Effects of Maternal Position during Induction of Combined Spinal-epidural Anesthesia for Caesarean Delivery

Abstract:【Objective】 To observe the effects of maternal position on caesarean delivery during induction of combined spinal-epidural anesthesia.【Methods】 Healthy women, at term of pregnancy, about to undergo an elective cesarean section under CSE(combined spinal-epidural) anesthesia, were randomly assigned to(group A) the full left lateral position for 10 minutes(n = 40) or to(group B) the supine position with 20°left lateral tilt(n = 40) or to(group C) the supine position with 20°foot lateral tilt(n = 40) from the initiation of the block and A wedge-shaped cushion about 10cm was placed under the right hip to tilt the pelvis .An anesthesiologist evaluated the cool sensory level of anesthesia, maternal heart, blood pressure, oxygen saturation, need for ephedrine.【results】 The severity of hypotension was greater in group C than that of group A and group B at the points of 4, 8, and 10 minute, and the severity of hypotension in group B was greater compared with that of group A.Ephedrine requirements before re-position for surgery were less in group A than that of group B and group C.Sensory block to cool sensation at the T5 dermatomal level was most quickly achieved in the group B.【Conclusion】 We demonstrated that the full left lateral position and the supine position with 20° left lateral tilt with A wedge-shaped cushion under the right hip reduced the incidence of early hypotention compared with the supine position with 20° foot lateral tilt.
