Incidence and Risk Factors of Acute Mountain Sickness among Qinghai-Tibet Railroad Passengers

Abstract:Objective:The Qinghai-Tibet railroad is the highest and longest railway of the world,it take about 24 hours travel the 2 000 km-long railroad,of which 85% is situated above 4 000 m,we thereby studied on the incidence and risk factors of acute mountain sickness in the passengers.Methods:Three subject groups were surveyed,160 lowland Han newcomers,62 Han immigrants who have been living at 2 200m to 2 500m,and 25 native Tibetans living at 3 700 to 4 200 m.We use Lake Louise scoring system(LLSS) for the diagnosis of AMS.Results:Incidence of AMS was 31% in Han newcomers,24% in Han altitude residents,and 0% in Tibetans.Particularly,passengers by train from Golmud(2 808 m) to Mt.Kunlun pass(4 768m) in less than 1.5 hours,after which 78% of the passengers reported symptoms,and 24% reaching the LLSS for AMS.Females and older subjects had a slightly greater risk for AMS.Conclusions:There is a higher incidence of AMS for the Duration of train running in Qinghai-Tibet railway line,and the incidence is associated with passenger's age,older and whether acclimation;there as,effective prevention of AMS in passengers by train and staged travel are importance for high altitude travel.
