Abstract:Objective:To discuss the therapeutic effectiveness on intra-articular injection of sodium hyaluronate for knee osteoarthritis and joint compound of sodium hyaluronate and compound betamethasone intra-articular injection treatment of knee osteoarthritis,to compare efficacy of the two methods and to discuss their short-term efficacy and relations to the severity of knee osteoarthritis.Methods:All atients with knee osteoarthritis were selected and randomly divided into two groups,group A and group B,and group A contained 43 knees(with 13 patients suffered OA on both knees),group B contained 62 knees(with 12 patients have OA on both knees).Patients in group A were treated by knee intra-articular injection of sodium hyaluronate of 2.5mL once per week,5 weeks per course treatment;while patients in group B were given intra-articular injection of 1mL compound betamethasone and intra-articular injection of 2.5mL sodium hyaluronate for the first week of treatment prior to treatment of intra-articular injection of 2.5mL sodium hyaluronate once per week for the remaining 4 weeks.Two weeks after the treatment,an observation table was formulated by using the corresponding Lequesne knee OA severity index assessment.The situation of group A patients and group B patients after first week of treatment and that of two weeks after the 5th week full course treatment were assessed by using a pointing system.In group A and group B before and after treatment were tested and evaluated by using paired t test method and then averaged sample t test to calculate rate of improvement.Patients in each group were divided into two sub-groups,one with mild to moderate OA and one with severe OA.According to the score before and after treatment,we assigned value of effective and ineffective to each patient and analyzed the data by using chi-square test method.In all test, α level was set to=0.05.Results:①All 80 patients received a return visit.Scores in group A and B before and after treatment were in normal Gaussian distribution which met paired t test standard,with no significant difference before treatment(P0.05),and a significant difference after treatment(P0.01) between group A and group B.②In group A,13 patients showed clinical remission(response rate ≥75%),31 effective(response rate 75%,≥ 30%) and 3 marked ineffective(response rate 30%);in group B,clinical remission was observed in 16 patients,43 effective and 3 cases ineffective.Chi-square test analysis showed that there were no significant differences in the treatment effectiveness between group A and B.③Chi-square test showed no significant difference(P0.05) in the treatment of the patients with mild to moderate and severe knee osteoarthritis when adopting intra-articular injection of sodium hyaluronate approach.Chi-square test also proved there was no significant difference(P0.05) in the treatment of the patients with mild to moderate and severe knee osteoarthritis when adopting compound of sodium hyaluronate and compound betamethasone intra-articular injection approach.Conclusion:Intra-articular injection of sodium hyaluronate treatment and an joint compound of sodium hyaluronate and compound betamethasone intra-articular injection treatment have a similar positive effect in the treatment of knee osteoarthritis.The short term effectiveness of the two approaches has no significant relations to the severity of illness.
