Long-term follow-up study on deep brain stimulation in a patient with posttraumatic dystonia and Literature review.

Abstract:Objective To examine the long-term efficacy of deep brain stimulation on subthalamic nucleus(STN) in the treatment of a patient with posttraumatic segmental dystonia.Methods The patient had suffered a severe closed head injury and was comatose for about twenty days at 17 years of age.Three years later,the patient developed segmental dystonia.This patient performed bilateral subthalamic nucleus deep brain stimulation at the age of 21 years.The BFMDRS score,movement score,disability score were 83,56 and 27,respectively.Follow up was started to evaluate the efficacy one month after operation.Results The dystonic symptoms began to be relieved one-week following stimulation and were significantly improved after three-month stimulation.The improvements of BFMDRS,movement scale and disability scale were 49%,50%,48% respectively on the third month.Chronic deep brain stimulation resulted in progressive improvement of dystonic symptoms.During 3.5 year follow-up,the conditions were stable without recurrence of dystonic symptoms.BFMDRS,movement score and disability score were 8,8 and 0,respectively.No adverse effects of stimulation were observed at the optimal setting.Medication was completely stopped after stimulation.Conclusions The present study suggests that deep brain stimulation on STN may be an effective treatment for posttraumatic segmental dystonia and may provide a new avenue of treatments for secondary dystonia.
