancer cells and stem cells,and their reproductive activities were also determined by immunofluorescence staining and cell culture respectively before and after blood was treated.The membrane of karyocyte scavenger was detected by light microscope and transmission electron microscope.Results (93.9±3.4)% of RBC passed through the karyocyte scavenger.The counts of nucleated cells decreased from 109/L to 105/L,and over 90% of cells were without cytoplasm.By immunofluorescence staining,we found that there were about 104/L of cancer cells in salvaged blood,in which over half were CK19 and CD133 positive,and 80% of blood samples were positive by cell culture.Contract to these,there was neither cancer cells nor clone found in treated blood.By light and electron microscopes,we found most of damaged nucleated cells were trapped in the first layer of karyocyte scavenger.Conclusion This karyocyte scavenger with high RBC recovery rate,can remove as high as 5 logs of nucleated cells in the blood.After its treatment,cancer cells cannot be found in blood,and can't live because of the shortage of adhesive characteristics.We,therefore,conclude that blood salvage can be safely applied during oncosurgery after blood is treated by this karyocyte scavenger.
