Age- and Gender-Specific in Direct Hospitalization Costs for Hemorrhagic Stroke Inpatients with Basic Medical Insurance Scheme for Employees in China

Abstract:This study was to explore whether the similar age and gender differences exist in direct hospitalization costs for inpatients with hemorrhagic stroke based on data supplied by China Health Insurance Research Association from 2010 to 2012. A nationwide, cross-sectional sampling of hemorrhagic stroke inpatients with disease code ICD-10 (I60,I61,I62) with basic medical insurance scheme for employees was extracted from China Health Insurance Research Association claim database.We use age and gender as variables, and analyze cost per hospitalization, cost per hospital day, out-of-pocket payment and reimbursment rate. This study contained 67.33% men and 32.67% wemen.For male inpatiens,cost per hospitalization, cost per hospital day, out-of-pocket payment and reimbursment rate were ¥25,255(95% CI, 23,857-26,652), 1,305(95% CI, 1,241-1,369), 6,896(95% CI, 6,449-7,343), 70.96%(95% CI, 70.4%-71.51%);but for female inpatients those were ¥25,771(95% CI, 23,911-27,631), 1,472(95% CI, 1,303-1,645), 7,155(95% CI, 6,487-7,823), 71.46%(95% CI, 70.67%-72.25%).Female spent more money than male, but there was no significance difference between the genders.With age increasing, cost per hospitalization, cost per hospitalization and out-of-pocket payment showed an inverse relation to the age, and the largest costs was under 40 years. There was a significant difference between different age groups (P<0.05).Reimbursment rate had a positive correlation with age.The averge reimbursement rate was 71.09%,and the highest reimbursment rate was 74.72% with people aged over 80 (P<0.05). Gender differences in direct hospitalization costs for hemorrhagic stroke inpatients with basic medical insurance scheme for employees do occur in China, but there was no significance difference. In order to reduce the disease economic risk of the older, China established various payment system for different people, which gave more reimbursement for seniors. This study have proved that older get more protection of the disease economic risk.
