Abstract:We analyzed the health expenditure data from 1978-2007 using ARFIMA models and predicted health expenditure as percentage of GDP in 2010.The result showed health expenditure as percentage of GDP in 2010 would be 4.73% and would still be lower than 5%,the WHO lowest standard.

Abstract:BACKGROUND: China's health financing system has changed from the government-led mode under the planned economy to the diversified mode under the market economy. Equity in health financing has been a national health priority. This study aimed to predict changes in total health expenditure (THE), government health expenditure (GHE), social health expenditure (SHE) and out-of-pocket health expenditure (OOP) in China from 2018 to 2022, and to provide a theoretical basis for health policy adjustment.METHODS: Based on health expenditure date of time series from 1978-2017, R3.5.1 software was used to construct the Autoregressive Integrated Moving Average (ARIMA) model.RESULTS: The model of THE, GHE, SHE and OOP are ARIMA (3.3.0), ARIMA (1.3.1), ARIMA (2.4.0), ARIMA (2.2.2). According to the simulation results, in 2022, China's THE is expected to reach 8473.00 billion Yuan, and the constituent ratios in GHE, SHE and OOP will be 25.49%, 51.25% and 23.26%, respectively. The proportion of THE to GDP will continuously increase from 2018-2022 at a reasonable pace, while THE itself will increase rapidly.CONCLUSIONS: China should take effective measures to control the excessive growth of THE, keep decreasing the OOP percentage, and improve the efficiency and fairness of the use of health funds.

Abstract:Objective: To estimate the total expenditure of CT and MRI examination services and their proportions in the national total health expenditure in 2005 and 2009 and provide basis for the CT,MRI allocation and expenditure control.Methods: By sampling the secondary and tertiary hospitals in Shanghai,Shaanxi,Hunan and Zhejiang,the data of CT and MRI examination services of these areas were obtained in 2005 and 2009.Epidata 3.1 was used for data entry and Excel 2007 was used for statistical analysis.Results: The total expenditure of CT examination services in 2005 and 2009 was 12.34 billion and 24.90 billion and their proportions in the national total health expenditure were 1.43% and 1.45%.The total expenditure was 4.54 billion and 9.61 billion for MRI examination,with the proportions of 0.52% and 0.56% respectively.Conclusion: The total expenditure of CT and MRI examination services increased relatively fast from 2005 to 2009,but their proportions in the national total health expenditure almost remained the same,so it can not be concluded that the growth of total expenditure of CT and MRI examination services increased the financial burden of patients.
Analysis of Growth Pattern of Medical Care Expenditure in China Based on EMD

Abstract:Objective:Using the EMD analysis method to analyze the retail price growth trend of health care costs in our country,and provide a theoretical basis for the growth of medical expenses.Methods:TCM and western medicines and health care products commodity retail price index data (2011.4-2016.7) were extracted from statistical yearbook of Chinese,descriptive analysis and EMD methods were used to explore the data.Results:We successfully extracted five IMFs and one RES from the original time series.The fluctuation cycles and variance contribution rates for each IMF and RES are IMF1 (3.41 months,22.25％),IMF2 (5.31 months,13.61％),IMF3(10.6 month,10.22％),IMF4(21.3 months,11.03％),IMF5 (32.4 months,5.68％),RES (37.12％) respectively.The contribution of high frequency component is 29.67％,the contribution of low frequency component is 36.47％,and the natural growth rate is 33.86％.Conclusion:The health care costs rose at a high speed for a long time in China.The price growth volatility has obvious periodicity.The impact of stochastic events,major policies of the state and the natural growth's contribution for prices growth is closer,but major policies of the state is most important.

Abstract:Objective To analysis the fee of laboratory examination service for patients in order to provide some data for implement scientific management and control of laboratory examination expenses. Method The auto regression time series model was established and evaluated based on the average expenses of laboratory examination from Jan, 1994 to Jun, 2006 in a large-scale hospital of Beijing. Result The average expenses of laboratory examination fluctuated along the trend risen from Jan, 1994 to Jun, 2006 and increased about 3.2 times since Jan, 1994. It may continually rise slowly in the next half year of 2006 according to this model. The errors of prediction on the average expenses of laboratory examination from Jan to Jun of 2006 are around 1.03 %～3.44% by means of this model. Conclusion It is practical to apply the approach of time series model predict the average expense of the laboratory examination and it will provide some instructively information for the scientific control of medical cost and medical system reform.

Abstract:Background In the context of rapid economic and social development, there has been a continuous intensification of population aging, transformation of disease patterns, and wide application of new medical technologies. As a result, health expenditures in various countries have sharply soared. How to utilize limited medical resources to maximize the improvement of health levels has become a hot and challenging issue related to the well-being of all humanity. The relevant indicators of total health expenditure play a crucial role in monitoring and evaluating the fairness of health financing and health security in the region. Objective This study explores the changes in the main expenses that constitute China’s total health expenditure and uses indicators related to health expenditure to observe the changes and future development trends of China’s health expenditure. Based on this, the utilization of China’s health expenditure is monitored to identify possible problems, and thereby targeted suggestions for promoting the development of China’s health and wellness cause are put forward. Methods Based on the comparison of previous literature, this paper analyzes the changes and future development trends in China’s health expenditure by using the relevant indicators of China’s health expenditure through the structural variation analysis method and the gray prediction model. Results The results show that the scale of government, social, and out-of-pocket health expenditures has continuously expanded, with social health expenditures becoming the main funding source for total health expenditures. The burden of medical expenditures on individuals has been further reduced. In the institutional method of total health expenditures, hospital expenditures account for about 60% of the total and are the main component. The expenditures of health administration and medical insurance management institutions are the main driving force behind the growth of total health expenditures. However, the proportion of health expenditures in China’s GDP is relatively low, so more investment is needed in the healthcare sector, and the burden of individual medical expenses also needs to be continuously reduced. Discussion In the future, China should further increase its investment in the medical and health sector. Specifically, the government should persist in investing in fundamental medical and health services. Simultaneously, efforts should be made to establish a scientific cost control mechanism for pharmaceuticals and broaden financing channels for healthcare, such as accelerating the development of commercial health insurance.

Abstract:The supply and demand models of per capita total health expenditures are established with regression analysis using SPSS statistic software,based on the statistical data for 2001-2010.The results showed that:the growth of health expenditures is faster than that of GDP,but the elasticity is relatively small,only 1.026;the growth of health expenditures is mainly affected by the adjusted population and the disposable income of urban residents;the price index of medical &health care for urban resident and the number of health institutions cannot stably explain the growth of health expenditures;the issue on the growth of health expenditures should be discussed in the relationship between supply and demand.
Predicting the Development Trend of Health Expenditure Based on Methods of System Dynamics Simulation

Abstract:Objective:To explore the usage of system dynamics analyzing the influencing factors of total health expenditure(THE),forecast the trends of total health expenditure based on the method of System Dynamics and put forward relative countermeasures.Methods:According to the literature and expert consultation,the population,elderly,GDP,government health expenditures,drug costs and the number of health workers per thousand population were the important factors for THE.These factors were incorporated into system dynamics model for total health expenditure and conducted the simulation.Results:Based on the examination,the simulation results highly corresponded to historical data (2002-2014) and the official forecast (2015-2020),which showed that the model had good stability and reliability.The simulation results showed that steadily increasing total health expenditure would reach 7 457.12 billion yuan in 2025.Conclusion:The system dynamics approach had superior characteristics to other projection methods in terms of stability and reliability.In order to control the irrational growth of total health expenditure,it needed to increase the prevention investment,decrease the incidence of chronic diseases;sustainably improve the reform of public hospitals,implement the basic drug system;change the income-oriented performance appraisal to realize the labor value of medical staffs.

Abstract:Objective To forecast and analyze the hospital visits and the demand of the medical and health services in China from 2011 to 2015.Method ARIMA(1,2,2)was built to predict the hospital visit.Result In the coming five years,the hospital visits in China is trending to grow by an average rate of 9.32% annually.Conclusion It is essential to increase medical input,rationalize health recourse distribution and perfect medical security system in order to satisfy people's health need.

Abstract:This article investigates the variation in the effects of various determinants on the per capita health care expenditure. A total of 28 OECD countries are studied over the period 1990-2012, employing an instrumental variable quantile regression method for a dynamic panel model with fixed effects. The results show that the determinants of per capita health care expenditure do vary with the distribution of the health care expenditure growth, while the change patterns are dissimilar. Specifically, the lagged health spending growth has a significantly positive effect, with an effect that decreases towards the higher quantiles of growth of per capita health care expenditure. Per capita GDP has a significantly positive effect, both the short and long run income elasticities are smaller than one, and health care is a necessity. The density of physicians only has a significant negative effect at the lower tail of the distribution. The elderly population has the reverse effect at the lower and upper tails, and this shows an upward trend with the increase in health expenditure growth. Life expectancy has an effect similar to the proportion of the old. Variable representing Baumol's model of "unbalanced growth" theory has a significantly positive effect, and the change pattern of its influence shows a marked upward trend. However, one component of "Baumol variable", labor productivity, only shows significant effect in the low half of the distribution. More attention needs to be paid to the influence of determinants in health expenditure study.

Abstract:This paper investigates the variation in the effects of various determinants on the per capita health-care expenditure. A total of 28 Organisation for Economic Co-operation and Development countries are studied over the period 1990-2012, employing an instrumental variable quantile regression method for a dynamic panel model with fixed effects. The results show that the determinants of per capita health-care expenditure growth, involving the growth of lagged health spending, of per capita gross domestic product (GDP), of physician density, of elderly population, of life expectancy, of urbanization, and of female labor force participation, do vary with the conditional distribution of the health-care expenditure growth, while the changing patterns are dissimilar. Moreover, we show that Baumol's model of "unbalanced growth" has a significantly positive effect on per capita health spending growth, and its effect is quite stable over the entire distribution. However, the correlation between the components (wage growth and labor productivity growth) of the "Baumol variable" and health expenditure growth is more varied. As a comparison, only the growth of lagged health spending, per capita GDP, and the Baumol variable (or its components) are found related to health spending growth in conditional mean regressions. The prediction results were also quite different between the quantile regression dynamic panel instrumental variable models and linear panel data models. More attention needs to be paid to the varying influence of determinants in health expenditure study.

Abstract:Based on Cobb-Douglas production function,this article builds up an econometric model for the factors of total health expenditure in China using ordinary least square.Meanwhile,the author carries out(analysis) by using the whole country's statistics from 1978 to 2003.The conclusions are as follows: economy is a key expenditure driver;the reduction of government expenditure allocated to health since 1996 has a significant influence on total health expenditure;reduced-demand by doctor has less effect;ageing population doesn't have significant influence.Finally,the author brings forward some proposals according to the empirical study.

Abstract:Purpose The purpose of this paper is to study a fractional grey model FAGM(1,1,t) based on the GM(1,1,t) model and the fractional accumulated generating operation, and then predict the national health expenditure, the government health expenditure and the out-of-pocket health expenditure of China.Design/methodology/approach The presented univariate grey model is systematically studied by using the grey modelling technique, the fractional accumulated generating operation and the trapezoid approximation formula of definite integral. The optimal system parameters r and are evaluated by the particle swarm optimisation algorithm.Findings The expressions of the time response function and the restored values of this model are derived. The GM(1,1), NGM(1,1,k,c) and GM(1,1,t) models are particular cases of the FAGM(1,1,t) model with deterministic r and . Compared with other forecasting models, the results of the FAGM(1,1,t) model have higher precision.Practical implications The superiority of the new model has high potential to be used in the medicine and health fields and others. Results can provide a guideline for government decision making.Originality/value The univariate fractional grey model FAGM (1,1,t) successfully studies the China's health expenditure.

Abstract:Objective To study the dynamic correlation between per capita health expenditure and economic growth in our country,and to provide relevant evidence for the formulation of health economic policy. Methods The time series data from 1990 to 2015 were used. A VAR model was established between per capita health expenditure and per capita gross domestic product(GDP). Impulse response function and variance decomposition were used to analyze the dynamic relationships among variables. Results There is a long-term equilibrium relationship between per capita health expenditure and per capita GDP,with obvious two-way Granger causality. The impact of economic growth on per capita health expenditure has both short-term and long-term effects,while per capita health expenditure promote economic development in the short term. Conclusion It is suggested that the government increase investment in medical and health services,formulate a reasonable growth mechanism for health expenditure input,and strengthen supply-side reform of health input.

Abstract:Objective:To realize the allocation situation of health resource in Hubei,to provide some references for Health Administrative Department.Methods:According to System of Health Accounts of OECD and China’s current health services system,analyzing data by using Excel software and the results.Results:Accounting data of Total Health Expenditure by providers in Hubei from 2007 to 2010 is used to describe the overall level of Total Health Expenditure,and describe the health expenditure allocation across medical institutions,outpatient institutions,public health institutions and other institutions.Conclusion:Total Health Expenditure in Hubei had been on the rise for years,health resources allocation structure had a gradually reasonable tendency,the proportion of the hospital cost and overall proportion of public health agency cost were slightly declining,but proportion of medicine retail cost increased quickly.

Abstract:Objective To grasp the features of the macro health economic operation in China in the 1990s. Methods The method of retrospective analysis was used to compare the tendencies of increase in the health costs, medical expenses, costs of medical services and expenses for medicine in China from 1990 to 2000. Results The health costs in China were constantly on the increase, with an average annual increase rate of 13 33%. Medical expenses also showed a tendency of accelerated increase from 1993 on, with an average annual increase rate of 10 83%. The average annual rates of increase in costs of medical services and expenses for medicine were respectively 14 14% and 11 07%. From 1994 on, the rate of increase in the above indexes was apparently higher than the growth rate of GDP. Conclusion Although some effects have been achieved through total quantity control and structural readjustment, the health economic situation in China remains to be further improved when compared with developed countries.

Abstract:China's national pharmaceutical system has multiple Objectives: to develop the domestic pharmaceutical industry and encourage innovation, to control total pharmaceutical expenditure (TPE) which represents the largest component of total health expenditure (THE) in China, and to ensure equities for poor and uninsured patients. The challenges to the current system with distorted market incentives resulted in unaffordable drug costs and questionable prescribing practices that undermine public health. This study intended to examine the time trends and determinants of total pharmaceutical expenditure in China. Total pharmaceutical expenditures over 1990-2009 were explained by amount of prescriptions dispensed, proxy of health utilization, and price index over years including medical CPI. Data from China National Health Accounts Report was used. Time trends of TPE as share of THE (TPE/THE) and of GDP (TPE/GDP), and the relationship between TPE and GDP were examined descriptively. The growth of TPE was examined after adjusting for health utilization. Determinants of TPE/THE and TPE/GDP were investigated by time-series regression models. Descriptive analyses showed TPE/THE and TPE/GDP grew over the years 1990-2009. THE/GDP grew faster in the recent few years than TPE/GDP. TPE per visit still rose over years after adjusting for health utilization (inpatient and outpatient services). However, health utilization was not shown the growth pattern along with the GDP growth over 1990s. Time-series analyses showed TPE/THE was negatively influenced by GDP (p = 0.039) and medical CPI (p=0.021). TPE/GDP was positively influenced by price index of prescriptions (p=0.000) and amount of health service use including inpatient (p= 0.012) and outpatient visits (p=0.003). Both TPE and THE increased over time with the pace of GDP growth. The study provided evidence of increasing economic burden on patients imposed by TPE in China. Rapid growth in the China economy, however, may ameliorate the overall TPE burden.

Abstract:To make clear what's the feasible total expenditure on health(TEH) in China,the study analysed the data from WHO/WB by means of single factor analysis and panel data method to find out:The proportion of TEH accounts for GDP should be 5.5%in China by 2020,but considering the development of economy in China,the ratio can be 5.0%in 2～3 years and 5.5%finally.Since health can gain the average social profits,TEH should grow synchronously with GDP.

Abstract:With DEA-Malmquist method,this paper takes the input and output data of medical financial expenditure of 30 provinces(cities,autonomous regions) from 2002 to 2009 as panel data,and analyzes the different efficiency of the provincial medical financial expenditure.The results show that our overall medical expenditure efficiency is low relatively,but increasing,the eastern region medical expenditure efficiency is high relatively,and the center and the west are low relatively,particularly the center.

Abstract:Using the data of national health expenditures(NHE),related economic index and the government investment,this paper analyzes in what extent the government investment can impact the ratio between out-of-pocket(OOP) and national health expenditures(NHE),OOP and the disposable incoming.The result shows that it will be very effective in the short term.But it also suggests that it is difficult to keep the sustainable effect in the long term without the support of social health expenditures and the improvement of the income distribution.
