Abstract:Due to the advantages of high resolution, multi-scale, multi-dimension, low radiation and easy to integrate, optical imaging technology plays an important role in biomedical field. In the field of endoscopy, how to obtain, process and visualize the endoscopic image information is the core of the problem what optical imaging technology need to solve. The obtaining of trans-scale endoscopic image of patients in the medical clinical is more advantageous to the surgeon for the diagnosis of patients and can improve in accuracy of the operation. The review starts with the application of trans-scale optical imaging technology in the field of endoscopy, focusing on the different optical systems to obtain trans-scale images in clinical endoscopy, including trans-scale optical zoom system, multi-channel imaging system, fiber-scanning imaging system, and expounds its progress and future trends.

Abstract:During tumor resection, doctors use intraoperative biopsies to determine the tumor margin. However, the pathological procedures of traditional diagnostic methods, such as imprint cytology and frozen section analysis, are complicated and time-consuming. As this is not conducive to surgeries, their applications are limited to a large extent. Therefore, novel fast microscopy imaging technologies with resolutions comparable to those of pathological tissue sections are necessary. Stimulated Raman scattering (SRS), photoacoustic microscopy (PAM), multiphoton microscopy (MPM), and optical coherence microscopy (OCM) exhibit the advantages of high spatial resolution, large imaging depth, avoiding damage to biological tissues, label-free detection, and the availability of biochemical information of tissues. Additionally, they are superior to intraoperative biopsies owing to their fast imaging speeds. Therefore, they possess broad application prospects in tumor resection surgeries and the diagnosis of other diseases. This study briefly introduces the basic principles, structural characteristics, advantages and disadvantages, and the existing research status of SRS, PAM, MPM, and OCM in biomedicine. Furthermore, we propose a multi-mode hybrid detection technology that can be used for surgeries. The combination of the proposed technology with deep learning-based artificial intelligence can form the basis for intraoperative diagnosis in the future.

Abstract:These 7 articles from high‐quality Chinese academic medical centers focused on breast cancer present evidence of the growing expertise in this area in the most populous nation in the world. In addition to articles on risk stratification and prognosis at various stages of breast cancer, of particular interest is a study of high‐resolution full‐field optical coherence tomography and dynamic cell imaging for rapid intraoperative diagnosis during breast cancer surgery. This study demonstrates that these timely and tissue‐sparing optical imaging techniques are highly accurate in diagnosing breast cancer and nodal metastasis and compare favorably with routine histologic sections, but it is important to supplement the data in a future cohort involving breast‐conserving surgery.

Abstract:Recent advancements on interventional molecular imaging aimed to further complement the advantages of two imaging fields, namely, interventional radiology and molecular imaging. Interventional molecular imaging significantly improved the early diag-nosis of cancer, local treatment, and monitoring of tumor treatments. Interventional radiology has continuously extended the capabili-ties of the currently available molecular imaging techniques to (i) obtain deep-seated targets;(ii) thoroughly examine small targets;(iii) precisely guide the delivery of non-targeted imaging tracers or therapeutics;and (iv) selectively enhance the effectiveness of targeted imaging and treatment with high accuracy. Molecular imaging has been used to guide interventional therapies and assess the thera-peutic efficacies of medical interventions. The continuous efforts on interventional molecular imaging extend molecular imaging from benches or small animal laboratories to large animal suites and ultimately to certain clinical applications in humans and enhance the diagnosis and treatment of cancer.

Abstract:Optical coherence tomography (OCT) is a non-invasive imaging technique which has become the "gold standard" for diagnosis in the field of ophthalmology. However, in contrast to the eye, nontransparent tissues exhibit a high degree of optical scattering and absorption, resulting in a limited OCT imaging depth. And the progress made in the past decade in OCT technology have made it possible to image nontransparent tissues with high spatial resolution at large (up to 2mm) imaging depth. On the one hand, OCT can be used in a rapid, noninvasive way to detect diseased tissues, organs, blood vessels or glands. On the other hand, it can also identify the optical characteristics of suspicious parts in the early stage of the disease, which is of great significance for the early diagnosis of tumor diseases. Furthermore, OCT imaging has been explored for imaging tumor cells and their dynamics, and for the monitoring of tumor responses to treatments. This review summarizes the recent advances in the OCT area, which application in oncological diagnosis and treatment in different types: (1) superficial tumors:OCT could detect microscopic information on the skin's surface at high resolution and has been demonstrated to help diagnose common skin cancers; (2) gastrointestinal tumors: OCT can be integrated into small probes and catheters to image the structure of the stomach wall, enabling the diagnosis and differentiation of gastrointestinal tumors and inflammation; (3) deep tumors: with the rapid development of OCT imaging technology, it has shown great potential in the diagnosis of deep tumors such in brain tumors, breast cancer, bladder cancer, and lung cancer.

Optical Breast Imaging: A Review of Physical Principles, Technologies, and Clinical Applications

Abstract:Optical imaging involves the propagation of light through tissue. Current optical breast imaging technologies, including diffuse optical spectroscopy, diffuse optical tomography, and photoacoustic imaging, capitalize on the selective absorption of light in the near-infrared spectrum by deoxygenated and oxygenated hemoglobin. They provide information on the morphological and functional characteristics of different tissues based on their varied interactions with light, including physiologic information on lesion vascular content and anatomic information on tissue vascularity. Fluorescent contrast agents, such as indocyanine green, are used to visualize specific tissues, molecules, or proteins depending on how and where the agent accumulates. In this review, we describe the physical principles, spectrum of technologies, and clinical applications of the most common optical systems currently being used or developed for breast imaging. Most notably, US co-registered photoacoustic imaging and US-guided diffuse optical tomography have demonstrated efficacy in differentiating benign from malignant breast masses, thereby improving the specificity of diagnostic imaging. Diffuse optical tomography and diffuse optical spectroscopy have shown promise in assessing treatment response to preoperative systemic therapy, and photoacoustic imaging and diffuse optical tomography may help predict tumor phenotype. Lastly, fluorescent imaging using indocyanine green dye performs comparably to radioisotope mapping of sentinel lymph nodes and appears to improve the outcomes of autologous tissue flap breast reconstruction.

Abstract:Cancer poses a significant threat to human health,with tumor invasion and metastasis being the leading causes of mortality.Circulating tumor cells(CTCs)and other circulating entities are pivotal in the metastasis process.Consequently,tracking these cells and particles is critical for understanding metastasis.Optical in vivo flow cytometry(IVFC),an innovative laser-based technique,facilitates non-invasive tracking of CTCs and tumor-associated particles,proving invaluable in cancer researches,especially in metastasis studies.This study focuses on the applications of IVFC in tumor metastasis research.We discuss the detection principles of IVFC,highlighting various optical techniques such as fluorescence emission,photoacoustic effects,and computer vision.The categorization of IVFC technologies,including fluorescence in vivo flow cytometry,photoacoustic flow cytometry,and in vivo imaging flow cytometry,is detailed.We review its application in research on different cancers such as liver,prostate,breast cancer,and melanoma.The paper concludes with future prospects for IVFC in studying tumor metastasis.

Abstract:Background: The review of patents reveals that with the high incidence of breast cancer, the therapeutic methods are further becoming increasingly important. In traditional breast interventional procedures, particle was manually injected into the body of cancer patients by a doctor according to the image. However, the operation could not be performed with image navigation in real-time, and the manual implantation can also produce large errors. Objective: Breast intervention robot with image-guided is one of the research contents of the medical intervention therapy robot, which is a combination of medical technology, material science and robot technology, to assist surgeons to complete the breast interventional procedures, such as biopsies and particle’s implantation. It’s possible to accurately complete the operation under navigation in real-time. Method: The breast intervention robots are designed with sufficient degrees of freedom to achieve the function in various navigation environment under considering material, structure and driver compatibility of the robot. Results: Many scholars have focused on the study of breast interventional robots in MRI (Magnetic Resonance Imaging), CT (Computed Tomography), and ultrasound navigation, and have achieved some research results. Besides, the mortality of breast cancer is gradually reduced, and the cure rate is progressively increased owing to the improvement of treatment equipment. Conclusion: The methods of cancer treatment have gradually been developed towards minimally invasive, precision and individual. Thus, minimally invasive surgery based on image navigation is widely used. In addition, the combination of robot and medical image technology, which makes the positioning more accurate, will lead to the success rate of the operation greatly improved. Abstract:The use of artificial intelligence (AI) to assist biomedical imaging have demonstrated its high accuracy and high efficiency in medical decision-making for individualized cancer medicine. In particular, optical imaging methods are able to visualize both the structural and functional information of tumors tissues with high contrast, low cost, and noninvasive property. However, no systematic work has been performed to inspect the recent advances on AI-aided optical imaging for cancer theranostics. In this review, we demonstrated how AI can guide optical imaging methods to improve the accuracy on tumor detection, automated analysis and prediction of its histopathological section, its monitoring during treatment, and its prognosis by using computer vision, deep learning and natural language processing. By contrast, the optical imaging techniques involved mainly consisted of various tomography and microscopy imaging methods such as optical endoscopy imaging, optical coherence tomography, photoacoustic imaging, diffuse optical tomography, optical microscopy imaging, Raman imaging, and fluorescent imaging. Meanwhile, existing problems, possible challenges and future prospects for AI-aided optical imaging protocol for cancer theranostics were also discussed. It is expected that the present work can open a new avenue for precision oncology by using AI and optical imaging tools.

Abstract:We have witnessed rapid development of fluorescence molecular imaging of solid tumors for cancer diagnosis and image-guided surgery in the past decade. Many biomarkers unique to cancer cells or tumor microenvironment, such as cell surface receptors, hypoxia, secreted proteases and extracellular acidosis have been characterized, and can be used to distinguish cancer from normal tissue. A variety of optical imaging probes have been developed to target these biomarkers to improve tumor contrast over the background tissue. Unlike conventional anatomical and molecular imaging technologies, fluorescent imaging method benefits from its safety, high-spatial resolution and real-time capability, and therefore, has become a highly adoptable imaging method for tumor detection and image-guided surgery in clinics. In this review, we summarize recent progress in 'always-ON' and stimuli-activatable fluorescent imaging probes, and discuss their potentials in tumor detection and image-guided surgery.

Abstract:There have been substantial advancements in optical spectroscopy-based imaging techniques in recent years. These developments can potentially herald a transformational change in the diagnostic pathway for diseases such as cancer. In this paper, we review the clinical and engineering aspects of novel optical spectroscopy-based imaging tools. We provide a comprehensive analysis of optical and non-optical spectroscopy-based breast cancer diagnosis techniques vis-à-vis the current standard techniques such as X-Ray mammography, ultrasonography, and tissue biopsy. The recent advancements in optical spectroscopy-based imaging systems such as Transillumination Imaging (TI) and the various types of Diffuse Optical Imaging (DOI) systems (parallel-plate, bed-based, and handheld) are examined. The engineering aspects, including mechanical, electronics, optics, automatic interpretation using artificial intelligence (AI), and ergonomics are discussed. The abilities of these technologies for measuring several cancer biomarkers such as hemoglobin, water, lipid, collagen, oxygen saturation (SO2), and tissue oxygenation index (TOI) are investigated. This article critically assesses the diagnostic ability and practical deployment of these new technologies to differentiate between the normal and cancerous tissue.

Abstract:Lung cancer is a major threat to human health and a leading cause of death. Accurate localization of tumors in vivo is crucial for subsequent treatment. In recent years, fluorescent imaging technology has become a focal point in tumor diagnosis and treatment due to its high sensitivity, strong selectivity, non-invasiveness, and multifunctionality. Molecular probes-based fluorescent imaging not only enables real-time in vivo imaging through fluorescence signals but also integrates therapeutic functions, drug screening, and efficacy monitoring to facilitate comprehensive diagnosis and treatment. Among them, near-infrared (NIR) fluorescence imaging is particularly prominent due to its improved in vivo imaging effect. This trend toward multifunctionality is a significant aspect of the future advancement of fluorescent imaging technology. In the past years, great progress has been made in the field of NIR fluorescence imaging for lung cancer management, as well as the emergence of new problems and challenges. This paper generally summarizes the application of NIR fluorescence imaging technology in these areas in the past five years, including the design, detection principles, and clinical applications, with the aim of advancing more efficient NIR fluorescence imaging technologies to enhance the accuracy of tumor diagnosis and treatment.

Abstract:Cancer is a major threat to human health. Diagnosis and treatment using precision medicine is expected to be an effective method for preventing the initiation and progression of cancer. Although anatomical and functional imaging techniques such as radiography, computed tomography (CT), magnetic resonance imaging (MRI) and positron emission tomography (PET) have played an important role for accurate preoperative diagnostics, for the most part these techniques cannot be applied intraoperatively. Optical molecular imaging is a promising technique that provides a high degree of sensitivity and specificity in tumor margin detection. Furthermore, existing clinical applications have proven that optical molecular imaging is a powerful intraoperative tool for guiding surgeons performing precision procedures, thus enabling radical resection and improved survival rates. However, detection depth limitation exists in optical molecular imaging methods and further breakthroughs from optical to multi-modality intraoperative imaging methods are needed to develop more extensive and comprehensive intraoperative applications. Here, we review the current intraoperative optical molecular imaging technologies, focusing on contrast agents and surgical navigation systems, and then discuss the future prospects of multi-modality imaging technology for intraoperative imaging-guided cancer surgery.

The study of breast cancer with Near Infrared light

Abstract:Early detection of the breast cancer is one of the keys to decrease the mortality of this disease. Now more and more attention is paid on development of non-invasive mammography. To study the feasibility of breast imaging with near infrared light, a frequency-domain optical scanner of breast cancer was designed and developed in our lab, based on the theory of interaction of Near Infrared (NIR) light and soft tissues. In this paper, the imaging prototype as well as the imaging processing system based on Matlab is described. The results of phantom experiments using the prototype of this scanner show a resolution in millimeter scale. The initial clinical trials show the feasibility of our prototype in clinical study. The prototype can scan a breast in 4 minutes. Images of the same breast obtained both with the prototype and with x-ray mammography are presented.

Abstract:In recent years, potential new therapeutic options have been yielded for the immunotherapy of invasive cancers. The most important breakthrough in breast cancer research today is to understand the immune features and profiles of triple negative breast cancer and then design novel immune-modulatory targeted therapy. The major findings are the use of immune checkpoint inhibitors, which have obtained good curative effect in neoadjuvant treatment for early stage triple negative breast cancer and salvage treatment for metastatic triple negative breast cancer. This paper summarizes the advances of immunotherapy and its application in triple negative breast cancer with emphasis on immune checkpoint inhibitor and its combination with chemotherapy, radiotherapy and targeted therapy.

Abstract:Near-infrared-II (NIR-II) fluorescence visualization is an in vivo optical imaging technique with large imaging depth and high resolution. Compared with visible (360-760 nm) and near-infrared-I (760-900 nm) windows, the light absorption of bio-tissues in the second near-infrared window (900-1880 nm) improves significantly, suppressing the scattering background in two-dimensional array detection, thus remarkably improving imaging quality. Currently, scientists have developed various luminescent probes, imaging systems, and theranostics pathways corresponding to the NIR-II window. The experimental animals cover from rodents to non-human primates. The clinical surgery of human liver tumors guided by NIR-II fluorescence imaging has also been initially evaluated. With light absorption and photon scattering in the visible and near-infrared regions in biological tissues, this study expounds on the mechanism of imaging quality improvement on NIR-II fluorescence imaging. It briefly introduces several typical NIR-II luminophores and reviews the developed multimodal NIR-II optical imaging methods. The multi-functional theranostics platform guided by precious NIR-II optical imaging is also discussed. These aim to provide references for further development and accelerate clinical translation.

Abstract:In recent years, breast cancer incidence has increased year by year, which makes research on treatment methods critical. Intervention surgery is a new development in breast cancer treatment. However, the operation is complicated and lasts for a long time. The doctor’s operation can no longer meet clinical needs. The breast intervention robot has the characteristics of minimal invasiveness, operability, and high flexibility so that it can compensate for the shortcomings of traditional interventional surgery. The combination of the robotic system and medical imaging technology is an important means to determine the lesion location and plan the puncture path. This article mainly reviews the breast intervention robot under image navigation. Based on existing literature, image navigation methods are divided into MRI, ultrasound, and CT for the introduction. First, we summarized the principles and technologies associated with materials, sensors, and actuators used in the MRI-guided breast intervention robot. In particular, the actuation is analyzed and compared in detail. Subsequently, the ultrasound-guided and CT-guided breast intervention robots are introduced. After that, we discuss the image-guided positioning technology. Finally, we summarize the research progress and trends in the future development of breast intervention robots under image navigation.
