Abstract:Although nasopharyngeal carcinoma (NPC) is rare in other parts of world, it is endemic in certain regions, especially in Southern Chinese. The seventh edition of the American Joint Committee on Cancer (AJCC) staging manual defines new rules for classifying nasopharyngeal carcinoma (NPC). The Chinese 1992 staging system had also been revised one year ago. The new name was Chinese 2008 staging system, which was characteristic as its N staging criteria using lymph node variables basing on magnetic resonance imaging (MRI) instead of palpation examination. The current study conducted in Southern China, compares the two in predicting NPC prognosis. A total of 799 patients presented with untreated nondisseminated NPC who had received MRI scan of the nasopharynx and neck were studied retrospectively. The clinical materials and information of imaging were collected. All patients were restaged according to the AJCC staging system and Chinese 2008 staging system of nasopharyngeal carcinoma. Distribution of patients, survival and prognostic value were compared. The median follow-up for the whole group was 36 months (range 2-54). By two staging systems, patient distributions in T, N and clinical stage were similar. Only 71 (8.9%) patients had N0 disease, which caused the percentage of patients with stage I was merely 1.8%. Using AJCC system, 3-year disease specific survival rates for stage I, II, III and IV were 100%, 93.8%, 83.9% and 73.8%, respectively. Statistical significance was obtained among adjacent stages except for stage I and stage II. Corresponding survival curves were similar by the Chinese 2008 system. Free from local failure survival curves for stage T1, T2 and T3 were not statistically divided by the two staging systems. For the Chinese 2008 system, the distant failure-free survival curves for N0 and N1a were overlap, and survival curves showed better separation between N1b, N2, and N3. However, for AJCC staging system, no statistical significance was found for distant metastasis rates between N0 vs. N1, N2 vs. N3. Both systems have the similar prognostic value. However, it appeared that the Chinese 2008 system was slightly better in N classification, when considering its new reliable criteria. There were no statistical differences for local relapse rates between T1, T2 and T3. The percentage of patients with stage N0 and stage I were small, when retropharyngeal lymph node (RLN) was defined as N1 disease.
