Effect of Different Dosage of Remifentanil on Cardiovascular Response Induced by Modified Electric Convulsive Therapy

Abstract:AIM To determine the suitable dosage of remifentanil in attenuating cardiovascular response induced by modified electric convulsive therapy (MECT). METHODS Sixty ASA I - IT patients in the age of 20 to 60 years, weighing 45 to 70 kg, scheduled for MECT, were randomly divided into three groups according to the dosage of remifentanil. Patients in each group (n = 20) were intravenously administered with propofol 1.5 mg·kg~(-1), remifentanil (1 μg·kg~(-1) in group Ⅰ , 2μg·kg~(-1) in group Ⅱ and 3 μg·kg~(-1) in group Ⅲ , respectively) and succinyleholine 1mg·L~(-1) before electroconvulsive therapy. Systolic blood pressure (SBP). mean arterial pressure (MAP) and heart rate (HR) were measured before anaesthesia (T_0), before electro-convulsive therapy ( T_1) , at 1 min ( T_2) , 3 min ( T_3) and 5 min ( T_4) after electroconvulsive therapy. RESULTS SBP, MAP, HR and systolic rate pressure product (RPP) were significantly increased at T_2 and T_3 (P < 0.05) in the group Ⅰ , were not changed remarkably at T_2, T_3 and T_4 in the group Ⅱ , but were significantly decreased at T_1, T_3 and T_4 in the group 1 (P < 0.05). SBP, MAP, HR and RPP at T_2, T_3 and T_4 in the group Ⅲ were lower than those in the group I and group Ⅱ (P < 0.05). SBP, MAP and RPP at T_2 and T_3 in the group Ⅱ were lower than those in the group I (P < 0.05). CONCLUSION Remifentanil 2 μg·kg~(-1) and 3 μg·kg~(-1) can attenuate the cardiovascular response induced by MECT effectively, while remifentanil 2 μg·kg~(-1) can keep steady hemodynamics.
