Abstract:BACKGROUND:With the more and more new synthesis of artificial materials,abdominal wall defects are well repaired; however,there are few studies on battle would-induced abdominal wall defect. OBJECTIVE:To compare the efficacy and safety of acellular dermis matrices (ADM),polypropylene mesh (PPM),and conventional method for repairing battle would-induced abdominal wall defect. DESIGN,TIME AND SETTING:A randomized controlled animal study was performed at the Animal Experimental Laboratory,Xijing and Tangdu Hospitals,the Fourth Military Medical University of Chinese PLA from January to August 2008. MATERIALS:ADM was provided by Yantai Zhenghai Biotechnology Co.,Ltd.,and PPM was purches. METHODS:A canine model of open abdominal injury was established on 27 adult hybrid dogs which were randomly divided into ADM,PPM,and conventional method groups. MAIN OUTCOME MEASURES:Samples from the wound of three groups were observed respectively at 1,3,and 6 months after surgery under optic microscope. Strength of extension was detected,too. RESULTS:All wounds were not ruptured,and lateral ventral syndrome was not formed. The interior surfaces of wound were bonding with viscera in PPM group,but not in ADM group. HE staining demonstrated that ADM was more favorable to the growth of fibrobast and microblood vessels than PPM and conventional method. After 6 months all ADM were gradually replaced with fibrobast and microblood vessels in ADM group; inflammatory cells were still observed in the PPM group at 6 months after surgery,showing a long-term effect of foreign substance; a number of scar could be seen in conventional method group,but a few of proliferation,and wound was recovered slowly. The examination for strength of extension showed that strength of extension of wound in the ADM group was the strongest among the three groups during 1-3 months,and that from PPM group was stronger than conventional method group. CONCLUSION:The repairing effect of ADM is equal to that of PPM,but both are superior to the conventional method; in addition,the safety of ADM is superior to that of PPM.
