Study on the risk of non-alcoholic fatty liver disease caused by high hemoglobin level:the impact of gender

Abstract:Objective To explore the relationship between hemoglobin level and the risk of non-alcoholic fatty liver disease(NAFLD)based on gender.Methods A cross-sectional study of on 61,646 subjects was conducted,which was more than 18 years old and underwent a routine health check-up in the Health Care Center,Renji Hospital,Shanghai Jiao Tong University School of Medicine between 1st January 2012 and 31st December 2012.The incidence of NAFLD was evaluated according to gender and level of hemoglobin.Results Of 61 646 subjects,the mean age was 44 years,and 34 961(56.7%) were men.,and 20 401(33.09%) were diagnosed with NAFLD by ultrasound.The prevalence of NAFLD increased steadily as hemoglobin level increased in both men and women(P<0.001for both).The prevalence of NAFLD was higher in men than that in women(44.7% vs 17.9%,χ2=4 900,P<0.001).The multivariate logistic forward regression showed that hemoglobin level had a positive correlation with the prevalence of NAFLD(For men,OR=1.31, 95%CI=1.18-1.45 in the second quartile;OR=1.38,95%CI=1.24-1.53 in the third quartile;OR=1.29,95%CI= 1.05-1.58 in the fourth quartile vs.control quartile;P<0.05,respectively.For women,OR=1.26,95%CI=1.09- 1.45 in the third quartile;OR=1.59,95%CI=1.06-2.38in the fourth quartile vs.the lowestcontrol quartile;P<0.05 respectively;Pvalue was more than 0.05 in the second quartile vs control quartile).Men had a greater risk of the prevalence of NAFLD(OR=3.7,95%CI=3.57-3.85,P<0.001) than that in women.Conclusion Hemoglobin level has a positive correlationis with the prevalence of NALFD.In addition to obesity and metabolic syndrome,high hemoglobin level is one of independent risk factors for NAFLD.The prevalence of NAFLD in men is 3.7 times higher than that in women.
