Abstract:A 25-year-old man was admitted to our hospital after 8 years of exertional dyspnea and palpitations and 2 years of repeated hemoptysis. Cardiac auscultation revealed P2 accentuation and grade 2/6 systolic murmurs at the lower-left sternal edge. An electrocardiogram showed sinus rhythm and hypertrophy of both ventricles. Chest radiography revealed cardiomegaly (cardiothoracic ratio, 0.57) and increased bilateral pulmonary vascularity. The patient had no knowledge of any notable abnormality at his birth or in childhood and said that he had been delivered at full term. Cardiac magnetic resonance imaging showed an anomalous origin of the right pulmonary artery (RPA) from the posterior wall of the ascending aorta just above the aortic valve, and an associated patent ductus arteriosus (Fig. 1). Cine magnetic resonance imaging demonstrated a loss of pulsation and decreased contractility in the proximal segments of both pulmonary arteries. Cardiac catheterization conirmed the presence of the anomaly and of marked pulmonary hypertension (Fig. 2). The mean pressure of the aorta was 95 mmHg; the RPA, 92 mmHg; and the left pulmonary artery, 90 mmHg. The oxygen saturation of the aorta was 0.94; the RPA, 0.91; and the left pulmonary artery, 0.88. B) Pulmonary angiogram shows the main pulmonary trunk traced solely to the ... Diverse hypotheses have been advanced to explain an anomalous RPA that arises from the ascending aorta. Edasery and colleagues thought that the abnormality resulted from asymmetric division of the truncus that leaves the right 6th arch on the wrong (that is, the aortic) side.1 Schneiderman suggested that the anomaly occurs after abnormal RPA migration and truncus septation, which does not happen simultaneously in normal development.2 This hemitruncus malformation is also believed to occur because of incomplete leftward migration of the right pulmonary artery during its formation.3 The anomalous RPA may arise from the ascending aorta close to the aortic valve (this is the proximal form, which occurs in 85% of patients), or from the ascending aorta proximal to the brachiocephalic artery or from the base of the artery itself (this is the distal form, which occurs in 15% of patients).3,4 Associated cardiac anomalies include tetralogy of Fallot, right aortic arch, ventricular septal defect, patent ductus arteriosus (the most common associated anomaly, found in our patient), and some complex associated lesions. Anomalous origin of the RPA is a rare congenital anomaly. Fewer than 65 cases of RPA arising from the ascending aorta have been reported in the medical literature, and 95% of those cases were reported during the 1st year of the subject's life.1 To our knowledge, only 8 cases in adults have thus far been documented, and our patient had the longest documented survival of any untreated male with this condition.5,6 However, because of his pulmonary vascular obstructive disease, our patient was deemed inoperable except for heart–lung transplantation. The mortality rate, without surgery, of RPA arising from the ascending aorta may exceed 70% during the 1st year of life because of congestive heart failure or early, irreversible, pulmonary vascular obstruction.5 Early clinical diagnosis greatly advances the opportunity for surgical correction. Although most patients with this anomaly present with signs of heart failure or respiratory distress during infancy, an accurate diagnosis cannot be made solely from a patient's medical history, physical examination, chest radiograph, and electrocardiogram. Furthermore, the aberrant RPA usually lacks distinctive auscultatory features. Therefore, it is important for physicians to consider the possibility of this anomaly when encountering a patient with congenital heart disease. Cardiac magnetic resonance imaging appears to be an effective method for the noninvasive diagnosis of an aberrant RPA.
