Effects of CVVH on the Prognosis of MODS Patients

Abstract:Objective To explore the effect of continuous veno-venous hemofiltration (CVVH) on the prognosis of multiple organ dysfunction syndrome(MODS) patients. Methods 516 sepsis patients with acute organ failures from 20 university hospitals were collected. Patients were divided into two groups according to hemofiltration or no hemofiltration. Mortality of two groups from different severity stages was compared. Results Patients with hemofiltration had a higher mortality than those without hemofiltration(P0.01), when their APACHE-Ⅱ scores were no more than 30. Patients with hemofiltration had a lower mortality than those without hemofiltration, when their APACHE-Ⅱ scores were more than 30,but there were no statistical differences(P0.05).Compared with patients without CVVH, patients undergoing CVVH with creatinine clearance rate between 40 mg/min and 80 mg/min had a higher mortality(P0.05). Conclusion Whether patients with severe sepsis would benefit from hemofiltration relates to severity of disease. MODS patients with APACHE-Ⅱ score ≤30 or with creatinine clearance rate between 40 mg/min and 80 mg/min would not benefit from this therapy, but it still remains to be studied on whether MODS patients with APACHE-Ⅱ30 would benefit from hemofiltration.
