Use of RECIST criteria in evaluation of therapeutic response in breast cancer metastases:with comparison of WHO criteria

Abstract:Background and purpose:Since 1980's,WHO guidelines have been widely used for tumor response evaluation.The value of RECIST guidelines,which was developed on the basis of WHO guidelines in 2000,needs to be further verified in the clinical practice.The purpose of our study was to evaluate the RECIST criteria in assessment of treatment response in breast cancer patients with metastases and compared with WHO criteria.Methods:CT images of 67 breast cancer patients who had metastases were respectively enrolled in 3 phase-Ⅲ clinical trials,the evaluation took place before and after initiation of treatment.Lesions were selected and measured on digitized images with use of electronic calipers,for each lesion the maximum diameter,maximum perpendicular diameter were determined.Measurements of all lesions were summed up to obtain the patient response,which was categorized as:complete response,disappearance of lesions;partial response,>30% decrease in tumor diameter(uni-dimensional),or >50% reduction in tumor area(bidimensional);progressive disease,>20% increase in tumor diameter,or >25% increase in tumor area;stable disease,neither complete response,partial response,nor progressive disease.Differences in response assessment between the two techniques were compared statistically according to the χ2 test.Results:A total of 205 lesions(lung metastases,119;enlarged lymph nodes,53;liver metastases,29;chest wall metastases,4)were analyzed.These two techniques were concordant in 61 classifications(91.0%).The response assessment produced by RECIST measurement was improved compared with those of WHO measurement in 4 patients,and worse in 2 patients,respectively.There is no significant difference between the total efficiency judged by these two criteria.Conclusions:Uni-dimensional measurements are in concordant with bidimensional measurement,and are appropriate for assessment of treatment response in breast cancer patients with metastases.
