Abstract:Objectives: To summarize our clinical experience on salvage surgery and immediate reconstruction in patients with advanced craniomaxillofacial tumours, including skull base reconstruction with adjacent or free flaps, functional reconstruction after total glossectomy and laryngectomy with pedicled or free flaps, carotid artery resection with reconstruction. Methods: One hundred and one patients with advanced craniomaxillofacial tumours were reviewed, including 35 patients with advanced oral cancers (group A, total glossectomy and total laryngectomy, and immediate reconstruction), 38 patients with tumours involving the skull base (group B, radical resection and immediate reconstruction) and 11 patients with carotid arteries involvement (group C, carotid resection with immediate reconstruction). The clinical data were obtained from the patient’s charts including pathological diagnosis, clinical stage, surgical method, postoperative complication and their follow-up records. Results: Successful radical salvage surgery and reconstruction was performed in all patients with post-operative complication rate of 20.0% (group A), 23.7% (group B) and 21.4% (group C), respectively. The 3- and 5-year survival rate were 38.7% and 23.1% (group A), 51.6% and 47.6% (group B), 76.5%(3-year, group C), respectively. Conclusions: Radical surgery is suggested for patients with high risks of craniomaxillofacial tumours, reconstruction with pedicled or free flaps is also suggested for those patients.
