Obstetric Outcome Analyses of 132 Cycles in Vitro Fertilized Pregnancies Complicated by Moderate and Severe Ovarian Hyperstimulation Syndrome

Abstract:To analyse the obstetric outcome of pregnancies complicated by moderate and severe Ovarian Hyperstimulation Syndrome (OHSS) in assisted reproductive techniques (ART). Retrospective study. As the study group, 72 pregnant patients in 132 patients undergoing IVF who were hospitalized for moderate and severe OHSS during January 2004 to December 2005 were analyzed retrospectively, and 861 pregnant patients undergoing IVF who were not complicated with OHSS as a contemporaneous control group. (1) A total 132 patients who had moderate and severe OHSS after IVF treatment were identified, of whom 87 (65.9%) patients had undergone ET. The clinical PR was 82.8% (72/87). In the study group, the rates of singleton delivery, twins delivery, miscarriage, fetal death in uterus and ectopic separately were 53.5%, 25.4%, 16.9%, 1.4% and 1.4%, respectively; and the rate of fetal malformation was 1.4%. As comparison, in the control group, the rate were 55.0%, 19.5%, 18.7%, 0.6% and 6.1%, respectively; and the rate of fetal malformation was 2.4%. Between the study group and the control group, neither the rate of singleton and twins delivery were significantly different (P>0.05), nor the rates of delivery (including premature and mature) and pregnancy loss (including miscarriage and fetal death in uterus) were significantly different (P>0.05), but the miscarriage rate in second trimester was higher than that in early trimester in OHSS patients (9.9%–7.0%), lower in non-OHSS patients (5.4%–13.3%), but they were not significantly different between patients with and without OHSS (P=0.075). (3) Between the study group and the control group, neither the rate of premature and Low birth weight were significantly different (P>0.05), nor the rates of perinatal complications (including PIH, GDM, placental abruption, et al.) were significantly different (P>0.05). The rate of pregnancy loss, premature delivery and perinatal complications did not increase in OHSS patients compared with patients without OHSS.
