A Preliminary Application of the Imaging-Based Splint for Orthognathic Surgery

Abstract:PURPOSE:The purpose of this study was to introduce a stereolythographic method for surgical wafer fabrication.METHODS:Fifteen consecutive patients with craniofacial deformities seeking for orthognathic surgery were included in this study.All patients had previously undergone 3-dimensional computed tomography(CT) scannnings(0.625mm slice thickness) of the craniofacial skeleton.The data from CT scan with DICOM format were input into an interactive Simplant CMF software(Materialise Medical,Leuven,Belgium).Preoperative planning including segmentation,osteotomies and movements of the jaw bones was simulated using the Simplant CMF.The surgical wafers were fabricated by laser sintering based on the preoperative planning data.The model surgery for each patient was performed at the same time and the traditional surgical wafers were made.Two kinds of wafers were brought to the operating room during the surgery to compare the two methods of preoperative planning and fabrication of the wafers.RESULTS:Four one-jaw surgeries(bilateral ramus sagittal split osteotomy) and 11 two-jaw surgeries(Le Fort I osteotomy and bilateral ramus sagittal split osteotomy) were performed.The movement and fixation of bone segments were performed using the stereolythographic wafers firstly.Then the new position of bone segments were checked by the wafers made from the traditional model surgery.All the stereolythographic wafers worked well except for two cases with double-jaw surgery and one case with mandibular surgery.CONCLUSIONS:Within the limits of this study,the Simplant CMF software can provide the preoperative planning and fabrication of surgical wafer for standard Le Fort I osteotomy and bilateral ramus sagittal split osteotomy.But the application for the more complex surgery needs to be further studied.
