Abstract:It is essential for social interactions if one can process theperceived external world with appropriate cognitive control and well-regulatedemotion. However, compared with healthy individuals, patients withmajor depression disorder (MDD) usually exhibit compromised cognitiveand enhanced emotional processes, respectively; and these behavioralchanges have been shown to be associated with alterations in functionalbrain networks. Recent advances of MDD research have been increasinglyinvolving functional magnetic resonance imaging (fMRI) and focusingon brain networks of cognition and emotion, as well as the interactionof these two networks. While inconsistencies and debates noted, thepresent paper attempts to review and provide a summarized findingfor these recent fMRI studies. Converging results have shown thatMDD is associated with significant alterations in the fronto-limbicsystem. Specifically, compared with healthy controls, individualswith MDD usually show hyperactivation to negative facial stimuli inthe amygdala, reflecting increased withdrawal, the core syndrome ofdepression. Some studies have also attempted to use hyperactivitiesof amygdala as the biomarker and reported its predictive value forMDD. Besides amygdala, increased insula response to disgusting faceswas also reported in literature, emphasizing the role of disgust inemotional dysregulation. Cognitive abnormalities in depression mainlyconcern the default mode and executive control networks. In additionto alterations of regional activation in task state, individuals withMDD exhibit increased functional connectivity of the default modenetwork in resting state, suggesting enhanced self-referential processes.In particular, higher functional connectivity in the anterior defaultmode network corresponds with higher level of rumination. In the executivecontrol network, many previous studies observed a decreased activationof dorsolateral prefrontal cortex in depression, indicating impairedability of cognitive control. At the same time, recent research foundthat transcranial direct current stimulation in the left dorsolateralprefrontal cortex was helpful for working memory enhancement in individualswith MDD. While alterations were reported in the emotional and cognitivenetworks separately, interactive changes of these 2 modules have alsobeen examined when they are simultaneously probed. Most studies havenoted that when challenged with negative emotion, individuals withdepression show increased activation in amygdala and decreased activationin dorsolateral prefrontal cortex, indicating a biased processingresource allocation towards the emotional system. Based on the aforementionedfMRI studies of MDD, we suggest that future improvements may consideraspects of (i) matching depressive syndromes for optimized confoundingfactor control; (ii) incorporating multimodal approaches for bothbrain imaging and stimulation; as well as (iii) applying neuroimagingtogether with psychological therapies either for prognosis evaluationor neurofeedback training.
