Abstract:Objective:To study the curative effect of external application of traditional Chinese medicine(TCM) on lumbar disc herniation(LDH).Methods:116 LDH patients met the criteria were randomly divided into 2 groups,58 cases in each group.Patients in treatment group were administrated with external application of TCM,while the others in control group were administrated with physiotherapy of electromagnetic wave.The curative effects were evaluated after two courses of treatment.Results:The curative effect of treatment group was better than that of control group,there was statistical difference between the 2 groups(u=2.830,P=0.003).Conclusion:The method of external application of TCM has an exact curative effect on LDH,and it is worth popularizing in clinic.
Electro -Acupuncture Mainly at Jiaji Point in the Treatment of Prolapse of Lumbar Intervertebral Disc:An Analysis of 60 Cases

Abstract:Objective:To observe the clinical therapeutic effects of electroacupuncture at Jiaji(EX-B2) and sacral injection method on prolapse of lumbar intervertebral disc. Methods:One hundred and twenty cases were randomly dividid into electroacupuncture group and sacral injection group.The condition of pain and lumbar function were observed after treatment of 2 courses seperately by using Visual Analogue Scale and JOA Scale and the follow up carried on 3 months later. Results:The cured effective rate of 66.67%in electroacupuncture group was better than 48.33% in sacral injection group with statistics significance(P0.05);Both the analgesia effect and the improvement of lumbar function were obviously superior to pretherapy(P0.01),and electroacupuncture group was better than sacral injection group.Meantime,the electroacupuncture group also had advantages on curative and long-term effect. Conclusion:The electroacupuncture at Jiaji(EX-B2) is an effective way to treat prolapse of lumbar intervertebral disc and it's worth spreading in clinic.

Abstract:Objective: To explore the effect of the lumbar hernia of intervertebral discs and sciatica with the therapy of electric acupuncture.Methods: 120 cases of lumbar hernia of intervertebral discs and sciatica patients were distributed to the observation group(60) which were acquired lumbar traction,low frequency impulse electrotherapy and electric acupuncture treatment and the control group(60) which were acquired lumbar traction and low frequency impulse electrotherapy treatment.After two weeks of therapy,The pain of the visual analogue scale(VAS) in each group was recorded,the clinical assess of lumbar function was judged by the grade score table,the effect was judged by the clinical symptom and sign.Results: There was significant difference in acesodyne effect and lumbar development after 2 weeks of treatment in contrast to pretreatment in two groups(P<0.01).There was significant difference after 2 weeks of treatment in the acesodyne effect and lumbar development between the two groups(P<0.05).The experimental group got more cure rate and total effective rate than those of the control group(P<0.01).Conclusion: It was effective to the lumbar hernia of intervertebral discs in the lumbar traction and low frequency impulse electrotherapy,It could get better effect to the lumbar hernia of intervertebral discs and sciatica at the basis therapy of lumbar traction and low frequency impulse electrotherapy adding to the electric acupuncture.
Observations on the Efficacy of Electroacupuncture plus Point Injection in Treating Lumbar Intervertebral Disc Protrusion

Abstract:Objective To investigate the efficacies of three therapies,electroacupuncture,point injection and electroacupuncture plus point injection,in treating lumbar intervertebral disc protrusion.Methods Ninety patients were selected and allocated by simple randomization to three groups,electroacupuncture plus point injection,electroacupuncture and point injection,30 cases each.The criteria of therapeutic effects and a quantitative criterion(JOA and VAS) were used.A comparison was made between pretreatment and posttreatment within one group and between different groups.Results There was a statistically significant difference in the score between pretreatment and posttreatment in all the three groups(P<0.01).Scoring showed that the effect was significantly better in the electroacupuncture plus point injection group than in the electroacupuncture group or the point injection group(P<0.05).The cure rate was significantly higher in the electroacupuncture plus point injection group than in the electroacupuncture group(P<0.05).In the first course of treatment, the effect of electroacupuncture was superior to that of point injection(P<0.05 by VAS analysis);in the second course of treatment,the effect of point injection was superior to that of electroacupuncture(P<0.05 by VAS analysis).In the three groups,the cure rate was significantly lower in the patients with the duration of disease > twelve months than in those ≤ twelve months(P<0.05).The cure rate was significantly lower for liver-kidney depletion than for the other three types of syndromes.Conclusion Electroacupuncture plus point injection has clinically a good synergic effect in treating lumbar intervertebral disc protrusion,but the cure rate is lower in the patients with the duration of disease over twelve months and those with liver-kidney depletion.
Clinical Study of Qi-guiding Acupuncture at Points Selected According to Meridian Differentiation for Treatment of Lumbar Intervertebral Disc Herniation

Abstract:ObjectiveTo investigate the efficacy of meridian differentiation-based point selection plus qi-guiding acupuncture in treating lumbar intervertebral disc herniation.MethodFive hundred and forty-nine patients with lumbar intervertebral disc herniation were randomly allocated, using a random number table, to two groups. The treatment group of 280 patients received qi-guiding acupuncture at points selected according to meridian differentiation and the control group, electroacupuncture at points selected according to meridian differentiation.ResultThe total efficacy rate was 87.5% in the treatment group and 86.6% in the control group; there was no statistically significantdifference between the two groups (P>0.05). The cure rate was 51.8% in the treatment group and41.6% in the control group; there was a statistically significant difference between the two groups (P<0.05). ConclusionThe total efficacy rate of qi-guiding acupuncture at points selected according to meridian differentiation for lumbar intervertebral disc herniation is similar to that of electroacupuncture at points selected according to meridian differentiation, but the cure rate of the former is higher thanthat of the latter.

Abstract:To observe the relation between the course and therapeutic effect of electroacupuncture in treating prolapse of lumbar intervertebral discs, and the different effects improved by electroacupuncture between different clinical symptoms of lumbar intervertebral discs, according to the symptoms of lumbago, pain of leg or pains of both lower back and leg, the cases were divided into different therapeutic groups. Every group was treated for three courses of treatment. There was a significant difference between the therapeutic effect and the course of treatment ( P <0.01). There was a difference between the symptoms and the therapeutic effects ( P <0.05). Among the clinical symptoms of lumbar intervertebral disc improved by electroacupuncture, lumbago was most obvious, pain of leg was worst. The therapeutic effect of the second course of treatment was relatively marked.

Abstract:AIM:This study was intended to verify the advantages of electro-acupuncture therapy on improving the symptoms and side effects of lumbar intervertebral disc protrusion and to validate the clinical effect of electro-acupuncture in treating lumbar intervertebral disc protrusion. METHODS:①Totally 200 patients with lumbar intervertebral disc protrusion,who were treated at Sixth People's Hospital Affiliated to Shanghai Jiao Tong University and Shanghai Hospital of Traditional Chinese Medicine from January 2005 to January 2006,were enrolled. They all singed the informed consent. G6805-Ⅱ electro-acupuncture apparatus was provided by Shanghai Gaoxin Medical Instrument Company. 30# 1.5 cun and 3 cun stainless steel acupuncture needle was Brand Huatuo and offered by Suzhou Medical Article Factory. Mobic tablets were provided by Germany Bolingeyingehan International Company. ②The patients were randomly divided into observational group(n =100) and control group(n =100). ③The Jiaji(EX B 2),Yaoyangguan(Du 3),Huantiao(GB 30) and Yanglingquan(G 34) were applied in observational group with 30# 1.5 cun and 3 cun stainless steel acupuncture needle and then frequency of 4 Hz and current intensity of 2 mA G6805-Ⅱ electro-acupuncture apparatus,continuous wave for 20 minutes. Mobic was taken in the observational group,7.5 mg per day. The treatment in both the groups were given once a day for 10 days,with an interval of 5 days,then given for another 10 days. ④The improvement of lumbago,pain or numbness of lower limb,walking ability,sensory disability,raising straight leg and muscle strength was observed. RESULTS:①The total efficacy percent of observational group was higher than that of the control group(86.53%,75.00%,P < 0.01). The improvement of lumbago,pain or numbness of lower limb,walking ability,raising straight leg and muscle strength was better in the observational group than the control group(P < 0.01). ②There were poor appetite,nausea,abdomen pain,swollen,headache and dizziness in control group. But there had been no side effect in observational group besides bleeding. CONCLUSION:Electro-acupuncture therapy has good therapeutic effect on the symptoms and side effects of lumbar intervertebral disc protrusion.

Abstract:In clinical practice, lumbar disc herniation is also called lumbar disc herniation due to the rupture of the annulus fibrosus. The degenerative changes of the nucleus pulposus, annulus fibrosus, cartilage plate and other positions with different degrees and symptoms make it difficult to bear excessive external force, which leads to the increase of external load. For patients with lumbar disc herniation, acupuncture has the functions of dredging meridians, promoting blood circulation, and harmonizing qi and blood, and can improve local blood circulation through acupuncture. Relieve muscle spasm and dilate blood vessels, which can play a therapeutic or auxiliary role in the treatment of lumbar disc herniation. Acupuncture therapy is generally divided into general acupuncture, electroacupuncture, acupuncture plus moxibustion, acupuncture plus massage therapy, etc., which have good clinical effects. Therefore, this paper adopts the random control method to study and discuss the curative effect analysis of acupuncture, massage and rehabilitation nursing intervention on patients with lumbar disc herniation. There are many reports on the clinical observation of acupuncture in the treatment of lumbar protrusion. At the same time, supplemented by massage, it can open the channels of patients, regulate their qi and blood, promote blood circulation and remove blood stasis, relieve the muscle tension of patients, and at the same time act on protrusions to promote their movement or retraction, and improve the state of patients' back.

Clinical Analysis of A Composite Treatment with Silver Needles for Lumbar Intervertebral Disc Herniation

Abstract:Objective To evaluate the analgesic effects of warm acupuncture with silver needles, warm acupuncture with filiform needles and sinew injury-treating manipulation on lumbar intervertebral disc herniation. Method Ninety patients with lumbar intervertebral disc herniation were allocated to three groups, 30 cases each. The medication group received oral administration of celecoxib capsules, an anti-inflammatory and analgesic drug and the other two groups, warm acupuncture with silver needles and warm acupuncture with filiform needles, respectively. Random half patients of every group received sinew injury-treating manipulation as well. An evaluation was made using the Oswestry Disability Index(ODI) in every group before treatment(T0) and after one course of treatment(35th day, T1). Result After treatment, the improvement rate was 96.7% in the silver needle group, 90.0% in the filiform needle group and 83.3% in the medication group; there was a statistically significant difference among the three groups(P0.05) and the improvement rate was higher in the silver needle group than in the other two groups. Analysis of variance of factorial design showed that the ODI score changed in all the three groups after treatment compared with before(P0.05). All the three treatments had a good analgesic effect. There was a statistically significant post-treatment difference in the ODI score among the silver needle, filiform needle and medication groups(P0.05) and silver needles had a better analgesic effect. Warm needling moxibustion plus sinew injury-treating manipulation had a better relieving effect on the pain(P0.05). Acupuncture, medication and manipulation did not produce a marked reciprocal effect and their effects were mutually independent. Conclusion Warm acupuncture with silver needles plus sinew injury-treating manipulation can markedly relieve the pain in lumbar intervertebral disc herniation.

Abstract:Objective: The present study is to investigate the mechanism of electro-acupuncture in preventing and treating lumbar intervertebral disk displacement.Methods: Seventy-five Sprauge-Dawley rats were used to establish the models of lumbar intervertebral disk displacement by operation, and divided into five groups including electro-acupuncture group, western medicine group, acupuncture plus medicine group, model group and normal control group, then they were treated with corresponding therapeutic method. The expression of inducible nitric oxide synthase (iNOS) mRNA in the lumbar intervertebral disk tissues of rats was detected by RT-PCRResults: The expression of iNOS mRNA in the lumbar intervertebral disk tissues of rats in model group was compared with that in normal control group, the significant difference was found (P<0.01); While the expressions of iNOS mRNA in the lumbar intervertebral disk tissues of rats in electroacupuncture group, western drug group and acupuncture plus medicine group were all inhibited, and compared with the model group, the significant differences were also found (P<0.01).Conclusion: Electro-acupuncture could inhibit the expression of iNOS mRNA in the lumbar intervertebral disk tissue.
CLINICAL OBSERVATION ON LUMBAR INTERVERTEBRAL DISC HERNIATION TREATED BY TRACTION COMBINED WITH ACUPUNCTURE

Abstract:Objective: To observe the therapeutic effect of traction combined with acupuncture on lumbar intervertebral disc herniation (LIDH).Methods:Eighty-three patients diagnosed as LIDH were randomly divided into treatment group (n=42)and control group (n=41).In the treatment group, besides lumbar traction, electroacupuncture at Jiaji (EX-B 2, L3～5), Shenshu (BL 23), Qihai (CV 6), Mingmen (GV 4), Huantiao (GB 30), Chengshan (BL 57) and Yanglingquan (GB 34) was added.In the control group, only lumbar traction was applied.Results: In treatment group, the cure plus markedly effective rate was 80.95% with a total effective rate of 92.86%, while in control group, it was 39.02% with a total effective rate of 80.48%.The therapeutic effect in the treatment group was obviously better than that in the control group (P<0.001).Conclusion: Lumbar traction plus EA can effectively alleviate or even eliminate clinical symptoms and signs of lumbar intervertebral disc herniation.

Abstract:This study aimed for the analysis of the effect of acupuncture and moxibustion combined with needle-knife on pain and lumbar function in patients with lumbar disc herniation. From June 2019 to February 2021, the medical records of 126 patients with lumbar disc herniation admitted to the department of orthopedics of our hospital were selected and divided into the control group (n = 63) treated with acupuncture and moxibustion and the observation group (n = 63) treated with acupuncture and moxibustion combined with needle-knife according to different treatment regimens. After 4 weeks of treatment, the clinical efficacy, pain status, and lumbar function were compared between the two groups. The concentrations of relevant inflammatory factors (IL-6, IL-10, TNF-α, and MMP-2) in peripheral blood of the two patients before and after treatment were measured by enzyme-linked immunosorbent assay (ELISA). After treatment, the overall response rate was 93.65% in the observation group, which was higher than 80.95% in the control group (P < 0.05); the visual blurred score (VAS) scores of lower limbs and waist in the observation group were lower than those in the control group, while the expression of pain mediators serotonin (5-HT) and prostaglandin E2 (PEG2) was also lower than that in the control group (P < 0.05); the Oswestry disability index (ODI) in the observation group was lower than that in the control group, while the Japanese Orthopedic Association assessment treatment score (JOA) was higher than that in the control group (P < 0.05). After treatment, the concentration levels in peripheral blood (IL-6, IL-10, TNF-α, and MMP-2) were significantly lower in the observation group than in the control group (P < 0.05). Acupuncture and moxibustion combined with needle-knife is effective in the treatment of lumbar disc herniation, which helps to improve the clinical efficacy, relieve pain symptoms, promote the improvement of lumbar function, and contribute to the reduction of inflammatory factors.
The Efficacy of Acupuncture and Acupoint Massage in Enhancing Lumbar Mobility and Alleviating Pain in Individuals with Lumbar Disc Herniation

Abstract:Background: Lumbar disc herniation (LDH) is a prevalent condition in spinal surgery, and Traditional Chinese Medicine (TCM) acupuncture has gained clinical attention as a potential treatment for LDH in recent years. Objective: This study aimed to assess the therapeutic efficacy of acupuncture combined with acupoint massage in LDH patients. Methods: We enrolled a total of 135 LDH patients treated at our hospital from May 2021 to February 2023. Among them, 63 patients received acupuncture treatment (control group), while the remaining 72 received acupuncture combined with acupoint massage (observation group). We compared treatment efficacy and the time it took for lumbar stiffness, lower back and leg pain, bending and flexing difficulties, and other symptoms to disappear between the two groups. The Visual Analogue Scale (VAS) and Japanese Orthopedic Association Scoring System (JOA) were used to evaluate patients' pain levels and lumbar vertebral function before and after treatment. Additionally, we assessed patients using the Generic Quality of Life Inventory-74 (GQOL-74) and recorded their treatment satisfaction. Results: The observation group exhibited a slightly higher total effective rate compared to the control group, with a shorter time for the resolution of lumbar stiffness, lower back pain, leg pain, and other symptoms (P < .05). Furthermore, the observation group had lower VAS scores and higher JOA scores (P < .05). They also achieved higher GQOL-74 scores and reported greater treatment satisfaction (P < .05). Conclusions: Acupuncture combined with acupoint massage effectively alleviated clinical symptoms and pain in LDH patients, demonstrating significant clinical utility.
Clinical Observation on Qi-conducting Needling Method Combined with Acusector in Treating Lumbar Disc Herniation

Abstract:Objective: To observe clinical effects of Qi-conducting needling method combined with acusector in treating lumbar disc herniation (LDH). Methods: All 150 patients were randomized into two groups, 75 cases each group. The control group was treated by retaining acusector, the observation group accepted Qi-conducting needling method to promote Qi-circulation and then retaining acusector. The course was two weeks. Clinical effects were observed, VAS and JOA scores were counted and classified before and after treating. Results: Total effective rate of the observation group was 98.5％, higher than 94.5％ of the control group, and the difference had no statistical meaning (P＞0.05); the difference had statistical meaning in the comparisons of VAS and JOA scores and the classification before treating within the same group (P＜0.01), the difference had statistical meaning in the comparisons of VAS after treating between both groups (P＜0.05), the difference had no statistical meaning in the comparisons of JOA scores (P＞0.05). Conclusion: Qi-conducting needling method combined with acusector could relieve LDH patients' clinical symptoms including pain, numbness, sore and swelling and others.
Acupuncture treatment for lumbar disc herniation with scoliosis: A case report

Abstract:Lumbar disc herniation (LDH) is a common and prevalent clinical condition, and the number of patients has increased in recent years. Some patients with LDH may have scoliosis at the same time, which affects the mechanical joint structure of the spine and trunk and can seriously affect their quality of life. LDH with scoliosis is clinically treated with a stepwise approach, from conservative treatment, minimally invasive surgery, conventional decompression surgery, non-fusion fixation surgery, to fusion fixation surgery. 80% to 90% of patients can be cured or improved with non-surgical treatment. [ 1 ] Therefore, conservative treatment is the treatment of choice for LDH, which includes anti-inflammatory and pain relievers, neurotropics, Traditional Chinese Medicine (TCM), acupuncture and massage, physical therapy, and life coaching. [ 2 , 3 ]

Abstract:Objective:To evaluate the effectivenes and safety of percutaneous endoscopic intervertebral disk ablation for the treatment of lumbar intervertebral disk herniation.Method:58 patients who suffered form simple lumbar intervertebral disk herniations were confirmed.Among them,44 were L4/5 disk herniation,13 L5/S1 disk herniation,and one L3/4 disk herniation.Spine endoscope was inserted into disk space percutaneously via dilators under the monitor of C-arm X ray.Herniated intervertebral disks were ablated by using Ellman radiofrequency unit with patented Trigger-Flex electrodes.The whole procedures were directly monitored.Intervertebral pressures were significantly reduced after majority parts of the disks were ablated.Result:The effective rate of the operation was 90% after one to five months follow up study.36 patients(62%) showed significant improvement,16 patients(28%) showed improvement,and 6 patients(10%)showed no improvement after operation.No complication was observed after operation.Conclusion:It shows that the percutaneous endoscopic intervertebral disk ablation is a safe,effective and truely minimal invasive treatment for intervertebral disk herniation.

Abstract:Abstract. The effectiveness of acupuncture in the treatment of discogenic compression radiculopathy has been proven in many scientific works. Acupuncture has a positive effect not only on the course of radiculopathy, but also on the size of the hernias and protrusions of the intervertebral discs themselves. Purpose. To study the effectiveness of acupuncture in the treatment of intervertebral disc herniation in the lumbar region. Material and methods. Before and after treatment (after 3 and 6 months), the patient was assessed: neurological status, results of an electrokymographic study of peripheral nerves of the lower extremities, MRI of the spine. Acupuncture was carried out at acupuncture points of the meridian of the stomach, bladder, gallbladder, colon, for a course of 40 procedures. Results. A 40‐year-old patient suffered from prolonged pain for 8 months. Conservative treatment with drug therapy and physical therapy did not provide satisfactory improvement in symptoms. On MRI of the PC department, the IVD herniation at the L4 — L5 level was 12 mm. When using acupuncture, the pain syndrome decreased clearly after the first sessions. Sensory disturbances and motor deficits gradually regressed. After 40 sessions over 3 months, electromyographic indicators of the affected right peroneal nerve improved. An unexpected result was a reduction in the size of the IVD hernia by 50 % to 6 mm.
Clinical Trial on Lumbar Disc Herniation with Acupoint Injection in the Comprehensive Therapy of TCM

Abstract:Objective To observe the clinical effectiveness and safety on lumbar disc herniation (LDH)treated with TCM specific scheme,in which acupoint injection was the main therapy,combined with Chinese medicine iontophoresis and Chinese massage. Methods One hundred and twenty LDH patients were randomized into a treatment group and an observation group,60 cases in each one. In the treatment group, acupoint injection was combined with Chinese medicine iontophoresis and Chinese massage. In the control group,the intramuscular injection of cobamamide and oral administration of celecoxib were used. The treat-ment was given once every day,the treatment of 2 weeks made one session,2 sessions were required in the two groups. After treatment,clinical effects and the improvements in pain and lumbar function were observed in the two groups. Results After treatment,the total effective rate was 91. 53% in the treatment group and was 77. 27% in the control group,indicating the significant difference(P < 0. 05). The differences in VAS, ODI and JOA scores were not significant statistically(P > 0. 05). After treatment,for VAS and ODI,the re-sults in the treatment group were better than those in the control group,indicating the significant differences (P < 0. 05). For JOA,the scores for subjective symptoms and daily activity in the treatment group were better than those in the control group,indicating the significant differences(P < 0. 05). The differences in physical signs and bladder function were not significant statistically(P > 0. 05). Conclusion TCM specific scheme, in which acupoint injection is the main therapy,combined with Chinese medicine iontophoresis and Chinese massage,alleviates pain and improves limb function in LDH and has no apparent adverse reaction. This thera-py deserves to be promoted in clinic.
