Significance of genetic polymorphism of the thiopurine S-methyltransferase for individualizing 6-mercaptopurine chemotherapy

Abstract:OBJECTIVE To evaluate the activity and genotype of thiopurine methyltransferase(TPMT) for individualizing mercaptopurine(6-MP) therapy.METHODS Ninty-four patients were treated with 6-MP,19 cases with hematological crisis were analyzed to clarify the genotype and phentype of TPMT of this adverse effect.Other 75 cases with the same doses but without hematological crisis served as control group.An HPLC assay was used to measure activity of Erythrocyte TPMT.Allele-specific PCR(ASPCR) and PCR-restriction fragment length polymorphism(RFLP) were used for analysis of genotype frequency of TPMT*2(G238C),TPMT*3A(G460A/A719G),TPMT*3B(G460A) and TPMT*3C(A719G) respectively in each group.RESULTS All of 19 cases with hematological crisis showed severe WBC reduction,PLT reduction and alopecia.Activity of TPMT in case group was(6.13±2.13) U·mL-1 pRBCs,while that in control group was(15.33±2.32) U·mL-1 pRBCs,The total frequency of mutant TPMT alleles in case group(10.7%) is high than that of the contron group(1.2%).CONCLUSION Detection Phenotype and genotype of TPMT may be helpful for preventing side effects from antipurine metabolic drug overdose,and individualizing thiopurine drug chemotherapy.
