Abstract:[Objective]To understand the pathogen spectrum of HFMD in Hangzhou,analyze enterovirus(EV)71-type VP1 gene nucleotide and amino acid sequence,and understand the genetic characteristics of EV71 in Hangzhou.[Methods]98 confirmed cases of HFMD stool specimens were collected.All of the specimens were identified by reverse transcription polymerase chain reaction(RT-PCR)with EV71 and Coxsackie(Cox)A16-type-specific primers.Full length VP1 genes from these six EV7l strains were amplified by RT-PCR and sequenced.The sequences were compared with VP1 genes from EV7l in GenBank database.Sequence analysis and type identification were performed by bioinformatics.[Results]Of the RT-PCR,enterovirus 71(EV71)was diagnosed in 22.4% of the patients(22 of 98),and COXA16 was diagnosed in 27.6%(27of 98).The nucleotide and amitto acid homogeneity between of these 6 Zhejiang strains ranged from 92.2% to 98.5%and 99.0% to 100% respectively,there are no significant difference between severe and mild patients in the amino acid sequence.These 6 Ev7l isolated strains were within genotype C subgenogroup C4 in the phylogenetic tree.Compare with representative strains of all genotypes and subtypes,there was difference in the BC loop region between the amino acid sequence differences,and the SP70 region amino acid sequence was identical elsewhere.[Conclusion]The hand,foot and mouth diseases were mainly caused by the EV71 infection increasing proportion.In addition,the separation of EV71 in Hangzhou was belonged to subtype C genotype of the C4.There is still no evidence that this separation in Hangzhou 6 EV71 strains have enhanced virulence.
Analysis of the Complete Genomes of Enterovirus 71 Subtypes in China.

Abstract:Enterovirus 71 (EV-A71) is one of the most pathogens to hand, foot, and mouth disease (HFMD) as well as neurological complications in young children. Molecular characteristic of EV-A71 is important to prevent the virus outbreak. Here, the complete genomes of EV-A71 from China between 1998 and 2019 were downloaded from GenBank. The phylogenetic trees were developed by MEGA7.0 software, and the complete genetic epidemiological characteristics and amino acid mutations of EV-A71 from China were also analysed. The results showed that major epidemic EV-A71 subtype was C4b before 2004, while it turned to C4a after 2004 in mainland China, and C4 and B5 were major subtypes in Taiwan. VP1, VP4, 2C, 3C, 3D, and complete genome sequence can be used for virus genotyping, and VP1, VP4, and complete genomes have obvious advantages over other segments. There were many significant mutations in the viral complete genome sequence. This study indicated that the major C4 and B5 subtypes will contribute to the development of vaccines and drugs of EV-A71 for prevention and monitoring of EV-A71-associated HFMD in China.

Abstract:Objective To investigate the etiology composition of enterovirus (EV) in patients with severe hand, foot, and mouth disease (HFMD) in children. To assess the diagnostic value of cerebrospinal fluid (CSF) tests in severe HFMD, and to find the key laboratory tests for severe HFMD. Methods A total of 288 hospitalized cases of children clinically diagnosed with severe HFMD in Hangzhou Children′s Hospital were included from March to July 2016. Throat swabs were collected and enterovirus nucleic acids were detected by fluorescence quantitative reverse transcription (RT)-PCR. Synchronous CSF and serum samples were collected for EV-A71 and CV-sackievirus A16 (CV-A16)-IgM antibody detection. CSF samples underwent routine and biochemical tests. Normally distributed continuous variables were compared using t test. Non-normal distribution continuous variables were compared using Mann-Whitney U test. Differences between categorical variables were compared with χ2 test. Results The total positive rate of enterovirus nucleic acid EV-A71/CV-A16/EV by fluorescence quantitative RT-PCR in the 288 cases of children clinically diagnosed with HFMD was 83.7% (241/288), including EV-A71 55.2% (159/288), CV-A16 4.9% (14/288) and the other enterovirus 23.6% (68/288). Among the other enterovirus group, there were 29.4% CV-A6 (20/68) , 16.2% CV-A4 (11/68) and CV-A10 10.3% (7/68). The total positive rate of combined serum and CSF detection of EV nucleic acid and EV-A71 and CV-A16 IgM antibody was 98.3% (283/288). EV nucleic acid positive rate was 83.7% (24/288). The positive rates were statistically different (χ2 =37.289, P=0.000). The CSF nucleated cells count in EV-A71 positive subgroup was higher than those in CV-A16 positive subgroup and other enteroviruses subgroup (Z=-4.472 and -9.991, respectively, both P 400 mg/L) rate in EV-A71 positive subgroup was higher than those in CV-A16 positive subgroup and other enteroviruses subgroup (χ2=4.956 and 11.795, respectively, both P<0.05). The CSF nucleated cell counts and positive rates in EV-A71 IgM antibody-positive subgroup were both higher than those in antibody-negative subgroup (both P<0.05). The CSF protein level and elevated proportion in antibody-positive subgroup were both higher than those in antibody-negative subgroup (both P<0.05). The lactate dehydrogenase concentration in antibody-positive subgroup was significantly higher than those in antibody-negative subgroup (P<0.05). The EV-A71 IgM antibody in serum was significantly correlated with the antibody in CSF (r=0.600, P<0.05). Conclusions EV-A71 is still the most important pathogen of severe HFMD in Hangzhou in 2016. Other enterovirus such as CV-A6, CV-A4 increases compared to those in 2014 and 2015. The CSF routine and biochemical tests and the IgM antibody levels can serve as an important indicator for the diagnosis of children with severe HFMD. Abstract:Enterovirus 71(EV71),a member of human enterovirus family,was first isolated from faeces of a central nervous system infected infant in 1969.In recent 30 years,EV71 outbreaks every 2-3 years in the Asia-Pacific regions,and is the main pathogen causing severe hand-foot-mouth disease(HFMD).Severe HFMD caused by EV71 involves in central nervous system and respiratory system disorder.In general,EV71 associated complication is related to the viral invasion of central nervous system,leads to pulmonary edema or pulmonary hemorrhage.It is of great significance to study the clinical features and pathogenesis of EV71 infection,and then take positive and effective measures to block the occurrence of its complications.

Abstract:Enterovirus A71 (EV-A71) is one of the major causative agents of hand-foot-and-mouth disease (HFMD) that majorly affects children. Most of the time, HFMD is a mild disease but can progress to severe complications such as meningitis, brain stem encephalitis, acute flaccid paralysis (AFP) and even death. HFMD caused by EV-A71 has emerged as an acutely infectious disease of highly pathogenic potential in the Asia-Pacific region. In this review, we introduced the properties and life cycle of EV-A71, the pathogenesis and the pathophysiology of EV-A71 infection, including: tissue tropism and host range of virus infection, the diseases caused by the virus, as well as the genes and host cell immune mechanisms of major diseases caused by EV71 infection, such as encephalitis and neurological pulmonary edema. At the same time, clinicopathological characteristics of EV71 infection were introduced. There is currently no specific medication for EV-A71 infection, highlighting the urgency and significance of developing suitable anti-EV-A71 agents. This overview also summarizes the targets of existing anti-EV-A71 agents, including virus entry, translation, polyprotein processing, replication, assembly and release; interferons (IFNs); interleukin (IL); the mitogen-activated protein kinase (MAPK), phosphatidylinositol 3-kinase (PI3K), and protein kinase B (Akt) signaling pathways; the oxidative stress pathway; and the ubiquitin-proteasome system, et al. Furthermore, it overviews the effects of natural products, monoclonal antibodies, and RNA interference against EV-A71. It also discusses issues limiting the research of antiviral drugs. This review is a systematic and comprehensive summary of the mechanism and pathological characteristics of EV71 infection, the latest progress of existing anti-EV-A71 agents. It would provide better understanding and guidance for the research and application of EV71 infection and antiviral inhibitors.

Abstract:Hand,foot and mouth disease(HFMD)is mainly caused by intestinal enterovirus.It is a contagious disease that spreads mostly in children.Its transmission way is digestive tract,respiratory tract and close contact,especially with the highest incidence under the age three groups.The common HFMD generally deteriorates into severe condition in a short time with rapid progression,serious condition,poor prognosis and high incidence,which poses a serious threat to the health and safety of children.Among them,enterovirus 71(EV71)and coxsackievirus A16 are the main pathogens of HFMD.The main pathogen of severe HFMD is EV71.In recent years,severe HFMD is still popular nationwide,so it is particularly important to master its popular characteristics.The key to severe HFMD is prevention.At present,EV71 vaccines such as inactivated vaccine and attenuated live vaccine have been studied at home and abroad,but most of them are in stage Ⅳclinical trials. The future study of multivalent vaccines should be continued in order to prevent the occurrence of HFMD in a multidirec -tional manner.

Abstract:The aims of the present study were to investigate the genetic characteristics of enterovirus 71 (EV71) and coxsackievirus A16 (CVA16) strains in China and to evaluate the relationship between the genotypes of CVA16 and EV71 and their geographical distribution. A total of 399 stool specimens were collected from children with symptoms of hand, foot and mouth disease (HFMD) in Zhejiang Province. The presence of enteroviruses was determined using reverse transcription-semi-nested PCR targeted to the VP1 gene of all human enteroviruses and DNA sequencing. EV71 and CVA16, the major etiological agents of HFMD, were detected in 38.4% (38/99) and 35.4% (35/99) of HEV-A species-positive cases, respectively. Based on the phylogenetic analysis of the VP1 gene, EV71 strains identified in this study belong to subgenotype C4, and CVA16 strains herein were classified into clusters B2a and B2b within the genotype B2. Taking into consideration other published data, we conclude that the genetic characteristics of enteroviruses in China reflect the pattern of the endemic circulation of the subgenotype C4 to EV71 and clusters B2a and B2b within genotype B2 to CVA16, which have been continuously circulating in China since 1997. This observation indicates that the genetic characteristics of enteroviruses in China seem to depend on their special geographical and climatical features allowing them to be sustained with little external effect.

Abstract:Enterovirus 71 (EV71) is a small, positive-sense, single-stranded RNA virus in the genus Enterovirus, family Picornavirus. It causes hand, foot and mouth disease in infants and children, which in a small percentage of cases progresses to central nervous system infection, ranging from aseptic meningitis to fatal encephalitis. Sporadic cases of EV71 infection occur throughout the world, but large epidemics have occurred recently in Southeast Asia and China. There are currently no approved vaccines or antiviral therapies for the prevention or treatment of EV71 infection. This paper reviews efforts to develop antiviral therapies against EV71.

Abstract:Objective Through detecting and analyzing the VP1 region of enterovirus 71,to master the epidemiological and phylogenetic relationship of EV71,which provides support and basis for prevention and treatment of hand foot and mouth disease.Methods By RT-PCR amplification of EV71 VP1 region nucleotide sequence,comparative analysis of sequence was performed using DNASTAR and Primer Premier 5.0,and to build phylogenetic trees of the samples tested VP1 region sequences and GenBank EV71 genotype from reference strains gene sequence using Mega 3.1 software.Results The homology of nucleotide and amino acid sequence in 21 EV71 isolates were within 95.6％-100％ and 99.2％-100％,respectively;the analysis of phylogenetic tree showed that 21 strains of EV71 belong to C4a sub-genotype.Conclusion The sub-genotype of EV71 strains in Hangzhou area in 2016 is consistent with the results of some regions in domestic and abroad areas,so they are expected to become a vaccine candidate for further study.

Abstract:Enterovirus 71 (EV 71) is one of the major etiological agents of hand foot and mouth disease (HFMD), which is a neuro-tropic pathogen that has been associated with many severe central nervous system (CNS) diseases and complications in infected infants’ patients. Several studies showed that EV71-induced immune responses, cytokines storm, neural apoptotic cell death and specific receptors may be associated with the neural pathogenesis of HFMD. Here reviewed the progress of research on host potential factors. And this may help to better understand the neural pathogenesis of EV71-induced HFMD.

Abstract:Enterovirus 71(EV71)is the main pathogen of severe hand-foot-mouth disease. As the characteristics and the path?ological mechanism of EV71 gradually become clear,the therapeutic strategies of EV71 infection are clarified. Acting on specific tar?get can directly interfere the process of EV71 infection,while regulating the host immune can clean the virus,ease inflammation and protect uninfected cells. In addition,some cytokines like interferon with both broad-spectrum antiviral function and immune regulation are used in clinical. In addition,drug combination could enhance the antiviral effect and reduce toxicity and drug resistance. At pres?ent,most of the therapeutics are still in pre-clinical phase,while only a few are in clinical application. This article reviews the differ?ent types of EV71 infection treatment strategies and methods recently reported,in order to provide help for further research on anti-EV71 infection.

Epidemiology of enterovirus 71 infection in children with hand, foot, and mouth disease in Shanghai, 2010-2011

Abstract:Objective To understand the clinical epidemiology of enterovirus 71 (EV71) in children with hand,foot,and mouth disease (HFMD) in Shanghai during 2010 to 2011.Methods The demographic,etiological and clinical data of children with HFMD were analyzed retrospectively.EV71 was tested in stool samples by one-step quantitative reverse transcription-polymerase chain reaction (RT-PCR).The date were analyzed by Chi-square test.Results EV71 was detected in 820 (54.45％) of 1506 inpatients in 2010 and in 924 (59.84％) of 1544 inpatients in 2011,respectively.The detection rates in severe cases and uncomplicated cases were 86.31％ and 46.67％ (x2 =247.146,P＜0.01) in 2010 and 88.78％ and 48.75％ (x2 =201.664,P＜0.01) in 2011,respectively.The detection rate of EV71 was the highest (60％- 67 ％) during May and June when HFMD peaked.Among 1744 EV71-infected HFMD inpatients,the male-to-female ratio was 1.78 ∶ 1 ; the proportion of cases was the lowest in infant ＜6 months of age (0.46％,8/1744),and the highest in children 1 years of age (34.92％,609/1744); children aged 1-3 years accounted for 76.78％ (1339/1744);and migrant children accounted for 72.76 ％ (1269/1744).The demographic characteristics in severe cases were similar to those in general EV71-infected children.Nine severe cases of pulmonary edema/hemorrhage were all infected with EV71.Conclusions EV71 was a major pathogen causing the outbreaks of HFMD and severe complications in Shanghai in 2010 and 2011.Most severe cases and all critically severe cases were associated with EV71 infection.

Abstract:Introduction: Hand, foot, and mouth disease caused by enterovirus A71 (EV-A71) is more frequently associated with neurological complications and deaths compared to other enteroviruses. Areas covered: The authors discuss current understanding of the neuropathogenesis of EV-A71 based on various clinical, human, and animal model studies. The authors discuss the important advancements in virus entry, virus dissemination, and neuroinvasion. The authors highlight the role of host immune system, host genetic factors, viral quasispecies, and heparan sulfate in EV-A71 neuropathogenesis. Expert opinion: Comparison of EV-A71 with EV-D68 and PV shows similarity in primary target sites and dissemination to the central nervous system. More research is needed to understand cellular tropisms, persistence of EV-A71, and other possible invasion routes. EV-A71 infection has varied clinical manifestations which may be attributed to multiple receptors usage. Future development of antivirals and vaccines should target neurotropic enteroviruses. Repurposing drug and immunomodulators used in combination could reduce the severity of EV-A71 infection. Only a few drugs have been tested in clinical trials, and in the absence of antiviral and vaccines (except China), active virus surveillance, good hand hygiene, and physical distancing should be advocated. A better understanding of EV-A71 neuropathogenesis is critical for antiviral and multivalent vaccines development.
