Abstract:Based on the optical characteristic parameters and thermal characteristic parameters of bio-tissue and the relation between blood perfusion and bio-tissue's thermal damage, the transmission of laser through the biotissue, transient heat response and thermal damage zone of tissue are numerical simulated. By analyzing the effect of laser power input and the size of laser head on the curative effect of thermotherapy, some suggestions useful for tumour clinical treatment were proposed and discussed.

Abstract:Laser thermotherapy is a technique used for tumor treatment. It generates a local heating, causes thermal coagulation of living tissue and eliminates the tumor. Precise heating of tumor tissue with healthy minimum thermal injury to adjacent tissue is essential to thermotherapy. Understanding of heat transfer and optical-thermal interaction is important for control of temperature and design of thermotherapy. This study applies the Arrhenius damage model to describe the heat-induced change of optical properties. It calculates the distribution temperature, damage and optical-thermal response of bio-tissue during the laser treatment, and shows how these factors affect the effectiveness of laser thermotherapy. Similar research has been performed by Kim and coworkers [1996], Iizuka and coworkers [2000], and Whelan and coworkers [2000]. This study relaxes some conditions in previous investigations. It reveals the importance and the effect of size of the laser head.

Abstract:Various phase-changing thermal effects, i.e. evaporization, carbonization and melting, occur sequentially in the biological tissue irradiated by high-intensity laser. According to this practical thermal effect, a new heat transfer model for laser-tissue thermal interaction is proposed. In the model two heterogeneous tissues, i.e. carbonization layer and bio-tissue layer, are considered and two different phase-changing interfaces are introduced. Some parameters such as the temperature and moving velocity on the interfaces as well as the carbonization depth are obtained by numerical solution. The relationship between these parameters and the laser power density is studied. Numerical simulation shows that the laser-tissue thermal interaction model has two stages: the initial non-steady stage and the succeeding quasi-steady stage with the characteristic of constant parameters such as interface temperature, interface moving rate and carbonization zone etc. Some laser-tissue thermal damage experimental results are presented and discussed by contrast with the mathematical simulation results. The predicted carbonized depth and thermal damage depth coincide well with experimental observations.

Abstract:The use of lasers in medical applications has dramatically increased over the past half century. For treatment quality, the control of thermal damage is absolutely necessary. Living tissue is highly non-homogenous, and the effect of local non-equilibrium on the thermal behavior should be accounted. Therefore, this work uses the dual-phase-lag model to analyze thermal response for estimating thermal damage in laser-irradiated biological tissue. The transport behavior of laser light in the tissue is assumed as highly absorbed and strongly scattered, respectively. Thermal damage is assessed with the rate process equation. The effects of blood perfusion and metabolic heat generation on thermal response and thermal damage are explored. The reliability of the present results has been evidenced through comparison with the literature. The results show the DPL bio-heat transfer equation with the effect of blood perfusion, even with the space-dependent source term, can be reduced to the Fourier bio-heat transfer equation for τq=τT.

Abstract:Comprehension of thermal behavior underlying the living biological tissues helps successful applications of current heat therapies. The present work is to explore the heat transport properties of irradiated tissue during tis thermal treatment, in which the local thermal non-equilibrium effect as well as temperature-dependent properties arose from complicated anatomical structure, is considered. Based on the generalized dual-phase lag (GDPL) model, a non-linear governing equation of tissue temperature with variable thermal physical properties is proposed. The effective procedure constructed on an explicit finite difference scheme is then developed to predict numerically the thermal response and thermal damage irradiated by a pulse laser as a therapeutic heat source. The parametric study on variable thermal physical parameters including the phase lag times, heat conductivity, specific heat capacity and blood perfusion rate has been performed to evaluate their influence on temperature distribution in time and space. On this basis, the thermal damage with different laser variables such as laser intensity and exposure time are further analyzed.

Abstract:During laser-induced interstitial thermotherapy,the laser-tissue interaction includes two aspects:one is photon migration,the other is bioheat transfer.The distribution of photons and energy in tissue can be calculated with Monte-Carlo simulation based on transfer theory.With Pennes equation and Weinbaum Jiji model,temperature distribution and changes in tissue could be described.It is very important to unite the both models.Dynamical process of coagulation is monitored through changes of the tissue's scattering characteristics and new experimental results are discussed.It makes quantitative description more exact and noninvasive optical monitoring possible.

Abstract:Laser ablation is an efficient technique of thermal treatment for tumor, during which process, temperature of the tumor tissue will be elevated by the laser beam. Irreversible burn and thermal deformation appear in the tissue, which can induce the protein denaturation and cell membrane looseness to kill the cancer cells. In order to investigate the thermo-mechanical responses of the skin during therapy process, a mechanical model consisting of an elastic plate resting on viscoelastic foundation was proposed to describe the irradiated skin tissue which is subjected to a moving laser beam. The DPL bio-heat conduction model, Kirchhoff hypothesis and the Kelvin-Voigt model were employed in the present study. The governing equations were analytically solved with the usage of Green's function method and the closed-form solutions for the temperature, deformation and stress distribution in the tissue were obtained. A numerical example was given to illustrate the analytical solutions and representative results were shown in figures to make it more intuitive. The influences of laser moving speed and the phase lags of heat flux vector and temperature gradient were discussed in detail.

Abstract:It is important to enhance the understanding and accurately assessing heat laser-induced thermal conduction and thermal damage to biological tissues. In order for ensuring the personal safety and accuracy of treatment efficiency in medical applications, in the present paper, the Cattaneo-Vernotte model for non-Fourier heat conduction subjected to laser heat source is present and simulated by Discontinuous Galerkin Finite Element Method (DGFEM). The simulated results exhibit the accuracy and consistency with the comparison to the analytical solutions. Furthermore, thermal damage process is combined with non-Fourier heat conduction to show the influence of damage parameter assessed with the variation of thermal relaxation time. It is indicated that the thermal damage is significantly different compared to that of the classical heat conductive model.

Abstract:A generalized dual phase lag (DPL) bioheat model based on the nonequilibrium heat transfer in living biological tissues is applied to investigate thermal damage induced by laser irradiation. Comparisons of the temperature responses and thermal damages between the generalized and classical DPL bioheat model, derived from the constitutive DPL model and Pennes bioheat equation, are carried out in this study. It is shown that the generalized DPL model could predict significantly different temperature and thermal damage from the classical DPL model and Pennes bioheat conduction model. The generalized DPL equation can reduce to the classical Pennes heat conduction equation only when the phase lag times of temperature gradient (τ T ) and heat flux vector (τ q ) are both zero. The effects of laser parameters such as laser exposure time, laser irradiance, and coupling factor on the thermal damage are also studied.

Abstract:We consider an inverse problem arising in laser-induced thermotherapy, a minimally invasive method for cancer treatment, in which cancer tissues are destroyed by coagulation. For the dosage planning, numerical simulations play an important role. To this end, a crucial problem is to identify the thermal growth kinetics of the coagulated zone. Mathematically, this problem is a nonlinear and nonlocal parabolic heat source inverse problem. The solution to this inverse problem is defined as the minimizer of a nonconvex cost functional in this paper. The existence of the minimizer is proven. We derive the Gateaux derivative of the cost functional, which is based on the adjoint system, and use it for a numerical approximation of the optimal coefficient. A Distribution-Based Method for Thermal Damage Model Analysis and Optimization in Brain Laser Interstitial Thermal Therapy

Abstract:MR-guided laser interstitial thermal therapy (LITT) has been regarded as a promising treatment option in neurosurgical oncology. One major advantage of MR-guided LITT is that temperature measurement can be provided to monitor the ablation in nearly real-time. The intraoperative temperature images make it possible to estimate thermal damage during the ablation. Several thermal damage models have been developed and used in LITT procedure, but the model parameters were mainly derived from laboratories in vitro experiments and varied in a wide range. As the frequency of MR-guided LITT procedure increases, there is a growing need to optimize thermal damage models utilizing clinical data. In this study, we dug into the relationship between the temperature history and postoperative ablation image collected in brain LITT surgery. Compared with the previous studies where the thermal damage models could only predict two tissue states: normal and ablated, our study considered three tissue states for thermal damage prediction: ablated, transitional and normal. We noticed that for different tissue states, the damage values calculated according to the Arrhenius formula were in different distribution patterns. Therefore, we developed a distribution-based thermal damage model. MR images of 15 patients were used in our study. The results showed that the proposed method could predict three tissue states and had higher prediction accuracy than the traditional Arrhenius model in clinical application.

Simulation of the Cooling Effect of Blood Flow in Laser-Induced Interstitial Thermotherapy

Abstract:A two-layer model with a single blood vessel was developed to investigate the cooling effect for blood flow during laser-induced interstitial thermotherapy(LITT).The laser energy distribution was simulated using the Monte Carlo method.The Pennes bio-heat equation and the energy equation for the blood vessel were solved to calculate the temperature distribution.The corresponding thermal damage distribution was determined by the Arrhenius rate process formulation.Different sizes of blood vessels with different velocities were compared.The results show that the blood vessel diameter and the average blood velocity are the crucial factors in the temperature determination and thermal damage distributions near the blood vessel.LITT may not work when the laser power is less than 5 W and the blood vessel diameter is larger than 1 mm if the therapy plan is based on a model without blood vessels.

Abstract:In this paper, a time-fractional dual-phase-lag (DPL) bio-thermoelastic model is presented to solve the thermoelastic response of a biological tissue during hyperthermia treatment by a moving laser heating. To improve the elimination rate of diseased tissue and reduce the side effects to surrounding normal tissue in hyperthermia, it is necessary to study the thermoelastic response of biological tissues under high-speed and high-intensity laser heating. The laser heating process causes a large instantaneous temperature difference and heat flow in the tissue, which leads to abnormal diffusion of heat conduction. The classical Fourier heat conduction model cannot describe this anomalous diffusion phenomenon due to infinite heat propagation velocity. In order to describe anomalous diffusion in tissue, the fractional derivative is introduced into the DPL heat conduction model. In order to solve the elastic deformation of tissue under heating, the biological fractional thermoelastic theory is established by combining the fractional DPL model with one-dimensional thermoelastic theory. The physical significance of the phase lags and fractional order is elucidated. The thermoelastic responses are compared with those based on existing models. Finally, the effects of blood perfusion rate and laser velocity on temperature distribution and thermal stress response during percutaneous laser thermal ablation are analyzed.

Abstract:Laser ablation is a minimally invasive therapeutic technique to denature tumors through coagulation and/or vaporization. Computational simulations of laser ablation can evaluate treatment outcomes quantitatively and provide numerical indices to determine treatment conditions, thus accelerating the technique's clinical application. These simulations involve calculations of light transport, thermal diffusion, and the extent of thermal damage. The optical properties of tissue, which govern light transport through the tissue, vary during heating, and this affects the treatment outcomes. Nevertheless, the optical properties in conventional simulations of coagulation and vaporization remain constant. Here, we propose a laser ablation simulation based on Monte Carlo light transport with a dynamic optical properties (DOP) model. The proposed simulation is validated by performing optical properties measurements and laser irradiation experiments on porcine liver tissue. The DOP model showed the replicability of the changes in tissue optical properties during heating. Furthermore, the proposed simulation estimated coagulation areas that were comparable to experimental results at low-power irradiation settings and provided more than 2.5 times higher accuracy when calculating coagulation and vaporization areas than simulations using static optical properties at high-power irradiation settings. Our results demonstrate the proposed simulation's applicability to coagulation and vaporization region calculations in tissue for retrospectively evaluating the treatment effects of laser ablation.

Abstract:When high power laser irradiates the skin-tissue,the skin-tissue will be damaged.This paper based on the interaction theory mechanism of laser-biotissue,the 2-D heat transfer model of laser-irradiated three layers skin-tissue was established.By using the blood perfusion rate in the Pennes' bioheat transfer equations,and finite method was used to solve the numerical simulation results for temperature distribution.Study result shows that the thermal damage of vivo skin-tissue irradiated by laser take place firstly at the center of the beam of light,and the temperature decrease in the z and radial direction.There is evident difference in temperature between consider the heterogeneous thermal properties of living tissues and not consider it.

Abstract:This paper based on the interaction theory mechanism of laser-biotissue,the 2-D heat transfer model of laser-irradiated invitro human skin-tissue was established.Finite method was used to solve the numerical simulation results for temperature distribution for the Pennes′ bioheat transfer equations.Experiments about damage in invitro human skin-tissue irradiated by continuous laser are carried out.It is found that when the irradiation time reaches 6.83s,the heated evaporization is occurred.When the irradiation time reaches 11.20s,the heated carbonization is occurred.When the irradiation time reaches 25.30s,the heated combustion is occurred.The result of theoretical analysis exhibits good agreement with the experimental result.

Abstract:Laser irradiation in conjunction with surface cooling,called Low Invasive Laser Coagulation (LILC),provides a promising therapy for superficial cancer,which can cauterize the inner tissue with the tissue surface undamaged.In order to provide a further insight into this therapy,this paper developed a transient finite difference numerical model of the LILC method. It was then used to simulate the light propagation and heat transfer inside the tissue. The effects of various optical coefficients (such as scattering and absorption coefficients,anisotropy parameter,etc.) and thermophysical properties (such as surface convective coefficient,etc.) and physiological coefficients (such as blood perfusion rate and metabolic heat generation,etc.) on the thermal behavior of the LILC method were discussed in detail. The results show that the size and location of cauterized tissue under the skin can be bontrolled by properly adjusting the absorption and scattering coefficients and irradiation power and so on.

Abstract:The laser irradiation of living tissues poses a risk of thermal damage, making it a critical factor in medical procedures such as laser surgery and thermal therapies. Effectively predicting and managing this damage, particularly in hyperthermia therapy, is essential for maximizing treatment efficacy while protecting surrounding healthy tissues. In this context, theoretical and computational models of biological heat transfer, especially the enhanced Pennes bioheat transport equation, have attracted significant research interest. This study contributes to the field by providing a novel analytical solution to the refined Pennes bioheat model, incorporating the three-phase lag (TPL) concept. The research examines heat transfer in a one-dimensional region, where the outer surface is exposed to laser heating while the inner surface remains thermally insulated. It explores the mechanical effects of thermal shock induced by laser treatment, focusing on heat generation patterns across different laser intensities in diseased human skin tissues. To validate the model, numerical inverse and Laplace transform techniques were applied, producing results consistent with existing literature. The findings not only advance the theoretical understanding of bioheat transfer but also enhance the safety and effectiveness of laser-based medical therapies.

Abstract:The thermal damage of a three-dimensional bio-tissue model irradiated by a movable laser beam was studied in this work. By employing the DPL biological heat conduction model and Henriques’ thermal damage assessment model, the distribution of burn damage of vivo human tissue during laser therapy was analytically obtained. The influences of laser moving velocity, laser spot size, phase lags of heat flux and temperature gradient were discussed. It was found that the laser moving speed and the laser spot size greatly influence the thermal damage degree by affecting the energy concentration degree. The increases of the laser moving speed and laser spot size can enlarge the irradiated region and reduce the burn degree. A greater phase lag of temperature gradient led to lower accumulation of thermal energy and lower burn degree. However, the increment of heat flux phase lag leads to the thermal energy accumulation and more serious burn degree in the irradiated region.
