Abstract:BACKGROUND:Treatment of the disrupted intrasynovial flexor tendon is troublesome and can be complicated by the rupture of weak repairs and the formation of adhesions. The central issue underlying the unsatisfactory outcomes is the lack of sufficient healing capacity, which prohibits aggressive postoperative tendon motion. Transfer of genes that are critical to healing by means of an efficient vector system offers a promising way of strengthening the repair. The purpose of the present study was to transfer the basic fibroblast growth factor gene through the adeno-associated viral-2 vector to injured digital flexor tendons and to investigate its effects on the healing strength of the tendon and on adhesion formation in a clinically relevant injury model.METHODS:One hundred and twenty-eight long toes from sixty-four white leghorn chickens were used. The flexor digitorum profundus tendons were cut completely in the digital sheath area and were repaired with the modified Kessler method. In Group 1, a total of 2 x 10(9) particles of adeno-associated viral vector harboring the basic fibroblast growth factor gene were injected into both ends of the cut tendon. In Group 2, the same amount of adeno-associated viral vector carrying the luciferase gene was injected. In Group 3 (the non-injection control group), the tendons were sutured without any injection. At the end of two, four, eight, and twelve weeks, the toes were harvested and the tendons were tested for determination of the load-to-failure strength. At the end of eight and twelve weeks, the energy required to flex the toes was tested. The morphology regarding healing status and adhesions around the tendon were evaluated at two, four, eight, and twelve weeks.RESULTS:The ultimate strength of repaired tendons that had been treated with adeno-associated viral vector-basic fibroblast growth factor was significantly greater than that of tendons that had been treated with the sham vector or simple repair both during the early healing period (two weeks, p < 0.01; four weeks, p < 0.01) and a later period (eight weeks, p < 0.05). At four weeks, the strength of tendons that had been treated with adeno-associated viral vector-basic fibroblast growth factor (8.9 +/- 1.9 N) was significantly greater than that of tendons that had been treated with sham vector (6.1 +/- 1.0 N) (p < 0.01) or simple suture (5.7 +/- 1.1 N) (p < 0.001). Statistically, the grading of adhesions was the same among all three groups at four and eight weeks, but at twelve weeks it was significantly less severe for tendons that had been treated with adeno-associated viral vector-basic fibroblast growth factor than for those that had been treated with simple suture (p < 0.05). The energy that was required to flex the toes after treatment with adeno-associated viral vector-basic fibroblast growth factor was not increased at eight or twelve weeks compared with that in the controls.CONCLUSIONS:The present study demonstrates that basic fibroblast growth factor gene transfer to digital flexor tendons by means of adeno-associated viral vector-2 significantly increases healing strength during the critical tendon healing period but does not increase adhesion formation.
Basic Fgf or Vegf Gene Therapy Corrects Insufficiency in the Intrinsic Healing Capacity of Tendons

Abstract:Tendon injury during limb motion is common. Damaged tendons heal poorly and frequently undergo unpredictable ruptures or impaired motion due to insufficient innate healing capacity. By basic fibroblast growth factor (bFGF) or vascular endothelial growth factor (VEGF) gene therapy via adeno-associated viral type-2 (AAV2) vector to produce supernormal amount of bFGF or VEGF intrinsically in the tendon, we effectively corrected the insufficiency of the tendon healing capacity. This therapeutic approach (1) resulted in substantial amelioration of the low growth factor activity with significant increases in bFGF or VEGF from weeks 4 to 6 in the treated tendons (p < 0.05 or p < 0.01), (2) significantly promoted production of type I collagen and other extracellular molecules (p < 0.01) and accelerated cellular proliferation and (3) significantly increased tendon strength by 68–91% from week 2 after AAV2-bFGF treatment and by 82–210% from week 3 after AAV2-VEGF compared with that of the controls (p < 0.05 or p < 0.01). Moreover, the transgene expression dissipated after healing was complete. These findings show that the gene transfers provide an optimistic solution to the insufficiencies of the intrinsic healing capacity of the tendon and offers an effective therapeutic possibility for patients with tendon disunion.
Effects of Transforming Growth Factor-Beta 1 and Vascular Endothelial Growth Factor 165 Gene Transfer on Achilles Tendon Healing

Abstract:Repaired Achilles tendons typically take weeks before they are strong enough to handle physiological loads. Gene therapy is a promising treatment for Achilles tendon defects. The aim of the present study was to evaluate the histological/biomechanical effects of Transforming growth factor-beta 1 (TGF-beta 1) and vascular endothelial growth factor 165 (VEGF(165)) gene transfer on Achilles tendon healing in rabbits. Bone Marrow-Derived Mesenchymal Stem Cells (BMSCs) were transduced with adenovirus carrying human TGF-beta 1 cDNA (Ad-TGF-beta 1), human VEGF(165) cDNA (Ad-VEGF(165)), or both (PIRES-TGF-beta 1/VEGF(165)) Viruses, no cDNA (Ad-GFP), and the BMSCs without gene transfer and the intact tendon were used as control. BMSCs were surgically implanted into the experimentally injured Achilles tendons. TGF-beta 1 distribution, cellularity, nuclear aspect ratio, nuclear orientation angle, vascular number, collagen synthesis, and biomechanical features were measured at 1, 2, 4, and 8 weeks after surgery. The TGF-beta 1 and TGF-beta 1/VEGF(165) co-expression groups exhibited improved parameters compared with other groups, while the VEGF(165) expression group had a negative impact. In the co-expression group, the angiogenesis effects of VEGF(165) were diminished by TGF-beta 1, while the collagen synthesis effects of TGF-beta 1 were unaltered by VEGF(165). Thus treatment with TGF-beta 1 cDNA-transduced BMSCs grafts is a promising therapy for acceleration and improvement of tendon healing, leading to quicker recovery and improved biomechanical properties of Achilles tendons. Abstract:PURPOSE Delivery of growth factors that may substantially increase the healing rate of injured digital flexor tendons is a new arena of application of gene therapy. So far adenoviral, adeno- associated viral (AAV), and liposome-mediated plasmid vectors have been used to deliver growth factor genes to tendons or tenocytes, but tissue reactions of these vectors in tendons, in particular those in contrast of healing responses of the injured tendons, were unknown. The purpose of this study was to compare tissue reactions of these vectors in tendons and those of healing responses of injured flexor tendons. METHODS Flexor digitorum profundus tendons within sheath area of paws of 4 New Zealand white rabbits were used as a model. Adenoviral vector (Ad5-lacZ), 1×1010 viral particles, was injected into each of 9 tendons, and AAV2-luciferase, 1×1010 viral particles, was injected into each of 9 tendons. pCMV-b mixed with 3.5 μL of lipofectamine was injected into each tendon. In other 9 tendons, the tendons were cut through volar 2/3 transversely and repaired. At 3, 7, and 14 days, the tendons were harvested and stained with hematoxylin and eosin. Normal tendons were harvested, serving as control. RESULTS Tissue reactions of the liposome- plasmid vectors in tendons were the most prominent among 3 vectors tested. The adenoviral and AAV2 vectors showed similar tissue reactions at the same viral titers tested, but AAV2 had less obvious tissue reactions in the endotenon area than adenoviral vectors. Early stage inflammatory changes were remarkable in the injured tendons. Compared with these tissue reactions, the reactions elicited by the vectors were less severe. CONCLUSIONS The tested 3 gene delivery systems had less severe tissue reactions in flexor tendons compared with inflammatory changes in injured flexor tendons in early stage. Among 3 vector systems tested, adenoviral and AAV vectors elicited less tissue reactions than liposome-plasmid vectors. AAV2 causes less severe reactions in the endotenon area than adenoviral vectors. From the perspective of tissue reactions, adenoviral and AAV2 vectors, in particular AAV2, are advantageous gene delivery systems for future investigation of delivery of genes to promote flexor tendon in vivo.

Abstract:BACKGROUND:Exogenous basic fibroblast growth factor (bFGF) plays an important role in the ligament tissue healing process, and the use of transgenic methods to transfect exogenous genes into cells can promote the secretion of bFGF. OBJECTIVE:To observe phenotypic changes and the bFGF protein expression after bFGF recombinant adenovirus was used to transfect rabbit bone marrow mesenchymal stem cells (BMSCs). METHODS:Passage 2 BMSCs were divided into three groups:Ad.bFGF-eGFP group, Ad.eGFP group and control group. Under a phase contrast microscope we observed the changes in cellmorphology. The expression of bFGF protein in BMSCs was determined by enzyme-linked immunosorbent assay (ELISA). The proliferative curve was detected by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT). RESULTS AND CONCLUSION:The transfected cells showed a uniform phenotype of fibroblasts. MTT colorimetric assay revealed that more proliferative activity of transfected BMSCs was shown in the Ad.bFGF-eGFP group than in the Ad.eGFP group and control group. ELISA results showed that expression of bFGF protein was higher in the Ad.bFGF-eGFP group than in the Ad.eGFP group and control group (P<0.05). BFGF recombinant adenovirus can induce the differentiation of BMSCs into fibroblasts, increase proliferative ability and promote the expression of bFGF protein.

Methods: Forty-two flexor digitorum profundus tendons of 6 New Zealand white rabbits were used. Eighteen tendons were divided into 3 groups of 6 each and injected with different vectors: adenoviral vector, AAV2-luciferase vector, or pCMV-beta vector with liposome. Another 12 tendons were cut and repaired. At 3, 7, and 14 days, the tendons were harvested and stained with hematoxylin and eosin. Normal flexor tendons were harvested as controls. Results: The tissue reactions of the liposome-plasmid vector in tendons were the most prominent among the 3 vectors tested. The adenoviral vector elicited a moderate degree of tissue reaction. The AAV2 vector caused remarkable reactions in epitenon but almost no reactions in endotenon. Early-stage tissue reactions were more robust in the injured tendons. Compared with early-stage inflammatory and healing responses, the reactions elicited by these vectors were less severe. Conclusions: The 3 gene delivery systems tested elicit less severe tissue reactions in flexor tendons compared with early-stage inflammatory changes in injured tendons. Adenoviral and AAV vectors elicit less severe tissue reactions than liposome-plasmid vectors. The AAV2 vector appears to cause almost no reaction in endotenon. In terms of tissue reactions, the adenoviral and AAV2 vectors, in particular AAV2, are suitable gene delivery systems for future gene transfer to the tendon in vivo.

Abstract:Advances in allograft processing have opened new horizons for clinical adaptation of flexor tendon allografts as delivery scaffolds for antifibrotic therapeutics. Recombinant adeno-associated-virus (rAAV) gene delivery of the growth and differentiation factor 5 (GDF-5) has been previously associated with antifibrotic effects in a mouse model of flexor tendoplasty. In this study, we compared the effects of loading freeze-dried allografts with different doses of GDF-5 protein or rAAV- Gdf5 on flexor tendon healing and adhesions. We first optimized the protein and viral loading parameters using reverse transcription polymerase chain reaction (RT-PCR), enzyme-linked immunosorbent assay (ELISA), and in vivo bioluminescent imaging. We then reconstructed flexor digitorum longus (FDL) tendons of the mouse hindlimb with allografts loaded with low and high doses of recombinant GDF-5 protein and rAAV- Gdf5 and evaluated joint flexion and biomechanical properties of the reconstructed tendon. In vitro optimization studies determined that both the loading time and concentration of the growth factor and viral vector had dose-dependent effects on their retention on the freeze-dried allograft. In vivo data suggest that protein and gene delivery of GDF-5 had equivalent effects on improving joint flexion function, in the range of doses used. Within the doses tested, the lower doses of GDF-5 had more potent effects on suppressing adhesions without adversely affecting the strength of the repair. These findings indicate equivalent antifibrotic effects of Gdf5 gene and protein delivery, but suggest that localized delivery of this potent factor should also carefully consider the dosage used to eliminate untoward effects, regardless of the delivery mode.

Abstract:Collagen content and cross-linking are believed to be major determinants of tendon structural integrity and function. The current study aimed to investigate the effects of transforming growth factor (TGF)-β1 on the collagen content and cross-linking of Achilles tendons, and on the histological and biomechanical changes occurring during Achilles tendon healing in rabbits. Bone marrow-derived mesenchymal stem cells (BMSCs) transfected with the TGF-β1 gene were surgically implanted into experimentally injured Achilles tendons. Collagen proteins were identified by immunohistochemical staining and fiber bundle accumulation was revealed by Sirius red staining. Achilles tendons treated with TGF-β1-transfected BMSCs showed higher concentrations of collagen I protein, more rapid matrix remodeling, and larger fiber bundles. Thus TGF-β1 can promote mechanical strength in healing Achilles tendons by regulating collagen synthesis, cross-link formation, and matrix remodeling.
EFFECT OF LOCAL BASIC FIBROBLAST GROWTH FACTOR AND 5-FLUOROURACIL ON ACCELERATING HEALING AND PREVENTING TENDON ADHESION AFTER FLEXOR TENDON REPAIR

Abstract:OBJECTIVE:To assess the effect of basic fibroblast growth factor (bFGF) and 5-fluorouracil (5-FU) applied topically on the tendon adhesion and the healing process after the flexor tendon repair in Leghorn chickens. METHODS:Ninety male Leghorn chickens (weighing 3.0-3.5 kg) were randomly divided into 3 groups, with 30 chickens in each group. The flexor digitorum profundus tendons of the third right toes were transected and sutured directly. The repair site in group A was given 0.6 microL fibrin sealant (FS). In group B, the repair site was given 0.6 microL FS containing 500 ng bFGF. In group C, before the tendons were transected, they had been soaked in 5-FU solution, and then the same treatment as group B was given. Six specimens of the third toe were harvested to perform the macroscopical and histological examinations at 1, 2, 4, and 8 weeks, respectively, and to perform the biomechanical test at 8 weeks. RESULTS:All animals survived until the experiment was completed. All incisions healed smoothly. No rupture occurred in the repaired tendon. At 8 weeks, the adhesion degree was lighter in group C than in group B (P < 0.05), but there was no significant difference in the adhesion degree between group A and groups B, C (P > 0.05). At 1, 2, and 4 weeks after operation, the number of fibroblast cells of group A was significantly less than that of group B (P < 0.05), and the number of fibroblast cells of group C was significantly less than that of group A and group B in the tendon sheath and epitenon (P < 0.05); however, it was significantly more than that of group A in the tendon parenchyma (P < 0.05), and no significant difference was observed when compared with that of group B (P > 0.05). At 8 weeks, no difference was found among 3 groups (P > 0.05). The collagen fiber content of group A was significantly less than that of group B at 4 and 8 weeks (P < 0.05). In the sheath and epitenon, the collagen fiber content of group A was significantly more than that of group C at 4 weeks (P < 0.05); however, no significant difference was found between 2 groups at 8 weeks (P > 0.05). The collagen fiber content of group A was significantly less than that of group C in the parenchyma at 4 and 8 weeks (P < 0.05). At all time points, the collagen fiber content of group B was significantly more than that of group C in the sheath and epitenon (P < 0.05), but no significant difference in the parenchyma was observed between 2 groups (P > 0.05). The biomechanical tests showed that the gliding excursion of the tendon in groups A, B, and C was (3.51 +/- 0.56), (2.84 +/- 0.42), and (4.56 +/- 0.59) mm, respectively; the work of flexion was (14.08 +/- 1.85), (20.62 +/- 3.52), and (10.91 +/- 1.53) N x mm, respectively; and the ultimate tensile strength of the tendon was (11.26 +/- 1.83), (15.02 +/-2.20), and (14.40 +/- 1.57) N, respectively. There were significant differences in the gliding excursion of the tendon and the work of flexion among 3 groups (P < 0.05) and in the ultimate tensile strength of the tendon between group A and groups B, C (P < 0.05), but there was no significant difference in the ultimate tensile strength of the tendon between group B and group C (P > 0.05). CONCLUSION:Local single-use bFGF and 5-FU can not only effectively promote the healing of flexor tendon, but also significantly reduce tendon adhesion.
The Functions and Mechanisms of Basic Fibroblast Growth Factor in Tendon Repair

Abstract:Tendon injury is a disorder of the musculoskeletal system caused by overuse or trauma, which is characterized by pain and limitations in joint function. Since tendon healing is slowly and various treatments are generally ineffective, it remains a clinically challenging problem. Recent evidences suggest that basic fibroblast growth factor (bFGF) not only plays an important role in tendon healing, but also shows a positive effect in laboratory experimentations. The purpose of this review is to summarize the effects of bFGF in the tendon healing. Firstly, during the inflammatory phase, bFGF stimulates the proliferation and differentiation of vascular endothelial cells to foster neovascularization. Furthermore, bFGF enhances the production of pro-inflammatory factors during the early phase of tendon healing, thereby accelerating the inflammatory response. Secondly, the cell proliferation phase is accompanied by the synthesis of a large number of extracellular matrix components. bFGF speeds up tendon healing by stimulating fibroblasts to secrete type III collagen. Lastly, the remodeling phase is characterized by the transition from type III collagen to type I collagen, which can be promoted by bFGF. However, excessive injection of bFGF can cause tendon adhesions as well as scar tissue formation. In future studies, we need to explore further applications of bFGF in the tendon healing process.

Effectiveness of Microrna in Down-Regulation of Tgf-Beta Gene Expression in Digital Flexor Tendons of Chickens: in Vitro and in Vivo Study

Abstract:Purpose Transforming growth factor (TGF)-beta is considered to be responsible for the formation of scars such as adhesions around healing digital flexor tendons. We proposed to deliver microRNAs (miRNAs) to silence expression of the TGF-beta 1 gene and to investigate the effectiveness of miRNAs in down-regulation of the TGF-beta 1 gene in vitro and in vivo.Methods We designed and engineered 4 miRNAs according to genetic sequences of chicken TGF-beta 1. Four plasmid vectors harboring the respective engineered miRNAs and I control vector were constructed. We transfected 30 wells of cultured tenocytes with these vectors and harvested them 48 hours later The gene expression levels were quantified using real-time polymerase chain reactions. Subsequently, the miRNA that most effectively silenced TGF-beta gene in vitro was tested on 25 chickens in vivo The miRNA and control vectors were injected into the injured tendons. respectively. At 1 and 6 weeks after surgery. the tendons were analyzed for gene expression and protein productionResults In both in vitro and in vivo settings, delivery of miRNA to the tendon substantially down-regulated expression of the TGF-beta gene but did not affect expression of the collagen I gene. In the healing tendon. TGF-beta gene expression was significantly down-regulated by 50% to 60% at 1 and 6 weeks. At 6 weeks. the collagen III gene expression was significantly down-regulated by 55% at 6 weeks and the connective tissue growth factor gene was significantly down-regulated by 25%. At 6 weeks, TGF-beta protein was substantially decreased.Conclusions MicroRNA significantly down-regulates expression of the TGF-beta in vitro and in vivo. Application of miRNA did not down-regulate expression of the collagen I. but downregulated the collagen III gene. Abstract:Objective: To investigate bFGF expression and the relationship with the tendon healing process after insheathed chicken flexor digitorum tendon injury. Method: The right 3rd flexor digitorum longus tendons of thirty six Leghorn chickens were transected, resutured, and covered with the sheath. Specimens were obtained postoperatively at various time points(n=6, at each time point). The control tendons were harvested without operation(n=6). Histological observation was carried out. BFGF expressions in tendon were detected by RT-PCR and immunohistochemical methods. Result: After tendon injury, inflammatory cells infiltrated at the suture line at the begining, then fibroblasts congregated and secreted collagen, the collagen fibers were reshaped at last. These cell activities in parenchyma appeared later and were weaker than those in sheath and epitenon. BFGF expression was very low in tendons of control group, but high in those of experimental groups at different time points. The bFGF expressions in sheath and epitenon were higher than in parenchyma. Conclusion: BFGF plays a critical role in the healing process of chicken flexor digitorum tendon following injury.
Effect of basic fibroblast growth factor gene transfection with lentiviral vector on biochemistry of meniscal fibrochondrocytes

Abstract:Aim: Basic fibroblast growth factor (bFGF) is a kind of repair initial factor that is widely used. It could effectively promote the proliferation of various cells and matrix synthesis. In this study, the probability of gene therapy for meniscal injuries with lentiviral vector was explored. Methods: The experiment was conducted in the laboratory of Shanghai Microvascular Surgery of Extremities from May 2004 to November 2005. 1 The meniscal fibrochondrocytes were isolated from the 3 New Zealand rabbits (3 months old). At 60% confluence, the cultured first-generation meniscal fibrochondrocytes (ViraPower), which was named as experimental group, were mixed with the recombinant lentivirus carrying bFGF gene. At the same time, the fibrochondrocytes mixed with the lentivirus carrying no gene served as control group. The fibrochondrocytes in vacant group were cultured without any intervention. 2 After 48 hours transfection, the expression of bFGF was detected by ELISA; cell cycle, synthesis of collagen and pell proliferation at 2, 4, 6 and 8 days were detected by laser flow cytometry, 3H-pyrrolidinecarboxylic acid, and MTT methods, respectively Results: 1 After 48 hours co-culture, the expression of bFGF was only found in experimental group. Six days after co-culture, the cell proliferation (absorbance) was more obvious in experimental group than in control group and vacant group (P < 0.01). 2 The cell cycles of G1 phase, S period, prophase, and dividing phase were shorter in experimental group than that in control group and vacant group (P < 0.05). 3 The synthesis of collagen was increased in experimental group compared than that in control group and vacant group. Conclusion: The gene therapy with the reconstructed lentiviral vectors could transfer bFGF gene into meniscal fibrochondrocytes effectively, and enhance cell proliferation and extracellular matrix synthesis, which could be used as a new way to treat meniscal injury.
Angiopoiesis and Bone Regeneration Via Co-Expression of the Hvegf and Hbmp Genes from an Adeno-Associated Viral Vector in Vitro and in Vivo.

Abstract:AIM:To investigate the therapeutic potential of adeno-associated virus (AAV)-mediated expression of vascular endothelial growth factor (VEGF) and bone morphogenetic protein (BMP).METHODS:Four experimental groups were administered the following AAV vector constructs: rAAV-hVEGF(165)-internal ribosome entry site (IRES)-hBMP-7 (AAV-VEGF/BMP), rAAV-hVEGF(165)-GFP (AAV-VEGF), rAAV-hBMP-7-GFP (AAV-BMP), and rAAV-IRES-GFP (AAV-GFP). VEGF(165) and BMP-7 gene expression was detected using RT-PCR. The VEGF(165) and BMP-7 protein expression was determined by Western blotting and ELISA. The rabbit ischemic hind limb model was adopted and rAAV was administered intramuscularly into the ischemic limb.RESULTS:Rabbit bone marrow-derived mesenchymal stem cells (BMSCs) were cultured and infected with the four viral vectors. The expression of GFP increased from the 7th day of infection and could be detected on the 28th day post-infection. In the AAV-VEGF/BMP group, the levels of VEGF165 and BMP-7 increased with prolonged infection time. The VEGF(165) and BMP-7 secreted from BMSCs in the AAV-VEGF/BMP group enhanced HUVEC tube formation and resulted in a stronger osteogenic ability, respectively. In rabbit ischemic hind limb model, GFP expression increased from the 4th week and could be detected at 8 weeks post-injection. The rAAV vector had superior gene expressing activity. Eight weeks after gene transfer, the mean blood flow was significantly higher in the AAV-VEGF/BMP group. Orthotopic ossification was radiographically evident, and capillary growth and calcium deposits were obvious in this group.CONCLUSION:AAV-mediated VEGF and BMP gene transfer stimulates angiogenesis and bone regeneration and may be a new therapeutic technique for the treatment of avascular necrosis of the femoral head (ANFH).
[Effect of Platelet-Derived Growth Factor-Bb on the Healing and Adhesion of Rat Tendon].

Abstract:OBJECTIVE:To study the effect of platelet-derived growth factor-BB (PDGF-BB) gene transfected rat tendon cells on the healing and adhesion of rat tendon.METHODS:A model of heel tendon injury was reproduced in 90 rats. They were randomly divided into three groups: experiment group [with injection of 20 µL rat tendon cells (1 × 10(8) cell/mL) transfected with PDGF-BB gene into the injured tendon ends], control group [with injection of 20 µL non-transfected rat tendon cells (1 × 10(8) cell/mL) into the injured tendon ends], and blank control group (without treatment), with 30 rats in each group. Heel tendon ends were sutured with 6-0 thread by modified Kessler method and immobilized with tube-type plaster of Paris cast for one week. Rat tendon cells transfected with PDGF-BB gene were identified with gene sequencing and RT-PCR. Tendon tissue sample was harvested 3 days or 1, 2, 4, 8 week(s) after operation (POD or POW) for morphology and histology observation, and bio-mechanical test. The degree of tendon adhesion, the number of Fb and collagen fiber content in tissue, maximum tensile strength and sliding distance of tendon, and concentration of PDGF-BB in tendon tissue among groups were compared. Data were processed with t test.RESULTS:(1) PDGF-BB mRNA expressed stably in PDGF-BB gene transfected tendon cells as testified by RT-PCR and gene sequencing. (2) Obvious edema and inflammatory cells infiltration were observed in each group on POD 3, but they were less pronounced in experiment group. And the changes in all groups were ameliorated gradually. The difference in grading of tendon adhesion was not obvious among groups in POW 4 and 8. (3) Fb number in experiment group in POW 2, 4, 8 was respectively fewer than that of control group and blank control group (with t value respectively 2.94, 4.26, 5.76 and 4.00, 3.83, 6.12, P < 0.05 or P < 0.01). (4) Collagen fiber content in rat tendon of experimental group in POW 4 was (43 ± 6)%, which was significantly lower as compared with that of control group [(55 ± 8)%] and blank control group [(61 ± 8)%] (with t value respectively 2.94 and 4.41, P < 0.05 or P < 0.01). (5) The largest sliding distance of tendon in experiment group in POW 4 and 8 were (3.25 ± 0.33) and (3.65 ± 0.21) mm, which were significantly longer than those in control group [(2.29 ± 0.40), (2.21 ± 0.37) mm] and blank control group [(2.01 ± 0.23), (1.89 ± 0.24) mm] (with t value respectively 4.53, 8.29 and 7.55, 13.52, P values all below 0.01). There was no statistical significant difference among the three groups in the maximum tensile strength of tendon (with t value respectively 0.41, 0.41, 0.77, 0.72, P values all above 0.05). (6) Content of PDGF-BB in tendon tissue of experimental group on POD 3 and in POW 2, 4 were (12.95 ± 1.36), (8.32 ± 0.94), (9.10 ± 1.06) ng/mL, all significantly higher than those in control group [(1.13 ± 0.21), (2.07 ± 0.48), (3.85 ± 0.39) ng/mL] (with t value respectively 21.04, 14.50, 11.39, P values all below 0.01).CONCLUSIONS:PDGF-BB gene transfected rat tendon cells can promote endogenous healing of tendon and prevent tendon adhesion.
Tendon Healing in Vivo: Gene Expression and Production of Multiple Growth Factors in Early Tendon Healing Period

Abstract:Purpose The actions of growth factors during healing of injured flexor tendons are not well characterized, although information pertinent to some individual growth factors is available. We Studied gene expression and protein production of a number of growth factors at several time points during the early healing period in a chicken model.Methods Seventy-four long toes of 37 white Leghorn chickens were used. The flexor digitorum profundus tendons of 60 toes were surgically repaired after complete transection and were harvested for analysis 3, 5 7, 9, 14, and 21 days after surgery. The expression of 6 growth factors was studied at 4 time points after surgery with real-time quantitative polymerase chain reactions, and production and distribution of 3 growth factors at all 6 time points were studied by immunohistochemical staining with antibodies. Fourteen tendons that had no surgery served as day 0 controls. Tendon healing, status was also assessed histologically.Results Throughout the early tendon healing period, connective tissue growth factor (CTGF) C and transforming growth factor beta (TGF-beta) showed high levels of gene expression. Levels of gene expression of vascular endothelial growth factor (VEGF) and insulin-like growth factor 1 (IGF-1) were high or moderately high. Expression of the TGF-beta gene was upregulated after injury, whereas the basic fibroblast growth factor (bFGF) gene was downregulated at all postsurgical time points and expressed at tile lowest levels among 6 growth factor genes 2 to 3 weeks after surgery. The platelet-derived growth factor B (PDGF-B) gene was also minimally expressed. Findings of immunohistochemistry corresponded to TGF-beta, bFGF, C, and IGF-1 gene expression.Conclusions In this model. up to 3 weeks after surgery, gene expression and production of TGF-beta are high and are Upregulated in this healing period. However. expression of the bFGF gene and protein is low and decreases in the healing tendon. The CTGF, VEGF, and IGF-1 genes are expressed at high or moderately high levels, but PDGF-B is minimally expressed.
PDGF-loaded microneedles promote tendon healing through p38/cyclin D1 pathway mediated angiogenesis

Abstract:Tendon injury is one of the most serious orthopedic diseases often leading to disability of patients. Major shortages of tendon healing are due to its multiple comorbidities, uncertainty of therapeutic efficacy and insufficient of angiogenesis. With a deeper understanding of angiogenic mechanism of tendon healing, we investigated an innovative microneedle patch loaded with platelet derived growth factor (PDGF) to achieve a constant systemic administration of PDGF to enhance topical tendon healing. Rat achilles tendon injury model was performed as in vivo animal models. Histological staining showed an enhancement of tendon healing quality, especially angiogenesis. Biomechanical studies demonstrated an increase of tendon stiffness, maximum load and maximum stress with treatment of PDGF-loaded microneedles. Furthermore, MAPK/p38/Cyclin D1 pathway and angiogenesis were found to play an important role in tendon healing process by using a biological high throughput RNA-sequence method and bioinformatic analysis. The high throughput RNA-seq tendon healing results were confirmed by histochemical staining and western blot. These results suggest the novel therapeutic potential of PDGF-loaded microneedle patch in tendon surgery.

Abstract:Graft remodeling following anterior cruciate ligament (ACL) reconstruction requires a long period of recovery before it is capable of withstanding physiological loads. Graft revascularization is extremely important in the remodeling process. In ACL reconstruction, the local administration of vascular endothelial growth factor (VEGF) significantly increased revascularization of the graft, but did not significantly affect the mechanical properties of the graft after implantation (Ju et al., 2006; Yoshikawa, et al., 2006). Our previous studies showed that transforming growth factor-β1 (TGFβ1) could promote improvements in mechanical strength in Achilles tendon regeneration, by regulating collagen type I and type III synthesis, cross-link formation, and matrix-remodeling (Hou et al., 2009). The current study aims to investigate whether the co-expression of TGFβ1/VEGF(165) could beneficially affect the remodeling of ACL grafts. Bone marrow-derived mesenchymal stem cells (BMSCs), transfected with an adenoviral vector encoding TGFβ1, VEGF(165) or TGFβ1/VEGF(165), were surgically implanted into experimental ACL grafts, with non-transfected cells as a control. HE and toluidine blue staining, vascular number, and biomechanical features were analyzed at 3, 6, 12, and 24 weeks after surgery. The results suggest that TGFβ1 expression, in the TGFβ1/VEGF(165)-transfected BMSCs, could accelerate the remodeling of the reconstructed ligament. The cross-talk between TGFβ1 and VEGF(165) has positive consequences, as TGFβ1/VEGF(165)-transfected BMSCs significantly promoted angiogenesis of the reconstructed ligament at 3, 6, 12 weeks, with the best mechanical properties being achieved at 24 weeks. Furthermore, co-expression of these genes is more powerful and efficient than single gene therapy.
TAT-Mediated Acidic Fibroblast Growth Factor Delivery to the Dermis Improves Wound Healing of Deep Skin Tissue in Rat

Abstract:BACKGROUND: The definition of deep tissue injury was derived from multiple clinical cases as "A purple or maroon localized area of discolored intact skin or blood-filled blister due to damage of underlying soft tissue from pressure and/or shear". Acidic fibroblast growth factor (aFGF) significantly improves wound healing under diabetic conditions. However, to date, the therapeutic application of aFGF has been limited, due to its low delivery efficiency and short half-life.METHODOLOGY/PRINCIPAL FINDINGS: Using an animal model of magnet-induced pressure ulcers, transactivator of transcription protein (TAT)-aFGF was evaluated for transdermal delivery and wound healing. Immunohistochemistry and Western blotting were also performed to determine the expression of transforming growth factor (TGF)-β1, α-smooth muscle actin (α-SMA), CD68, proliferating cell nuclear antigen (PCNA) and TGF-β-receptor II (TGF- βRII) in cultured human dermal fibroblasts. We found that that mice treated with TAT-aFGF had higher accumulation of aFGF in both dermis and subcutaneous tissues compared with mice treated with aFGF alone. In the remodeling phase, TAT-aFGF treatment decreased the expression of α-SMA to normal levels, thereby facilitating normal wound healing processes and abrogating hypertrophic scarring. In human dermal fibroblasts, TAT-aFGF reversed the suppressive effect of TNF-α on α-SMA expression and restored TGF-βRII and TGF-β1 expression.CONCLUSIONS/SIGNIFICANCE: Our results demonstrate that TAT-aFGF has a favorable therapeutic effect on the healing of subcutaneous deep tissue injury.
