Abstract:Objective To investigate the therapeutic effect of No. 2 burn and scald prescription on burn and scald. Methods 242 patients with burn and scald were randomly divided into two groups. 160 patients in treatment group were treated by No.2 burn and scald prescription;82 patients in control group were treated by Jingwanhong,2-3 times a day, with exposure therapy for both groups. Results The total effective rate was 100% in treatment group and 69.5% in control group, there was a significant difference between two groups (P0.01). The therapeutic effect in treatment group was obviously superior to that in control group. Conclusion The No.2 burn and scald prescription is effective in treating burn and scald, with shorter treatment course, simple application, lower cost and better therapeutic effect, without side effect, thus which deserves wide-spread clinical application.

Abstract:Objective:To investigate the effect of indirect calorimetry on nutritional treatment of patients with severe burns, and to explore the influence of this method on nutritional indicators and prognosis of patients.Methods:The research method of retrospective analysis matching control was adopted. Fifteen patients with severe burns who were injured in the explosion of the 6.13 Wenling Tank Truck in the Department of Intensive Medicine of our hospital from June 13 to July 31, 2020 were served as a measured group, and all of the patients in this group used the indirect heat measurement method of Indirect Calorimetry, IC once a week to determine energy demand and guide nutritional intervention. At the same time, 18 patients with severe burns treated by our hospital from June 2018 to June 2019 were included in the formula group, which were treated with the third military medical university burn nutrition formula (KJ/d-4104 x body surface area M2 x 105 x burn area) to estimate energy demand. Nutrition-related indicators such as albumin, retinol binding protein, hemoglobin and absolute value of lymphocytes at the time of hospitalization and weeks 1-6 after hospitalization were compared. Death-ending events were analyzed using the Kaplan-Meier curve.Results:There was no significant difference in nutritional index between the measured group and the formula group when they were admitted to the hospital. Albumins 32.79±3.88, 33.71±5.42, 35.56±10.09 g/L of the measured group were significantly higher than those of the formula group 25.69±5.76, 25.70±5.29, 29.26±5.40 g/L at week 1, 2 and 3 after admission (all P< 0.05). Retinol binding proteins 28.71±12.24, 27.71±13.02, 34.6±11.24, 45.09±15.90, 46.11±10.82 mg/L in the measured group were significantly higher than those in the formula group 11.06±10.99, 17.08±12.06, 17.73±14.11, 21.56±13.26, 21.60±13.43 mg/L at week 1, 2, 4, 5 and 6 after admission (all P< 0.05). There were no significant differences in absolute values of hemoglobin and lymphocytes between the two groups. The survival rate of the measured group was 80.0% (12/15) and that of the formula group was 55.6% (10/18). Using Breslow test, there was a significant difference between the two sets of survival curves ( P=0.033). Conclusion:Using the IC method to determine energy demand to guide the nutritional support treatment of patients with severe burns can improve the nutritional status of patients at an early stage and improve the early survival rate.
Economic Evaluation of Early Enteral Nutrition in Severely Burned Patients

Abstract:Objective: To evaluate the clinical effectiveness and cost of early enteral nutrition (EN) in severely burned patients. Methods: In a rando mized controlled clinical trial, 41 burned inpatients with total burn surface area (TBSA) over 30% were randomly assigned to EN group (n = 21) and conventional therapy (CONT) group (n = 20). The two nutritional support protocols were similar in expenditure of calorie and nitrogen intake within 7 days of treatment. Serum levels of visceral proteins were measured. Prognostic inflammatory and nutritional index (PINI) was postulated from formula. Results: Prealbum in was significantly higher in the EN group than in the CONT group [(115.8 ± 31.0) mg/L vs. (69.4 ± 17.3) mg /L, P < 0.01]. PINI was significantly lower in EN group than in CONT group ( 162.9 ± 89.4 vs. 343 ± 149, P < 0.01). A cost of ¥ 382.06 was saved in EN group. Conclusion: Early enteral nutritional support improved nutrition al status compared to conventional nutritional support, and was good in cost-effectiveness.
ECONOMIC ANALYSIS OF ENZYMATIC DEBRIDEMENT VERSUS STANDARD BURN CARE: A RETROSPECTIVE ANALYSIS

Abstract:When compared to standard surgical management, rapid enzymatic debridement of deep burns reduces the need for surgery while achieving similar long-term results. However, few studies have directly compared the costs of standard surgical and enzymatic burn care. We conducted a study comparing the care costs of 44 adult burn patients treated before (n=22) and after (n=22) introducing rapid bromelain-based enzymatic debridement (BED) of deep burns. Mean age was 59 years, 54% were male, and mean total body surface area (TBSA) was 23.5%. Burn etiology included flame and scalding burns (8). Groups treated with standard of care and enzymatic debridement were comparable in terms of age, sex and TBSA. Burn management with BED significantly reduced total debridement costs as well as grand total costs when compared with traditional surgical care. Such reduction was mostly related to lower costs associated with reduced surgical care and less facilities and resources consumption in the BED group.
Curative Effect and Safety Evaluation of Nanometer Silver in Treatment of Ⅱ Degree Burn Wound

Abstract:Objective To observe the infection prevention, wound healing effect and safety of nanometer silver in treatment of Ⅱ degree burn wound. Methods The patients with 6%-10% TBSA Ⅱ degree burn wound ( deep or superficial) were randomly divided into test group ( treated with nanome-ter silver, n =35) and control group (treated with 1% sulfadiazine silver, n =35). Each group was reg-ulated to change dressing and medication one time a day for seven days on 5% superficial Ⅱ degree burn wound. Then, 1% iodophors and gauze were used for change dressing. Before and after change dressing, bacterial culture was done in two groups to observe the time for wound healing. The blood collected before change dressing and at days 1,7 and 14 after change dressing and urine within 24 hours were employed for detecting serum silver level and mean silver level in urine by using inductively coupled plasma mass-spec-trum ( thermoelectricity of American Ⅹ Series Ⅱ ). In the meantime, the liver and renal function was e-valuated at days 7 and 14. Results Positive rate of bacterial culture in test group and control group was 3% and 3.1% respectively after treatment, which showed a decrease compared with the levels before treatment (12. 1% and 15.6% respectively), with no statistical difference. The wound healing time of test group was (9. 18 ± 1.9) days, which was shorter than ( 12.9 ± 1.3 ) days in control group, with sta-tistical difference (P<0.01). Before treatment, the silver level of blood was ( 1.55 ± 1.26) μL and ( 1.54 ± 1.28 ) μg/L respectively in test group and control group, and silver level of urine within 24 hours was ( 1.67 ± 1.05 ) μg and ( 1.87 ± 1.37) μg respectively test group and control group, with no statistical difference (P > 0. 05 ). Silver medication could elevate the serum silver level ( P < 0.01 ), with lower level in test group than control group (P <0. 01 ). Average silver level of urine within 24 hours showed similar change with that in the serum. The patients showed normal renal function, with no abnormal change of ALT. Conclusions For Ⅱ degree burn wound, nanometer silver can more significantly short-en wound healing time compared with sulfadiazine silver. Moreover, nanometer silver has higher degree of safety on silve metabolism and is an ideal medcation for burn wound.

Abstract:Objective To study the application of skin soft tissue expansion in repairing scar after deep second degree burns. Methods A total of 120 patients with scar after second degree burn treated in our hospital from February 2014 to April 2016 were selected, they were randomly divided into the observation group and the control group, 60 cases in each group. The Patients in the observation group were treated with skin and soft tissue expansion. The patients in the control group were treated with composite skin grafting. The total effective rate, infection rate, wound healing time, wound skin revascularization time and pain score were compared between the two groups. Results After treatment, the total effective rate of the observation group was 95.0%, which was significantly better than that of the control group(83.3%), the difference was statistically significant(P＜0.05). The wound healing time, skin blood circulation recovery time, pain score and infection rate of the observation group were significantly less than those of the control group(P＜0.05). Conclusion In the plastic repair of burn scar, the effect of skin and soft tissue expansion is obviously better than composite skin grafting, which can reduce the pain and infection rate of patients, improve the total clinical efficiency, and is worthy of clinical application.

Abstract:Objective: To explore the effect of irradiated biological dressing (IBD) on second degree burn wounds. Methods: Eighty patients with second-degree burns who were treated in our department were selected and randomly divided into IBD group and traditional dressing (TD) group by random number table method. The dressing change, wound healing, comfort and adverse reactions of patients in the two groups were compared and analysed. Results: The number of dressing changes in the IBD group was significantly less than that in the TD group, and the pain degree of dressing changes was significantly lower than that in the TD group (P<0.05). The dressing comfort of the IBD group was higher than that of the TD group, the secondary trauma score was lower than that of the TD group, the wound scar hyperplasia score was lower than that of the TD group, and the healing time was shorter than that of the TD group (P<0.05). There was no statistically significant difference in adverse reactions between the two groups (P>0.05). Conclusion: IBD can promote the healing of second-degree burn wounds, improve patient comfort, reduce secondary trauma and wound scarring, and improve patients' quality of life.
Cost Analysis of Pure Hypochlorous Acid Preserved Wound Cleanser versus Mafenide for the Irrigation of Burn Wounds

Abstract:Over 40,000 patients in the United States (US) require hospitalization for burns annually. The treatment regimen can cost more than $6,000 a day and requires the use of numerous supplies to ensure the graft takes for successful wound healing. Irrigation of the wound is a critical step for burn treatment, yet little is known about the cost-effectiveness of different irrigation modalities. In a recent study, pure hypochlorous acid preserved wound cleanser (pHA) was shown to be safe and effective compared to mafenide. This study estimated the associated costs of two common wound irrigation modalities, pHA and mafenide solution, for the treatment of patients with burns. In this study, a patient-level Monte Carlo simulation model using data from a randomized control trial (RCT) was used to conduct the cost analysis from the US Hospital perspective. Based upon 100,000 simulated patients, pHA was expected to save $133 ($123 to $144, 10th to 90th percentile) for the hospital compared to using a mafenide solution over 14 days. Adoption of pHA should be considered a cost-saving strategy when treating patients with burns.
Cost-Effectiveness Comparison Between Topical Silver Sulfadiazine and Enclosed Silver Dressing for Partial-Thickness Burn Treatment

Abstract:The standard treatment of partial-thickness burns includes topical silver products such as silver sulfadiazine (SSD) cream and enclosed dressings including silver-impregnated foam (Mepilex Ag; Molnlycke Health Care, Gothenburg, Sweden) and silver-laden sheets (Aquacel Ag; ConvaTec, Skillman, NJ). The current state of health care is limited by resources, with an emphasis on evidence-based outcomes and cost-effective treatments. This study includes a decision analysis with an incremental cost-utility ratio comparing enclosed silver dressings with SSD in partial-thickness burn patients with TBSA less than 20%. A comprehensive literature review was conducted to identify clinically relevant health states in partial-thickness burn patients. These health states include successful healing, infection, and noninfected delayed healing requiring either surgery or conservative management. The probabilities of these health states were combined with Medicare CPT reimbursement codes (cost) and patient-derived utilities to fit into the decision model. Utilities were obtained using a visual analog scale during patient interviews. Expected cost and quality-adjusted life years (QALYs) were calculated using the roll-back method. The incremental cost-utility ratio for enclosed silver dressing relative to SSD was $40,167.99/QALY. One-way sensitivity analysis of complication rates confirmed robustness of the model. Assuming a maximum willingness to pay $50,000/QALY, the complication rate for SSD must be 22% or higher for enclosed silver dressing to be cost effective. By varying complication rates for SSD and enclosed silver dressings, the two-way sensitivity analysis demonstrated the cost effectiveness of using enclosed silver dressing at the majority of complication rates for both treatment modalities. Enclosed silver dressings are a cost-effective means of treating partial thickness burns.

Analysis of curative effect of insulin external application on burn wounds of diabetic patients with different depths

Abstract:To explore the curative effect of insulin external application on burn wounds of diabetic patients with different depths. A retrospective analysis of 114 diabetic burn patients in the First Hospital of Hebei Medical University from June 2019 to June 2022. According to the different treatment methods, they were divided into study group (insulin therapy) and control group (conventional therapy) with 57 cases in each. The wound healing time, dressing changes, scar healing after wound healing and adverse events were compared between two groups. Pain level, serum inflammatory factors, vascular endothelial growth factor (VEGF) and oxidative stress factors before and after treatment were compared. The wound healing time (17.23 ± 2.18 vs 20.31 ± 2.09 days) and the number of dressing changes (7.01 ± 1.23 vs 8.93 ± 1.32 times) in study group were significantly lower than those in control group (P < 0.05). Before treatment, there was no difference in pain level, VEGF, interleukin-1 (IL-1), tumour necrosis factor-α (TNF-α), malondialdehyde (MDA) and superoxide dismutase (SOD) between two groups (P > 0.05). However, the pain level, scar healing, IL-1, TNF-a and MDA in study group were significantly lower than those in control group after treatment (P < 0.05). And the VEGF and SOD in study group was significantly higher than that in control group (P < 0.05). External application of insulin can shorten the wound healing time of diabetic patients with different depths, reduce the number of dressing changes, promote scar healing after wound healing, relieve pain and reduce the level of inflammatory factors, which is worthy of clinical promotion.
EFFECTS OF EARLY TOTAL ENTERAL NUTRITION ON SEVERELY BURNED PATIENTS

Abstract:Objective: To evaluate the clinical effectiveness and safety of early total enteral nutrition (TEN) in severely burned patients. Methods: Forty one burned patients with total burn surface area over 30% were randomly assigned to TEN group (n=21) and CONT group (n=20). The two nutritional support protocols were similar in calorie and nitrogen intake within 7 d of treatment. Serum levels of visceral proteins and TNF-α were measured. Results: Contrasting to a significant reduction (from 96.0±31.8mg/L to 69.4±17.3 mg/L, P0.01) of prealbumin in CONT group, remarkable increase (from 99.9±25.1mg/L to 115.8±31.0 mg/L) was observed in TEN group. C-reaction protein(CRP)and TNF-αlevels were significantly lower in the TEN group than in the CONT group. PINI increase was significantly higher in CONT group than in TEN group. Side-effects were similar in both groups. Conclusions: Early total enteral nutritional support improved nutritional status compared to conventional nutritional support,and was good in safety and practice in severely burned patients.
607 Cultured Epidermal Autografts versus Spray Keratinocytes for Massive Burns—a Cost Effectiveness Analysis

Abstract:Abstract Introduction Massive burn injuries are challenging to treat surgically due to limited donor skin. Autologous epidermal grafts such as cultured epidermal autografts (CEAs) and spray keratinocyte suspensions are successful strategies for wound closure when donor sites are limited. There are no investigations to date that describe the differential outcomes or costs between these competing strategies. Cost-effectiveness analysis is required to guide payors, hospitals, and policy makers in determinations of care. Methods A cost-effectiveness analysis compared CEAs with spray keratinocytes in adult burn patients with >50% total body surface area deep partial thickness burns. Hybrid Monte Carlo simulation and Markov modeling studied cost-utility from the payer perspective. Study data were derived from randomized clinical trials and cohort investigations of CEAs and spray keratinocytes. The time horizon was 1 year with 1 month cycling. Model costs included the product, surgery (both physician and facility fees), and hospitalization. Surgical costs were derived from Medicare reimbursement fee schedules. Costs of epidermal autografts were determined using currently available marketing material. All costs were inflated to 2023 US dollars, with a standard 3% discounting rate. Model utilities were derived from the Vancouver Scar Scale (VSS) with 1 as the best outcome and 0 as the worst outcome. The incremental cost-effectiveness ratio (ICER) was derived from the cost difference between the two strategies divided by the difference in adjusted VSS. Deterministic and probabilistic sensitivity analyses were performed varying all model parameters. Results CEAs achieved successful wound closure in 73% of simulations compared to 88% for spray keratinocytes. Compared to treatment with CEAs, treatment with spray keratinocytes resulted in cost savings of $254,743, with no compromise in overall utility of treatment based on VSS. As such, spray keratinocyte treatment was the dominant strategy. This finding was robust upon sensitivity analyses. Conclusions Spray keratinocyte suspensions were a dominant strategy to CEAs (i.e. cost saving without compromising utility). Applicability of Research to Practice Payers and providers should consider the cost utility of spray keratinocytes as the dominant treatment strategy for massive burn epidermal grafting. CEA manufactures may want to consider cost reductions to be more economically competitive with alternative strategies.

Abstract:Objective: To observe and explore the clinical therapeutic effects of a self-developed novel low-temperature boric acid wet dressing on the face and neck wounds of patients with superficial burns. Methods: A prospective randomized controlled study was conducted. One hundred cases of superficial burn (sunburn) patients who met the inclusion criteria were admitted to Department of Dermatology of Chongqing Hospital of Traditional Chinese Medicine from October 2016 to June 2018, the course of sunburn was less than 15 days. According to the of random number table, the patients were divided into new dressing (ND) group (30 males and 20 females, aged (55±14) years) and conventional dressing (CD) group (28 males and 22 females, aged (59±12) years). Patients in ND group were treated with a self-developed new low-temperature boric acid wet dressing, and patients in CD group were treated with normal temperature boric acid solution wet dressing, 3 times a day. The Eczema area and severity index (EASI) score and Visual Analogue Scale (VAS) score of patients before and 14 days after treatment (patients discharged from hospital within 14 days were recorded on the day of discharge), the number of cured patients at 6, 8, 10, and 11 to 14 days after treatment, and the efficacy were compared between the patients in 2 groups. The nurse operators were investigated by self-made convenience questionnaire, and the time and convenience required of 2 dressings were compared. Data were statistically analyzed with independent sample t test or Mann-Whitey U test. Results: Before the treatment, the EASI and VAS score in those two groups showed no significantly difference (t=1.576, 1.492, P>0.05). At 14 days after treatment, the EASI score (2.4±0.4) points in ND Group was significantly lower than (4.6±0.7) points in CD Group (t=3.552, P<0.01); the VAS score (0.51±0.12) points in ND Group was significantly lower than (0.98±0.19) points in CD Group (t=3.496, P<0.01). At 14 days after treatment, the cured time of sunburns in ND Group was significantly shorter than that in CD Group (Z=-6.690, P<0.01); the treatment effects of ND Group showed better than that in CD Group (Z=3.387, P<0.01). The time for nurses operating ND was significantly shorter than that in CD (Z=-5.575, P<0.01); the nurses also believed the operation of ND was more convenient than CD (Z=-4.304, P<0.01). Conclusions: Compared with that of CD, the application of ND can shorten the recovery time and improve the treatment efficiency. At the same time, the application of ND can significantly reduce the time of nursing operations, and the material is easy to use. This new material is worthy of clinical promotion for the treatment of superficial burns.
Cost Analysis of a Novel Enzymatic Debriding Agent for Management of Burn Wounds

Abstract:Introduction. Given its efficacy and safety, NexoBrid™ (NXB) has become part of our therapeutic options in burns treatment with satisfactory results. However, no cost analysis comparing NXB to the standard of care (SOC) has been carried out as of today. Aim. To assess the cost of treatment with NXB and compare it to the SOC cost. Methods. 20 patients with 14-22% of TBSA with an intermediate-deep thermal burn related injury were retrospectively and consecutively included. 10 of these patients were treated with the SOC, while the other 10 with NXB. The cost analysis was performed in accordance with the weighted average Italian Health Ministry DRGs and with Conferenza Stato/Regioni 2003 and the study by Tan et al. For each cost, 95% confidence intervals have been evaluated. Results. Considering the 10 patients treated with NXB, the overall savings (total net saving) amounted to 53300 euros. The confidence interval analysis confirmed the savings. Discussion. As shown by our preliminary results, significant savings are obtained with the use of NXB. The limit of our study is that it is based on Italian health care costs and assesses a relative small cohort of patients. Further studies on larger multinational cohorts are warranted.

Abstract:Objective: To explore the effects of multidisciplinary comprehensive intervention in integrated treatment of patients with extremely severe burns. Methods: One hundred and ten patients hospitalized in our center from July 2013 to August 2017 met the criteria for inclusion in this study, and their medical records were retrospectively analyzed. According to the medical model at that time, 56 patients hospitalized from July 2013 to July 2015 received routine comprehensive treatment led by doctors, and they were included in the conventional intervention group. From August 2015 to August 2017, 54 patients were treated with integrated multidisciplinary interventions performed by a team consisting of physicians, intensive care nurses, burn nurses, intravenous infusion nurses, wound stoma nurses, and rehabilitation therapists, and they were included in the integrated intervention group. Acute Physiological and Chronic Health Assessment Ⅱ (APACHE Ⅱ) scores on admission and 4 weeks after treatment, incidences of pressure injury, wound sepsis, lung infection, and catheter-related infection during hospitalization, length of stay in intensive care unit (ICU) and total length of hospital stay, and Abbreviated Burn Specific Health Scale (BSHS-A) scores at discharge and 3 months after discharge were analyzed. Data were processed with t-test and chi-square test. Results: The APACHE Ⅱ scores of patients in the two groups were close on admission (t=0.573, P>0.05). Four weeks after treatment, the APACHE Ⅱ scores of patients in the two groups were obviously lower than those on admission within the same group (t=5.697, 4.853, P<0.01), and the score of the integrated intervention group was obviously lower than that of the conventional intervention group (t=2.170, P<0.05). No pressure injury was observed in patients of any group during hospitalization. The incidences of wound sepsis, lung infection, and catheter-related infection of patients in the integrated intervention group were 18.5% (10/54), 3.7% (2/54), and 9.3% (5/54), respectively, significantly lower than 42.9% (24/56), 21.4% (12/56), and 26.8% (15/56) of the conventional intervention group, χ2=4.073, 6.075, 3.962, P<0.05. The length of stay in ICU of patients in the integrated intervention group was (50±5) d, obviously shorter than (62±4) d of the conventional intervention group (t=2.852, P<0.01). The total length of hospital stay of patients in the integrated intervention group was (115±8) d, obviously shorter than (140±7) d of the conventional intervention group (t=16.885, P<0.01). At discharge and 3 months after discharge, the scores of BSHS-A, physical function, psychological function, and general health status of 50 patients in the integrated intervention group were significantly higher than those of 48 patients in the conventional intervention group (t=2.886, 3.126, 2.416, 2.544, 2.033, 3.471, 2.588, 2.210, P<0.05 or P<0.01), while the scores of social function of patients in the two groups were close (t=1.570, 1.350, P>0.05). Conclusions: Integrative treatment of patients with extremely severe burns by multidisciplinary comprehensive intervention can significantly alleviate the severity of burns, reduce the incidences of wound sepsis, lung infection, and catheter-related infection, shorten the length of stay in ICU and total length of hospital stay, and improve the quality of life.

[A cost-effectiveness analysis of comprehensive rehabilitation treatment of hand burn]

Abstract:Objective: To observe the effect of comprehensive rehabilitation treatment on hand burn, and to make a cost-effectiveness analysis. Methods: Sixty-two patients with ninety-eight affected hands were divided into rehabilitation group (32 cases, 48 hands) and control group (30 cases, 50 hands). Patients in rehabilitation group received comprehensive rehabilitation treatment at early stage after burn; patients in control group were given instructions for function training at the same time. The functions of the hands to be restored including grasp, hold, pinch, nip, forearm pronation and supination, fetching, laying, and writing abilities of patients in both groups were quantitatively evaluated with Carroll's upper extremity function test before treatment and 5 months after. Direct medical costs of patients in both groups within 5 months were respectively added up to make a cost-effectiveness analysis. Results: In rehabilitation group, function of digital opposition, palmar opposition, holding, and pinching of 37 hands recovered well, with which patients could pick food, put on clothes, go to toilet, and self-care etc. independently. Function of digital opposition, palmar opposition, holding, pinching half recovered in 7 hands, accompanied with well recovered of metacarpophalangeal function, but recovery of function of interphalangeal joint was less satisfactory. Although patients could grasp and hold, they were still poor in fine and harmonized activities. Joint ranges of motion of 4 hands were poor with limited function, and this was resulted from not strictly following treatment for remaining granulation wound. In control group, 23 hands received reconstructive surgery, 14 of them recovered with good function, but were poor in most of fine and harmonized activities. Severe claw hands were found in 13 hands. The ratio between total mean cost value and total function increment value in rehabilitation group (181 +/- 11) was obviously lower than that in control group (298 +/- 30, P < 0.01). Conclusions: Comprehensive rehabilitation treatment at early stage after hand burn has a good effect on prevention and treatment of hand deformity, promoting recovery of hand function and improving hand appearance. It is also less costly.

Abstract:Objective: To explore the effect of early eschar dermabrasion combined with antimicrobial soft silicone foam dressing (hereinafter referred to as foam dressing) in treating the deep partial-thickness burn wounds in children. Methods: This study was a randomized controlled trial. From June 2021 to December 2022, 78 pediatric patients with deep partial-thickness burns who met the inclusion criteria were admitted to the Department of Burns in Guiyang Steel Plant Employees Hospital. According to the random number table, the pediatric patients were divided into two groups, with 38 cases left in combined treatment group (with 20 males and 18 females, aged 26.00 (16.75, 39.75) months) and 39 cases in foam dressing group (with 21 males and 18 females, aged 19.00 (14.00, 31.00) months) after the exclusion of one dropped-out child in follow-up. The pediatric patients in combined treatment group underwent eschar dermabrasion of the wound within 48 hours after injury, the wound was covered with foam dressing after operation, and the dressing was replaced once every 7 days; for the pediatric patients in foam dressing group, the wound was sterilized within 48 hours after injury and covered with foam dressing, and the dressing was replaced once every 2 to 3 days. After the wound healing, the children in both groups were routinely applied with silicone gel twice a day for 3 weeks before started wearing elastic sleeves for more than 18 hours a day, and continuously for over than 6 months. The degree of pain during dressing change was evaluated using the children's pain behavior inventory FLACC. The adverse reactions during the treatment period, number of dressing changes, and wound healing time were observed and recorded. Six months after wound healing, the Vancouver scar scale (VSS) was used to evaluate the condition of the wound scar. Results: When changing dressing, the FLACC score for pain of pediatric patients in combined treatment group was 3.5 (2.0, 5.0), which was significantly lower than 6.0 (5.0, 8.0) in foam dressing group (Z=-5.40, P<0.05). During the treatment period, no adverse reactions such as wound edema, fluid accumulation, or peripheral skin rash allergies occurred in any pediatric patient in both groups. The number of dressing changes of pediatric patients in combined treatment group was 3 (3, 4) times, which was significantly less than 8 (7, 10) times in foam dressing group (Z=-7.58, P<0.05). The wound healing time of pediatric patients in combined treatment group was (19±5) days, which was significantly shorter than (25±6) days in foam dressing group (t=-4.48, P<0.05). Six months after wound healing, the VSS score for scar of pediatric patients in combined treatment group was 5 (2, 8), which was significantly lower than 7 (5, 10) in foam dressing group (Z=-3.05, P<0.05). Conclusions: Compared with using foam dressings alone, early eschar dermabrasion combined with foam dressings can reduce the number of dressing changes, alleviate the pain during dressing changes, and shorten the wound healing time in treating children with deep partial-thickness burns, and effectively alleviate scar hyperplasia by combining with anti-scar treatment post burns.

Abstract:BACKGROUND Body parts such as the face and hands are highly exposed during daily life and burns may accumulate in these areas. In addition, residual wounds, scar hyperplasia and contracture often exist in the late stage of a deep burn in these areas, which may affect patients' appearance, movements, and mental health. However, inadequate attention has been paid to this issue which can result in problems, such as difficulty in healing, possibility of carcinoma, chronic pain and a heavy mental burden. AIM To investigate the long-term medical treatment of patients with severe burns at exposed sites following a mass burn casualty event. METHODS A retrospective analysis of 13 patients with severe burns at exposed sites was performed to determine their respective long-term medical treatment. A combined wound dressing scheme consisting of traditional Chinese and Western medicine was introduced to repair residual wounds. Active and passive functional exercises with massage, Chinese herbal baths and compression fixation were proposed to ameliorate the condition of the hands. A combination of physical, chemical and photoelectrical measures was adopted for anti-scar treatment. A psychological intervention and recovery guide was provided which corresponded to the patients' psychological status. RESULTS Compared to patients who did not simultaneously receive the same treatment, patients who underwent systematic treatment recovered with a lower wound infection rate (P< 0.05), a shorter healing time (13.6 +/- 3.2 d) compared with (19.1 +/- 3.5 d) and more bearable pain during wound dressing at three days, one week and two weeks after a Chinese herbal bath (P< 0.05). Satisfactory results were achieved with regard to restored function of patients' joints and blood supply to nerve endings, closure of the eyelids and the size of mouth opening tended to be normal, and only 7.1% of patients were diagnosed with severe scar hyperplasia and contracture deformity compared with 30.7% in the control group. In addition, the color, thickness, vascular distribution and softness score of the scars improved (P< 0.01), and the effects of the psychological intervention was remarkable as shown by the Self-Rating Anxiety Scale and Self-Rating Depression Scale. CONCLUSION A better prognosis can be achieved in patients in the late stage of a burn with active residual wound repair, limb functional exercise, anti-scar and psychological rehabilitation.
