Abstract:Objective:The typical clinical characteristics of drug dependence are compulsive drug seeking and drug taking. Evidences from clinical studies suggest that the craving for addictive substance of drug addicts keeps out of self-control even after long-term withdrawal,while the physiological symptoms might have disappeared for months or years. They perform with little or inadequate forethought and then continue drug taking. It has been proved that stress can induce drug craving. But there are rare studies on effects of stress on impulsive choice of heroin addicts at different abstinent time. The purpose of the present study was to investigate the effects of stress on decision-making in heroin addicts at different abstinent time. Methods:In the present study,we assessed decision-making performance in heroin addicts at different abstinent time by Iowa Gambling Task (IGT). The subjects provided written informed consent before testing. All subjects were 20 to 45 years old and heroin addicts had to be with a DSM-Ⅳ diagnosis of substance dependence to opiates. All subjects completed HAMA,BDI,BIS questionnaires and tests of attention and working memory. The subjects who had been drug free for 15 d,30 d,3 months,12 months,24 months and matched control were challenged with the Trial Social Stress Test (TSST) or a control condition to test the effects of stress on decision-making performance in a counterbalance design. Blood pressure,heart rate,differential emotion and salivary cortisol were monitored at pre-CONTROL,post-CONTROL,pre-TSST and post-TSST test time points. The subjects who had been drug free for 30 d,12 months and 24 months were randomly assigned to placebo or 40 mg propranolol. One hour after administration of placebo or propranolol orally,heroin addicts were challenged with the TSST or a control condition in a counterbalance design. Blood pressure,heart rate,differential emotion and salivary cortisol were monitored at time points described above. Results:The enhancement of decision-making performance after stress exposure was found in all heroin groups,but not in normal control group. In addition,this enhancement was blocked by 40 mg propranolol in 30 d,12 months and 24 months groups. Conclusion:The present study displayed that stress enhanced the impulsive choice in heroin addicts after withdrawal from drug taking,and the enhancement of stress induced could be blocked by propranolol. These findings have implications for behavioral and pharmacological interventions targeting impaired decision-making in heroin addiction.
