Abstract:Diffusion tensor imaging (DTI) uses the diffusion motion of water molecules to describe the spatial structure of brain.DTI,which can obtain the infiltration of brain tumors into normal physiological structures,makes the diagnosis of diseases change from macroscopic to microscopic,from morphology to both morphology and metabolism,bringing new opportunities for the diagnosis and treatment of brain tumors.In this review,we outlined the theoretical basis and current technical limitations of DTI,and comprehensively discussed its research values in brain tumors radiotherapy treatment and its prognostic value.We believe that DTI can provide important imaging information for radiotherapy planning of brain tumors.

Abstract:Objective:This article is to investigate the potential of DWI in cervical lymph node lesions and focus on the technique parameters that affect the quality of DW imaging.Methods:Conventional MR imaging(spin echo T_1WI,T_2WI and STIR) was used to scan 30 patients with histologically proven malignant tumors(16) and inflammatory lesions(14) on a GE 1.5T scanner using cervical phased array coil.Six B values(30 cases) were acquired with diffusion gradients applied in all three orthogonal directions using body coil.Diffusion scan time was 40～52 seconds.Results:All benign and malignant lymph nodes were visible in DWI and showed significant hyperintensity signal.Calculated ADC malignant lymph nodes was statistically different from that of benign lymph nodes.When various of scan parameters were mixed rationally,MRI may improve signal-noise-ratio of image quality and lessen geometric malformation.Conclusion:Diffusion weighted imaging is a rapid and feasible method in detecting cervical lymph node lesions and shows a potential for distinguishing between benign and malignant lymph nodes.

Abstract:Background and purpose:With the increased number of advanced tumors,it is important to evaluate the actual pre-and post-treatment changes in metastatic diseases,such as tumor size and tumor quantity.Previously,CT and/or MRI scan were usually applied,only a few patients could pay for PET(positron emition tomography).But there were several problems we should face,including redundantly exposure to radiation,too long verification time and heavy financial burden.Therefore we evaluated the application of whole body diffusion weighted imaging in diagnosing of tumor and metastatic tumor.Methods:sixty-eight patients(49 male,19 female,age from 29 to 84 years with mean age of 61 years) with a variety of tumors were investigated by combined CT and/or MRI scan,seventeen patients were compared with positron emition tomograph(PET).All tumors were classified into four groups,according to longest diameter of <1 cm,≥1-<2 cm,≥2-<3 cm and ≥3 cm.At the same time,the positive detection rates between different parts of the lesions were analyzed.Results:The detection rate of the four groups were 30%,78%,96% and 100%,respectively.The skeletal system had the highest sensitivity and accuracy in every part of body.There was difficulty in diagnosing lung cancer with longest diameter less than 1 cm as well as tumors located in neck and pelvic with longest diameter less than 2 cm.Conclusion:Whole body diffusion weighted imaging is secure,convenient,effective and economic for screening wide-ranging tumor focus.After improvement of parameters,WBDWI could be as a new effective whole body examination technique.

Abstract:<p>The diffusion‐weighted imaging signal is derived from the motion of water molecules. It represents the physiological characteristics of tissue and is an essential imaging sequence for prostate cancer. Whole‐body diffusion‐weighted imaging facilitates the assessment of pathological conditions throughout the body. The concept of diffusion‐weighted whole‐body imaging with background body signal suppression and technological advances has led to routine clinical deployment of whole‐body diffusion‐weighted imaging as a one‐step staging tool for prostate cancer. Furthermore, whole‐body diffusion‐weighted imaging has the potential to reveal the state of osseous lesions, for which conventional imaging techniques are suboptimal for monitoring the response to therapy, and extraosseous lesions, and holds promise as a new imaging‐based therapeutic approach. This article reviews the basics of whole‐body diffusion‐weighted imaging, and focuses on application of whole‐body diffusion‐weighted imaging in the clinical management of prostate cancer at various stages in the course of the disease.</p>

Application of 3.0T-MRI DWI in evaluating the efficacy of synchronous radiotherapy and chemotherapy for advanced lung cancer

Abstract:Objective To assess the clinical applications of 3.0 T magnetic resonance (MRI) diffusion weighted imaging (DWI) in evaluating the efficacy of synchronous radiotherapy and chemotherapy for advanced lung cancer. Methods 59 patients with lung cancer confirmed by biopsy were examined with 3.0 T-MRI DWI. The values of apparent diffusion coefficient (ADC) and tumor cell density (TCD) were measured. We analyzed the ability of DWI to display the lesions of lung cancer and compared the correlation between the ADC value and TCD of lung cancer. Results 59 cases of lung cancer showed obviously high signal on DWI. The mean average ADC of the cancer was (993±170) ×10-6 mm2/s, which was significantly less than the ADC mean of the pectoralis major muscle (1388±148) ×10-6 mm2/s. There was statistically significant difference between the two (P<0.001). There was a difference in the mean value of ADC between non small cell lung cancer (NSCLC) and small cell carcinoma (SCLC) (P=0.005<0.05). A total of 45 TCD values were obtained. There was a significant statistical difference in TCD between NSCLC and SCLC (P=0.047 < 0.05).There was a negative correlation between the ADC value and pathological TCD of lung cancer (P=0.001, correlation coefficient =-0.534). Conclusion The involvement area of lung cancer can be clearly demonstrated by DWI examination. It is not only beneficial to the differential diagnosis of SCLC and NSCLC, but also can reflect the TCD value of the tumor indirectly theseby providing, reliable basis for monitoring therapeutic effect and prognosis of lung cancer.

Abstract:Diffusion-weighted magnetic resonance imaging (DWI) provides functional information and can be used for the detection and characterization of pathologic processes, including malignant tumors. The recently introduced concept of “diffusion-weighted whole-body imaging with background body signal suppression” (DWIBS) now allows acquisition of volumetric diffusion-weighted images of the entire body. This new concept has unique features different from conventional DWI and may play an important role in whole-body oncological imaging. This review describes and illustrates the basics of DWI, the features of DWIBS, and its potential applications in oncology.

Abstract:OBJECTIVE: To investigate the feasibility of whole body diffusion weighted imaging (WB-DWI) in screening metastasis.METHODS: WB-DWI was performed in 24 patients diagnosed with various types of primary tumors. The three-dimensional maximum intensity projection reconstruction and black-and-white flip technique were used to observe metastatic lesions, and the results were compared with those of bone scintigraphy.RESULTS: By WB-DWI scanning sequence at b = 800 s/mm2, all the bone lesions found by bone scintigraphy in the cohort were well identified, and other lesions of soft tissue and organs were also well demonstrated. Its screening capability was equivalent with bone scintigraphy in screening metastases in bones (P = 0.062).CONCLUSION: WB-DWI was practicable with the parameter settings attempted in metastases screening.

Abstract:Objective To explore the the clinical application value of 3.0 T magnetic resonance imaging ( MRI) diffusion weighted imaging (DWI) in advanced gastric cancer.Methods 60 patients with advanced gastric cancer confirmed by ultrasonic gastroscopy and pathology underwent 3.0 T MRI DWI examination.The displaying ability of DWI for gastric cancer lesions and lymph nodes around the stomach was analyzed .By measuring the normal stomach and tumor involving the area of the ADC values , we compared the differences between the two parameters .Results MRI DWI was able to display the focal area of gastric cancer , swollen lymph nodes around the stomach in all 60 patients, which obviously showed high signal .The mean ADC value of cancer was 1.444 ±0.029 ×10-3 mm2/s, the average ADC value of lymph node was 1.375 ±0.205 ×10-3 mm2/s.The normal stomach wall exhibited low signal , the average ADC value of normal stomach was 2.088 ±0.194 ×10-3 mm2/s, with significant difference compared with and the former two parameters ( P <0.001).After chemotherapy, the tumor ADC value increased and the tumor ADC value was 1.572 ±0.261 ×10-3 mm2/s, showing statistical significant difference compared with that before treatment ( P <0.001).Conclusion MRI DWI examination can effectively detect the status of the affected the scope and stomach and lymph node enlargement in patients with advanced stomach cancer .By comparing ADC values before and after new adjuvant chemothera-py for gastric carcinoma , we can effectively evaluate its curative effect .

Abstract:Diffusion-weighted magnetic resonance imaging (DWI) provides qualitative and quantitative information about the random motion of water molecules in biological tissues and is able to give functional insight into tissue architecture and pathological changes on a cellular level. This technique has the major advantages of not requiring the administration of contrast agents and not exposing the patient to ionizing radiation. Recent technological advances have led to the development of diffusion-weighted whole-body imaging with background body signal suppression (DWIBS) that allows screening of the whole body in 25 minutes. DWI and DWIBS have both revealed great potential in the field of oncology and proved to be useful for detecting and characterizing tumors and evaluating treatment response. This article reviews the basic principles and experimental setup of DWI and DWIBS and illustrates its potential application to the assessment of extracranial tumors. In addition, current limitations and challenges of this promising imaging procedure are discussed.
Application Of Whole Body Diffusion Weighted MR Imaging for Diagnosis and Staging of Malignant Lymphoma

Abstract:Objective To evaluate the clinical impact of whole body diffusion weighted imaging (WB-DWI) on diagnosis and staging of malignant lymphoma. Methods Thirty-one patients with suspected lymphadenopathy were enrolled. WB-DWI was performed by using short TI inversion recovery echo-planar imaging sequence with free breathing and built-in body coil. Axial T2-weighted imaging images of the same location were used as reference. The results of WB-DWI were compared with pathological results and other imaging modalities. The mean apparent diffusion coefficient (ADC) values of different kinds of lymph nodes were compared. Results WB-DWI was positive in all 18 cases with lymphoma, 5 cases with metastatic lymph nodes and 4 of 8 cases with benign lymphadenopathy. The mean ADC value of lymphomatous, metastatic and benign lymph nodes was (0.87 ± 0.17) × 10−3, (0.98 ± 0.09) × 10−3 and (1.20 ± 0.10) × 10−3 mm2/s. There was significant difference in ADC value between benign lymph nodes and other two groups (P < 0.01). The sensitivity, specificity and accuracy of WB-DWI in diagnosis of lymphoma were 100% (18/18), 30.8% (4/13) and 71.0% (22/31). When an ADC value of 1.08 × 10−3 mm2/s was used as the threshold value for differentiating malignant from benign lymph nodes, the best results were obtained with sensitivity of 87.8% and specificity of 91.3%. Sixteen of eighteen cases (88.9%) of lymphoma were accurately staged in accordance with clinical staging. Conclusions WB-DWI is a sensitive, but less specific technique for diagnosis of lymphoma. It is difficult to differentiate lymphomatous from metastatic lymph nodes using WB-DWI. However, it is a valuable imaging modality for staging of patients with malignant lymphoma.

Abstract:Conventional imaging modalities,such as CT and MRI,have been extensively applied to the diagnosis,staging,evaluation and follow-up in the head and neck cancer.However,there is still a way to go in the staging of lymph node and differential diagnosis of recurrence and post-radiation effects.Diffusion weighted imaging, a new technique in MRI,can provide more detailed anatomic information,and can play a role in the following aspects: pathologic diagnosis,staging of lymph node,early prediction of therapeutic response and differential diagnosis of recurrence and post-radiation effects,which is quite valuable in the selection and optimization of therapeutic strategies of head and neck cancer.In this article,we reviewed the current applications of diffusion weighted imaging in the management of head and neck cancer.
Advanced Diffusion-Weighted MRI for Cancer Microstructure Assessment in Body Imaging, and Its Relationship With Histology

Abstract:Diffusion-weighted magnetic resonance imaging (DW-MRI) aims to disentangle multiple biological signal sources in each imaging voxel, enabling the computation of innovative maps of tissue microstructure. DW-MRI model development has been dominated by brain applications. More recently, advanced methods with high fidelity to histology are gaining momentum in other contexts, for example, in oncological applications of body imaging, where new biomarkers are urgently needed. The objective of this article is to review the state-of-the-art of DW-MRI in body imaging (ie, not including the nervous system) in oncology, and to analyze its value as compared to reference colocalized histology measurements, given that demonstrating the histological validity of any new DW-MRI method is essential. In this article, we review the current landscape of DW-MRI techniques that extend standard apparent diffusion coefficient (ADC), describing their acquisition protocols, signal models, fitting settings, microstructural parameters, and relationship with histology. Preclinical, clinical, and in/ex vivo studies were included. The most used techniques were intravoxel incoherent motion (IVIM; 36.3% of used techniques), diffusion kurtosis imaging (DKI; 16.7%), vascular, extracellular, and restricted diffusion for cytometry in tumors (VERDICT; 13.3%), and imaging microstructural parameters using limited spectrally edited diffusion (IMPULSED; 11.7%). Another notable category of techniques relates to innovative b-tensor diffusion encoding or joint diffusion-relaxometry. The reviewed approaches provide histologically meaningful indices of cancer microstructure (eg, vascularization/cellularity) which, while not necessarily accurate numerically, may still provide useful sensitivity to microscopic pathological processes. Future work of the community should focus on improving the inter-/intra-scanner robustness, and on assessing histological validity in broader contexts. Diagnostic capability of whole-body diffusion-weighted imaging in malignant tumors compared with positron emission tomography CT

Abstract:Objectives To evaluate the diagnostic capability of whole-body diffusion-weighted imaging (WBDWI) with 1.5 T MR scanner in malignant lesions and compare it with 18F-FDG PETCT.Methods Thirty-five patients with pathologically proved malignant tumors were enrolled in this study.All patients underwent both WBDWI and PET-CT,and were followed up for more than 6 months.The final diagnosis was determined based on the results of pathology and clinical follow-up.The value of ADC and SUVm~ of each lesion were measured.The ROC curve,sensitivity,specificity and accuracy of the two methods were compared.Kappa analyses were used to compare the diagnostic consistency between both methods.Areas under the curve were compared by Hanley & McNeil.Categorical variables were tested with the Chi-square test or McNemar test.Results The area under the ROC curve of WBDWI,PET-CT,eADC,and SUVmax were 0.952,0.970,0.844 and 0.858,respectively.There was no significant statistical difference between WBDWI and PET-CT,eADC and SUVmax (Z =2.153,1.195,P ＞ 0.05),but Az (WBDWI) was significantly larger than Az(SUVmax) (Z =7.456,P ＜ 0.01),Az (PET-CT) was significantly larger than Az (SUV) (Z =6.821,P ＜ 0.01).The sensitivity,specificity and accuracy were 86.6％,95.3％ and 89.5％ in WBDWI,and 94.4％,94.3％ and 94.3％ in PET-CT.Kappa value of the diagnostic consistency between WBDWI and PET-CT was 0.745.Conclusions WBDWI had a high sensitivity,specificity and accuracy in detecting malignant lesions,and a substantial consistency with PET-CT,which could provide an appropriate choice for radiation-sensitive population.

Abstract:Magnetic resonance imaging (MRI) has been used in prostate cancer (PCa) diagnosis for over a decade. Diffusion weighted imaging (DWI) is one of the methods of the MRI techniques that could provide qualitative as well as quantitative information at a cellular level. DWI potentially could develop into a powerful technique to improve the accuracy of MRI to evaluate the PCa. In this article, we will present the basics of DWI, clinical application, as well as the limitations and the future directions of DWI of PCa.
Clinical Application Study on Malignant Metastatic Diseases Between DWIBS and PET/CT

Abstract:Background and purpose: Diffusion-weighted whole-body imaging with background body signal suppression (DWIBS) can be used for magnetic resonance imaging systemic examination, especially in examing the metastatic lesions, lymph node and bone diseases, and the imaging result is similar with PET. This study aimed to evaluate the application value of magnetic resonance DWIBS and positron emission tomography with computed tomography (PET/CT) on malignant metastatic diseases. Methods: Thirty-six patients confirmed with malignant tumors accompanying metastasis by the pathology of operation or biopsy underwent both DWIBS imaging and PET/CT, chi-square test and Kappa test were used for comparing the detection results of metastasis by these 2 imaging methods. Results:Among the 36 malignant tumor patients with 238 metastatic lesions, 218 (91.6%, 218/238) lesions in DWIBS and 209 (87.8%, 209/238) lesions in PET/CT were detected, with 200 lesions detected by the two methods simultaneously, and the concordance rate was 88.7%(211/238);but there was no statistical signiifcance between this two methods (χ2=1.843, P=0.157). Kappa test showed a fair concordance rate between DWIBS and PET/CT (P=0.000).There were different significance between DWIBS and PET/CT in detecting metastatic lesions of brain and bone (P=0.005 and 0.031);But there was no signiifcant differences (P=0.309 and 1.000) in detecting metastatic lesions of lymph nodes and liver. Conclusion:DWIBS could detect metastatic lesions effectively, and there is ifne consistency with PET/CT. DWIBS is more sensitive than PET/CT in detecting metastatic lesions of brain and bone, so DWIBS could be chosed for screening metastatic lesions according to the characteristics of different primary tumors.
Clinical Value of Whole-body Magnetic Resonance Diffusion Weighted Imaging on Detection of Malignant Metastases

Abstract:OBJECTIVE: To evaluate the value of whole-body diffusion weighted imaging (WB-DWI) on detection of malignant metastasis.METHODS: Forty-six patients with malignant tumors underwent WB-DWI examinations between April 2007 and August 2007 in our hospital. Before WB-DWI examination, the primary cancers of all the patients were confirmed by pathology, and the TNM-stage was assessed with conventional magnetic resonance imaging (MRI) or computed tomography (CT). WB-DWI was performed using short TI inversion recovery echo-planar imaging (STIR-EPI) sequence. Abnormal high signal intensities on WB-DWI were considered as metastases. The results of WB-DWI were compared with other imaging modalities. For the assessment of the diagnostic capability of WB-DWI, WB-DWI were compared with CT for demonstrating mediastinal lymph node metastases and lung metastases, and with conventional MRI for demonstrating metastases in other locations.RESULTS: WB-DWI demonstrated 143 focuses, 14 of which were diagnosed to be benign lesions in routine imaging. The number of bone metastases depicted on WB-DWI and routine imaging was 85 and 86; lymph node metastases was 17 and 18; liver metastases was 14 and 14; lung metastases was 4 and 8; and brain metastases was 6 and 8, respectively. WB-DWI failed to detect 12 metastatic lesions including 3 osteoplastic bone metastases, 4 lung metastases, 3 mediastinal lymph node metastases, and 2 brain metastases. Four metastatic lesions including 2 deltopectoral lymph nodes and 2 rib metastases were detected with WB-DWI alone, all of which evolved greatly during clinical follow-up for more than 6 months. WB-DWI had higher detection rates for metastatic lesions in liver, bone, and lymph nodes than those in lung and brain (chi2=30, P<0.001).CONCLUSIONS: WB-DWI could detect most of metastatic lesions that were diagnosed with conventional MRI and CT. The limitations of WB-DWI might be had high false-positive rate and low efficiency in detecting mediastinal lymph node, brain, and lung metastases.

Abstract:Diffusion-weighted imaging (DWI), by adopting a special sequence to give prominence to scattered phases caused by water diffusion, allows in vivo demonstration of micro-diffusion process in macro-imaging. It is necessary to take diffusion trait of lesion as an important imaging character to study. ASSET-EPI technique improve the quality of spatial resolution and image quality of diffusion weighted image; increasing numbers of excitation approximately eliminate EPI and ASSET related artifacts; separated breath hold acquisition technique helps to acquire better diffusion images of abdomen. Clinical applications of DWI at a high b values (800-1000 s/mm2) had gained success in many fields, such as breast, stomach, liver, rectum, uterus and prostate and so on, which showed a promising prospect. Clinical research had showed that, signal changes in DWI could, to certain extent, reveal abnormality with yet no morphologically distortion; and apparent diffusion coefficient (ADC) value, a quantitative index, may provide a rather accurate evaluation for undetectable foci by naked eyes. Besides, relative index such as ratio of ADC value of lesion to surrounding tissue, also showed value in reflect specific histological characteristic of lesion in clinical application. Combined applications of DWI with other imaging techniques will be helpful to evaluate its clinical value objectively and fully understand the disease in a profound inspection.
Clinical application of body diffusion weighted MR imaging in the diagnosis and preoperative N staging of cervical cancer

Abstract:Objective: To evaluate the clinical impact of body diffusion weighted imaging (DWI) on the diagnosis and preoperative N staging of cervical cancer. Methods: Twenty-four patients (mean age 37.9 years old) with proved cervical cancer by cervical biopsy and 24 female patients with other suspected pelvic abnormalities received preoperative body DWI scan. Results of body DWI were compared with pathological findings. The apparent diffusion coefficient (ADC) values of normal cervix and different pathological types of cervical cancer were compared. ADC value of normal or inflammatory lymph nodes was also compared with that of metastatic ones. Student's t test was used for statistical analysis. Results: There were 5 adenocarcinomas and 19 epitheliomas showed with biopsy results, and DWI showed 21 cervical lesions out of them (87.5%). ADC values of the normal cervix (n = 24), epithelioma (n = 19), and adenocarcinoma (n = 5) were (1.73 +/- 0.31) x 10(-3), (0.88 +/- 0.22) x 10(-3), and (1.08 +/- 0.12) x 10(-3) mm2/s, respectively. Statistical analysis showed significant difference in ADC value between normal cervical tissue and either tumor tissues (both P < 0.01). In patients had lymphadenectomy (n = 24), totally 67 lymph nodes including 16 metastatic lymph nodes were pathologically analyzed, and DWI showed 66 (98.5%) out of them. ADC values of normal/inflammatory and metastatic lymph nodes were (1.07 +/- 0.16) x 10(-3) and (0.77 +/- 0.13) x 10(-3) mm2/s (P < 0.01). Receiver operating characteristic (ROC) curve of ADC value of metastatic lymph node showed that area under curve was 0.961. Conclusions: ADC value in cervical carcinoma is lower than that in normal cervix, and ADC may have predictive value in subtype discrimination. ADC value may improve the preoperative characterization of lymph node metastasis. And at least abdominal and pelvic DWI scan is suggested for N staging evaluation in such patients.

Abstract:Functional imaging with diffusion-weighted imaging (DWI) is a complementary tool to conventional diagnostic magnetic resonance imaging sequences. It is being increasingly investigated to predict tumor response and assess tumor recurrence. We elucidate the specific technical modifications of DWI preferred for gynecological imaging, including the different b-values and planes for image acquisition. Additionally, we discuss the problems and potential pitfalls encountered during DWI interpretation and ways to overcome them. DWI has a wide range of clinical applications in malignant and non-malignant gynecological conditions. It provides supplemental information helpful in diagnosing and managing tubo-ovarian abscess, uterine fibroids, endometriosis, adnexal torsion, and dermoid. Similarly, DWI has diverse applications in gynecological oncology in diagnosis, staging, detection of recurrent disease, and tumor response assessment. Quantitative evaluation with apparent diffusion coefficient (ADC) measurement is being increasingly evaluated for correlation with various tumor parameters in managing gynecological malignancies aiding in preoperative treatment planning. Newer advanced DWI techniques of diffusion tensor imaging (DTI) and whole body DWI with background suppression (DWIBS) and their potential uses in pelvic nerve mapping, preoperative planning, and fertility-preserving surgeries are briefly discussed.
