Abstract:Human epilepsy is a common neurological disease which is characterized by temporary brain dysfunction caused by epileptic discharges.Blood oxygenation level-dependent functional MRI has the capabilities to detect the normal physiological activities and abnormal epileptic activity,which can be used for clinical evaluations of epilepsy by localizing the eloquent functional areas and epileptogenic focus.In addition,fMRI holds the advantage to investigate the brain networks in epilepsy.The application of fMRI contributes to the presurgical diagnosis and promotes the understanding of neuro-phathophysiology of epilepsy.

Abstract:Studies in animal models of epilepsy and pre-surgical patients have unanimously found a strong correlation between high-frequency oscillations (HFOs, > 80 Hz) and the epileptogenic zone, suggesting that HFOs can be a potential biomarker of epileptogenicity and epileptogenesis. This consensus includes the definition and standard detection techniques of HFOs, the localizing value of pathological HFOs for epileptic foci, and different ways to distinguish physiological from epileptic HFOs. The latest clinical applications of HFOs in epilepsy and the related findings are also discussed. HFOs will advance our understanding of the pathophysiology of epilepsy.

Abstract:Epilepsy is a chronic neurologic disorder caused by abnormal discharge of neurons in the brain. Imaging examination is of great significance in the diagnosis and treatment of epilepsy, but some patients failed to find any apparent abnormality in conventional magnetic resonance imaging. Recently, a variety of imaging post-processing techniques are applied in clinical practice, which have significantly improved the detection rate of epileptic lesions. In this paper, the research progress of various imaging post-processing techniques in magnetic resonance imaging negative focal epilepsy is reviewed.

Abstract:Hepatic encephalopathy (HE), characterized by a wide spectrum of clinical manifestations, ranging from behavior abnormality, conscious disorder and even coma, is a consequence of liver dysfunction in both acute and chronic hepatic diseases. Minimal hepatic encephalopathy (MHE) refers to a subgroup of cirrhotic patients without clinical overt hepatic encephalopathy symptoms but with abnormalities in neuro -cognitive functions. HE/MHE can have a far -reaching impact on quality of life and prognosis. The exact neuropathology mechanism was still unclear. Recently, functional MRI (fMRI) has been increasingly applied for investigating the neuro-pathophysiological mechanism of HE. This paper will review the fMRI research applied on uncovering the neuropathology mechanism of HE.

Abstract:Abstract While functional MRI (fMRI) studies have mainly focused on gray matter, recent studies have consistently found that blood-oxygenation-level-dependent (BOLD) signals can be reliably detected in white matter, and functional connectivity (FC) has been organized into distributed networks in white matter. Nevertheless, it remains unclear whether this white matter FC reflects underlying electrophysiological synchronization. To address this question, we employ intracranial stereotactic-electroencephalography (SEEG) and resting-state fMRI data from a group of 16 patients with drug-resistant epilepsy. We find that BOLD FC is correlated with SEEG FC in white matter, and this result is consistent across a wide range of frequency bands for each participant. By including diffusion spectrum imaging data, we also find that white matter FC from both SEEG and fMRI are correlated with white matter structural connectivity, suggesting that anatomical fiber tracts underlie the functional synchronization in white matter. These results provide evidence for the electrophysiological and structural basis of white matter BOLD FC, which could be a potential biomarker for psychiatric and neurological disorders.

[Resting Functional MRI with Default Mode-Based Functional Connectivity Analysis for Localization of Epileptic Activity].

Abstract:OBJECTIVE:Using the functional connectivity analysis based on the underlying neurophysiological characteristic that epileptic discharges can induce change of brain default mode, to develop a technique for epileptogenic localization using functional MRI (fMRI) without simultaneous electroencephalography (EEG).METHODS:A data-driven method that jointly employed independent component analysis and functional connectivity analysis was used for the resting functional MRI data analysis of 12 focal epileptic patients. The independent components were ranged according to the coefficients of the negative correlation between independent component time course and the signal temporal course in the region of posterior cingulate cortex. The results were comparatively studied with simultaneous EEG-fMRI.RESULTS:In the 10 successful results from 12 patients underwent EEG-fMRI examination, the outcomes of eight subjects were concordant with pathological foci. While the results of all 10 patients processed by data-driven method were concordant with pathological foci, besides the other patients who failed to perform EEG-fMRI examination. Meanwhile, the default mode was well mapped in all patients.CONCLUSIONS:The default mode-based functional connectivity analysis can localize the epileptogenic foci effectively without simultaneous EEG, besides to detect the default mode of epileptic patients.

Abstract:Accurate epileptogenic zone (EZ) or seizure onset zone (SOZ) localization is crucial for epilepsy surgery optimization. Previous animal and human studies on epilepsy have reported that changes in blood oxygen level-dependent (BOLD) signals induced by epileptic events could be used as diagnostic markers for EZ or SOZ localization. Simultaneous electroencephalography and functional magnetic resonance imaging (EEG-fMRI) recording is gaining interest as a non-invasive tool for preoperative epilepsy evaluation. However, EEG-fMRI studies have reported inconsistent and ambiguous findings. Therefore, it remains unclear whether BOLD responses can be used for accurate EZ or SOZ localization. In this study, we used simultaneous EEG-fMRI recording in a rat model of 4-aminopyridine-induced acute focal seizures to assess the spatial concordance between individual BOLD responses and the SOZ. This was to determine the optimal use of simultaneous EEG-fMRI recording in the SOZ localization. We observed a high spatial consistency between BOLD responses and the SOZ. Further, dynamic BOLD responses were consistent with the regions where the seizures were propagated. These results suggested that simultaneous EEG-fMRI recording could be used as a noninvasive clinical diagnostic technique for localizing the EZ or SOZ and could be an effective tool for mapping epileptic networks.
Changes in default mode network functional connectivity of resting-state functional magnetic resonance imaging in partial epilepsy

Abstract:Objective To explore changes of brain function among patients with partial epilepsy in resting state by using the blood oxygenation level dependent functional magnetic resonance imaging (fMRI) techniques. Methods fMRI scanning was performed in resting state among 60 patients with partial epilepsy and 60 gender, age and educational levels matched normal controls. The functional connectivity analysis was applied to calculate the default mode network (DMN) functional connectivity in resting-state fMRI. SPM5 was used to analyze differences in functional connectivity between the two groups(P<0.001, cluster＞50). Results Left precuneus and adjacent posterior cingulate cortex (Pcu/PCC), angular gyrus, and cingulate gyrus were involved in the DMN of epileptic patients. By contrast, the DMN of controls included left Pcu/PCC, right angular gyrus, bilateral medial frontal lobe and temporal lobe. Compared with normal controls, patients with partial epilepsy showed a significantly decrease in functional connectivity of DMN region such as left inferior parietal lobule, supramarginal gyrus, parahippocampa gyrus and superior temporal gyrus, and bilateral uncus, while no regions were found increased functional connectivity in patients group. Conclusions Patients with partial epilepsy show abnormal changes in functional connectivity of DMN in resting state by fMRI, which may associate with the potential pathophysiological mechanisms of epilepsy. The findings demonstrate that the resting-state fMRI might detect the extensive changes of brain function in partial epilepsy with negative results of conventional MRI, suggestive of fMRI as an effective and non-invasive method to explore brain function in epilepsy. Abstract:Objective: To investigate the ictal epileptic activation using simultaneous electroencephalogram functional MRI(EEG-fMRI),and to study the neural mechanism and the roles of the cerebral cortex and subcortical structures in the generation and propagation of the epileptic activity.Methods: A patient with partial epilepsy was retrospectively found to have slight seizures during the simultaneous EEG-fMRI examination.Two sessions of the ictal functional MRI data were processed and analyzed using the general-linear model.Results: Consistent activation patterns were obtained from the two sessions of data.The positive activation was found over the bilateral parietal lobes,insula cortex,bilateral striatum,thalamus and some nuclei of the brain stem,as well as over the vermis and right cerebellar hemisphere.The maximum intensity point was in the left parietal lobe.Conclusion: The seizure activation is originated from the cerebral cortex,and then propagates through the reciprocal circuits made up of the cortex and the subcortical structures.The cerebral cortex and the brain stem reticular formation are the main structures that propagate the seizure to contralateral hemisphere.The subcortical structures play important roles in the seizure propagation and seizure manifestation.

Abstract:Epilepsy is a common neurological disorder; 1% of people worldwide have epilepsy. Differentiating epileptic seizures from other acute neurological disorders in a clinical setting can be challenging. Approximately one-third of patients have drug-resistant epilepsy that is not well controlled by current antiepileptic drug therapy. Surgical treatment is potentially curative if the epileptogenic focus is accurately localized. Diffusion-weighted imaging (DWI) is an advanced magnetic resonance imaging technique that is sensitive to the diffusion of water molecules and provides additional information on the microstructure of tissue. Qualitative and quantitative analysis of peri-ictal, postictal, and interictal diffusion images can aid the differential diagnosis of seizures and seizure foci localization. This review focused on the fundamentals of DWI and its associated techniques, such as apparent diffusion coefficient, diffusion tensor imaging, and tractography, as well as their impact on epilepsy in terms of differential diagnosis, epileptic foci determination, and prognosis prediction.

Detection of focal epileptic activity using simultaneous EEG-functional MRI

Abstract:Objective: To localize the epileptogenic foci of the focal epilepsy preoperational using combined simultaneous electroencephalogram (EEG) and functional MRI (fMRI). Methods: The EEG and functional MRI data of 11 focal epileptic patients were acquired by means of simultaneous EEG-monitored functional MRI before surgery. Observed the brain activation caused by interictal epiletiform discharges (IEDs). Analyzed the findings combined with EEG findings, pathology and postoperative follow-up. Results: Ten patients (10/11) showed significant activation nearby the original lesion. It was concordant with EEG findings. Pathological confirmed the existence of seizures caused pathological changes. Follow-up confirmed 11 cases of patients have fine surgery effect. Conclusion: The BOLD activation of IEDs can be used to localize the epileptogenic foci in focal epilepsy. The simultaneous EEC-fMRI that combines the advantages of two modalities of neuroimaging has the potential for the application in epilepsy research.

Abstract:Over 30% epileptic patients are refractory to medication, who are amenable to neurosurgical treatment. Non-invasive brain imaging technologies including video-electroencephalogram (EEG), magnetic resonance imaging (MRI), and magnetoencephalography (MEG) are widely used in presurgical assessment of epileptic patients. This review mainly discussed the current development of clinical MEG imaging as a diagnose approach, and its correlations with the golden standard intracranial electroencephalogram (iEEG). More importantly, this review discussed the possible applications of functional networks in preoperative epileptic foci localization in future studies.

Abstract:Electrophysiological recording and metabolic imaging are two complimentary techniques in preoperative evaluation of epilepsy surgery. High frequency electrical brain activity has been increasingly used in localizing seizure onset zone (SOZ) owing to the development of stereo-EEG (SEEG) technique. However, SEEG recording has no capacity of whole brain coverage as functional MRI. In this study, we performed cross-modal validation of epileptogenic network defined by SEEG and that by resting-state fMRI on individual brain of epilepsy patients. Epileptogenicity index (EI) based on abnormal high frequency SEEG signals was employed to define the epileptogenic network in electrophysiology. Meanwhile, taking the SEEG sites with highest EI as the seed, resting-state functional MRI and connectivity analysis were used to define the epileptogenic network in hemodynamics. The spatial consistency of these two networks was measured by ROC, and the average AUC reached 0.87, which proved a good consistency between electrophysiological and hemodynamic networks in epileptic brain. The difference part in resting-state fMRI network may supplement the SEEG electrode coverage, to reveal more information about epileptic network.
Altered DMN functional connectivity and regional homogeneity in partial epilepsy patients: a seventy cases study

Abstract:PURPOSE: Clinically diagnosed partial epilepsy is hard to be functionally diagnosed by regular electroencephalograph (EEG) and conventional magnetic resonance imaging (MRI). By collecting transient brain regional signals, blood oxygenation level-dependent (BOLD) function MRI (BOLD-fMRI) can provide brain function change information with high accuracy. By using resting state BOLD-fMRI technique, we aim to investigate the changes of brain function in partial epilepsy patients.METHODS: BOLD-fMRI scanning was performed in 70 partial epilepsy and 70 healthy people. BOLD-fMRI data was analyzed by using the Regional Homogeneity (ReHo) method and functional connectivity of Default Mode Network (DMN) methods. The abnormal brain functional connectivity in partial epilepsy patients was detected by Statistical Parametric Mapping 8 (SPM8) analysis.RESULTS: Compared to healthy group, epilepsy patients showed significant decreased ReHo in left inferior parietal lobule/pre- and post-central gyrus, right thalamus/paracentral lobule/Cerebellum anterior and posterior Lobe, bilateral insula. The DMN functional connectivity regions decreased significantly in right uncus, left Inferior parietal lobule, left supramarginal gyrus, left uncus, left parahippocampa gyrus, and left superior temporal gyrus, in epilepsy patients, compared to healthy controls.SIGNIFICANCE: This study clarified that both ReHo and functional connectivity of DMN decreased in partial epilepsy patients compared to healthy controls. While left inferior parietal lobule was detected in both ReHo and DMN, many other identified regions were different by using these two BOLD-fMRI techniques. We propose that both ReHo and DMN patterns in BOLD-fMRI may suggest networks responsible for partial epilepsy genesis or progression.
A Plot Study of Resting State-fMRI:Preoperative Localization of Epileptogenic Zone in Epilepsy with Malformations of Cortical Development

Abstract:Objective To investigate whether resting state-fMRI (RS-fMRI) based on local consistency (ReHo), am?plitude low-frequency fluctuate (ALFF) and fALFF can add meaningful information on preoperative localization of epilepto?genic zone in patients with malformations of cortical development (MCD). Methods Ten epilepsy patients with MCD were studied with RS-fMRI using a 3.0 T scanner. The resting state data were preprocessed and analyzed using SPM8 and REST to generate the activation map. Abnormal ReHo, ALFF and fALFF related blood oxygen level dependent (BOLD) signal changes were compared to video EEG (VEEG),PET,MRI findings and the final result of a comprehensive evaluation-de?fined epileptogenic zone. For operated patients, postoperative resection and histology were compared to BOLD responses. Re?sults The results of spike localization of RS-fMRI were consistent with VEEG, PET, MRI findings and final comprehensive evaluation-defined epileptogenic zone in 6, 8, 6, 7 of 10 investigations. Six operated patients (including two negative results of MRI examination) revealed local abnormal changes but not visible on structural MRI, which was confirmed cortical malfor?mations by pathology after operation (2 heterotopia and 4 cortical dysplasia). Conclusion RS-fMRI may help to delineate the epileptic focus in epilepsy patients with MCD.

Abstract:Epilepsy can be defined as a dysfunction of the brain network, and each type of epilepsy involves different brain-network changes that are implicated differently in the control and propagation of interictal or ictal discharges. Gaining more detailed information on brain network alterations can help us to further understand the mechanisms of epilepsy and pave the way for brain network-based precise therapeutic approaches in clinical practice. An increasing number of advanced neuroimaging techniques and electrophysiological techniques such as diffusion tensor imaging-based fiber tractography, diffusion kurtosis imaging-based fiber tractography, fiber ball imaging-based tractography, electroencephalography, functional magnetic resonance imaging, magnetoencephalography, positron emission tomography, molecular imaging, and functional ultrasound imaging have been extensively used to delineate epileptic networks. In this review, we summarize the relevant neuroimaging and neuroelectrophysiological techniques for assessing structural and functional brain networks in patients with epilepsy, and extensively analyze the imaging mechanisms, advantages, limitations, and clinical application ranges of each technique. A greater focus on emerging advanced technologies, new data analysis software, a combination of multiple techniques, and the construction of personalized virtual epilepsy models can provide a theoretical basis to better understand the brain network mechanisms of epilepsy and make surgical decisions.
Comparative study of Independent Components Analysis and Combined EEG Functional MRI in Epileptic Focus Detection

Abstract:Objective To research the functional MRI in focal epilepsy using the method of independent components analysis,and to estimate its application value in the research of detecting epileptic focus.Methods The resting functional MRI data of 18 sessions of 12 focal epileptic patients were analyzed by spatial independent components analysis after preprocessing.The components of ICA were sorted according to the time course multiple regressions with hypothesis-driven mode matrix,which was designed by the interictal epileptiform discharges(IEDs) points detected with simultaneous EEG functional MRI.The spatial and temporal character of the components of interesting were investigated.Results Compared with conventional general-linear model(GLM) approach,ICA was found to have better capability to detect epileptic IED-related BOLD activation(GLM vs ICA: 55% vs 72%).Moreover,ICA could detect abnormalities in the resting-state default-mode network in epilepsy.Conclusion The method of independent analysis has the ability to detect the BOLD activation of IEDs of focus epileptic patients,which has a good perspective in the application of the epilepsy research.

Abstract:In the past few years, one of the most significant developments in magnetic resonance imaging (MRI) is the use of MR imaging to non-invasively map human cortical function without the use of exogenous contrast agents . Since its introduction in 1992, functional magnetic resonance imaging (fMRI) has emerged as a powerful tool for studying neuronal function and generated an enormous amount of interest among neuroscientists, NMR scientists, and clinicians. The purpose of this paper is to outline the principle of fMRI and the associated technical issues and illustrate the utility of fMRI with representative applications.The basis of fMRI is the blood oxygenation level dependent (BOLD) contrast which is derived from the fact that deoxyhemoglobin is paramagnetic and changes in the local concentration of deoxyhemoglobin within the brain lead to alterations in the magnetic resonance signal. Neuronal activation within the cerebral cortex causes an increase in blood flow without a commensurate increase in oxygen extraction.
