Abstract:Endometrial evaluation by ultrasound is a convenient and noninvasive practice used during in vitro fertilization (IVF) cycles to analyze factors including endometrial thickness (EMT), endometrial pattern, endometrial volume, and endometrial and subendometrial blood flow. These factors are all believed to contribute to endometrial receptivity and IVF success. The relationship between EMT and IVF success has been well studied; however, no consensus has been reached, and the association remains controversial. This meta-analysis aimed to evaluate the association between EMT and IVF success rate. The outcomes recorded included pregnancy, implantation, miscarriage, live birth, ongoing pregnancy, and ectopic pregnancy after IVF. The electronic databases PubMed, EMBASE, and the Cochrane Library were searched for studies identifying the association between EMT and any of the aforementioned outcomes. Studies were evaluated for inclusion based on a preset criterion involving participants, exposure and controls, outcomes, study design, and publication language of English. Pregnancy, ongoing pregnancy, miscarriage, implantation, and live birth rates were extracted from selected studies and assigned as dichotomous frequency data. Pooled odds ratios (ORs) for study outcomes were calculated with the random-effects model. Subgroup analyses were conducted according to country, study design, cutoff value of EMT, treatment strategy, and study quality. A total of 30 studies were included in this analysis including a total of 88,056 IVF cycles. Pooled ORs showed that lower EMT was associated with lower pregnancy rate compared with those with high EMT (OR, 0.61; 95% confidence interval [CI], 0.52-0.70; P < 0.001). A lower EMT was also found to be associated with a lower implantation rate (OR, 0.49; 95% CI, 0.32-0.74; P = 0.001). Lower EMT compared with higher EMT was found to be associated with a lower live birth or ongoing pregnancy rate (OR, 0.60; 95% CI, 0.48-0.73; P < 0.001). Pooled data found no significant association between EMT and miscarriage rate; however, sensitivity analysis indicated there was a statistically significant association after excluding several studies. Subgroup analysis found EMT may be associated with an increased risk of miscarriage when the cutoff value of EMT was defined as 0.8 mm or less, in patients treated by both IVF and intracytoplasmic sperm injection and in patients receiving fresh embryo transfer. No significant association between EMT and ectopic pregnancy rate was observed. The results of this meta-analysis found that a lower EMT was associated with a reduced pregnancy rate, lower implantation rate, and lower ongoing pregnancy and live birth rate. The pooled results showed no significant association between EMT and miscarriage or ectopic pregnancy rates.
